Abstract

Abstract

Abstract

Background: Atrial Septal Defect (ASD) is one of the most common congenital heart diseases, occurring in about 25% of children and accounting for 6-10% of all congenital heart conditions. ASD is more frequently observed in females with a ratio of 2:1 and often does not show symptoms in infants, unless there is an increase in right ventricular volume. Case Report: A 7-year-old girl was referred with complaints of shortness of breath and cyanosis at the fingertips, accompanied by palpitations. Initial diagnosis was established through physical examination and echocardiography, which revealed a secundum ASD measuring 1.9 cm. The ASD closure surgery was performed but had to be halted due to intraoperative bradycardia. The patient then underwent subsequent therapy with Lisinopril and Spironolactone and was in good condition at the last follow-up. Discussion: Management of ASD, whether through surgical or catheter-based intervention, is recommended to prevent further complications such as paradoxical embolism and pulmonary hypertension. This case underscores the importance of timely diagnosis and treatment to prevent long-term health impacts in patients with ASD.

Abstract

Background: Ovarian cancer is considered a silent killer disease, mainly due to the late diagnosis. It is often diagnosed at an advanced stage, thus increasing the mortality rate. Ovarian cancer can happen to any age, with different characteristics according to the age group, the most often being the epithelial type. Methods: This research is a descriptive study with a quantitative method. This study aims to determine the characteristics and histopathological features of ovarian cancer in Dr. Ramelan Navy Hospital from January 2019 to December 2021. There were 155 samples out of 635 patients, with the information acquired through medical records. Results: The highest incidence of ovarian cancer occurs in the 40-60-year-old group at 52.9%. Among all participants, 85.8% of patients were not employed. Of most histopathological features in epithelial ovarian cancer, 34.8% were the serous carcinoma subtype. The majority of these cases were diagnosed at stage IIIC at 21.3%, and the most prevalent treatment for ovarian cancer was surgery and chemotherapy at 49.7%.Conclusions: The most common type of ovarian cancer is epithelial ovarian cancer, a subtype of serous carcinoma, with histopathological features of round nucleated anaptic cells, prominent nucleoli, forming acini, papillae, and thin connective tissue stroma.

Abstract

BACKGROUND: Glaucoma can lead to irreversible blindness, but normotension glaucoma (NTG) often shows no early symptoms. However, to date, there is limited knowledge regarding the potential parameters for early detection. Therefore, in this study, various metabolic parameters and biomarkers which may contribute to NTG in metabolic syndrome (MetS) patients were evaluated.METHODS: A retrospective cross-sectional study was conducted in the National Cardiac Center Harapan Kita Hospital. Mets were confirmed following the IDF criteria. NTG was determined based on normal intraocular pressure (IOP) with a mean defect (MD) of the visual field, thinning of the retinal nerve fiber layer (RNFL), and enlargement of cup disc ratio (CDR) by ocular coherence tomography (OCT). Metabolic parameters results of waist circumference (WC), body weight, body height, body mass index (BMI), blood pressures, HbA1c, fasting blood glucose (FBG), lipid profiles; and biomarkers including thiobarbituric acid reactive substance (TBARS) and ferric reducing antioxidant power (FRAP), leptin, adiponectin, high-sensitivity C-reactive protein (hs-CRP), and endothelin-1 (ET-1) were analyzed. Statistical analysis was performed using comparative and correlative tests.RESULTS: From 29 subjects, 19 subjects (65.5%) were included in the NTG group and 10 subjects (34.5%) were included in the non-NTG group. In the NTG group, we found significant correlation between MD with systolic blood pressure (p=0.035); CDR with ET-1 (p=0.049); and CDR with adiponectin (p=0.010). The non-NTG group had a significant correlation between MD with BMI (p=0.043); CDR with LDL (p=0.042); and CDR with TG (p=0.031).CONCLUSION: There are correlation between adiponectin and ET-1 with NTG in MetS patients. Therefore, they might be considered as potential early detectors for NTG in MetS patients.KEYWORDS: normal tension glaucoma, metabolic syndrome, biomarker, endothelin-1, adiponectin

Abstract

Introduction: Congenital Heart Disease (CHD) is one of the most common congenital disabilities, with an incidence of 8-10 per 1000 live births in full-term infants and ten times higher in preterm infants. In Indonesia, the incidence of CHD is estimated at around 8 per 1000 live births.  Understanding the anatomy and pathophysiology of the defect that occurs contributes to the accuracy of management planning.  Purpose: This study aims to identify the characteristics of pediatric patients with CHD featuring left-to-right shunt accompanied by left ventricular enlargement at RSPAL dr. Ramelan Surabaya from 2020 to 2023. Methods:  The descriptive study uses secondary data (medical records) of pediatric CHD patients with left-to-right shunt and left ventricular enlargement, specifically focusing on ventricular septal defect (VSD) and patent ductus arteriosus (PDA) at the pediatric clinic of RSPAL dr. Ramelan.  Results: Based on medical records from 45 samples, the majority of CHD patients were aged 1-12 months (23 children). CHD with VSD was more common in males (18 children), while PDA was more common in females (11 children). Most CHD patients showed no initial complaints (39 children) and did not have other types of congenital heart diseases, with 23 children having VSD and 10 having PDA. Additionally, most patients did not have comorbidities (39 children). Echocardiographic results indicated that most patients had an ejection fraction (EF) greater than 75% (27 children) and a fractional shortening (FS) greater than 45% (23 children). The main management involved echocardiographic evaluation and medication.  Conclusion: The predominant age group for CHD cases at the pediatric clinic of RSPAL dr. Ramelan was 1-12 months, with echocardiographic results showing normal left ventricular function, with EF >75% and FS >45%.
