Abstract

Anticoagulants are prescribed very frequently, but their use is often associated with adverse drug reactions. The optimal monitoring of heparin is unknown, either by aPTT or anti-Xa levels. This paper compares which anti-Xa or aPTT most closely correlates with patients on intravenous heparin. This review also compares the achievement of target goals of unfractionated heparin (UFH) using aPTT and anti-Xa tests. Literature searching was started in June 2024 and ended in July 2024 through some online scientific databases: Scopus, PubMed, Google Scholar, Science Direct, and Willey Online Library. This review includes fifteen studies, at least involving 2,938 patients or 94,038 sets of measurements. The review demonstrates that anti-Xa outperforms aPTT in achieving the therapeutic range of unfractionated heparin, time to therapeutic goal, and dose modification.

Abstract

Background: Stroke is a clinical manifestation of impaired brain function, both focally and globally, which can be severe and last for 24 hours. Damage to brain cells can cause cognitive, sensory and motoric dysfunction and hinder functional abilities from daily activities to communication caused by brain cells in stroke patients. Aims this study to determine the relationship between cognitive function and stroke severity at Bethesda Hospital, Yogyakarta. Methods: This study used a retrospective cohort method in 118 ischemic stroke patients with mild to moderate levels of cognitive function within 1-3 months after the first attack using secondary stroke registry. The patient's cognitive function was measured by the MMSE when the patient was discharged from hospital and stroke severity was measured by the NIHSS when the patient was first admitted to the hospital. Results: Of the 118 patients, the majority of patients in this study were male (59.3%), aged ≥ 60 years (57.6%). In this study, no significant relationship was found between cognitive function and stroke severity (p = 0.106). Gender, age, history of hypertension, history of diabetes mellitus, history of dyslipidemia, and the patient's education level did not find a significant association with cognitive function as assessed by the MMSE score and the severity of stroke as assessed by the NIHSS score. Conclusion: There was no significant relationship between cognitive function as assessed by the MMSE score and stroke severity as assessed by the NIHSS score. Abstract

Background: Drug-induced gingival overgrowth has been reported as adverse effect of certain medication, including amlodipine, in susceptible individuals. The exact mechanism of gingival hypertrophy remains unclear and may be related with multifactorial factors. Gingival hypertrophy related with speech and mastication problem, poor oral hygiene and aesthetic appearance. Case Presentation: We report the case of a 64-year-old male with post stroke and hypertension who developed gum hypertrophy following amlodipine use for 3 years. Maintenance of oral hygiene and substitution of offending medication was performed as the first step in management. Conclusion: A case of DIGO associated with amlodipine on post stroke patient with hypertension that causing difficulties in mastication and diffuse gingival enlargement. Abstract

Introduction: The high incidence of death in patients with COVID-19 requires a risk stratification tool to assess the patient's condition and determine appropriate medical action. The 4C mortality score is a stratification tool that uses patient demographics, clinical observations and laboratory parameters to describe the patient's risk of death. Method: This study used a nested case-control method with secondary data on patients with confirmed COVID-19. The sample with 71 confirmed COVID-19 patients was categorized into groups of patients who died (cases) and patients who recovered (controls). Data analysis using the independent T test and bivariate using the chi-square method to see the relationship between the 4C mortality score and the incidence of patient death.Results: It was found that the 4C mortality score in the group of patients who died was higher compared to patients who recovered (mean = 8.20 vs 4.05). A significant relationship was found between the 4C mortality score and the incidence of death in COVID-19 patients (OR = 22.696; 95% CI = 2.82-182.9; p = 0.0034)Conclusion: It can be concluded that the 4C mortality score of medium and high COVID-19 patients is associated with the risk of patient death.

