Abstract

Osteoarthritis (OA) is a disease caused by the wear of joints, demage of cartilage in the outer layers caused by repeated used of joints can lead to disability. According to WHO, 8,1% of total population in Indonesia suffering from OA. This study aims to research the characteristics of OA patients in outpatient in RSUD Dr. Pirngadi Medan in 2015, descriptive case series design. Data were analyzed by chi-square test, Fisher, and Kolomogorov-Smirnov. The population was 967 OA patient in outpatient and the sample was 96 patient. The research showed that the highest proportion of OA patients at age group 45 - 59 (39,6%), female (80.2%), Muslim (54.2%), senior secondary education (69.8%), from the city of Medan (74%), the location of the genus joint (53.1%), the largest other disease is hypertension (29.2%), with pharmacological (91.7%), OA patient the number of visit was ≤ 3 times (63,5%) and cost source from JKN (87.5%). There was no significant difference between age and the joint location. There was no significant difference between the gender and the joint location and there was no significant difference occupation and the joint location. To the risk population of OA in order to carry out examination of the bones to prevent damage to bones or joints are more severe. Patient with OA, particulary 45-65 years old and have obesity should not do more activity, especially in the genus joint and is recommended for reducing weight through proper diet. Patient with OA should receive treatment of physiotherapy rather than taking non-steroidal anti-inflammatory drug to prevent an increase in hypertension. To RSUD Dr. Pirngadi Medan, field the complements medical record OA patients by add BMI patients and history of injury patient. Key words: OA, Patient Characteristic

Abstract

Pneumonia is an infectious disease that is a major cause of death in children under five in the world. In 2016 Indonesia was the 7th highest in the world of mortality rate that caused by pneumonia.The case of pneumonia in West Sumatra on 2015 is 85% and Pariaman city is 39% which is the city with the third highest of pneumonia cases in the province of West Sumatra. The purpose of this study was to determine the characteristics of children under five hospitalized pneumonia patients in RSUD Pariaman West Sumatra. This research used case series design with the population is all ofchildren under five hospitalized pneumonia patients whose data was recorded in the medical records in RSUD Pariaman West Sumatra on 2016 - February 2017. The population were 108 patients and all were taken to be sampled. The data was collected from secondary data which is processed by univariate analyze by describing the variables and bivariate analyze using Chi-square test and Mann-Whittney test. Based on sociodemographic the highest proportion in the age group 0-11 months (38%), male (60.2%), Minangness (100%), Islam (100%), outside of Pariaman City (51.9%). The highest proportion found in children under five with good nutritional status (57.4%), incomplete basic immunization status (63%), with other diseases (82.4%), the average length of treatment for 6 days and return outpatient (54.6%). There is a significant difference of age based on immunization status (p=0,001), the average length of treatment based on the nutritional status (p=0,007) and other diseases suffered by (p=0,002). The result of this research are there is a significant difference of age based on immunization status, the average length of treatment based on the nutritional status and other diseases suffered by. Department of Health Promotion and Children's Ward in RSUD Pariaman to be more active in providing information about pneumonia to parents so they dont underestimate their child's illness, recognize the signs and symptoms of pneumonia and even prevent the occurrence of it. They also should be more active in providing information about the importance of complete basic immunization for children aged <1 year.     Keywords        : Children under five, Pneumonia of children under five, Patients characteristics

Abstract

Typhoid fever is a systemic infection caused by Bacteria Salmonella typhi, transmitted through the consumption of contaminated food/water. In 2015, Sumatera Utara Province is in the 28order out of 34 provinces with the success percentage of the Total Environmental Based SanitationProgram/STBM 8.49%. InDr. RM. DjoelhamHospital, typhoid fever is 1 of10 largest diseases of inpatient with the total of patients 108 people in 2016. This study aims to determine the characteristics of patients with typhoid feverhospitalized at Dr. RM. Djoelham Hospital of Binjai City in 2016. This research is descriptive with case seriesdesign, Population of this research is all data of inpatient typhoid fever which are 108 in total. The research sample use the whole population. Statistical analysis use Chi-Square test, Mann-Whitney test, Kruskal-Wallis test, and Exact Fisher test. The higest proportion of people with typhoid fever based on sociodemography is in 12-25 years old group (38.9%), female (56.5%), Java (70.4%), Islam (81,4%), students (55.5%), unmarried (74.1%), in Binjai City (99.1%),being treat directly/no referral (32.4%), fever (16.7%), average length of stay 2.88 days (3 days), recurrent typhoid (6 persons), JKN (86.1%), clinical recovery (54.6%). The Chi-Square test, show no significant difference betweenthe condition when being out of hospital based on clinical symptoms (p=0,822). The Mann-Whitney test, show significant difference between the average length of stay based on the cost source (p=0.011). Kruskal-Wallis test,show no significant difference between the length of stay based on the condition when being out of the hospital (p=0.176). The Exact Fisher test, show no significant proportion difference between the cost source based on the condition whe being out of hospital (p=0.329). Recommendedto doctors, nurses, midwives, and other health workers to encourage patients with typhoid fever to complete treatment so that there will be no recurrent tifoid or Carrier. Abstract

