Abstract

Background: Dementia is a chronic disease that will affect the function and the dependency of patient, thus requiring assistance from the caregiver to fulfill their daily needs. Family caregivers providing care to dementia patients are influenced by knowledge. Knowledge is one of the abilities needed in caring for dementia patients. Objective: The aim of this study is to give the description about caregiver’s knowledge that treating patients with dementia. Method: This study is a descriptive quantitative study using cross sectional design. Selection of sample was done by using non probability purposive sampling technique. The sample consisted of 53 family caregivers that have been recorded in medical record of Dr. Sardjito Hospital Yogyakarta on May 2018 by using inclusion criteria i.e. 1) took care a patient with dementia, 2) aged more than 18 years old, and 3) willing to become the respondents of this study. Data were obtained by using dementia knowledge assessment scale (DKAS) questionnaire which has been translated to Indonesian language. Data were analyzed using univariate analysis. Results: Majority respondents were women and aged more than 65 years old and has been taking care the patient more than 6 months. The majority type of dementia of the patients was vascular. Their average knowledge score were 23,25 from the maximum 50 total score. The highest average score was in domain 3, i.e. care consideration (8,17 ± 2,64), and the lowest one is in domain 2, i.e. communication and behavior (4,19 ± 1,82). Conclusion: Family caregivers’ knowledge about dementia that consist of several domain were still categorized as low.Keywords: dementia; family caregiver; knowledgeABSTRAKLatar belakang: Demensia merupakan penyakit kronis yang memengaruhi fungsi kerja dan kemandirian dari pasien, sehingga dibutuhkan peran caregiver dalam perawatan dan pemenuhan kebutuhan sehari-hari. Family caregiver memberikan perawatan kepada pasien demensia dipengaruhi oleh pengetahuan. Pengetahuan merupakan salah satu kemampuan yang diperlukan dalam merawat pasien demensia.  Tujuan: Mengetahui gambaran pengetahuan family caregiver dalam merawat pasien demensia. Metode: Penelitian ini merupakan penelitian deskriptif kuantitatif dengan menggunakan rancangan cross sectional. Pemilihan sampel dilakukan dengan teknik sampling nonprobability purpossive sampling. Sampel terdiri dari 53 family caregiver pasien demensia yang tercatat klinik memori RSUP Dr. Sardjito pada bulan Mei 2018 dengan kriteria inklusi, yaitu 1) family caregiver pasien demensia, 2) usia lebih dari 18 tahun, dan 3) bersedia menjadi responden penelitian. Data diperoleh dengan menggunakan kuesioner data demografi dan Dementia Knowledge Assesment Scale (DKAS) yang telah diterjemahkan ke dalam bahasa Indonesia. Analisis data menggunakan analisis univariat. Hasil: Mayoritas family caregiver rata-rata berusia lebih dari 65 tahun, perempuan, dan telah merawat pasien lebih dari 6 bulan. Jenis demensia yang paling banyak diderita pasien adalah demensia vaskular. Pengetahuan family caregiver pasien demensia rata-rata adalah 23,25 dari skor total maksimal 50. Rata-rata skor pengetahuan family caregiver tentang demensia tertinggi adalah pada domain 3 care consideration (rata-rata 8,17 ± 2,64) dan terendah pada domain 2 communication and behaviour (rata-rata 4,19 ± 1,82). Kesimpulan: Pengetahuan family caregiver tentang demensia masih dalam kategori kurang.

