Abstract

Hypocalcemia can present with a variety of clinical symptoms and indications. In situations of nerve-related hypocalcemia, muscle cramps and tetany are signs of increased excitability in either peripheral nerves or the central nervous system. Here, we described the case of a 58-year-old woman experiencing ongoing pain and cramping in both her upper and lower extremities. Through careful physical, neurophysiological, and radiological examinations, we were able to identify hypocalcemia as the primary cause of her severe muscle spasms and cramps, known as spasmophilia. Further detailed history-taking revealed a past total thyroidectomy. Administration of adequate calcium supplementation successfully relieved the symptoms.

Abstract

Tuberculosis (TB) is a widespread infectious disease caused by Mycobacterium Tubercle Bacilli. In 15-20 % cases of active pulmonary TB, infection spreads to extrapulmonary foci, including the nervous system complications. Ischemic stroke related tuberculosis without meningitis is rare. We report a 74-year-old female presented with rapid onset confusion, unconsciousness, and left hemiparesis. She had been recently diagnosed with active TB 3 months ago and treated with combination anti tuberculosis regiment. Her recent acute neurological manifestations began within 24 hours prior to admission in the hospital. The brain CT showed  middle cerebral artery territory infarction. The risk factors tracing found no significant abnormality. The only abnormal finding was high CRP. This report describes rare case of a patient with active TB and who presented with acute ischemic stroke.

Abstract

There are varying reports of 6 to 16% of patients with symptoms similar to trigeminal neuralgia having intracranial tumors. Acoustic neuromas are benign cerebellopontine angle (CPA) tumors that are most frequently responsible for TN-like symptoms. It has been claimed that magnetic resonance imaging can detect brain tumors with great accuracy (92% to 93%). We describe a case of an adult male patient who had trigeminal neuralgia and temporomandibular dysfunction, which were cured; nonetheless, an acoustic neuroma in the CPA was later discovered.

Abstract

Forearm fracture defined as fracture that occurs in the forearm, where the radius and ulna bones are both broken. Forearm fracture is the most common type of fracture that occurs in children and adults. Traffic accident is one of the cause of this fracture. The initial approach in non-complicated forearm fracture is closed reduction with cast immobilization (conservative therapy). The aim of this study is to analyze the success rate of closed reduction of the forearm fracture at Bethesda Hospital of Yogyakarta. The study also examines the relationship between the type of fracture and the related success rate of closed reduction on forearm fracture as well as the distribution of forearm fracture at Bethesda Hospital, Yogyakarta. This is a descriptive study with cross-sectional design. Radiographs and medical records of patients with forearm fracture at Bethesda Hospital, Yogyakarta were analyzed retrospectively in the period of 2007-2013. The measured variables are the change in the degree of angulation, apposition changes, gap occurrences, and the presence of overlap between radiological examinations before and after closed reduction. Medical records are also analyzed after reduction is done. Data is statistically analyzed using Chi-Square method. The result shows that closed reduction of forearm fracture performed by emergency team of Bethesda Hospital Yogyakarta during the period of 2007-2013 was quite successful and there is a significant relationship between the type of fracture with the success rate of closed reduction of forearm fracture (p=0.003). Thus, closed reduction is a mainstay initial approach in the patients with forearm fracture. If it is not successful, then surgery can be performed.

Abstract

Stroke adalah penyebab kedua kematian di seluruh dunia serta menyebabkan disabilitas fisik dan neurologis jangka panjang. Keparahan defisit neurologis dini adalah penentu yang paling penting dari hasil fungsional akhir pada stroke iskemik akut. Tujuan dari penelitian ini adalah membandingkan efektivitas penggunaan sitikolin dan asam folat dalam memperbaiki disabilitas neurologis pasien stroke iskemik akut. Penelitian ini dilakukan dengan cara narrative review dan dilakukan dalam empat tahap yaitu penentuan kata kunci, kriteria inklusi dan eksklusi, seleksi literatur, dan analisis hasil. Pencarian literatur dilakukan melalui database elektronik seperti PubMed, Scopus, Science Direct, Wiley Online Library dan Springer Link. Berdasarkan review yang dilakukan diperoleh data bahwa sitikolin terbukti neuroprotektor yang aman dan efektif diberikan pra, pasca serta pengobatan stroke jangka panjang sebab mampu menurunkan disabilitias neurologis. Pada penggunaan asam folat juga cenderung terbukti efektif untuk mencegah stroke, namun biasanya dikombinasi dengan vitamin B12 atau vitamin B6. Selain itu, data terkait fungsi asam folat dalam menurunkan disabilitas neurologis pasca stroke atau sebagai pengobatan jangka panjang masih sangat terbatas.

Abstract

Painful neuropathic pain is a challenging chronic pain to treat. It is heterogeneous in symptoms and could be resistant to the available treatments regimen.  Current pharmacological treatments fail to achieve adequate pain relief in a most patients. The previous review showed that only less than 50% of patients can achieve good pain reduction with standard adjuvant treatment. The available adjuvants analgesic only focus in the symptom control, and do not interfere with the progressing damage of the nerve. Vit D insufficiency is quite frequent in type 2 diabetes patients.diabetes, particularly those with symptoms of DPN. The studies also showed that low serum vitamin D levels are an independent predictor of DPN development. Vitamin D supplementation is necessary for diabetic neuropathy patients since it promotes the synthesis of neurotrophins and neurotransmitters. Additional vitamin D therapy have big role in nerve growth factor and the regulation of neurotrophin and Ca2+ homeostasis in neurons, and provides protection for neurons in the peripheral nervous system.  In this review, we do systematically search the studies about Vitamin D for the treatment of painful diabetic neuropathic condition. We used PubMed, Cochrane, Clinical Key, and search Google Scholar for papers that used vitamin D phrases. and painful diabetic neuropathy as our major database for this  review and  we make a systematic table to explain our review. However, there is still an unmet need in the management of neuropathic pain. The unmet needs maybe caused by the gap between pharmacological treatmnet in pain reduction in painful diabetic neuropathy patients. Therefore, in this review we discuss about the potential use of vitamin D as an add-on therapy to closing the gap in the management of neuropathic pain.