Based on Riskesdas Sumatera Utara (2007), the proportion of Typhus abdominalis in Medan city is 600 per 100,000 population. In 2016 the proportion of Typhus abdominalis cases in children whom hospitalized at RSU Sundari Medan was 86.6%. The research aims to find out the characteristic of Typhus abdominalis patient in children whom hospitalized at RSU Sundari Medan in 2016. The research is descriptive research with case series design. The population is data of Typhus abdominalis in childrenwhom hospitalized at RSU Sundari Medan was 845 patient data, and the samples is 272 patient data taken by simple random sampling. Data analysis using chi-square test and Mann whitney test. The highest sociodemographic proportions: age group 6-18 years 45,6%, males 55,9%, Islam 89,3%, not school 54,4%, and live in Medan city is 91,9%. Proportion based on clinical symptoms when admitte, the highest symptoms was fever 100.0%, no complication 89.5%, extraintestinal complication type 76,9%, tubex® test lab diagnostic result average score 4.36, average treatment duration 5 , 07 days, not own expense (BPJS, etc) 79,4%, go home treatment 77,2%. There was a significant difference between the average treatment duration based on the source of cost (p = 0.0001). There was a significant difference between the proportion of circumstances at home by source of cost (p = 0.0001). There was a significant difference betweenaverage treatment duration and the proportion of circumstances at home based on source cost. The hospital to further improve the handling of patients with Typhus abdominalis in children so as to reduce the incidence, career cases, and the occurrence of complications, and the hospital medical record to better complement of data patients with Typhus abdominalis such as tribe, education, and complication status. Abstract

ABSTRACT                Hepatitis B is a still a public health problem and a cause of death. WHO 2012, Hepatitis B virus (HBV) infects 2 billion of the world's population, about 360 million of whom are chronic Hepatitis B and 600 thousand die from it. To determine the characteristics of Hepatitis B patients in hospital Dr. Pirngadi Medan 2015-2016 Conducted a research with case series design. Population and sample were 120 patients and recorded in hospital medical records. Univariate data were analyzed descriptively while bivariate data were analyzed using Chi-square test, t-test and Anova with 95% CI. Proportion based on sosiodemographics were groups of age >45 years is 56,7%, male 75,8%, islam 83,3%, high school 49,6%, self employed 40,8%, married 78,3% and living in Medan 62,5%. Level of Bilirubin is not normal 79,2%, level of SGOT is high 43,3%, level of SGPT is high 45,8%, level of albumint is not normal 87,5%, type Acute Hepatitis B 50,8%, , average of treatment duration is 8,73 days, not to beneficiaries 47,5 and returned with outpatient 73,4%. There was significant difference between sex and type Hepatitis B (p=0,001), type Hepatitis B and level of Bilirubin (p=0,034), type Hepatitis B and level albumint (p=0,001), average of treatment duration and type Hepatitis B (p=0,002). It’s hoped the hospital Dr. Pirngadi Medan to give advice for Hepatitis B patients acute and chronic to check up serologic and liver function regularly, and it’s hoped the next researcher will research about one of characterictics or some risk factors are more specific. Abstract

Prevalence of communicable diseases and non-communicable diseases is double burden diseases in Indonesia. Diabetes mellitus (DM) is one of the non-communicable diseases that continues to increase in number and tend to be more at risk of pulmonary tuberculosis (TB) infections. The prevalence of type II DM with pulmonary TB in Indonesia by 2013 is 400 / 100,000 inhabitants. The purpose of this research is to know the characteristics of DM type II with pulmonary TB in RSUD Dr. Pirngadi Medan in 2016. Research is descriptive with case series design. Population is all data of DM type II patients with pulmonary tuberculosis with many sample that is 112 patient. Univariate data were analyzed descriptively while bivariate data were analyzed using Chi-Square test.           The results showed the highest proportion of patients with type II DM and pulmonary tuberculosis were age group ≥ 40 years (92%), male (60,7%), Bataknese (63,4%), Islam (60,7%), SMA (equal to 76.8%), entrepreneur (38.4%), Medan City (58%), HbA1C (70.5%), HbA1C ≥ 7% (uncontrolled) %), Smear positive pulmonary TB (82.1%), new cases (37.5%) and relapse cases (37.5%), heart and blood vessel disorders (36.6%), Oral Hypoglycemic Drugs OHO) (64.3%), BPJS / ASKES / KIS (90.2%), becoming outpatient / PBJ (83%). There was no significant difference between the proportion of HbA1C levels based on the proportion of type II DM treatment (p = 1,000) and there was a significant difference between the proportion of HbA1C levels based on the proportion of sputum microscopic pulmonary examination results (0.0001).               The results showed a difference between the proportion of HbA1C levels based on the proportion of sputum microscopy pulmonary examination results. It is desirable for the hospital to check HbA1C levels for DM type II patients with pulmonary tuberculosis and to the patient to be treated immediately and prevent transmission to others and is expected to perform routine HbA1C examination.Keywords: Type 2 DM, Pulmonary TB, characteristic of the patient

