Abstract

The aim of implementing Community Service is the implementation of the Family Centered Care Model in the Children's Inpatient Room using invasive injections and infusions at Dr Soetomo Hospital, Surabaya. This community service was carried out at the Children's Inpatient Installation at Dr. RSUD. Soetomo Surabaya consists of 4 rooms. The rooms are the sleeping room, Nakula Sadewa room, Bona 1 room, and Bona 2 room with a total of 92 nurses. The four rooms are children's inpatient rooms that provide low-care nursing care so services with an integrated family-oriented care (FCC) system have not yet been established. The method used in this community service is to involve the family, each nurse will carry out invasive procedures on children with stages of preparation, implementation, and reporting. Implementation results Almost all patient respondents received family-centered care in the good category, 85 people (92.4%). Meanwhile, there were only 7 patients who underwent family-centered care in the sufficient category (7.6%). This shows that patients and nurses at IRNA Children's Hospital Dr. Soetomo Surabaya have implemented a family-centered care system in nursing care and health services.

Abstract

Toddlers’ caries prevalence is influenced by some factors such as social status, economic status, parents' educational background, and dietary pattern. Caries in toddlers have a negative impact on their quality of life. The objectives of this research are to analyze the differences in Oral Health Related Quality of Life (OHRQoL) by gender. Analytical observational research design with a cross-sectional approach. The research population is toddlers with their parents in the Silo II Health Centre work area. The sample is 3-5 years toddlers as many as 71 respondents. The variables studied were Oral Health Related Quality of Life (OHRQoL) of male and female toddlers with the Ecohis index measuring instrument. The research data were tabulated and analyzed using the Mann-Whitney test. The majority of respondents were female toddlers (57.7%), the age range of 60-71 months (57.7%), parents’ last education is elementary school (49.3%), with an income of less than 2 million per month (59.1%). There are no differences in Oral Health Related Quality of Life of male and female toddlers with a p-value of 0.404. There are no differences in Oral Health Related Quality of Life between female and male toddlers

Abstract

Background Microcytic hypochromic anemia is the most common type of anemia found in Beta Thalassemia Trait and Iron Deficiency Anemia, posing a diagnostic challenge due to their similar presentations. Diagnostic errors among them can lead to incorrect treatment, potentially resulting in fatal outcomes. In Indonesia HPLC (High Performance Liquid Chromatography) usage as gold standard for discrimination between these diseases are expensive. Discrimination indices offer an alternative for cheaper and effective initial screening. However, a comprehensive performance evaluation of those indices, such as Matos and Carvalho indices, alongside Mentzer index, Green and King index, England and Fraser index, RBC index, Shine and Lal index, and Srivastava index, has not been conducted in Indonesia. This study aims to determine the best discriminative performance index based on the highest Youden’s indes value between those seven indices. Methods Study consisted of 30 subjects of beta thalassemia trait and 35 subjects of iron deficiency anemia. Index calculations were performed using blood profile formulas and compared with gold standard test results to find each index sensitivity, specificity, and Youden’s index value. Results Matos and Carvalho indices exhibited superior discriminatory performance, achieving 80% sensitivity, 77.1% specificity, and a Youden's index of 57.14%. Among other indices, the RBC index demonstrated the highest sensitivity (90%), while the Green and King index excelled in specificity (97.14%). MCV and MCH values did not significantly differ between BTT and IDA groups. Conclusions The study's findings underscore the efficacy of Matos and Carvalho indices in discriminating BTT and IDA in this study population, highlighting their potential as valuable tools in initial screening efforts.

