Abstract

Background: Infectious diseases have become one of the world’s serious problems including in Indonesia. Infectious diseases can originate from the community and hospital environment. Health workers performing inappropriate medical procedures could also be a cause of transmission of infectious diseases. Infection prevention and control nurses (IPCNs) as the pioneers of prevention and infection control in hospitals have not optimally played their roles.   Purpose: This study aimed to explore the roles of IPCNs and their constraints in preparing for emerging infectious diseases. Methods: The present study employed a qualitative design with the hermeneutic phenomenological approach. The samples were seven IPCNs for the primary participants, and two Infection Prevention and Control Officers (IPCOs) and 13 Infection Prevention and Control Link Nurses (IPCLNs) as the triangulation participants. The data were collected using in-depth semi-structured interviews analyzed using a content analysis technique.   Results: The results identified two themes: (1) IPCNs played roles as clinical practitioners, activity coordinators, administrators, and educators, and (2) the roles of IPCNs had not been optimal due to the lack of support from the hospital management, insufficient infrastructure, weak monitoring and evaluation, and the unavailability of appropriate rewards and  punishment.Conclusion: The majority of participants in this study agreed that IPCNs had tried well to play their roles. However, various obstacles were encountered, which hindered the IPCNs in performing their roles.

Abstract

Background: Communication is an interaction process that is highly essential in building nurse-patient interpersonal relationships. Good nurse communication-patient will have a good impact on the treatment plan and recovery for the patient. Improper communication can lead to mistaken nursing care provision and even lead to injury and death. Objective: This literature review aimed to know the efforts to improve nurse-patient communication. Design: This literature review used narrative review design. Data Sources: The databases used in narrative review are Scopus, Science Direct, ProQuest, and PubMed. The journal is limited to the publication year 2016 - 2020 with the area of ​​the nursing medicine journal, full text Portable Document Format (PDF) journal. This literature review uses 4 articles appropriate the inclusion criteria. The study of articles and data extraction were carried out separately by the researcher, if there were differences, the results were taken by consensus. Review Methods: The review method for this literature review using a narrative method by classifying the similar extracted data according to the research findings. Results: The databases used in the literature review are Science Direct, Scopus, PubMed and ProQuest. The journal is limited to the 2016 – 2020 publication year with the area of ​​nursing medicine journals, improving nurse communication, full text open access pdf journals, research designs. This literature review was identified with the number of science direct: 45, Scopus: 242, PubMed: 36 and ProQuest: 492, then screened and found 4 articles that matched the inclusion criteria. The study of articles and data extraction were carried out separately by the researcher, if there were differences, the results were taken by consensus. Conclusion: Nurse communication training has a positive impact where nurses have skills in developing knowledge and capability in improving nurse communication performance. Good communication nurse-patient can increase the patient's cure rate.

Abstract

Background: Evidence-Based Nurse Practice (EBNP) is used to enhance the quality of nursing by considering internal evidence, external evidence, and patient assessment. In reality, applying EBNP has many problems such as knowledge level, lack of time, facilities, and management support such as training and seminar. The development of technology gives chances for the massive promotion of EBNP. Objective: This application is newly made and its pilot study aims at finding the answer to the usability and accessibility of online application START EBP. Design: This study used a cross-sectional study method. The target population in this study was 112 clinical nurses. The sampling technique used was purposive sampling. The sample size in the pilot study was 12 respondents. This research is a kind of research and development study. The new application is Online START EBP that uses seven levels of applying EBP. The data analysis carried out was usability test. Results: The results of 12 respondents selected from 112 populations by purposive sampling shows that the users are easier to learn and memorize the interface of the application START EBP in total averaged scored 3.24 point. The reliability results show an alpha value of 0.920, so the reliability is in the perfect category. Conclusion: The START EBP online application is easy to use because it is easy to understand by users.

