Abstract

A high prevalence of malnutrition in hospitals leads to the increasing of more attention paid for nutrition management. Enteral nutrition feeding in a proper way will give advantages such as protecting nutrition status in order not to go worse and preventing or reducing the occurrence of metabolic complication and infection. It requires specific knowledge and expertise in nutrition area, management of enteral nutrition feeding is a part of nurse’s responsibilities. This study was aimed to obtain a description of technical feeding of enteral nutrition to adult patient at DR. Sardjito Hospital Yogyakarta. The type of this study was descriptive explorative by means of quantitative method, data was collected through observation to research subject who are the patients undergoing enteral nutritional therapy with a total number of 21 patients. This study was done within December 19th 2005 – January 9th 2006. The result showed that a high percentage of enteral nutrition feeding done by family was 40 actions (63.49%) while that done by nurses was 23 actions (36.51%). Enteral nutrition feeding that was considered as properly done was only 20 actions (31.75%) while that considered as improperly done was 43 actions (68.25%). The most frequent things that were not done in enteral nutrition feeding were checking the intestinal distortion before and after the nutrition feeding, aspirating residual volume of stomach before nutrition feeding, considering the rate of feeding, and some did not set the position of semifowler (30o – 45o) during enteral nutrition feeding. This study showed that most of enteral feeding had not been done based on theoretical standard.

Abstract

It is disconcerting that until today there are no agencies or research to recommend the appropriate indicators of home nursing care of stroke patients according to Indonesian conditions. Hence, this research aimed to verify the validity and reliability of quality indicators of home nursing care for stroke patient that resulted from Delphy process. Confirmatory factor analysis was carried out for validity and reliability testing with data collected from 350 respondents. Based on the value of loading factors, 62 indicators were declared valid and five indicators were invalid. This study successfully proved the validity of 62 indicators of quality home care for stroke patients. Further research is needed to pilot project this indicator in a larger area.

Abstract

Women with cancer are at risk for sexual dysfunction characterized by problems with sexual desire, sexual arousal, lubrication, orgasm, sexual satisfaction, and pain during sexual intercourse. The literature review shows that most studies have focused on correlation analysis between factors, and no studies have attempted to identify a causal relationship between factors of sexual dysfunction. This study aims to determine the causal mechanism between factors of sexual dysfunction in cancer patients using a causal algorithm called the Stablespec Specification Search for Cross-Sectional Data With Latent Variables (S3C-Latent). The causal algorithm has been implemented into the R software package called Stablespec. The computation of the model is done in parallel using the CPU server. The result of this study is that there are a causal relationship and association with a high-reliability score of sexual dysfunction factors. We hope that the causal model obtained can be a scientific reference for doctors and health workers in making decisions so that the quality of life of female cancer patients who experience sexual dysfunction can be improved.

Abstract

Background: Cancer patients can experience both physical and non-physical problems such as psychosocial, spiritual, and emotional problems, which impact the quality of life. Previous studies on quality of life mostly have employed multivariate analyses. To our knowledge, no studies have focused yet on the underlying causal relationship between factors representing the quality of life of cancer patients, which is very important when attempting to improve the quality of life.  Objective: The study aims to model the causal relationships between the factors that represent cancer and quality of life.Methods: This study uses the S3C-Latent method to estimate the causal model relationships between the factors. The S3C-Latent method combines Structural Equation Model (SEM), a multi objective optimization method, and the stability selection approach, to estimate a stable and parsimonious causal model.Results: There are nine causal relations that have been found, i.e., from physical to global health with a reliability score of 0.73, to performance status with a reliability score of 1, from emotional to global health with a reliability score of 0.71, to performance status with a reliability score of 0.82, from nausea, loss of appetite, dyspnea, insomnia, loss of appetite and from constipation to performance status with reliability scores of 0.76; 1; 0.61; 0.76; 0.72; 0.70, respectively. Moreover, this study found that 15 associations (strong relation where the causal direction cannot be determined from the data alone) between factors with reliability scores range from 0.65 to 1.Conclusion: The estimated model is consistent with the results shown in previous studies. The model is expected to provide evidence-based recommendation for health care providers in designing strategies to increase cancer patients’ life quality. For future research, we suggest studies to include more variables in the model to capture a broader view to the problem.

Abstract

Patients with cancer can potentially experience the negative impacts of treatment. Physical conditions due to illness and therapy can affect the patient's body image. This study aims to find a causal model among body image factors of patients with cancer using the S3C-Latent Method. The measurement of body image of patients with cancer used the BIS questionnaire. One hundred and ninety-nine patients with cancer participated in this study. The results showed the existence of causal relationships between behavior to cognitive factors and duration of illness with reliability scores of 0.8 and 0.6, respectively; from gender to affective factors, illness duration, behavior, and cognitive factors with reliability scores of 0.6, 0.8, 0.65, and 1, respectively. There are also causal relationships from age to affective factors, duration of illness, and cognitive factors with reliability scores of 0.8, 0.7, and 0.9, respectively. The results also showed that affective factors are associated with behavior, cognitive factors, and duration of illness, with reliability scores of 1, 1, and 0.9, respectively. The results showed further the association of cognitive factors and illness duration with a reliability score of 1. We expect that the estimated causal model will serve as a scientific reference for medical experts in developing a better intervention such as treatment.

