Abstract

ABSTRACT The use of mobile health applications (mHealth) for self-medication is increasingly common, especially among young adults. In Mataram, the demand for digital healthcare tools has surged, particularly in the wake of the COVID-19 pandemic, which has changed how healthcare is accessed and managed. This study aims to analyze the behavior of young adults in Mataram in using mobile health applications for medication in 2024. It seeks to understand the motivations, factors influencing usage, and the impact of these applications on self-medication practices. A qualitative approach using phenomenological research was employed, involving in-depth interviews with young adult users of mHealth applications. Data were collected from key informants who shared their experiences and perceptions about using these applications for health management. The results indicate that young adults in Mataram use mHealth applications primarily for convenience, cost-efficiency, and ease of access to health information. While the applications support self-medication, they also raise concerns about the accuracy of information and the need for professional healthcare guidance. The COVID-19 pandemic further accelerated the use of these applications, although some users still prefer in-person consultations for more complex health issues. mHealth applications have proven to be beneficial in enhancing access to healthcare and enabling self-medication among young adults. However, there is a need for improved regulations and supervision to ensure the safe and accurate use of these technologies, alongside greater professional involvement in health decision-making. Keywords: Mobile Health, Self-Medication, Young Adults, Mataram, Telemedicine, COVID-19, Health Behavior, Technology In Healthcare.  ABSTRAK Penggunaan aplikasi mobile health (mHealth) untuk swamedikasi semakin umum, terutama di kalangan dewasa awal. Di Mataram, permintaan terhadap alat kesehatan digital semakin meningkat, khususnya setelah pandemi COVID-19, yang telah mengubah cara akses dan pengelolaan layanan kesehatan. Penggunaan aplikasi mobile health (mHealth) untuk pengobatan semakin umum, terutama di kalangan dewasa awal. Di Mataram, permintaan terhadap alat kesehatan digital semakin meningkat, khususnya setelah pandemi COVID-19, yang telah mengubah cara akses dan pengelolaan layanan kesehatan. Pendekatan kualitatif dengan metode penelitian fenomenologi digunakan dalam penelitian ini, yang melibatkan wawancara mendalam dengan pengguna aplikasi mHealth dewasa awal. Data dikumpulkan dari informan kunci yang berbagi pengalaman dan persepsi mereka tentang penggunaan aplikasi ini untuk manajemen kesehatan. Hasil penelitian menunjukkan bahwa dewasa awal di Mataram menggunakan aplikasi mHealth terutama untuk kenyamanan, efisiensi biaya, dan kemudahan akses terhadap informasi kesehatan. Meskipun aplikasi ini mendukung swamedikasi, aplikasi ini juga menimbulkan kekhawatiran terkait keakuratan informasi dan perlunya bimbingan profesional dalam pengambilan keputusan kesehatan. Pandemi COVID-19 semakin mempercepat penggunaan aplikasi ini, meskipun sebagian pengguna masih lebih memilih konsultasi langsung untuk masalah kesehatan yang lebih kompleks. Aplikasi mHealth terbukti bermanfaat dalam meningkatkan akses layanan kesehatan dan mendukung swamedikasi di kalangan dewasa awal. Namun, dibutuhkan regulasi dan pengawasan yang lebih baik untuk memastikan penggunaan teknologi ini aman dan akurat, dengan tetap melibatkan tenaga medis dalam pengambilan keputusan kesehatan. Kata Kunci: Mobile Health, Swamedikasi, Dewasa Awal, Mataram, Telemedicine, COVID-19, Perilaku Kesehatan, Teknologi Dalam Layanan Kesehatan.

