Abstract

One plant that can be used as an alternative treatment for hyperuricemia is the cherry plant. The usefulness of these plants is due to the secondary metabolite compounds that can inhibit the enzyme xanthine oxidase. Cherry mistletoe is thought to have the same flavonoids, saponins, alkaloids, terpenoids, and tannins as the cherry plant, which can act as inhibitors. The aim of the study was to determine the inhibitory effect of an ethanol extract of cherry mistletoe leaves on the xanthine oxidase enzyme. This study was conducted as  an in vitro experimental study. Cherry mistletoe leaves were extracted with 96% ethanol. The extract was done with phytochemical screening, followed by an inhibition test on the xanthine oxidase enzyme using a spectrophotometer at a wavelength of 400 nm. Allopurinol was a positive control. The ethanol extract of cherry mistletoe leaves contains flavonoids, saponins, alkaloids, terpenoids, and tannins. The IC50 value of the ethanol extract of cherry mistletoe leaves was 23.44 mg/L. The ethanol extract of cherry mistletoe leaves has an inhibitory effect on the xanthine oxidase enzyme with moderate activity. Accordingly, the cherry mistletoe leaf extract can be considered as an alternative inhibitor of the xanthine oxidase enzyme in the control of hyperuricemia.

Abstract

BackgroundGastrointestinal stromal tumors are the most common mesenchymal tumors in the gastrointestinal tract. Based on tumor size andmitotic index, WHO classified GIST into three prognostic groups which were benign, uncertain malignant potential, and malignant.Some studies revealed that there was variation in association between age, gender, tumor location, histopathologic type and prognosisof GIST patients. This study aimed to identify and analyze the association between clinicopathologic characteristics and WHOprognostic group in GIST patients at RSUP Dr. Mohammad Hoesin Palembang.MethodsThis study used 30 medical records of GIST patients during 01 January 2014 to 27 December 2018 in the Department of AnatomicPathology RSUP Dr. Mohammad Hoesin. Clinicopathologic characteristics and WHO prognostic group data were collected. Theassociation between clinicopathologic characteristics (age, gender, tumor location, histopathologic type) and WHO prognostic groupwere analyzed by chi-square test.ResultsFrom 30 subjects, 40% of the patients aged 52-58 years.There was no gender predilection. A total of 33.3% of the GISTs were locatedin the small intestine. Majority of histopathologic type was spindle cell type (93.3%). Bivariate analysis showed there were no significantassociation between clinicopathologic characteristics: age (p=0.811), gender (p=0.682), tumor location (p=1.000), histopathologic type(p=0.469) and WHO prognostic group.ConclusionThere was no significant association between clinicopathologic characteristics and WHO prognostic group in GIST patients at RSUPDr. Mohammad Hoesin Palembang. We suggest doing a cohort study about association between clinicopathologic characteristics andsurvival rate in GIST patients.

Abstract

ABSTRACT The increasing rate of cesarean section has been associated with a higher incidence of postoperative complications, prolonged recovery, and reduced patient comfort. Enhanced Recovery After Cesarean Surgery (ERACS) was introduced as an evidence-based perioperative care pathway to accelerate postoperative recovery while improving patient-centered outcomes. This descriptive observational study was conducted from September to November 2024 at a secondary-level hospital in Indonesia and included all patients who underwent cesarean section using the ERACS protocol during the study period. Primary data were collected through structured interviews using a standardized questionnaire, while secondary data were obtained from medical records. A total sampling technique was employed, resulting in 203 eligible participants. Data were analyzed using descriptive statistics. Most participants were aged 20–35 years, multiparous, at term gestation, and had a hospital stay of two days. Overall, patient satisfaction with ERACS was high across all evaluated domains. Very high satisfaction was reported for preoperative, intraoperative, and postoperative hospital services. Most patients also expressed high satisfaction with the adequacy of information provided, postoperative nausea and vomiting control, pain management, early mobilization, timely urinary catheter removal, and appropriate initiation of oral intake, with the highest level of satisfaction observed for early oral intake. These findings indicate that cesarean section performed using the ERACS protocol provides high patient satisfaction throughout the perioperative and postoperative periods, supporting ERACS as an effective, evidence-based, and patient-centered approach to enhancing postoperative recovery and improving the overall patient experience following cesarean delivery. Keywords: Cesarean Section, ERACS, Patient Satisfaction, Enhanced Recovery, Obstetric Surgery.  ABSTRAK Peningkatan angka persalinan dengan operasi seksio sesaria berkaitan dengan meningkatnya risiko komplikasi pascaoperasi, pemulihan yang lebih lama, serta berkurangnya kenyamanan pasien. Enhanced Recovery After Cesarean Surgery (ERACS) dikembangkan sebagai pendekatan pelayanan perioperatif berbasis bukti yang bertujuan mempercepat enhanced recovery sekaligus meningkatkan luaran yang berpusat pada pasien. Penelitian ini menggunakan desain observasional deskriptif yang dilaksanakan pada September hingga November 2024 di salah satu rumah sakit tingkat sekunder di Indonesia. Sampel penelitian meliputi seluruh pasien yang menjalani persalinan dengan operasi seksio sesaria menggunakan protokol ERACS selama periode penelitian. Data primer diperoleh melalui wawancara terstruktur menggunakan kuesioner baku, sedangkan data sekunder diperoleh dari rekam medis. Teknik total sampling menghasilkan 203 responden yang memenuhi kriteria penelitian, dan data dianalisis secara deskriptif. Sebagian besar responden berusia 20–35 tahun, multipara, berada pada usia kehamilan aterm, serta menjalani lama rawat inap selama dua hari. Secara keseluruhan, tingkat kepuasan pasien terhadap penerapan ERACS tergolong tinggi pada seluruh aspek yang dievaluasi. Tingkat kepuasan yang sangat tinggi ditemukan pada pelayanan praoperatif, intraoperatif, dan pascaoperatif. Sebagian besar pasien juga menyatakan sangat puas terhadap kecukupan informasi yang diberikan, pengendalian mual dan muntah pascaoperasi, pengelolaan nyeri, pelaksanaan mobilisasi dini, waktu pelepasan kateter urin, serta ketepatan pemberian asupan oral, dengan tingkat kepuasan tertinggi terdapat pada waktu dimulainya pemberian asupan oral. Temuan ini menunjukkan bahwa penerapan protokol ERACS mampu memberikan tingkat kepuasan pasien yang tinggi pada seluruh komponen pelayanan perioperatif dan pascaoperatif, sehingga mendukung ERACS sebagai pendekatan pelayanan yang efektif, berbasis bukti, dan berpusat pada pasien dalam mempercepat pemulihan serta meningkatkan pengalaman pasien setelah persalinan dengan seksio sesaria.  Kata Kunci: Seksio Sesaria, ERACS, Kepuasan Pasien, Enhanced Recovery, Operasi Obstetri.