Abstract

Background: A family caregiver requires high social support to reduce stress levels and improve welfare in treating people with dementia. Research about social support for family caregiver in treating dementia has rarely been conducted. Objective: To describe social support for family caregiver in treating dementia patients. Method: This research was descriptive quantitative research and cross-sectional study design. Data was collected in May 2018. The samples in this study were 53 family caregivers of dementia patients who came to the Memory Clinic of Dr. Sardjito Hospital in Yogyakarta. Sampling technique was using purposive sampling. Social Support Questionnaire (SSQ) was equipped as research instrument, while data analysis was using univariate analysis. Results: Respondents who received social support came from primary, secondary, and tertiary sources were 44 respondents (83%). The average value of emotional-support satisfaction level reached 4,85 (± 0,59), while instrumental support was 4.79 (± 0,64), esteem support was 4,79 (± 0,77), and informational support was 4,87 (± 0,57).  Conclusion: All forms of social support which were received by the family caregiver are close to category 5 (satisfied). Mostly, social support sources came from children, siblings, relatives, and health workers.ABSTRAKLatar belakang: Seorang family caregiver membutuhkan dukungan sosial yang tinggi dalam melakukan perawatan pada pasien demensia, untuk mengurangi tingkat stres dan meningkatkan kesejahteraan. Penelitian terkait gambaran dukungan sosial family caregiver dalam melakukan perawatan pada pasien demensia masih jarang dilakukan.  Tujuan: Untuk menggambarkan dukungan sosial yang diterima family caregiver dalam merawat pasien demensia. Metode: Penelitian ini merupakan penelitian kuantitatif yang bersifat deskriptif, dengan rancangan penelitian cross sectional. Pengambilan data dilakukan pada bulan Mei 2018. Sampel dalam penelitian ini adalah 53 orang family caregiver pasien demensia yang datang ke Klinik Memori RSUP Dr. Sardjito Yogyakarta. Teknik pengambilan sampel menggunakan purposive sampling. Instrumen penelitian menggunakan Social Support Questionnaire (SSQ). Sedangkan analisis data menggunakan analisis univariat. Hasil: Responden yang menerima dukungan sosial berasal dari sumber primer, sekunder, dan tersier, sebanyak 44 responden (83%). Rata-rata tingkat kepuasan dari dukungan emosional yang diterima responden sebesar 4,85 (± 0,59), dukungan instrumental sebesar 4,79 (± 0,64), dukungan penghargaan sebesar 4,79 (±0,77), dan dukungan informasional sebesar 4,87 (± 0,57).  Kesimpulan: Semua bentuk dukungan sosial yang diterima family caregiver bernilai mendekati kategori 5 (puas). Mayoritas sumber dukungan sosial berasal dari anak, kakak/adik, kerabat, dan petugas kesehatan.

Abstract

Background: Cancer is the second leading cause of death globally. Patients with cancer have various problems, both physicaly and physiologicaly, that are caused by the cancer itself or the effect of its treatment. One of the most common problems is cancer related fatigue (CRF). Music therapy is a kind of intervention to decrease CRF in cancer patient. Objective: To understand the effect of Self-Selected Individual Music Therapy (SeLIMuT) intervention toward CRF and to measure relative risk between SeLIMuT and CRF. Method: The design of this study was cohort retrospective with analytic observational approach. Research was conducted in IRNA 1 RSUP Prof. Sardjito Yogyakarta, Teratai and Bougenville unit of RSUD Prof. Margono Soekarjo Purwokerto. Respondents were divided into non exposure group with standard care in hospital (n = 21) and exposure group (n = 21) with SeLIMuT therapy four times in two days within 15-20 minutes. CRF was assessed in both group using visual analogue scale (VAS) Fatigue and had been put in observational sheet. This study used independent T test to measure the effect of SeLIMuT on CRF. The relative risk was calculated using RR with 95% confidence interval (CI). Results: The result of the study showed that there was difference in average pre post in both groups with score 8,33 in SeLIMuT group and -1,53 in non SeLIMuT group with score of p = 0,00 (p < 0,05). CRF occurred in SeLIMuT group was decreased after intervention. The relative risk was measured using RR and the result was 5,00.  Conclusion: SeLIMuT intervention is an effective intervention to reduce CRF.ABSTRAKLatar belakang: Kanker merupakan penyebab kematian kedua di dunia. Pasien kanker mengalami banyak permasalahan fisik dan psikis, baik karena efek proses penyakit maupun dari efek samping pengobatan. Masalah yang banyak timbul pada pasien kanker adalah cancer related fatigue (CRF). Salah satu intervensi yang digunakan untuk mengatasi CRF adalah terapi musik. Tujuan: Mengetahui pengaruh Self-Selected Individual Music Therapy (SeLIMuT) terhadap CRF. Metode: Penelitian diambil dari data sekunder penelitian SeLIMUT menggunakan metode observasional analitik dengan pendekatan cohort retrospective yang dilakukan di IRNA 1 RSUP Prof. Sardjito Yogyakarta serta bangsal Teratai dan Bougenville RSUD Prof. Margono Soekarjo Purwokerto. Responden dibagi dalam kelompok intervensi (n = 21) yang menerima SeLIMuT sebanyak empat kali dalam dua hari masing-masing selama 15-20 menit dan kelompok kontrol (n = 21) yang hanya mendapatkan perawatan standar rumah sakit. Visual analogue scale (VAS) fatigue digunakan untuk menilai outcome CRF yang dilakukan sebelum (pretest) dan setelah (posttest). Uji pengaruh SeLIMuT terhadap CRF menggunakan independent T test. Uji besar risiko menggunakan relative risk (RR) dengan confidence interval (CI) sebesar 95%. Hasil: terdapat perbedaan rerata selisih pre-post pada kelompok intervensi sebesar 8,33 dan kelompok kontrol -1,53 dengan nilai p = 0,001 (p < 0,005). Hal ini menunjukkan bahwa terapi SeLIMuT berpengaruh terhadap penurunan CRF pada pasien kanker. RR didapatkan sebesar 5,00. Hal ini berarti bahwa SeLIMuT memiliki efek yang cukup besar terhadap penurunan CRF. Pasien kanker dengan SeLIMuT berpeluang tidak mengalami CRF kategori tidak lelah-lelah ringan 5x lebih banyak dibanding kelompok yang tidak mendapat intervensi. Kesimpulan: SeLIMuT dapat berpengaruh positif terhadap penurunan cancer related fatigue pada pasien kanker.

