Abstract

Diabetes Mellitus (DM) is a chronic metabolic disease characterized by elevated blood glucose levels and is linked to an increased risk of hypercholesterolemia. Garlic (Allium sativum Linn), which contains bioactive compounds such as saponins and flavonoids, has potential for managing these conditions. This study aimed to evaluate the effects of garlic extract on two parameters: blood cholesterol levels in Wistar rats fed a high-fat diet and blood glucose levels in rats induced with streptozotocin (STZ). A laboratory experimental post-test-only control group design was used. For cholesterol assessment, hypercholesterolemic rats were treated with garlic extract at doses of 0.05, 0.10, or 0.20 g/head/day, alongside negative and positive control groups. For glucose assessment, STZ-induced diabetic rats received garlic extract at 100, 250, or 500 mg/kgBW/day, with corresponding control groups. Garlic extract significantly reduced mean cholesterol levels in a dose-dependent manner (130.60 mg/dL, 121.80 mg/dL, and 112.00 mg/dL for 0.05, 0.10, and 0.20 g/head/day, respectively). In contrast, mean blood glucose levels showed a marginal decrease with higher doses (282.80 mg/dL, 271.20 mg/dL, and 269.27 mg/dL for 100, 250, and 500 mg/kgBW/day), but no statistically significant differences were observed among groups (P = 0.706, one-way ANOVA). These findings suggest that garlic extract exhibits a significant cholesterol-lowering effect in hypercholesterolemic rats but does not markedly reduce blood glucose levels in STZ-induced diabetic rats at the tested dosages.

Abstract

The most significant metric in the health care system is patient safety, which is meant to serve as a guide in providing the best possible care and lowering patient occurrences. The study's objective is to examine the impact of patient safety management on the standard of care provided in the inpatient unit at X Hospital. Qualitative research methodology is used. The patient safety team, the nursing committee, and the head of quality improvement and patient safety made up the five informants for the September 2022 study. Through observations and interviews, data was gathered. Collaizzi is used in data analysis to reduce data, show data, and draw conclusions.The research results show that there are five themes, namely that the role of the organization in determining patient safety management has been supported by mentors and experts. This is shown by the results of interviews, namely that there are mandatory, meetings, in-house training, reviews and also adequate human resources. Ensuring that patient safety management is supported by mentors and experts, this is shown by the results of interviews, namely that there are mandatory requirements, meetings, in-house training, reviews and also adequate human resources. It is recommended that hospital management provide training and socialization about patient safety programs on a regular basis, provide sanctions for violating the implementation of Standard Operating Procedures and propose a report form using an online system.

Abstract

Tuberculosis (TB) remains a major global public health threat, with Indonesia ranking second worldwide in new cases at 759,000. Puskesmas Paal V in Jambi City recorded 312 cases in 2023 with a treatment success rate of 85.6 percent, still below the National Tuberculosis Control Program target of 90 percent. This study aimed to analyze the characteristics and risk factors of TB occurrence at Puskesmas Paal V during the period January 2023 to April 2026 using multivariate logistic regression integrated with GIS-based spatial mapping. A retrospective cohort design was applied to 285 microbiologically confirmed pulmonary TB patients, selected through total sampling. Data from the e-TB Manager register, medical records, and validated questionnaires were analyzed using Chi-Square tests, logistic regression, and spatial analysis including Moran's I and Getis-Ord Gi*, processed through SPSS 26.0 and QGIS 3.28. Cases were dominated by males (65.6 percent) and the 25 to 44 year age group (57.5 percent), with 69.1 percent presenting at an advanced stage. Significant risk factors included household contact history (AOR 4.31), smoking behavior (AOR 3.14), housing density (AOR 2.45), Diabetes Mellitus (AOR 2.61), and malnutrition (AOR 2.08), collectively explaining 45.2 percent of the variability in TB occurrence. Spatial analysis identified three clustered hotspots (Moran's I 0.45), underscoring the importance of area-based intervention alongside individual risk factors.