Abstract

Background: World Health Organization (WHO) has established a Directly Observed Treatment Short-Course (DOTS) strategy. Carrying out the DOTS strategy, the public health center is running the TB Prevention and Treatment program, in Indonesia it is called prevention and countermeasures of tuberculosis (P2TB) but in reality, the P2TB program has not been effectively implemented. Health workers who play an important role in carrying out their roles and duties in society are nurses. One of the efforts to improve the performance of nurses in TB treatment is the application of transformational leadership. The purpose of this literature review is to examine the results of the past 10 years of research related to the influence of transformational leadership on nurse performance in tuberculosis treatment. Design: This study design is a literature review to search and review article from database and theory underlying this study or guidance in this literature review using Preferred Reporting Items for Systematic Reviews and PRISMA. Data Sources: The data base used in the article searches used include: Science Direct, PubMed, Google scholar, EBSCO using the keywords transformational leadership, nurse performance, tuberculosis treatment. The articles used are those between the years 2009-2019. Results: The main component of transformational leadership to improve The performance of nurses to play a more optimal role in an effective and efficient TB treatment effort is an idealized influence, which is that nurses have charismatic, in the fact that nurses in providing nursing care in the community must provide good paragons and become role models in the field of good health to individuals, families, groups and communities about how to live clean and healthy as behavior that can be imitated by the community, inspirational motivation for nurses as health teacher educators or health counselling which is a part of health promotion, the nurse's intellectual stimulation empowers communities to solve health problems in the community through small group discussions, and individual consideration, in this case the nurse's duty is as an advice provider and counseling is highly important and beneficial for the community. In order to achieve the fulfillment of the roles and responsibilities of nurses, there must be inner motivation among nurses in order that they can provide decent motivation to the community. Conclusion: Transformational leadership can be applied to improve the performance of nurses in TB treatment efforts.

Abstract

The holistic nursing service is based on the concept that humans are sick not only physically that cured by drug delivery, but also pay attention to other aspects of mental where the patient needs motivation and spirit to cope with illness, social where the patient wants to meet and gather with family/friends and spiritual where the patient wants to pray and pray for healing. Spiritual care is an important part of the overall care provided to improve the quality of life of patients. The role of nurses is now more involved on treatment measures. Proper methods in clinical supervision are necessary for the implementation of spiritual care to be as important as physical care, one of them using drill method. This study aims to analyze the influence of drill methods in the team leader's clinical supervision on the implementation of nurses spiritual care. The research method used is quasy experiment with pretest-posttest research design with control group design. The population of nurse research in inpatient room of PKU Muhammadiyah Yogyakarta and PKU Muhammadiyah Gamping Hospital. A total of 32 nurses were taken as samples through consecutive sampling technique. To see the implementation of spiritual care used observation sheet refers to the label Nursing Interventions Classification, spiritual care is observed before and after the application of drill methods in clinical supervision. Data were analyzed by paired t-test. The results showed the average of nurses' spiritual care before the drill method applied to the intervention group was 6,56 and 6,13 in the control group, after drill method applied in the intervention group was 17,44 and 6,50 in the control group. The results of statistical tests showed that there was a significant difference in nurse's spiritual care before and after the application of drill methods in the intervention group. It is recommended for the hospital to improve the implementation of clinical supervision to the nurses related to non-physical skills using drill method.

Abstract

The high demand for role and function as a nurse can potentially lead to burnout.The impact of burnout causes patients to have secondary infections that are not caused by the disease, decrease patient satisfaction and quality of care and mortality. Efforts that can be done to reduce the nurse's burnout is through the application of an effective leadership style. Caring leadership as a leadership style can facilitate the needs of nurses so that it will improve performance. This study aims to determine the effect of caring leadership of the head of space on the burnout of nurses. The method used is quasi experiment pre and post design with control group. A total of 46 nurses were taken as samples through purposive sampling fron 426 nurses. Data analysis using pairred t-test. The results showed that there was difference of average burnout value before and after applied caring leadership with p-value=0.000, which means there is influence of head room caring leadership against nurse's burnout. Implementation of caring leadership should be done consistently for all nursing activities in the hospital, given the caring leadership can reduce the burnout of nurses that will ultimately have an impact on the quality of nursing services.

Abstract

Healthcare-Associated Infections (HAIs) are infections that patients get while undergoing treatment and medical procedures in the hospital or other healthcare facilities after more than 48 hours. Improving the quality of hand hygiene practices can reduce pathogen transmission that causes HAIs. Infection and Prevention Control Nurses (IPCNs) as the pioneers of infection prevention and control should play an active role in cultivating hand hygiene in the hospital environment. Unfortunately, until the present time, there is evidence that IPCNs have not played their roles optimally in making hand hygiene a culture in the hospital. This study aimed to explore the experiences of IPCNs in cultivating hand hygiene at the inpatient wards in a regional hospital in Semarang, Indonesia. This study used a qualitative research design with a phenomenological approach. The samples were recruited using purposive sampling technique, consisting of three IPCNs as the main participants, and one Infection Prevention and Control Officer (IPCO) and three Infection Prevention and Control Link Nurses (IPCLNs) as the triangulation participants. Data were collected using semi-structured in-depth interviews and analyzed using the content analysis. The results revealed two themes. First, the appropriate education implementation and its documentation are important to provide an adequate understanding of medical and non-medical staffs, patients and families so that they are willing to perform hand hygiene properly. Second, the implementation of hand hygiene requires complete infrastructures and facilities, proper monitoring and evaluation system, as well as appropriate role models and motivation so that hand hygiene can become a positive culture. This study concludes that IPCNs have positive and negative experiences while trying to cultivate hand hygiene. IPCNs understand and agree that their ability to provide proper education and sensitivity to the factors affecting the implementation of hand hygiene can improve the people’s understanding and compliance with hand hygiene. In the end, hand hygiene will eventually become one of the positive cultures in the hospitals. Keywords: IPCNs, hand hygiene