Abstract

Background: Health in the elderly does not merely refer to a condition free from disease but should also have elements regarding functionality and independence. Purpose: This study aimed to measure Activities of Daily Living (ADLs) and to identify the factors affecting ADLs among the elderly population in Sleman District. Methods: A cross-sectional study was carried out using secondary data from the Multidimensional Elderly Care project, nested on the Health Demographic and Surveillance System in Sleman, involving 549 elderly participants. The independence of these elderly people was measured using an Activities of Daily Living Scale questionnaire. Factors measured were cognitive (using the Mini-Mental State Examination questionnaire), psychological (using the Geriatric Depression Scale or Cornell Scale for Depression in Dementia questionnaires), economic (using the Financial Management Behavior Scale questionnaire), and nutritional status (using the Mini Nutritional Assessment questionnaire). The data were analyzed using the chi-square test, Fisher test, and Poisson test for bivariate analysis, while multivariate analysis using a logistic regression test. Results: The prevalence of elderly dependence was 14.03%. Elderly dependence was significantly related to age, job, residence, financial management, dementia, depression, malnutrition, and stroke disease in the bivariate analysis. However, after multivariate analysis, only age (>86 years) (PR = 4.31; 95% CI = 1.91–9.72), malnutrition (PR = 6.62; 95% CI = 3.79–11.57), and stroke (PR = 3.06; 95% CI = 2.03–4.61) were still shown to be related to elderly dependence. Conclusion: Increasing age, malnutrition, and stroke in elderly people lead to higher dependency in their ADLs.

Abstract

Background: The cancer prevalance is predicted to raise. Cancer affects patient's quality of life, such as psychological, sexual, social function, and daily activities. The appropriate nursing interventions are needed for each type of cancer to improve nursing outcomes and quality of life.Objective: To identify the types of nursing intervention that could improve the quality of life of patients with cancer.Method: Four electronic databases, such as PubMed, Sciencedirect, Ebsco-host, and Clinical-Key, were explored to find original research articles which focused on the types of nursing intervention in caring for patients with cancer. Fourteen articles were included in the study, and each of article was analyzed using The Joanna Briggs Institute (JBI) Appraisal tool to achieve the purpose of the study.Results: The study found that psychological nursing interventions, physical nursing intervention including exercise/physical activity, and technology application in nursing intervention were contributed significantly to enhance patients’ quality of life.Conclusions: Nursing interventions which are psychological, physical, and using technology application can improve the quality of life of patients with cancer significantly. ABSTRAKLatar belakang: Angka kejadian kanker diprediksi semakin meningkat. Pasien kanker akan mengalami perubahan efek psikologi, seksual, fungsi sosial serta aktivitas keseharian yang memengaruhi Quality of Life pasien. Diperlukan intervensi keperawatan yang tepat sesuai jenis kanker untuk meningkatkan Quality of Life pasien kanker.Tujuan: Studi ini bertujuan untuk mengidentifikasi jenis intervensi keperawatan dalam meningkatkan quality of life pasien kanker.Metode: Pencarian literatur dilakukan untuk mendapatkan artikel berupa original research pada 4 database yaitu PubMed, Sciencedirect, Ebsco-host, Clinical-Key. Didapatkan  sebanyak 14 artikel yang mengukur quality of life pasien kanker sesuai kriteria inklusi. Setiap artikel dilakukan analisis menggunakan The Joanna Briggs Institute (JBI) Appraisal tool untuk mencapai tujuan penelitian.Hasil: Penelitian ini menemukan bahwa  intervensi keperawatan psikologis, intervensi keperawatan fisik  berupa exercise/physical activity, dan intervensi keperawatan dengan aplikasi teknologi memberikan kontribusi pada kualitas hidup pasien kanker. Semua intervensi keperawatan tersebut secara statistik dapat meningkatkan Quality of Life pada pasien kanker. Kesimpulan: Intervensi keperawatan yang berupa psikologis, fisik dan menggunakan aplikasi teknologi terbukti dapat meningkatkan quality of life pasien kanker secara signifikan.