Abstract

The implementation of Family Centered Care requires collaboration between nurses and patient parents with a holistic approach and philosophy in preventing hospitalization. However, nurses in pediatric inpatient settings have high work responsibilities due to their complex role in the care process which must focus on pediatric patients and parents, which causes nurses to have an increasingly heavy workload. This study aims to analyze the implementation of Family Centered Care on nurses' workload. Method: This study used a cross-section design with a sample of 92 patients and pediatric nurses at RSUD Dr. Soetomo. This research used a purposive sampling technique for pediatric nurses and a simple random sampling technique for families of patients in the pediatric ward. Research data was collected through questionnaires from the variables of implementing family center care and nurse workload. Data on the variables of FCC implementation and workload were analyzed using a description test and the relationship between FCC implementation and workload was tested using a logistic regression test. Research data was processed using the description test and logistic regression test with a p-value ≤ 0.05. Results: The research results show that the implementation of family center care has a relationship with workload (0.001). Conclusion: The implementation of family-centered care can form a collaboration with patients and patient families thereby reducing the workload of pediatric nurses in providing nursing care.

Abstract

The implementation of bedside handover can make patients actively involved in handover activities, so that there is effective communication practice by nurses to patients. The development of bedside handover is considered to increase patient satisfaction. Objective: Based on this phenomenon, this research was conducted to find out whether the implementation of bedside handover is related to patient satisfaction at the Graha Amerta Installation at RSUD dr. Soetomo. This research uses a quantitative research design with a correlation research design and a cross-sectional approach. This research was conducted at 4th and 5th Floor of Graha Amerta RSUD Dr. Soetomo Surabaya in January – April 2024 with a sample of 40 patients determined through consecutive sampling. Based on the research results, the Spearman Rho statistical test results obtained a value of p = 0.022 or p < 0.05, which means there is a relationship between the implementation of the SBAR bedside handover method and nurse satisfaction at Graha Amerta Dr Soetomo Hospital Surabaya. The results of the Spearman Rho statistical test obtained a correlation coefficient value of 0.361. The higher the level of implementation of the SBAR bedside handover method, the higher the patient satisfaction at Graha Amerta RSUD Dr Soetomo Surabaya.

Abstract

Technological developments have an impact on developments in the world of health, especially information technology in hospitals. The use of EMR makes medical records more orderly and improves health services that are faster, more precise, valuable and accountable. User usage and satisfaction will influence individual impact. Methods: An analysis of the relationship between EMR documentation and nurse satisfaction is needed. This study used a cross-sectional, The sampling technique in this research used purposive sampling correlational design with a sample of 80 patient at RSUD Dr. Soetomo. Research data was collected through questionnaires from the EMR documentation performance variables and nurse satisfaction. The results of the validity test of the EMR Documentation Performance instrument obtained a Parson correlation value in the range 0.590-0.926 > r table 0.514 and the results of the reliability test in this research showed a Cronbach's Alpha value of 0.966 so it is reliable. results of the validity and reliability of the patient satisfaction instrument Cronbach α reliability coefficient was high, ranging from 0.900 to 0.940. Research data was processed using the Spearmen rho test with a p value <0.05. Results: The results of the study showed that EMR documentation performance was related to patient satisfaction (0.009). Discussion: Good electronic medical record documentation performance can increase patient satisfaction.

Abstract

The quality of nursing services for hemodialysis patients is determined by the role and performance of nurses based on spiritual intelligence. Nurses use spiritual intelligence when providing nursing care to hemodialysis patients so that patients feel satisfaction from actions that have an impact on the quality of service. Therefore, this study aims to analyze the relationship between nurses' spiritual intelligence and service satisfaction in the hemodialysis room. This study used a cross-section design with a sample of 35 nurses and 153 hemodialysis patients at RSUD Dr. Soetomo. This research used a total technique for nurses and a simple random sampling technique for patients in hemodialysis installations. Research data was collected through a questionnaire from the spiritual intelligence variable with a valid test of 0.460-0.906 and a reliable test of 0.361 as well as a patient satisfaction questionnaire related to service quality with a valid test of 0.870-0.982 and a reliable test of 0.785. Research data was processed using a logistic regression test with a p-value ≤ 0.05. The results of the logistic regression test show that the p-value is 0.001, so it can be interpreted that H0 is rejected. This shows that there is a relationship between nurses' spiritual intelligence and the quality of hemodialysis services. Spiritual intelligence will create optimal nurse performance in taking action. The performance of nurses in hemodialysis tasks is quite complex with the number of patients being unbalanced with the number of nurses which influences the quality of nursing services.