Abstract

ABSTRACT The advancement of information and communication technology has significantly impacted various sectors, particularly healthcare. Mobile health (mHealth) applications, particularly those offering telemedicine, are emerging as tools to provide accessible healthcare services remotely. This study focuses on the factors influencing the implementation of mHealth applications for telemedicine among young adults in Mataram, Indonesia. The purpose of this research is to analyze the factors that influence the adoption of mobile health applications for telemedicine among young adults in Mataram, using the Technology Acceptance Model (TAM). A quantitative research design was used, involving a survey of young adults (aged 18-40 years) in Mataram who have used telemedicine applications. The data was collected using a structured questionnaire with Likert scale items to measure perceptions of usefulness, ease of use, and other factors as per the TAM framework. The study found that perceived usefulness and ease of use were significant predictors of the intention to use telemedicine applications among young adults in Mataram. Social influence and trust also played crucial roles in enhancing the adoption of telemedicine. The findings suggest that improving the usability and highlighting the benefits of telemedicine applications, along with addressing privacy concerns, could facilitate wider adoption among young adults in Mataram. Keywords: Mobile Health, Telemedicine, Technology Acceptance Model, Young Adults, Mataram.  ABSTRAK Perkembangan teknologi informasi dan komunikasi telah membawa dampak besar dalam berbagai sektor, termasuk kesehatan. Aplikasi mobile health (mHealth), khususnya yang menawarkan telemedicine, semakin muncul sebagai solusi untuk memberikan layanan kesehatan jarak jauh yang lebih mudah diakses. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang mempengaruhi implementasi aplikasi mobile health untuk telemedicine di kalangan dewasa awal di Kota Mataram, Indonesia. Tujuan penelitian ini adalah untuk menganalisis faktor-faktor yang mempengaruhi adopsi aplikasi mobile health untuk telemedicine di kalangan dewasa awal di Kota Mataram dengan menggunakan pendekatan Technology Acceptance Model (TAM). Penelitian ini menggunakan desain kuantitatif dengan survei pada dewasa awal (usia 18-40 tahun) yang telah menggunakan aplikasi telemedicine. Data dikumpulkan menggunakan kuesioner terstruktur dengan skala Likert untuk mengukur persepsi kegunaan, kemudahan penggunaan, dan faktor lainnya sesuai dengan kerangka teori TAM. Penelitian ini menemukan bahwa persepsi kegunaan dan kemudahan penggunaan merupakan prediktor signifikan dari niat menggunakan aplikasi telemedicine di kalangan dewasa awal di Mataram. Pengaruh sosial dan kepercayaan juga berperan penting dalam meningkatkan adopsi telemedicine. Temuan ini menunjukkan bahwa meningkatkan kemudahan penggunaan dan menyoroti manfaat aplikasi telemedicine, serta mengatasi kekhawatiran terkait privasi, dapat mempercepat adopsi di kalangan dewasa awal di Mataram. Kata Kunci: Mobile Health, Telemedicine, Technology Acceptance Model, Dewasa Awal, Mataram.

Abstract

The administration of anesthesia is a medical procedure fraught with considerable risk. The prevalence of anesthesia-related mortality is often understated, reminiscent of the so-called iceberg phenomenon. On-the-ground realities reveal that due to a shortage of anesthesiologists, nurse anesthetists frequently perform anesthetic procedures under the guidance of anesthesiologists. The annulment of Minister of Health Regulation Number 32 from the year 2013 has resulted in vulnerabilities within the practice of anesthesia nursing. Given the significant consequences associated with anesthetic procedures, it is imperative to have legal safeguards in place for nurse anesthetists. This study utilizes a descriptive-analytical framework and adopts a normative juridical approach, employing deductive reasoning. The research relies on secondary data, which is qualitative and gathered through a comprehensive review of relevant literature. The study's analytical process makes use of normative qualitative methods. In the context of this study, nurse anesthetists receive their delegated authority from physicians and must operate within the constraints set forth by the Medical Practice Act, Health Personnel Law, and Health Minister Regulations related to medical practice permits. Nurse anesthetists assign their tasks based on authority that is delegated in line with legislation No. 38 from 2014 concerning nursing. The specialist anesthesiologist has both legal and moral responsibility for providing protection to nurses executing tasks under such delegated authority since their duties are performed according to physicians' orders and directives. There exists a pressing need to revisit Ministerial Regulation No. 18 from 2016, concerning the licensing and execution of anesthesia management, in order to clarify the roles and functions proprietary to anesthesiologists as determined by their expertise. It is incumbent upon the Minister of Health to promptly issue new regulations pertaining to nurse anesthesia practice, ensuring that healthcare services are provided without any legal discrepancies.