Abstract

Background: Stroke is the fifth leading cause of death in the world, stroke does not only have a negative impact on patients but also has an impact on family caregivers. This study aims to describe the burden experienced by family caregivers in caring for stroke patients. Methods: This research uses a mixed method with sequential explanatory approach. The research was conducted from August to October 2019 involved 106 family caregivers using a purposive sampling technique. Burden experienced was measured with the Zarit Burden Interview (ZBI) questionnaire. Quantitative data analysis was performed by SPSS and qualitative data analysis with Opencode. Results: Burden experienced was at a mild level, there was a significant relationship to education variables (p = 0.007, r = -0.297), family relationships (p = 0.001, r = 0.001), number of caregivers (p = 0.001, r = 0.364), and the patient's duration of stroke (p = 0.006, r = -0.322). There are three themes in this study, that is difficulties in caring, motivation for caring and sources of support. Conclusion: Although quantitatively the family caregiver experiences a mild burden, qualitatively the family caregiver reports complaints in caring for almost all aspects, so treatment is needed to resolve this burden.

Abstract

Background: Dementia symptoms gradually hinder patients’ ability to perform daily activities, necessitating support from various parties particularly family caregivers. Self-efficacy is believed to have a positive influence on family caregivers, contributing to improved mental health, adaptive coping, and reduced psychological distress when dealing with the challenges associated with dementia patient care. Despite these potential benefits, research on family caregiver self-efficacy in caring for dementia patients in Indonesia remains limited, making this study necessary. Objective: To explore the self-efficacy of family caregivers in treating dementia patients. Methods: The study is quantitative descriptive approach employed a cross-sectional design involving 53 family caregivers of dementia patients from the Memory Clinic of RSUP Dr. Sardjito, Indonesia, selected through purposive sampling. The research instrument used was the Caregiver Self-efficacy Scale (CaSES). Univariate analysis was employed using mean and standard deviation scores for numeric ratio data. Results: The study identified four domains assessing family caregivers’ self-efficacy, with an average value of 3,09 ± 0,84. The domains included self-maintenance 3,18 ± 0,81, emotional connectivity 3,05 ± 0,87, instrumental caregiving 3,15 ± 0,82, and resilience 2,99 ±0,85. Conclusion: The results indicated that family caregivers exhibit relatively high levels of self-efficacy in treating dementia patients. Notably, family caregivers demonstrated the highest self-efficacy in the self-maintenance domain and the lowest in the resilience domain.ABSTRAKLatar belakang: Gejala demensia secara bertahap menghambat kemampuan pasien untuk melakukan aktivitas sehari-hari sehingga memerlukan dukungan dari berbagai pihak terutama pengasuh keluarga. Self-efficacy diyakini memiliki pengaruh positif bagi family caregiver dalam hal peningkatan kesehatan mental, penyesuaian diri yang adaptif, dan pengurangan tekanan psikologis saat menghadapi tantangan yang terkait dengan perawatan pasien demensia. Meskipun memiliki potensi manfaat tersebut, penelitian mengenai self-efficacy family caregiver dalam merawat pasien demensia di Indonesia masih terbatas, sehingga penelitian ini menjadi penting. Tujuan: Untuk mengeksplorasi self-efficacy family caregiver dalam merawat pasien demensia. Metode: Penelitian ini deskriptif kuantitatif menggunakan desain cross sectional dengan melibatkan 53 pengasuh keluarga pasien demensia dari Klinik Memori RSUP Dr. Sardjito, Indonesia, yang dipilih melalui purposive sampling. Instrumen penelitian yang digunakan adalah Caregiver Self-efficacy Scale (CaSES). Analisis univariat digunakan dengan menggunakan nilai mean dan standar deviasi untuk data rasio numerik. Hasil: Penelitian ini mengidentifikasi empat domain self-efficacy dengan nilai rata-rata sebesar 3,09 ± 0,84. Domain-domain tersebut meliputi self-maintenance 3,18 ± 0,81, emotional connectivity 3,05 (SD ± 0,87, instrumental caregiving 3,15 ± 0,82, dan resilience 2,99 ± 0,85. Kesimpulan: Hasil penelitian menunjukkan bahwa para family caregiver memiliki tingkat self-efficacy yang relatif tinggi dalam merawat pasien demensia. Secara khusus, para pengasuh keluarga menunjukkan self-efficacy tertinggi dalam domain self-maintenance dengan terendah domain resilience.

