Abstract

This study aimed to analyze the association between age, sex, occupation, smoking status, and body mass index (BMI) with the incidence of lumbar disc herniation (LDH) among patients with low back pain. This analytical observational study used a cross-sectional design with medical record data from 69 patients attending the Orthopedic and Traumatology Clinic at Dr. M. Djamil General Hospital, Padang, from 2020 to 2021. The results showed that 63.8% of patients were diagnosed with LDH. Smoking status (p=0.043; OR=3.21) and BMI (p=0.035; OR=3.39) were significantly associated with LDH, while age, sex, and occupation showed no significant relationship. These findings indicate that smoking and elevated BMI are important modifiable risk factors for LDH, highlighting their role in prevention strategies.

Abstract

Laryngopharyngeal reflux (LPR) is a condition caused by the movement of stomach acid back into the pharynx, larynx and upper digestive tract. One of the diagnostic tools that can be used in LPR is the Reflux Symptom Index (RSI), where RSI can also be used to evaluate symptom improvement in patients before and after administering drug therapy. This study aims to determine the average RSI score before and after giving therapy to LPR patients at Dr M Djamil Hospital, Padang. This research is a retrospective analytical study by taking medical record data on LPR patients at Dr M Djamil Hospital Padang using consecutive sampling techniques. Data analysis was carried out univariately to determine the frequency distribution and bivariately using the paired T-test to analyze the average RSI score. The results of this study were that the majority of LPR sufferers were >50 years old (52.9%), the majority gender was female (76.2%), and the most common complaints were a lump in the throat (100%) and heartburn (100%). The average RSI score before giving therapy was found to be 22.12 ± 6.827 and after giving therapy was 13.18 ± 7.844. Based on the data analysis carried out, the p<0.001. The conclusion of this study was that there was a significant change in the average RSI value before and after administration of lansoprazole and ranitidine therapy in LPR patients.

Abstract

Bitter melon (Momordica charantia) has been empirically proven for antimicrobial properties that can be used as an alternative medicine, while the changes that occur in Escherichia coli-infected mouse intestine after administering bitter melon have not been widely studied. The aim of this study was to determine the effect of Momordica charantia administration on histopathology of intestine of E. coli infected mice. This research was post-test control randomized group study involving 20 mice’s small intestine paraffin blocks which were divided into 4 groups consisted of control and different dosage of bitter melon juice, observed subsequently under a microscope to measure the of the small intestine lining and assess inflammation using modified Barthel Mandja scoring. This study found significant changes the height of mucosa and villi of mice small intestine after administration of 100 grams of bitter melon juice and the muscle layer and crypts after administration of 200 grams of bitter melon juice. The Barthel Manja score showed decrease of inflammation degree among P2 group, while increased inflammation degree found in P3 group. It can be concluded that administration of bitter melon juice has the potential to improve the intestinal histopathology of mice infected with Escherichia coli.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a widespread global health problem, defined by increasing insulin resistance and gradual deterioration of β-cell function. Vitamin D is posited to influence glucose metabolism, although the evidence about its impact on insulin dynamics is inconsistent. Objective: This study evaluated the effect of vitamin D supplementation on insulin levels in a streptozotocin (STZ)-induced diabetic rat model. Methods: Twenty-five Sprague Dawley rats were randomly assigned to five groups: A (healthy control), B (healthy + vitamin D 415 IU/day), C (T2DM control), D (T2DM + 415 IU/day), and E (T2DM + 1,100 IU/day). Diabetes was induced using a high-fat diet followed by STZ injection. Vitamin D supplementation was given orally for 30 days. Serum insulin and vitamin D were measured by ELISA, and group differences were analyzed using one-way ANOVA with Bonferroni post hoc testing. Results: Insulin concentrations differed significantly across groups (p = 0.022). Healthy rats receiving vitamin D (Group B) exhibited higher insulin levels compared with both diabetic controls (p = 0.004) and diabetic rats supplemented with the same dose (p = 0.005). Higher-dose supplementation in diabetic rats (Group E) showed only a nonsignificant trend toward increased insulin (p = 0.224). No significant correlation was observed between serum vitamin D and insulin levels (r = 0.171, p = 0.415). Conclusion: Vitamin D supplementation markedly enhanced insulin secretion in healthy rats but did not improve insulin output in diabetic rats. These findings suggest that vitamin D’s stimulatory effects on insulin may be more pronounced in healthy or early metabolic conditions and less effective in established diabetes.