Abstract

Low motivation can affect the quality of nurses' performance, especially in carrying out the duties and roles of nurses in hospitals. One of the hospital personnel who is vulnerable to decreased motivation is nurses. Therefore, support is needed to increase motivation in nurses so that they can improve nurse performance, especially in providing health education to patients in hospitals. Objective: To analyze factors that influence nurse motivation based on self-determinant theory towards the implementation of health education to pre-operative patients at Dr. Soetomo Hospital, Surabaya. Methods: This study used a correlation analytic design with a cross-sectional approach. The sample in the study was 115 nurses taken using a simple random sampling technique. The instruments used were intrinsic and extrinsic motivation questionnaires, as well as a questionnaire on the implementation of health education. Data were analyzed using a logistic regression test. Results: There is a significant relationship between extrinsic factors including: rewards, awards, punishments, internal rules, external rules towards nurse motivation in providing health education to pre-operative patients with a significant p-value of the extrinsic motivation variable of 0.003. Conclusions: Nurses' motivation in providing health education to pre-operative patients is influenced by various intrinsic and extrinsic factors, therefore hospitals should provide interventions that can increase nurses' motivation so that health education to patients can be carried out properly.

Abstract

Factors that can affect the quality of sleep in pre-school-aged children are hospitalization anxiety, when experiencing anxiety, sympathetic nerves can increase so that it can interfere with the sleep process. The aim of this study was to analyze the relationship between hospitalization anxiety and sleep quality in pre-school aged children (3-6 years) at Mutiara Hati Gedeg Hospital, Mojokerto. The research method uses a correlation analytic design with a cross sectional study. The population of this research is pre-school age children as many as 40 respondents. The research sampling technique uses purposive sampling with the Lemeshow formula. The research was conducted on March 31, 2023 in the Children's Room of Mutiara Hati Hospital. The research instrument used the DASS questionnaire for anxiety and the PSQI questionnaire for sleep quality which had been modified by researchers with validity and reliability test results of 0.692 for hospitalization anxiety and 0.561 for sleep quality and Cronbach's Alpha 0.920 for hospitalization anxiety and 0.967 for sleep quality. The results of this study indicate that there is the most anxiety about hospitalization, namely moderate anxiety for 16 respondents (40%) and most pre-school age children experience moderate sleep quality as many as 18 respondents (45%). Spearman's rho statistical test results p value 0.002 <α 0.05, in the sense that there is a relationship between hospitalization anxiety and sleep quality. The benefits of accompanying parents, close family and siblings are that children are protected, feel comfortable and safe by providing positive support to children.

Abstract

Blood disorders occur in one or several parts of the blood that affect the nature and function, and blood disorders can be acute or chronic. Blood disease consists of several types, such as anemia. Anemia is the most common hematologic disorder associated with a decrease in the number of red blood cells or hemoglobin, causing a decrease in the ability of the blood to carry oxygen throughout the body. Patients with anemia in Indonesia have increased for the age of 15-24 years. This study aimed to conduct a screening test for anemia using machine learning. It is expected to know the process of knowing the type of anemia suffered. The machine learning technique used to identify the cause of anemia is divided into four classes, namely Beta Thalassemia Trait, Iron Deficiency Anemia, Hemoglobin E, and Combination (Beta Thalassemia Trait and Iron Deficiency Anemia or Hemoglobin E and Iron Deficiency Anemia). This study would apply the K-Nearest Neighbor (KNN) and Random Forest (RF) methods to build a model on the data collected. The evaluation results using a confusion matrix in the form of accuracy, precision, recall, and f1-score against the KNN and RF methods are 79.36%, 59.40%, 62.80%, and 62.80%. In comparison, the RF is 87.30%, 90.89%, 78.40%, and 81.00%. From the results of comparing the two methods, the Graphic User Interface (GUI) implementation using python applies the RF method. The classifier that gets the highest value among all these parameters is called the best machine learning algorithm to perform screening tests for anemia.