Abstract

Home-based care can be effective, efficient, safe, and affordable, if this care is performed using a patient-centered approach. Patient-centered care prioritizes the patient’s needs and preferences. The issue of patient-centered care remains the focus of various parties. This study aimed to explore the expectations of patients who have experienced a stroke, their family caregivers as recipients of home-based care services and healthcare workers as home-based care providers. This study used a qualitative explorative model. Semi-structured interviews were conducted with 19 participants, in addition to 4 focus group discussions. Data was analyzed using a content analysis approach with open code 13 software and the results were presented in COREQ systematics for qualitative studies. There were four main themes present in the results: physical well-being, self-actualization, psychological condition and balance between family burden and coping. Physical limitations were highlighted as the most burden some for the majority of the participants. At the time of the study, the majority of the patients, their family and health workers were diligently working towards to achieving the best outcome regarding physical well-being. Physical well-being is a fundamental need. Reaching this goal, could lead to better self-actualization, better physiological condition and improving the burden on families. This study may be useful as preliminary data to compile a home-based care construct indicators for stroke patients.

Abstract

Patients with cancer are at risk of death. There has been a lack of research on cancer-related training for health cadres. Meanwhile, they can potentially prevent cancer deaths because of their proximity to the community. This study aimed to determine the effectiveness of cancer educational videos on cadres’ knowledge and self-efficacy in Yogyakarta. This was quasi-experimental research with a nonequivalent control group design. The purposive sampling technique was used to obtain data from 61 health cadre respondents. The inclusion criteria: older than 19 years old, a woman, and uses WhatsApp. The Cancer Knowledge and General Self-Efficacy Scale Questionnaires measurement instruments were used. Data analysis was conducted using an independent sample t-test, repeated ANOVA, and paired comparison post hoc. There was a significant difference in respondents' knowledge in the control and intervention groups (p<0.05). The intervention group’s knowledge increased, and there was a moderate increase in knowledge in the control group. There was no significant change in cadres’ self-efficacy in the intervention group (p>0.05), but there was a significant increase (p<0.05) in the control group. Therefore, cadres’ training using cancer educational videos is effective for increasing cadres' knowledge only and not self-efficacy.

Abstract

Patients with Coronavirus Disease 2019 (COVID-19), particularly those with a severe condition, might not survive. Pandemic situation challenges the healthcare providers in addressing palliative care to the patients. This paper aimed to describe the importance of providing palliative care for patients with severe COVID-19 in Indonesia. We used a case scenario to illustrate the common condition experienced by a patient with severe COVID-19. Health care providers in Indonesia could address palliative care for patients with COVID-19 by focusing on controlling the symptoms, avoiding futile intervention, and connecting the patients and their families. Nurses need to consider the patients’ needs for family supports, even though not in physical or psychosocial support, and help the patients who need end-of-life care to be dying with dignity. Communication technology must be utilized optimally by healthcare providers to realize the communication among the patients, families, and health workers.