Abstract

Background: Due to the critical importance of the COVID-19 pandemic in the educational system, medical academics must consider safe and appropriate clinical clerkship learning methodologies and models implemented during the pandemic.Aims: This study aimed to construct and evaluate virtual neurology examination as a tool for medical students undergoing clinical clerkships.Learning Media Review: This learning media review was action research conducted at Bethesda Hospital Yogyakarta, Indonesia. This study was conducted in two stages: developing a virtual neurology examination and evaluating student feedback. This study enrolled 12 medical students, the majority of whom were female (58%). The majority of medical students agreed that the virtual neurology assessment was a good overall experience (83%), with the majority stating that the image and sound quality were good (75%) and the virtual neurology examination cases were quite varied (66.5%). Nine medical students (75%) strongly supported the continuation of the virtual neurology examination program, while three medical students (25%) supported its continuation.Conclusion: This study indicates that virtual neurology examination is feasible and can serve as a helpful learning tool for medical students undergoing clinical clerkships during the COVID-19 pandemic.

Abstract

Nocturnal Leg Cramps (NLC) are involuntary lower limb contractions that are painful and occur during long rest periods. Magnesium is thought to have potential in the treatment of NLC as one of the precipitating factors of NLC is low levels of certain minerals, such as magnesium deficiency. This study aimed to assess the effectiveness of magnesium adjunctive therapy in reducing cramping pain intensity in patients with NLC. This study was a randomized clinical trial, open-label, controlled group that was followed up for 2 weeks. 30 subjects who have been diagnosed with the NLC short-form adaptation of ICSD 2005 were divided into 2 groups; (1) the intervention group who was given standard NLC therapy (calcium and gabapentin) with additional therapy of magnesium citrate 100 mg (Hi-Mg100) one tablet a day, (2) the control group who was only given standard NLC therapy. NLC cramping pain was measured using the Numeric Rating Scale (NRS) before the administration of therapy (baseline) and at week 2 after therapy. The results obtained were the addition of magnesium to standard therapy provided a significant reduction in cramping pain intensity between before and after treatment based on the Wilcoxon signed rank test (p=0.000). However, there was no statistically significant difference effect between the two therapy groups based on the Mann-Whitney test (p=0.073). In conclusion, magnesium adjunctive therapy was not significantly more effective in reducing cramping pain than standard drug therapy in patients with NLC.

Abstract

Osteoarthritis  is a form of arthritis that commonly occurs in the hands, hips and knees and affects the patient's level of pain. Osteoarthritis treatment currently uses conservative methods with NSAID therapy to reduce pain. However, long-term use has an impact on the digestive tract. Currently, traditional medicine is in great demand, such as the gods crown extract. The content of the gods crown extract can provide an alternative analgesic without side effects on the digestive tract. The purpose of this study was to measure the effectiveness of gods crown add-on therapy at the pain level of osteoarthritis patients at Bethesda Hospital, Yogyakarta. This study uses secondary data from the randomized clinical trial research design that has been conducted. The study was conducted within 2 weeks with 30 osteoarthritis patients as subjects at Bethesda Hospital, Yogyakarta. The instruments used were the Numerical Rating Scale and Subjective Global Assessment questionnaires. The results showed that there was a significant difference in the average pain level before and after therapy, both in the additional therapy group of Mahkota Dewa extract (p=0.001) and in the standard therapy group (Ibuprofen) (p=0.001). In addition, there was a significant difference in the average pain level where the additional crown of gods group had a larger average difference than the standard therapy (Ibuprofen) (p=0.013). This shows that the additional therapy of Mahkota Dewa extract is effective in reducing the level of pain which is greater in osteoarthritis patients at Bethesda Hospital Yogyakarta compared to the standard Ibuprofen therapy group.

Abstract

The pharmacokinetic profile of valproic acid in pediatric subjects in Asian populations is lacking. One of the most useful ways to develop a personalized dosing strategy is to use population pharmacokinetics. The focus of this systematic review is to describe the population pharmacokinetic profile of valproic acid (VPA) and determine the variables influencing pharmacokinetic models of the drug in children with epilepsy that have been published in Asian journals. A comprehensive literature search was carried out from 2014 to 2024 on Science Direct and PubMed.  Keywords used valproic acid, valproic, VPA, sodium valproate  and population pharmacokinetic, pharmacokinetic model. In this review, four studies were considered. The pharmacokinetic model was developed by looking at several covariates that significantly affect the population pharmacokinetics of VPA using TDM data from pediatric epilepsy patients. The clearance value and volume of distribution of VPA can be seen to be in the range of 0.148 - 3.19 L/h, and 0.317 - 24 L/KgBB. In addition, subjects with SCN1A A/A genotype showed a decreased therapeutic effect of VPA. Significant covariates affecting the pharmacokinetics of VPA were age, dose, and the use of comedication with other anti-seizure drugs.