Abstract

ABSTRACT Breast cancer is one of the women’s health problem in Indonesia. Based on esimation Globocan International Agency of research on Cancer in 2012 explain insidens rate breast cancer of 40 per 100.000 women. Based on medical record data from Dr. Pirngadi Hospital medan in 2011-2013 there was 775case of breast cancer which hospitalized. The purpose of this study is to know characteristic of breast cancer in Dr. Pirngadi Hospital Medan in 2016. The research is a descriptive research  with case series design. The population totaled to 775 peoples and the sample totaled to 106 people. Data obstained medical records, data analyzed by Chi-Square and Mann-Whitney. The result show the highest proportion of breast cancer patients at the age of 47-52 years (27,3%), women (100%), senior high school (76%), Islam (66,7%), Javanese (51,3%), housewife (62,0%), married (80,0%), residence outside Medan (75,3%), the main complainment of lumps in breast area (39,3%), stage IIIB (32,7%), BPJS (99,3%), chemoterahphy (76,7%), average length of stay 5 days, home ambulatory (58,0%). The result of statistical test show there was a difference average length of stay of breast cancer patients based on treatment. There was no difference betwen proportion of age by clinical stage, the medical management by the clinical stage, the average treatment duration by clinical stage, medical management by the circumtance when returning home, medical management by the source of cost, clinical stage by the circumtance when returning home. Women aged 25 years are advised to perform breast self-examination and checking her self to the hospital or a doctor if find a lump in breast area. Dr. Pirngadi general hospital Medan is suggested to complete the breast cancer patient status card include family history of disease sufferers. Abstract

Parkinson's disease attacked millions of people in the world, or about 1% of the world's total population. The cause of Parkinson's disease is unclear, but one of them is an environmental factor. Environmental factors that lead to this disease is pesticide exposure. The purpose of this research to analyze the relationship of personal hygiene and the household insecticide use with Parkinsonism in farmers. This research was an observational analytic with cross-sectional design. The sample amounted to 60 people and taken by simple random sampling technique. The data obtained through interviews and observations using a questionnaire. This study resulted that 53.3% of farmers had Parkinsonism. As many as 21.7% of farmers had poor personal hygiene, and 63.3% of farmers were household insecticide use. The chi-square test showed that there was a significant relationship between personal hygiene with Parkinsonism (p = 0.0001), and there was a significant relationship between household insecticide use with Parkinsonism (p = 0.001).

Abstract

The aims of the study is to find out the effect of lighting in family room. This type of research uses observational analytic studies with case-control study designs. The result of the study shows that there is the effect of lighting in the family room (p = 0.010, OR = 4.125, 95% CI = 1,473 - 11,555), lighting in the room (p = 0.009, OR = 3.167, 95% CI = 1,398-7,174) with the incidence of pulmonary TB, while the incidence of pulmonary TB disease, whereas occupancy density had no effect (p = 0.263). The result of multivariate analysis showed that lighting in the family room had the strongest influence with TB disease (p = 0.001 and Exp (B) of 24.560).

Abstract

Stroke is one of the most dominant non-communicable disease complications that can cause death and disability, the number of sufferers continues to increase. The number one cause of death stroke in Indonesia with a proportion of 15.4%. Many factors can cause a stroke, namely factors that can be modified and factors that cannot be modified. This study aims to determine the determinants of hypertension, obesity, smoking of the incidence of stroke in hospitalized patients at the Regional General Hospital of Rokan Hulu Regency, Riau. This type of research is an observational analytic study with a case-control study design with a total sample of 100 people (50 cases and 50 controls) with a ratio of 1: 1. Sampling was done by purposive sampling and using a questionnaire. The independent variables are hypertension, obesity, and smoking. Hypertension using a sphygmomanometer, Obesity using Body Mass Index (BMI) and smoking using a questionnaire. Data analysis was performed by bivariate analysis using simple logistic regression tests and multivariate analysis using multiple logistic regression tests. The results of bivariate analysis with risk factors for stroke events, the most dominant risk factors were hypertension with a value of p = 0.001 and (OR = 5.441, 95% Cl = 2,210-13,397), Obesity with a value of p = 0.026 and (OR = 1,712, 95% Cl = 0,690-4,244), and smoking with p value = 0,037 and (OR = 1,822, 95% Cl = 0,717-4,632). Hypertension is the most dominant risk factor for the incidence of stroke in hospitalized patients in Rokan Hulu District Hospital, while the increased risk of stroke with obesity and smoking is not statistically significant in this study.
