Abstract

Background: There have been various studies into the family caregivers’ experiences in taking care of advanced-cancer patients. But a study exploring the burden among family caregivers has not yet been conducted in Indonesia, a country which has strong family bonds among family members. Objective: This present study aimed to identify the burden among family caregivers of advanced cancer patients. Methods: This study was a cross-sectional study conducted from December 2016 to February 2017 on 178 consenting family caregivers and advanced cancer patients, selected using a purposive sampling technique. The Caregiver Reaction Assessment (CRA) was used to measure their burden. Data were analyzed using descriptive analyzes and bivariate analyzes. Results: The burden among family caregivers was 2.38 ± 0.38 (mean range 1-5). The highest burden was in the disrupted schedule domain. Conclusion: Our findings identified that the burden among family caregivers was at the medium level. The length of care per day and family support are potential targets for preventative intervention strategies to reduce the burden among the family caregivers.

Abstract

Background: Patients with advanced cancer require treatment and the fulfillment of their needs, based on the results of assessments regarding their physical symptoms, psychological and spiritual needs. Palliative care should be delivered with a person-centered care approach. It is important to consider the patients’ reports of their carings’ outcomes. Comparisons between the patients’ and nurses’ perceptions of palliative care outcomes can be used to improve the quality of palliative care. Objective: The purpose of this study is to compare the patients’ and nurses’ perceptions of palliative care outcomes during the patients’ hospitalization. Methods: This is a comparative descriptive study with a cross-sectional design. Data were collected from May to June 2018 from a total of 106 patients with advanced cancer, and 61 nurses. The versions of the Palliative care Outcome Scale (POS) for patients and nurses were used to measure the palliative care outcomes of the patients’ and nurses’ perceptions. Result: The study found a significant difference between the perceptions of the patients and nurses for the palliative care outcomes, particularly in the information availability domain (p = 0.001), the other symptoms domain (p = 0.029), and the anxiety feelings domain (p = 0.030), while the other seven domains had no significant differences between both groups (p > 0.05). Conclusion: The anxiety feelings, other symptoms and information availability domains are the aspects of palliative care which need more attention from health care providers, especially nurses, when caring for patients with advanced cancer.

Abstract

Background: Nurses have a key role in promoting patients’ involvement in decision-making process of palliative care to improve their dignity and satisfaction. However, there is a dearth of studies exploring this involvement, especially in public health centers in Indonesia. Objective: This study aimed to explore the perception of nurses in decision-making process and to understand what type of decision-making made by health care providers in palliative care. Methods: This was a descriptive explorative study with qualitative approach. Participants were selected using purposive and snowball sampling. Data were collected using Focus Group Discussion (FGD) and in-depth interview. Content analysis method by inductive approach was used for data analysis. Consolidated criteria for reporting qualitative research (COREQ) was also used. Results: Eight themes emerged in this study, namely: (1) Collecting information about patients’ current physiological condition, (2) Creating alternative strategies according to the patients’ current physiological condition, (3) Establishing implementation type to be performed, (4) Providing information to the patients, (5) Discussing decision-making between patients and family, (6) Choosing/rejecting an action by patients, (7) Performing selected actions, and (8) Evaluating action. Three points that are different from the existing theory were: (i) there was a discussion between patients and family, (ii) the absence of the process of collecting information after a patient rejects to act, and (iii) there were three types of decision-making: paternalistic, shared, and informed decision-making. Conclusion: This study serves as an input for nurses to pay more attention in decision-making process in palliative care in patients with cancer, and to encourage patients to give contribution in decision-making as part of shared decision-making.

Abstract

Family caregivers, especially in Asian countries, have a profound role in caring for a sick family member. However, there are wide variations between the Asian and western world in terms of culture and facilities. Therefore, the problems and needs of family caregivers between those two regions may also be distinct, and it is important to explore and elaborate based on our empirical evidence. In Indonesia, motives and values in caregiving and religion become the wheel-power of the family caregivers in providing care. This affects action and consequences for caregivers. This paper attempts to elaborate on common ethical dilemmas that usually face by family caregivers in Indonesia. Unfortunately, family caregivers typically are not prepared to make those challenging decisions. Therefore, we recommend not only that family caregivers need to be involved in the caring process, but also their issues and ethical dilemmas should be assessed and addressed by health care professionals, especially nurses, who have the most frequent contact with patient and family caregiver.