Abstract

Introduction: The COVID-19 pandemic is a disease outbreak caused by SARS-CoV-2; as of July 20, 2021, there were 2,950,058 positive cases and 76,200 deaths in Indonesia. COVID-19 patients with comorbidities are mostly hypertension (50%), diabetes mellitus (37.2%), and heart disease (16.7%). This study aims to prove how comorbidities affect the mortality of COVID-19 patients at Pertamina Jaya Hospital, Central Jakarta.Method: This study is a retrospective cohort analysis. The sample comprised 500 patients, 250 comorbid groups, and 250 without comorbidities. The secondary data source is medical records of COVID-19 patients at Pertamina Jaya Hospital from April 2020 to September 2021. Data analysis using univariate, bivariate, and multivariate.Result: Multivariate analysis with logistic regression resulted in a value of (32.197 95% CI 9.714 - 106.718) so that there is an influence on the risk of death with a comorbid history in COVID-19 patients. The relative risk value proves that hypertension has a 3-fold risk of death (3.124 95% CI 1.441 - 6.773), diabetes mellitus is 2.5 times (2.521 95% CI 1.055 - 6.024), while heart disease is 2.3 times (2.325 95% CI 1.169 - 4.622).Conclusion: Hypertension, diabetes mellitus, and heart disease are comorbidities that are predictors of mortality for COVID-19 patients due to a higher risk of death.

Abstract

Background: Stroke is a major contributor to deaths and disabilities worldwide. Impaired cognitive function is often found in ischemic stroke patients. Disability due to stroke causes functional impairment in various quality of life domains. Cognitive function is one of the predictors of the quality of life. There are still not many studies on cognitive impairment and quality of life in ischaemic stroke patients in Indonesia. Objective: To compare the average quality of life of 6-12 months of stroke patients at the Bethesda Hospital Yogyakarta Neurology Center based on the presence of cognitive impairment Methods: The study used a retrospective cohort study design. Subject data were obtained from the Bethesda Hospital Yogyakarta stroke registry. All risk factor data (laboratory and clinical) and medication were obtained during admission. Cognitive impairment was measured using the Mini Mental State Examination (MMSE) and Clock Drawing Test (CDT). The questionnaire used was World Health Organization Quality Of Life BREF (WHOQOL-BREF). Results: From 113 patients, average quality of life of patients in domain 1, domain 2, and domain 3 was not significant (p > 0.05). A significant mean difference between cognitive impairment and quality of life was found in domain 4 (p = 0.025). Analysis of the confounding variables showed that a history of hypertension, National Institute of Health Stroke Scale (NIHSS) score, diabetes, and atrial fibrillation also had an effect on the patient's quality of life. Conclusion: Cognitive impairment does not have a significant effect on quality of life, especially in the domains of physical health, psychological health, and social relations, but has a significant effect on the environmental health domain of ischemic stroke patients.

Abstract

CORRELATION ANALYSIS OF TOTAL SYMPTOM SCORE, PAIN, AND QUALITY OF LIFE POST HIGH DOSE VITAMIN B1, B6, DAN B12 TREATMENT IN PERIPHERAL NEUROPATHYABSTRACTIntroduction: Peripheral neuropathy (PN) is a clinical condition in which nerves of the peripheral nervous system are damaged and is associated with various symptoms affecting the patients’ quality of life (QoL).Aims: To understand the  effect of Vitamin B1, B6, and B12 combination in mild to moderate PN.Methods: This is a prospective, open label, multicenter, single arm observational study involved 399 subjects with PN of different etiology. Subjects received the vitamin B1, B6, and B12 combination tablet orally once daily and were observed for 3 months. Total symptom score (TSS), visual analog score (VAS) and QoL were assessed, and the correlation between these parameters was analyzed.Results: Clinically significant reductions were observed from baseline to subsequent visits for TSS and VAS. Positive correlation  was observed between TSS and components of VAS. The study treatment was associated with a significant improvement in QoL parameters. Inverse correlation was observed between QoL and TSS as well as QoL and components of VAS. The study treatment was found to be well tolerated.Discussion: The correlation  analysis between different outcome measures demonstrated the beneficial effect of combination of vitamin B1, B6, and B12 in relief from symptoms and improvement in QoL of PN.Keyword: Correlation analysis, peripheral neuropathy, SF-8, TSS, VAS, vitamin B1, B6, and B12ABSTRAKPendahuluan: Neuropati perifer (NP) merupakan kondisi klinis akibat kerusakan pada sistem saraf tepi yang memengaruhi kualitas hidup (quality of life/QoL) pasien.Tujuan: Mengetahui efek pemberian kombinasi vitamin B1, B6, and B12 pada NP ringan hingga sedang.Metode: Penelitian observasional dan prospektif secara open label, multisenter, dan single arm, yang melibatkan 399 subjek penderita neuropati perifer dengan etiologi yang berbeda-beda. Subjek mengonsumsi tablet kombinasi vitamin B1, B6, and B12 secara oral satu kali sehari dan diamati selama 3 bulan. Dilakukan penilaian skor gejala total (total symptom score/TSS), visual analog scale (VAS), dan QoL, serta korelasi antara parameter-parameter tersebut.Hasil: Terdapat penurunan TSS dan VAS yang bermakna secara klinis antara baseline (awal) dengan kunjungan berikutnya. Terdapat korelasi yang positif antara TSS dengan komponen-komponen VAS. Pemberian perlakuan dalam penelitian berkorelasi secara bermakna dengan perbaikan parameter dalam QoL. Teramati juga adanya hubungan terbalik antara QoL dengan TSS serta QoL dan komponen VAS. Pengobatan pada penelitian ini juga terbukti dapat ditoleransi dengan baik.Diskusi: Analisis korelasi antara berbagai macam metode pengukuran yang berbeda menunjukkan manfaat dari pemberian kombinasi vitamin B1, B6, and B12 dalam mengurangi gejala dan perbaikan QoL pada pasien PN.Kata kunci: Analisis korelasi, neuropati perifer, kualitas hidup, SF-8, TSS, VAS, vitamin B1, B6, and B12