Abstract

Keselamatan pasien menjadi tolak ukur utama untuk menilai mutu pelayanan kesehatan yang sangat dipengaruhi oleh performa perawat sebagai tenaga medis, yang sering kali berinteraksi langsung dengan pasien. Penelitian ini bertujuan untuk menginvestigasi lebih lanjut elemen-elemen yang mendukung perawat dalam menjalankan tugasnya secara efektif untuk memastikan keamanan serta keselamatan setiap pasien di RSU Royal Prima Medan. Metode yang digunakan dalam penelitian ini adalah kuantitatif dengan pendekatan cross-sectional. Informasi didapatkan melalui kuesioner yang telah teruji untuk validitas dan reliabilitasnya. Data dianalisis dengan teknik univariat, serta bivariat dengan menggunakan uji Pearson, dan multivariat dengan regresi linear ganda. Hasilnya dari pengamatan yang dilakukan, sebanyak 60,7% perawat berada dalam kategori kinerja yang baik, menunjukkan standar profesionalisme yang terjaga. Semua variabel independen, yaitu pengetahuan (p=0,006), sikap (p=0,010), motivasi (p=0,003), supervisi (p=0,001), ketersediaan SOP (p=0,020), dan beban kerja (p=0,028), memiliki hubungan yang signifikan dengan kinerja perawat. Supervisi adalah faktor utama (B=0,325), sementara beban kerja memiliki pengaruh negatif (B=-0,230). Secara statistik, kita dapat melihat bahwa 68,5% dari kinerja perawat dipengaruhi oleh variabel-variabel yang kita amati dalam penelitian ini.  Disimpulkan bahwa peningkatan kerja perawat membutuhkan metode manajerial yang lengkap, terutama dengan meningkatkan supervisi dan mengelola beban kerja. Penelitian ini menunjukkan bahwa secara bersamaan tugas perawat dalam usaha mencegah kecelakaan pasien dipengaruhi secara signifikan oleh enam faktor, yaitu pengetahuan, sikap, motivasi, supervisi, keberadaan standar operasional prosedur, serta beban kerja. Di antara enam faktor tersebut, supervisi adalah faktor yang paling utama, sedangkan beban kerja memberikan dampak yang berlawanan (negatif) terhadap kinerja. Model regresi yang dibuat dalam penelitian ini dapat menjelaskan 68,5% variasi yang terjadi pada kinerja perawat.

Abstract

This study aims to analyze the effect of the Government’s Free Nutritious Food Program (MBG) on changes in learning concentration and nutritional status among students at SDN No. 075062 Doli-Doli, Mandrehe District, West Nias Regency. This research used a quantitative, quasi-experimental design with a pretest and posttest (no control group). The population consisted of 168 students, with a sample of 128 students selected using the Slovin formula and purposive sampling. Learning concentration was measured using a structured test, while nutritional status was assessed using Body Mass Index (BMI) based on anthropometric measurements. Data were analyzed using univariate methods and the Wilcoxon signed-rank test due to non-normal distributions. The results showed a significant increase in learning concentration (from pretest 30.19 to posttest 34.91; p < 0.001) and nutritional status (from pretest 15.33 to posttest 16.17; p < 0.001). These findings indicate that the MBG program significantly improves students' cognitive performance and nutritional status. The study concludes that school-based nutrition interventions are effective in supporting both health and educational outcomes, particularly in disadvantaged areas.

Abstract

The abundance of natural materials has attracted the attention of numerous researchers, who are now developing them into effective adsorbents for waste treatment applications. The waste stream in question includes laundry liquid waste from workshops, which is discharged directly into the environment via soil or drainage channels without any form of pretreatment. The lead content (Pb) in workshop laundry liquid waste is hazardous if discharged into the environment because it is difficult to degrade. This study investigates a filtration media composed of a combination of natural adsorbents—coconut shell charcoal, pumice stone, and hair waste as an option for treating workshop laundry liquid waste. This research was conducted in January 2021 and focuses specifically on its effectiveness in reducing Pb levels. Adsorbents are arranged in a 60 cm diameter barrel with a height of 40 cm for each adsorbent. Laboratory analysis for the lead test method according to SNI 6989.8.2009 with Atomic Absorption Spectrophotometry (SSA) shows a decrease in Pb levels starting from the control to the first repetition, second repetition, and third repetition, where the final results show that Pb levels are safe to be discharged into the environment because they are below the environmental quality standards based on the Regulation of the Minister of Environment of the Republic of Indonesia Number 5 of 2014 concerning Wastewater Quality Standards, which is a maximum of 0.03 mg/L. Filtration A showed a Pb level of 0.021 (50% decrease), and Filtration B showed a Pb level of 0.106 (80% decrease)