Abstract

Background: One method to identify the level of performance is using quality of care indicators. Even though many quality indicators of health care have been announced, those which specify in home care services are limited in that none of them describe the stroke patients’ condition. The purposes of this study are to determine through systematic literature review what methods can be used to assess the quality of home care that patients received and to identify what components can be used as a determinant of the quality of home care services. Method: Google Scholar, EBSCO, ProQuest and PubMed database websites were searched for articles and information. Results: The method that was used is a qualitative study using a literature review with quantitative analysis of a previously accepted research instrument with a questionnaire that has been widely available and considered reliable. The researchers identified thematic differentiation in grouping the quality indicators used by those articles, however most of them are classified as outcome indicators, one as process indicator, and none as structure or input indicator. Overall quality indicators in home care that are used by the three articles are based on the Home Care Quality Indicators Instrument (HCQIs). Conclusions: Several studies discussed home care quality indicators but no articles specifically analyze home care provision for stroke patients. Further research is needed to clarify the components indicators for stroke patients and more importantly, these indicators should be valid, and reliable.

Abstract

Background: Depression is a psychological distress that often occurs on cancer patients. Depression can increase patient perception about pain, resulting reduced drug efficacy and longer length of stay. Drug treatment for depression sometimes has side effects. Another intervention to decrease depression on cancer patients is music therapy. Music therapy might avoid polypharmacy in cancer patients. The purpose of this study was to determine the effects of self-selected individual music therapy toward depression of cancer patients. Methods: The method used in this study was the randomized control trial (RCT). Simple Random Sampling was assigned randomly by three researcher assistants to participants (N=70) of intervention and control group. The intervention used was self-selected individual music therapy (SeLIMuT). SeLIMuT was applied four times in 2 days with a duration of 15 minutes each session. Beck Depression Inventory (BDI) was employed for selection purposes. The data were analyzed using Mann-Whitney with p < 0.05 and 95% CI. The effectiveness of therapy was analyzed by the effect size test using abs r.Results: Based on the result of the bivariate analysis, there was an effect of giving self-selected individual music therapy to reduce depression with depression gap in both groups with p-value of 0.001 (p < 0.05). The value of the effect test obtained abs r = 0.82 (r > 0.5). At the end, the self-selected individual music therapy had moderate effect toward depression of cancer patients. Conclusions: Music therapy is safe, inexpensive, and easy to use, for and by patients. The nurse can safely recommend any of these interventions for depression on cancer patients. Specific selections or types of music may have different effects on different patients and may provide different effects at different time, so the use of self-selected individual music therapy is recommended.

Abstract

“Difficult to meet supervisor” and “lack of feedbacks”: student’s perception of problems in writing final year project in undergraduate program Objective: This study explores the role of mentoring felt by the students involved in the making. Specifically, we want to disclose what students consider helpful and what is considered to be less helpful for final project preparation. Methods: Respondents were 82 third year nursing students at Poltekkes Kemenkes Pontianak who were writing thesis. The questionnaire consisted of 16 questions consisting of (a) lecturer-student communication and interaction, (b) mentoring roles and responsibilities, and (c) quality of mentorship. The questionnaire consisted of a question following the items in Beck et al.’s research, measured by a Likert scale consisting of good, moderate, and less. Six students were taken for in-depth interviews, selected based on different counselors. Results: Of the 16 themes about mentoring, “pretty good” at most for all items. Item analysis showed support of several concerns that have been predicted. Although “like the guiding figure” and “feel the support of lecturers including two things that stand out in the opinion of students about thesis supervisor figure. Students reported two main issues in their mentoring process: “advisers are difficult to see” and “lack of meaningful input from advisers”. Conclusion: Student satisfaction in mentoring is influenced by the quality of the mentors. Students feel satisfied if the characteristics, roles and responsibilites of the supervisor in accordance with student expectations. This study ensures that there are limitations in the availability and seriousness of the lecturers in thesis coaching. Further studies on mentorship and support capacity are needed.

Abstract

Entrepreneurship in the nursing field is still rising giving greater opportunity for these nursing entrepreneurs. However, nurses need to identify the obstacles and the supports so that their businesses will run well and succeed. To identify the hindering and supporting factors for nurses in entrepreneurship. This study was a narrative review with literature research using electronic databases, which included Science Direct and PubMed between 2011-2021. The framework from the European Medical Writers Association developed by Ferrari was used to guide the study From the total of 11 identified and analyzed literatures, there were seven hindering factors (fear; knowledge and skill; prior working culture experience; decision-making policy and regulations; health insurance, colleague jealousy; and unprofessional attitudes) and two supporting factors (knowledge and skills; ability to seek opportunities). Conducting entrepreneurship in nursing still faces several obstacles but also has some supports, and therefore can be done by developing nurses’ self-potentials and professional attitudes.