Abstract

Ramsay Hunt Syndrome is a rare case.  This syndrome is a severe complication of the reactivation of varicella zoster virus in the geniculate ganglion. Facial paralysis is the main clinical presentation.  We present a rare case Ramsay Hunt Syndrome in a 48 year old female that referred to neurology clinic with very painful herpetic neuralgia, left ear discomfort, and facial palsy. The patient came after two weeks eruption of herpetic rash in the second and third branch of trigeminal nerve. Treatment as antiviral, steroid, gabapentin, and amitriptyline showed improvement and significantly reduced the pain in this patient.

Abstract

Chronic subdural hematoma is an old collection of blood between the arachnoid and dura mater that covers the brain surface. This condition is a common neurological emergency and commonly caused by craniocerebral trauma. Chronic subdural hematoma caused by leukaemia was rarely reported, and its pathogenesis and strategies for clinical treatment remain controversial. We report a rare case of chronic subdural hematoma caused by chronic myelocytic leukaemia (CML) in blast crisis. The patient was diagnosed as CML 8 months ago after ischemic stroke. The bone marrow cytology, flow cytology, and haematology consultation suggested chronic myelocytic leukaemia. The patient admitted again with headache and right side weakness. The routine blood examination showed blast crisis. The emergency neurosurgical consultation was performed, but the patient's condition deteriorated quickly after admission. The platelet levels continued to decrease after admission. In this case, early diagnosis and multidisciplinary treatment are important to minimalized the outcome and poor prognosis.

Abstract

Highlight: Tremors are the most common motor symptom in patients with Parkinson's disease. Abdominal discomfort is a prevalent non-motor symptom of Parkinson's disease. Comprehensive symptom management is essential for patients with Parkinson's disease.   ABSTRACT Introduction: Parkinson's disease is a neurodegenerative disorder characterized by motor and non-motor symptoms. The prolonged administration of levodopa in patients may induce the wearing-off phenomenon, where symptoms recur before the next dose. Studies on the symptom profile of Parkinson's disease, especially in wearing-off conditions, are crucial for further management. Objective: This study aimed to determine the motor and non-motor symptom profile of patients with Parkinson's disease. Methods: conducted. The Indonesian version of the Wearing-Off Questionnaire-19 (WOQ-19) was used to identify motor and non-motor symptoms. Data were collected from 30 Parkinson’s patients at Bethesda Yogyakarta Hospital from October to December 2023 using a purposive sampling technique. The sample size was calculated using OpenEpi V3.01. Results: The most prevalent motor symptom among patients with Parkinson's disease was tremor (100%), followed by bradykinesia (93.33%) and rigidity (86.67%). The most common non-motor symptoms reported were abdominal discomfort (56.67%) and body aches (50%). Conclusion:Patients have a variety of Parkinson's disease symptoms. Parkinson's patients most commonly experienced motor symptoms such as tremors, bradykinesia, and rigidity, along with non-motor symptoms like abdominal discomfort and body aches. The significant percentage of wearing-off seen in this study underscores its common occurrence and highlights the importance of early identification and management.
