Abstract

The quality of nursing services is a value for health services in society. Patients are consumers who always hope to receive good health services. Many incidents may occur if the nurse's workload is too high. To overcome the danger of patients falling, it is necessary to take action to reduce the risk of patients falling. So nurses will always try to prevent patients from falling by installing identification bracelets, triangular labels on patient mattresses, warning signs, adjusting mattress height, and mattress railings. Precautions must also be in accordance with SOP. Nurse workload is the allocation of working time in providing services. The high workload of nurses occurs due to the limited number of staff and uncertain job descriptions. This outreach activity will provide knowledge and courage to communicate about workload so that we can prevent the risk of patient falls. PkM activities were socialized using lecture, question and answer and discussion methods to PkM participants, namely 15 nursing staff. To measure changes in PkM participants' knowledge, pre and post-tests were given to participants. The PkM results show that PkM participants have experienced increased knowledge in preventing the risk of patient falls. Participants also have the courage to discuss with the leadership if the workload is too heavy so that they can get the best solution. Participants also become more diligent in processing their daily work.

Abstract

Oral health is an important aspect to pay attention to because it is part of the digestive system. Disorders in oral health function can impact other bodily health. The use of mouthwash is necessary to improve the cleanliness and health of the mouth, especially when brushing alone is not optimal. The active compounds in mouthwash enhance cleanliness and effectively inhibit the growth of pathogenic oral microflora. Cinnamon leaves have not been widely utilized and are often left unused or discarded. Previous research has been conducted on the potential of the compounds in cinnamon leaves. This research was carried out by determining the best essential oil content of cinnamon leaves through several preliminary treatments and varying distillation times. Active compounds such as cinnamaldehyde, L-linalool, and cinnamyl acetate were found in the cinnamon leaves, making them a potential ingredient for mouthwash formulation. The aim of this study is to produce an effective mouthwash that reduces the growth of pathogenic oral microflora using underutilized local materials. The mouthwash preparation method was modified from several active ingredient mouthwash preparation methods by varying the concentration of cinnamon leaf essential oil at 5%, 10%, and 15%. The formulations were then evaluated for organoleptic properties, stability, homogeneity, and antibacterial activity against S. mutans using the disc diffusion method. The results showed that the preparations remained physically stable and homogeneous during 30 days of storage at room temperature. The cinnamon leaf mouthwash exhibited antibacterial activity against S. mutans, with average inhibition zones of 15.25 mm, 19.06 mm, and 24.34 mm for formulations I, II, and III, respectively. Based on these results, it can be concluded that the cinnamon leaf mouthwash has the potential to inhibit the growth of S. mutans bacteria."

Abstract

Cancer is one of the diseases that is the main cause of death in many parts of the country. In treating this disease, it can be done through chemotherapy, which can destroy cancer cells. However, there are side effects that can be felt by patients, namely nausea and vomiting. These symptoms can interfere with the patient's quality of life so that they become less compliant in following the next therapy. To overcome this nausea and vomiting condition, a type of non-pharmacological therapy can be tried, such as giving ginger aromatherapy which is known to contain active compounds such as gingerol and shogaol which can relieve these symptoms. Quantitative research with a Quasy Experiment design with a One Group Pretest Posttest approach was chosen in this research. A sample of 23 people was determined using the purposive sampling method. Analysis was carried out on the characteristics of respondents and the effect of ginger aromatherapy on nausea and vomiting was tested using a paired sample t-test. The results showed a change in the mean value from 4.52 to 2.48 or a decrease of 2.04, with a t value of 4.97, and a p value of 0.000 which can be interpreted that ginger aromatherapy can reduce the level of nausea and vomiting in cancer patients after aromatherapy. Ginger aromatherapy has been proven to reduce nausea and vomiting levels, making it suitable for recommendation for cancer patients after chemotherapy, so it can provide comfort and improve the patient's quality of life.

Abstract

Stroke is a type of catastrophic illness characterized by high incidence and disability rates. It significantly impairs physical function and increases the risk of developing pressure ulcers. Pressure ulcers, or decubitus ulcers, occur due to prolonged pressure, friction, and moisture exposure, leading to skin tissue damage in areas of the body that bear weight during periods of immobility. Family support plays a crucial role in the rehabilitation of stroke patients, particularly in providing basic care, assisting with mobility, and preventing complications. This community engagement program (PkM) aimed to enhance family capacity in managing pressure ulcers through educational and practical interventions. Family involvement was facilitated through structured education and hands-on training to encourage active participation in supporting comprehensive patient recovery. The PkM activities were delivered using an educational and participatory approach to 25 participants, all of whom were family members of stroke patients currently hospitalized at Grandmed Hospital, Lubuk Pakam. The program implementation consisted of four stages: preparation, sample selection, execution, and evaluation. The results showed a significant improvement in participants’ knowledge, with an average increase of 32.6 points (59.1%) regarding the role of families in pressure ulcer care. Additionally, 92% of participants demonstrated competency in performing basic stroke care, and 96% expressed high satisfaction with the program. This PkM initiative effectively strengthened the role of families in managing pressure ulcers in stroke patients, thereby contributing to increased patient motivation and improved quality of life.

Abstract

Congestive Heart Failure (CHF) remains a major health problem worldwide, with high prevalence and significant mortality rates. This condition is characterized by the heart’s inability to pump blood adequately to meet tissue demands, leading to hemodynamic disturbances such as impaired respiration and reduced oxygen saturation. One of the simple nursing interventions that is believed to improve these conditions is body positioning. The combination of lateral and semi-fowler positions is expected to enhance venous return, optimize lung expansion, and support ventilation in CHF patients. This study aimed to evaluate the effectiveness of combining lateral and semi-fowler positions in improving hemodynamic status, specifically respiratory pattern and oxygen saturation, among patients with congestive heart failure at GrandMed Hospital, Lubuk Pakam.A pre-experimental design with a one-group pretest-posttest approach was employed. A total of 23 respondents were selected using purposive sampling based on inclusion criteria. Respiratory rate and oxygen saturation were measured before and after the intervention. Data were analyzed using the Wilcoxon Test with a significance level of α = 0.05.Prior to the intervention, most patients experienced tachypnea (78%) and low oxygen saturation (52.2%). Following the intervention, improvements were observed, with 60.9% of patients achieving normal respiratory patterns and 56.5% showing adequate oxygen saturation, while only 8.7% remained in the low category. Statistical analysis revealed significant differences in respiratory rate (mean difference = 0.391; Z = -3.873; p = 0.001) and oxygen saturation (mean difference = 0.783; Z = -3.873; p = 0.001).The combination of lateral and semi-fowler positions proved effective in reducing tachypnea and enhancing oxygenation in patients with congestive heart failure. This intervention may serve as a simple, practical, and applicable nursing measure to support hemodynamic stability in CHF patients.