Abstract

Background: The concept of patient centered health services encourages interprofessional collaboration practice (IPCP), including teaching hospitals. IPCP in teaching hospitals expected to be implemented properly as a role model for students. Students in academic stage are expected to understand the concept of interprofessional education (IPE) and apply IPCP concept during clinical practice. This study aimed to determine the implementation of IPCP and various factors related to IPCP in teaching hospitals.Methods: This study used a mixed methods-sequential explanatory design with the subjects of healthcare professionals at RSUD R Syamsudin SH recruited using systematic random sampling. The research began with collecting quantitative data to measure the implementation of IPCP using the Indonesian version of the Assessment of Interprofessional Team Collaboration Scale-II (AITCS-II), continued with collecting qualitative data with in-depth interviews to explored IPCP and various factors associated with its implementation.Results: A total of 320 respondents filled out the AITCS-II questionnaire and 11 participants were interviewed deeply. More than 66% of respondents had a good collaboration in each domain; 73,8% of respondents had a good collaboration in IPCP; profession background significantly contributed IPCP (p <0.05). Qualitative analysis was carried out with a semantic theme and obtained 3 themes: not ideal  IPCP implementation, perception of IPCP domains by healthcare professionals and barriers in implementing IPCP. Conclusion: The perception of IPCP that well implemented may resulted from improper understanding of IPCP. We still found barrier in implementing IPCP related to stereotypes, hierarchical culture, interprofessional communication and regulation.

Abstract

Permasalah fisik dan psikologis pada pasien kanker ginekologi mempengaruhi kualitas hidup mereka. Mengetahui kualitas hidup pasien kanker berperan penting dalam pemilihan agen terapeutik yang tepat. Penelitian dengan desain analitik komparatif rancangan cross sectional bertujuan menganalisa perbedaan kualitas hidup penderita kanker serviks dan kanker ovarium. Penentuan sampel menggunakan purposive sampling didapatkan sampel total pasien kanker serviks 34 responden dan pasien kanker ovarium 41 responden dari bulan September 2016-Februari 2017. Pasien yang telah menjalani kemoterapi siklus pertama dan bersedia menjadi responden akan di skrining menggunakan PPS, setelah itu diberikan kuesioner EORTC QLQ C-30 untuk mengukur kualitas hidup.Kualitas hidup pasien kanker serviks dan kanker ovarium secara umum tidak ada perbedaan bermakna, namun terdapat perbedaan bermakna padalama rawat ≤2 hari berbeda bermakna yaitu skala fungsi sosial (p=0,005), dan berdasarkan lama rawat >2 hari yaitu pada skala gejala kesulitan tidur (p=0,019).Adanya keluhan fisik yang dialami oleh penderita kanker serviks dan kanker ovarium menunjukkan semakin buruk prognosis penyakit yang dideritanya. Hal tersebut akan mempengaruhi masa perawatan pasien dan memperparah gejala psikis yang dialami pasien.Kata kunci: Kualitas hidup, kanker serviks, kanker ovarium, lama rawat inap, stadium lanjut

Abstract

Ibu dengan kondisi terminal memiliki kebutuhan akan pelayanan paliatif dengan melihat keadaan BIOPSIKOSOSIAL SPIRITUALNYA. Keadaan ini memungkinkan ibu mengalami penurunan keadaan psikologis yang dapat berdampak pada kesehatan ibu dan anak. Perawatan paliatif ini merupakan perawatan yang sangat komprehensif sehingga pelayanan yang diberikan dapat dipadukan dengan pendekatan teori Ella Joy Lehrman-Morthen dan Reva Rubin sehingga tujuan awal untuk mengetahui tidak hanya keadaan psikologis ibu akan tetapi keadaan janin juga dapat terpantau dengan baik.  Metode yang digunakan adalah analisa kualitatif dengan pendekatan literature review. Tujuan akhir dari pendekatan teori Ella Joy Lehrman-Morthen dan Reva Rubin dalam implikasi pelayanan paliatif secara berkesinambungan pada ibu dengan kondisi terminal yaitu ibu akan  dapat terus menjalankan peranannya tanpa ada keterbatasan dalam aktivitasnya. Tujuan dalam pelayanan ini dapat dicapai dengan pemberian komunikasi efektif pada pasien dan keluarga. Komunikasi efektif merupakan dasar dalam pengkajian sehingga kebutuhan dan permasalan juga penyelesaian dapat diberikan dengan maksimal. Pelayanan paliatif merupakan pelayanan komprehensif yang dapat menjadi wadah dalam implikasi pelayanan maternitas dengan menggunakan pendekatan teori Ella Joy-Lehrman dan Reva Rubin. Teori ini dapat meningkatkan kualitas hidup pasien dengan kondisi terminal sehingga secara BIOPSIKOSOSIAL SPIRITUAL pasien dapat memperoleh pelayanan baik maternitas maupun paliatif dengan maksimal. Kata kunci: pelayanan paliatif, teori Ella Joy-Lehrman dan Reva Rubin, ibu kondisi terminal