Abstract

The Family Posyandu (PosGa) is a family-based health service innovation aimed at improving community health care access and quality, but faced significant challenges in rural areas. This study analyzed PosGa implementation in the Kalijaga Health Center area, East Lombok Regency, using a qualitative descriptive approach. Data were collected through 17 in-depth interviews, direct observation, and documentation. Findings revealed key challenges, including inadequate human resources, limited infrastructure, insufficient cadre training, and inconsistent incentives. Low participation from adolescents and the elderly, along with weak cross-sector coordination, further hindered effectiveness. Enhancing infrastructure, cadre capacity, and coordination, with a focus on inclusivity for all age groups, could strengthen PosGa as a comprehensive community health service.

Abstract

The focus of the global problem that is of concern both in the world and in Indonesia is the HIV/AIDS case, that reported HIV/AIDS cases in districts/cities in Indonesia were 22,331 HIV cases and 4,534 AIDS cases. Meanwhile in NTB in 2022, there were 1471 accumulated AIDS cases and the addition of 192 new cases, in Central Lombok Regency there were 378 HIV cases and 220 AIDS cases as of September 2022, as well as 211 HIV cases in the MSM group. The strategy determined was to carry out in-depth interviews using the Snow Ball method with 4 informants, namely the VCT/KTH Mandalika Team at Praya Regional Hospital, Management at Praya Regional Hospital, MSM Clients at KTH at Praya Regional Hospital and the HIV Program Holder of the Central Lombok District Health Service. Test the validity of the data with Credibility, Continuous Observation, Triangulation, Transferability, Depreciability, Confirmability. This research has received ethical approval from the Ethics Committee of Qamarul Huda Badaruddin Bagu University with Number: 022/EC/FKES-UNIQHBA/YPPQH/VIII/2023. Management of HIV/AIDS Prevention Services in the MSM Group at KTH Mandalika RSUD Praya still needs to improve the effectiveness of HIV/AIDS prevention, more intensive strategic planning is needed to overcome the increase in HIV prevalence, involving an approach through a TEAM of Field Officers in providing more effective education and social support through VCT services and hotline counseling. Coordination and a stable organizational structure in KTH Mandalika RSUD Praya cannot be ignored, so that response efforts can run more focused. Staff training and development, especially those related to handling PLWHA, needs to be based on objective selection and unit needs, while the use of information technology, such as the SIHA 2.1 application and RS SIM, can be improved to ensure accurate recording and reporting. Management of BHP and ARV availability needs to be optimized through efficient coordination with the District and Provincial Health Services, and regular evaluations need to be carried out to increase distribution efficiency. Implementing the recommendations from 4 informants can have a significant positive impact in managing the management of HIV/AIDS prevention services at KTH Mandalika RSUD Praya, so as to reduce HIV transmission, and provide better support to PLWHA in the MSM community..

Abstract

Pelayanan antenatal care (ANC) merupakan intervensi penting untuk menurunkan angka kematian ibu dan bayi melalui deteksi dini risiko kehamilan, pemantauan kesehatan ibu, serta pemberian edukasi. Penelitian ini bertujuan mengeksplorasi kualitas pelayanan ANC di Puskesmas dengan pendekatan kualitatif desain studi kasus. Informan terdiri dari tenaga kesehatan (dokter, bidan, pimpinan Puskesmas) dan ibu hamil yang pernah menerima layanan. Data diperoleh melalui wawancara mendalam, observasi, dan telaah dokumen, kemudian dianalisis dengan metode tematik. Hasil penelitian menunjukkan bahwa kualitas layanan ANC masih menghadapi keterbatasan, terutama terkait ketersediaan tenaga bidan yang kompetensinya belum sesuai rasio kebutuhan. Sarana pemeriksaan seperti Doppler dan USG tidak dikalibrasi berdampak tidak akurasi, serta ruang periksa belum memenuhi standar pelayanan. Selain itu, faktor non-teknis juga memengaruhi mutu layanan, antara lain sikap petugas yang kurang ramah, minimnya edukasi kepada ibu hamil, serta lamanya waktu tunggu yang menimbulkan ketidakpuasan pasien. Kesimpulan menekankan pentingnya penguatan sumber daya, peningkatan kompetensi, dan perbaikan sistem pelayanan untuk mengoptimalkan kualitas ANC di Puskesmas.