Abstract

The Healing Garden Therapy (PGHT) program is intended to improve the physical, mental, social, and spiritual health conditions of patients, visitors, and staff at the Hospital. In this activity, in addition to being participants, health workers also play a role in organizing PGHT. This study contributes to health workers, especially nurses, to implement holistic services for patients. This study is an exploratory qualitative study to explore the perceptions of health workers in the implementation of PGHT. The sample of this study was 13 health workers who had been involved in the implementation of the PGHT at Panti Rapih Hospital Yogyakarta and St Carolus Jakarta, which were taken using purposive sampling techniques. The researcher conducted in-depth interviews with participants, with the help of a recording device and interview guidelines that had been prepared previously. The reporting of the research results has been based on the COREQ Checklist. The results obtained themes; The Healing Garden Therapy program in hospitals is educational and innovative; Has challenges, and opportunities and requires commitment from all components of the hospital in further program development; The Healing Garden Program in hospitals is a manifestation of holistic nursing; The Healing Garden Program in hospitals is very beneficial for patients, families/visitors and hospital employees; and A garden design is needed that supports the implementation of the Healing Garden Program in hospitals. The implementation of the Healing Garden Program is beneficial for patients, visitors, and health workers, so efforts need to be made to be implemented in every hospital.

Abstract

Background: Patients with cervical cancer undergoing CCRT therapy often experience a decline in quality of life such as physical complaints, decreased spiritual well-being, and poor mental health. Family support plays a crucial role for individuals with certain illnesses, including those with cervical cancer.Objective: To describe family support for woman with stage IIIB cervical cancer undergoing CCRT.Case report: Mrs. Y, 37 years old, had been married for 10 years with a parity status of P2A1. She was currently a housewife in a middle-class family. The patient came for a routine check-up at the ICC Oncology Clinic of Dr. Sardjito General Hospital with a diagnosis of stage IIIB cervical cancer, scheduled for CCRT, and anemia. She has undergone chemotherapy twice and radiation therapy 10 times. Additionally, she had no history of hypertension, diabetes mellitus, or previous cancer.Outcomes: CCRT therapy had several physical and psychological impacts on the patient. These were including diarrhea, weight loss, oral problems, frequent fatigue, tingling in the hands and feet, the menstrual cycle alteration, bleeding, anxiety, and sexual relationships modification. Throughout the treatment process, the patient had received strong family support in the form of informational, emotional, instrumental, and appreciation support.Conclusion: Family support enhances self-confidence, provides motivation, and offers significant encouragement to patients. Therefore, family support plays a crucial role in the treatment of cervical cancer, especially for patients undergoing CCRT therapy.INTISARILatar belakang: Pasien kanker serviks yang mendapat terapi CCRT sering dilaporkan mengalami penurunan kualitas hidup yang berkaitan dengan keluhan fisik, penurunan kesejahteraan spiritual, dan kesehatan mental yang buruk. Dukungan keluarga memiliki peranan penting pada individu dengan penyakit tertentu, termasuk individu dengan kanker serviks.Tujuan: Mengetahui gambaran dukungan keluarga pada wanita yang menderita kanker serviks stadium IIIB dengan CCRT.Laporan kasus: Ny. Y, berusia 37 tahun, sudah menikah selama 10 tahun dengan status paritas P2A1. Pekerjaan pasien saat ini sebagai ibu rumah tangga dan pasien termasuk keluarga kelas menengah. Pasien datang untuk melakukan kontrol rutin di Poliklinik Onkologi ICC RSUP Dr. Sardjito dengan diagnosis kanker serviks stadium IIIB pro CCRT dengan anemia. Pasien sudah menjalani kemoterapi sebanyak dua kali dan radiasi sebanyak 10 kali. Selain itu, pasien tidak memiliki riwayat penyakit hipertensi, DM,  dan kanker sebelumnya.Hasil: Terapi CCRT memberikan beberapa dampak bagi pasien, baik secara psikis maupun psikologis. Dampak yang dialami pasien seperti diare, berat badan turun, masalah pada mulut, sering merasa lelah, kesemutan pada tangan dan kaki, perubahan siklus menstruasi, perdarahan, perasaan cemas dan perubahan dalam hubungan seksual. Selama menjalani pengobatan, dukungan keluarga yang didapatkan pasien baik dukungan informasional, penghargaan, instrumental maupun emosional sudah baik.Simpulan: Dukungan keluarga meningkatkan rasa percaya diri, memberikan motivasi dan semangat yang besar bagi pasien sehingga dukungan keluarga mempunyai peranan penting dalam proses pengobatan kanker serviks terutama pasien dengan terapi CCRT.