Abstract

Background: Low independent nurse actions leads to poor health care quality. Therefore, understanding the factors affecting independent nursing action is necessity. Objectives: This study aims to analysis the independent nurse actions and its related and predicting factors. Methods: This study used a cross-sectional design with observational technique for data collection. There were 165 nurses recruited using simple random sampling in 14 inpatient wards of the General Hospital of dr. M. Haulussy Ambon, Indonesia. Spearman Rank Correlation and multiple regressions were used for data analysis. Results: Results of this study showed that there were significant relationships of age, gender, education, employment time, family burden, working experience, knowledge, workload, and nurse ability with independent nursing action with p-value <0.05. Nurse knowledge was the most dominant factor predicting independent nursing action with R-value 0.450. Conclusions: Independent nurse actions in daily living activities, caring & support, and rehabilitation were influenced by multiple factors such as nurse knowledge, ability, education and gender. Nurse knowledge is the most predicting factors affecting independent nursing action. Therefore, hospital and nurse managers need to well manage these predicting variables in order to improve independent nurse action that leads to the better quality of health service in the hospital.

Abstract

Background: The SBAR (situation, background, assessment, recommendation) method assists nurses in communicating information in nursing shift handover. Inaccurate shift handover can have a serious impact on patients due to poor communication. Optimal resource development can be done by coaching as the best guidance method from manager for directional discussion and guidance activity to learn to solve problem or do better job and build nursing leadership culture in clinical service. Objective: To analyze the effectiveness of coaching method using SBAR communication tool on nursing shift handovers. Methods: This was a quasi-experimental study with pretest posttest control group design. Fifty-four nurses were selected using a consecutive sampling, which 27 assigned in the experiment and control group. An observation checklist was developed by the researchers based on the Theory of Lardner to evaluate the effectiveness of the implementation of coaching using SBAR on nursing shift handover. Independent t-test, Mann-Whitney test and Wilcoxon test were used for data analyses. Results: There was an increase in coaching ability of head nurses in the implementation of SBAR in nursing handover after 2-weeks and 4-weeks of coaching. There was also a significant improvement of the use of SBAR on nursing shift handover in the experiment group (p <0.05) compared to the control group. Conclusion: Coaching using SBAR (situation, background, assessment, recommendation) communication tool was effective on nursing shift handovers. There was a significant increase of the capability of head nurses and nursing shift handovers after given coaching intervention.

Abstract

The COVID-19 pandemic experienced by people around the world has resulted in an increase in the number of patients and stressors for nurses in Mental Hospital Emergency Departments, and is exacerbated by the condition of mental patients who are unable to communicate properly. The quality of patient care in the ER is determined by the availability of the 5M element where nurses as a human element in the COVID-19 pandemic situation require more attention regarding psychological wellbeing. Psychological wellbeing of good nurses will have a positive impact on nursing services in the ER mental hospital. The purpose of this study was to explore the experiences and feelings of psychologic wellbeing of Surakarta mental hospital emergency room nurses during the COVID-19 period. The research method used a qualitative design on the emergency room nurse mental hospital used Colaizzi analysis method. The results of the analysis of the participant interviews obtained seven themes, namely the meaning of psychological wellbeing (feeling happy, grateful to accept one's own condition, comfortable, useful and productive), Feelings during the COVID-19 pandemic (feelings at the beginning of the pandemic and current feelings), How to adapt during the COVID-19 pandemic. -19 (increased self-readiness, mutual care and support for fellow nurses, fulfillment of facilities and improvement of service systems), Hope for the COVID-19 pandemic (Joking freely and the pandemic will end soon and no health workers will fall), Changes experienced during the COVID-19 pandemic 19 (changes in regulations and culture in the face of a pandemic, changes in methods of studying online), experiences during work (pleasant experiences and bad experiences), families who always support carers (spouses and children and parents), and the impact of the COVID-19 pandemic ( towards others and towards family). The conclusion now is that nurses feel more comfortable and calm in carrying out their duties. The nurses had made adaptations to reduce anxiety so as to improve the psychological wellbeing status of the nurses.
