Abstract

Penyakit Tuberkulosis membutuhkan pengobatan jangka panjang dan cenderung terkendala oleh ketidakpatuhan pasien. Self efficacy (SE) merupakan faktor penting kepatuhan menjalankan pengobatan jangka panjang. Penelitian ini bertujuan untuk mengidentifikasi SE pada pasien TB paru di Poli DOTS RSUD dr Slamet Garut untuk menyelesaikan pengobatan.Penelitian deskriptif kuantitatif ini melibatkan 96 pasien TB paru yang berobat ke Poli DOTS RSUD dr Slamet Garut yang direkrut menggunakan accidental sampling. Data dikumpulkan menggunakan kuesioner SE yang dimodifikasi dari Bagja (2016), berisi 26 pernyataan, data dianalisis dengan penyajian distribusi frekuensi dan mean.Hasil penelitian menemukan bahwa secara umum SE pasien TB di RSUD Garut dalam menyelesaikan pengobatan rendah (57.3%). Domain pemeriksaan dahak merupakan domain terendah dan mengatasi kebosanan merupakan aspek yang pasien paling merasa tidak yakin mampu mengatasinya. Proporsi pasien TB dengan SE rendah lebih banyak ditemukan pada pasien berusia dewasa awal (76,9%), perempuan (58,5%), menikah (63,2%), berpendidikan SD dan SMP (60,5-71,9%), pekerjaan sebagai petani (87,5%), berpenghasilan di bawah 1 juta/bulan (60,7-71,9%), BTA (+) (70,5%), pengobatan fase lanjutan pada bulan ke 3 (64,7%) dan tidak pernah mengikuti pendidikan kesehatan tentang pengobatan TB (59,2%).Kesimpulan bahwa pasien TB di Poli DOT RSUD Garut memiliki SE rendah. Menjadi penting bagi petugas kesehatan (dokter dan perawat) dan PMO (pengawas minum obat) untuk menjalankan perannya lebih intensif. Petugas kesehatan diharapkan mengembangkan program atau memberikan edukasi yang secara literatur lebih efektif yang dapat meningkatkan self efficacy khususnya terkait pemeriksaan dahak dan mengatasi kebosanan dalam mengkonsumsi obat TB sebagai upaya peningkatan keberhasilan pengobatan TB.

Abstract

Chronic kidney disease (CKD) is a global health problem that can lead to serious complications. In patients with CKD and diabetes mellitus (DM), there is an increased risk of falls, dependence in daily activities, and stroke. Although these risks are often associated with the elderly population, the phenomenon of multimorbidity is now also increasing trends among individuals of working age. This study aims to describe changes in fall risk levels, dependency levels, and stroke risk knowledge among CKD patients with DM and glaucoma during hospital care. Data collection was conducted through observation, interviews, physical examinations, and medical record reviews. The sample used was a 43 year old woman. The study results showed a significant improvement in reduced fall risk (MFS: 35 to 25), decreased dependency level (CDS: 53 to 66), and increased stroke risk knowledge (SRQ: 28 to 40). This study indicates that comprehensive interventions such as hemodialysis, wound care, pharmacological therapy, and education effectively improve functionality and understanding in productive aged patients with complex multimorbidity.

Abstract

Background: Patients with heart failure (HF) often experience cognitive impairment, which negatively affects their quality of life. An effective screening tool is essential for nurses and healthcare professionals to assess cognitive function as part of HF management. Although many instruments exist, none are specifically designed for patients with HF. Objective: This study aimed to map the instruments for screening cognitive function in patients with HF. Design: A scoping review. Data Sources: Articles published between 2019 and 2023 were searched in PubMed, ScienceDirect, and Google Scholar, with the last search conducted on 27 January 2024. Review Methods: The review followed the scoping review framework by Arksey and O’Malley and adhered to PRISMA guidelines for scoping reviews. Results: Of the 21 articles meeting inclusion criteria, six cognitive function screening instruments were used across various cognitive domains, effectively identifying cognitive impairment in both inpatient and outpatient HF settings. The Montreal Cognitive Assessment (MoCA) was the most frequently used tool, covering a broad range of cognitive domains. MoCA showed high efficacy with a kappa coefficient of 0.82, Cronbach’s alpha reliability of 0.75, sensitivity of 90%, and specificity of 87%. Conclusion: Instruments like MoCA, Mini-Cog, and TICS-m show promise for assessing cognitive function in patients with HF, each with specific strengths and limitations. MoCA is notable for its comprehensive coverage despite being time-consuming and having language barriers. Further research is needed to revalidate and improve the existing instruments. It is crucial for nurses and healthcare professionals to integrate these tools into regular patient management, highlighting the need for continued research in their application.

Abstract

Septic arthritis is a musculoskeletal infection that can lead to permanent joint damage if not managed promptly and adequately. Patients following arthrotomy commonly experience pain and decreased muscle strength due to immobilization, which contributes to impaired physical mobility. Gradual weight-bearing exercise is a rehabilitative intervention aimed at improving muscle strength, reducing pain, and enhancing ambulatory ability. This study employed a descriptive case study design involving a 47-year-old male patient diagnosed with post-arthrotomy of the right knee due to septic arthritis at RSUD Umar Wirahadikusumah in March 2025. The intervention was administered over four consecutive days with progressive loading adjusted to the patient’s tolerance. Evaluation was conducted using Manual Muscle Testing (MMT), Numeric Rating Scale (NRS), and Functional Ambulation Category (FAC). The results demonstrated an improvement in physical mobility, indicated by decreased pain levels, increased muscle strength, and gradual enhancement in ambulatory function. Therefore, gradual weight-bearing exercise is considered an effective nursing intervention in supporting the recovery of musculoskeletal function in post-arthrotomy patients with septic arthritis

Abstract

Coronary Artery Bypass Grafting (CABG) is an effective revascularization procedure; however, postoperative recovery extends beyond clinical outcomes and involves complex physical, psychological, social, and spiritual dimensions. Understanding patients’ lived experiences is essential to develop holistic and patient-centered care. This study aimed to synthesize qualitative evidence on the lived experiences of patients following CABG. A scoping review was conducted following the Arksey and O’Malley framework and reported in accordance with PRISMA-ScR guidelines. A systematic search was performed across PubMed, Scopus, Taylor & Francis, and EBSCO databases. Studies were selected using predefined inclusion and exclusion criteria based on the PCC (Population, Concept, Context) framework. A total of 14 qualitative studies published between 2015 and 2026, conducted across 10 countries, were included. Data were analysed using a descriptive and thematic synthesis approach, categorizing extracted data to identify recurring patterns, then synthesizing findings across physical, psychological, social, and spiritual dimensions of recovery. The synthesis identified four overarching themes: (1) psychospiritual transformation and meaning reconstruction, (2) gaps in information, communication, and patient involvement, (3) physical burden and functional limitations, and (4) social role disruption and structural barriers to recovery. The synthesis results show that post-CABG patient recovery is influenced by various aspects, including psychospiritual changes, physical limitations, lack of information and patient involvement, and social barriers in carrying out daily roles. Therefore, a more comprehensive treatment approach is needed to optimally support the patient’s recovery process.