Abstract

Nurse performance and turnover intention are critical issues in hospital human resource management, influenced by leadership style, job satisfaction, and workload. Objective: This study aimed to analyze the effects of transactional and transformational leadership styles, job satisfaction, and workload on nurse performance and turnover intention at Sabang Regional General Hospital (RSUD Kota Sabang). Methods: A quantitative analytic design with a cross-sectional approach was employed. The study population and sample comprised all staff nurses in the inpatient units of RSUD Kota Sabang using total sampling (n=120). Data were collected using standardized Likert scale questionnaires (1–5), adapted from the Multifactor Leadership Questionnaire (MLQ) by Bass & Avolio, the Job Descriptive Index (JDI), and a validated turnover intention instrument. Multiple linear regression analysis was performed following normality, linearity, and multicollinearity testing. Results: Simultaneously, all four independent variables significantly influenced nurse performance (Sig.=0.037; R²=3.8%) and turnover intention (Sig.=0.032; R²=2.8%). In partial analysis, transformational leadership demonstrated the strongest positive effect on performance (B=0.211; Sig.=0.002) and the strongest negative effect on turnover intention (B=-0.111; Sig.=0.030). Workload was the most dominant predictor of performance decline (B=-0.186; Sig.=0.035), while job satisfaction was the most significant predictor of nurse retention (B=-0.110; Sig.=0.014). Conclusion: Transformational leadership and workload management represent priority interventions for improving nurse performance and reducing turnover intention. The low R² values indicate the need for further research integrating organizational culture, remuneration systems, and individual factors into the explanatory model.

Abstract

This study aims to determine the effect of bay leaf ethanol extract on the histopathological picture of the kidneys of rats induced by doxorubicin. To determine the administration of bay leaf ethanol extract is more effective in protecting the heart than the positive control (Vitamin E). The bay leaves used were obtained from the Harjosari II Medan Amplas area of Medan city. The independent variable in this study was Salam Leaf Ethanol Extract, while the dependent variable was kidney histopathology. This research was conducted at the Pharmacology and Toxicology Laboratory of the Faculty of Pharmacy, University of North Sumatra. The results showed that the administration of vitamin E together with doxorubicin showed a better renal histopathological picture. Vitamin E as a basic antioxidant is able to reduce the damage caused by doxorubicin which is seen in the absence of widening in the Bowman capsule.

Abstract

An important indicator to determine the quality of service in a hospital is about the completeness of the hospital's medical record files. There are many problems arise due to incomplete medical record files and do not recorded neatly and regularly. The incident is due to lack of medical team awareness in recording all information or actions that would be given to the patient. The aim of this study is to analyse the effect of Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. This study uses quantitative research method which is analytical in nature. The total sampling of this study is 115 active nurses at RSU ESHMUN Medan. The result of the study informs that there are very strong and significant effect among Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. The variable that has the most influence on Obedience with filling in inpatient medical record files at RSU ESHMUN in 2023 is the Protection Motivation variable. It is recommended to RSU ESHMUN Medan to improve the administration pattern in order to all of nurses have the same job demands and receive the same support.

Abstract

Introduction:Stunting remains a major public health issue in Indonesia, including in the working area of UPTD Puskesmas H.A.H Hasan Binjai. Mothers’ perceptions and feeding practices are critical determinants influencing child growth and nutritional status. Understanding how mothers perceive the direct and indirect causes of stunting is essential to develop effective, culturally appropriate interventions at the community level. Methods:This study employed a qualitative exploratory design to explore mothers’ perceptions of stunting determinants. In-depth interviews were conducted with eight informants, comprising five mothers of toddlers, one community health volunteer, one village midwife, and one nutrition officer. Data were analyzed thematically through data reduction, data display, and conclusion drawing following the Miles and Huberman framework. Results:The findings indicated that most mothers had limited understanding of nutrition, emphasizing food quantity rather than quality. Exclusive breastfeeding practices were suboptimal due to misconceptions that watery breast milk lacks nutrition. Complementary feeding (MP-ASI) was monotonous and low in animal protein. Indirect determinants such as unstructured parenting, limited family support, and poor economic conditions contributed to stunting risk. The main barriers identified were economic hardship, time limitations, and limited access to health services. Mothers with positive perceptions demonstrated more consistent preventive behaviors, such as exclusive breastfeeding, providing diverse complementary foods, and active participation in community health programs. Conclusions:Mothers’ perceptions significantly influence stunting prevention behaviors. Low nutritional literacy, cultural beliefs, and economic challenges remain key barriers. Effective prevention efforts require family-based and culturally tailored nutrition education supported by community health cadres to enhance maternal understanding and sustainable child health practices.