Abstract

Support for mothers in caring for children experiencing anxiety during the preoperative phase is very important. Smartphones and family become sources of support for mothers in caring for children with anxiety during the preoperative phase. This study aims to analyze the influence of smartphone addiction and family support on maternal care ability in children experiencing anxiety in the preoperative phase. This research uses a cross-sectional design, where the population in this study is mothers with children experiencing anxiety in the preoperative phase. The sample in this study consisted of 132 mothers of children aged 3-6 years who were hospitalized and awaiting surgery. Participants were purposively selected from two hospitals in Mataram City, Indonesia. Data were collected using the Smartphone Addiction Scale-Short Version (SAS-SV), Family Support Scale (FSS), maternal care questionnaire, and the modified Yale Preoperative Anxiety Scale (m-YPAS). Multiple linear regression analysis was performed in SPSS version 25.0 to assess the influence of the variables. The results of the analysis showed that both smartphone addiction and family support are significant predictors of maternal caregiving ability. Partially, smartphone addiction has a weak negative effect on maternal caregiving ability (β = -0.038, p = 0.047), whereas family support has a strong positive effect (β = 0.974, p < 0.001). This study indicates that interventions that involve and empower the family as a support system for mothers can improve maternal caregiving ability.

Abstract

The West Lombok Regency implementation interventions and sensitive interventions to reduce stunting 3.73 percent from 2021 to 2022. The research aims to determine if there is a significant relationship between specific intervention programs for chronically energy-deficient pregnant women and the reduction of stunting incidents. The objective is to evaluate the impact of specific intervention programs on pregnant women with chronic energy deficiency. The quantitative cross-sectional method involved 294 respondents, with the research conducted from September to November 2023. Conclusions: (1) There is a significant relationship with the provision of supplementary food, with an odds ratio of 0.264 times. (2) There is a significant relationship with the provision of blood supplement tablets, with an odds ratio of 0.379 times. (3) There is no relationship with pregnancy examinations. (4) There is a significant relationship with the administration of TT/TD immunizations, with an odds ratio of 0.440 times. (5) There is no relationship with HIV/TB/Malaria/Hepatitis services. (6) There is no relationship with the mother's class program. (7) Independent variables explain the dependent variable by 17.5 percent, and 82.5 percent is explained by variables not studied. Recommendations for the West Lombok Regency government: The provision of supplementary food requires intensive monitoring of the target population. Increasing the coverage of pregnancy examination programs and blood tablet consumption requires an application system. Increasing the coverage of TT/TD immunization integrated with a health application. HIV/TB/Malaria/Hepatitis services require human resource planning, equipment and disposable material, and reporting systems. Mother's class activities require innovation and modification.

Abstract

The suboptimal competency development program for healthcare workers in regional general hospitals affects the fulfillment of professional competency requirements and integration as a superior, international-class referral hospital chosen by the community with excellent and comprehensive services. This study aims to explore the implementation of the healthcare worker competency development program through input, process, and output, as well as identifying problems encountered and solutions to overcome them at the NTB Provincial Hospital. This study uses a Qualitative Descriptive approach using the Donabedian framework (input, process, and output) with in-depth interviews, observation, and document analysis methods. The informants numbered 14 people, determined by purposive sampling, namely selecting several health workers who have experience, strategic positions, or perspectives that are considered capable of providing in-depth insights regarding training needs. Data analysis used the interactive model of Miles and Huberman involving data collection, data reduction, data presentation, and drawing conclusions. This study found that the implementation of healthcare worker competency development has been running normatively, but there are problems with the lack of number and type of competency development and its uneven distribution. The large number of employees, types of health workers and the many varied service units demand a very important need in the development of health worker competencies, problems with the Training Needs Assessment (TNA) have not been carried out properly, implementing personnel or training organizers who have not been trained, lack of infrastructure for certain specific training and limited budgets, and there is no binding policy to implement SPM 20JP. The development of health worker competencies at the NTB Provincial Hospital has not reached an optimal level due to limited budget allocations, the large number of personnel accompanied by the diversity of types of services that require must be a priority agenda because of the importance of equitable and planned competency development to ensure the quality, effectiveness, and sustainability of health services.