Abstract

Cervical cancer remains a major cause of cancer-related morbidity and mortality among women worldwide. In addition to persistent human papillomavirus (HPV) infection, dysregulation of cell survival and cell death mechanisms contributes to cervical cancer development and progression. Apoptosis and autophagy are two important biological processes involved in regulating cancer cell survival and death; however, the interaction between these processes remains complex and context-dependent. This review aimed to examine the crosstalk between apoptosis and autophagy in cervical cancer, with a particular focus on the roles of BAX, BCL-2, and cleaved Caspase-3. A narrative literature review was conducted using PubMed to identify publications related to apoptosis–autophagy crosstalk in cervical cancer, with particular attention to studies discussing BAX, BCL-2, cleaved Caspase-3, and selected autophagy-related regulators published between 2020 and 2025. Articles were selected based on relevance to the review topic and were synthesized descriptively to provide an overview of current evidence. The reviewed studies indicated that increased apoptotic activity was frequently associated with BAX upregulation, BCL-2 downregulation, and activation of cleaved Caspase-3. In contrast, autophagy exhibited a context-dependent role and functioned either as a mechanism of cancer cell death or as a protective process that promotes cancer cell survival. The interaction between BCL-2 and Beclin1, together with the PI3K/Akt/mTOR signaling pathway, emerged as important components of apoptosis–autophagy crosstalk. The conclusions was the balance between apoptosis and autophagy appears to influence cervical cancer cell responses to therapeutic interventions. BAX, BCL-2, and cleaved Caspase-3 may provide useful molecular information for future investigations of cervical cancer biology and therapeutic responses

Abstract

Linezolid remains a key component in drug-resistant tuberculosis (DR-TB) regimens but is associated with toxic optic neuropathy (TON). Although mitochondrial dysfunction has been implicated in linezolid-induced neurotoxicity, the contribution of mitochondrial DNA (mtDNA) variants remains uncertain, with limited data from Indonesian populations. This study aimed to evaluate clinical risk factors and mitochondrial DNA polymorphisms associated with linezolid-related TON in DR-TB patients. This cross-sectional analytical study was conducted at Dr. Mohammad Hoesin Hospital in Palembang from January to March 2025 and involved 51 DR-TB patients receiving linezolid therapy, selected through consecutive sampling. Demographic and clinical data were collected, and mtDNA polymorphisms (G3010A and A2706G) were examined using PCR-RFLP, chosen based on previous studies linking these variants to linezolid toxicity in Asian and African populations. TON was identified in 22 of 51 patients (43.1%) based on standardized ophthalmological criteria. Diabetes mellitus (PR=2.078, 95% CI=1.191–3.624, p=0.03) and treatment regimen (PR=12.469, 95% CI=1.821–85.398, p=0.00) demonstrated statistically significant associations with TON, whereas age (p=0.635), sex (PR=0.713, 95% CI=0.384–1.322, p=0.29), HIV status (PR=1.600, 95% CI=0.672–3.809, p=0.40), and DR-TB type (PR=1.438, 95% CI=0.735–2.812, p=0.28) were not associated. All subjects had a wild-type genotype for G3010A and a mutant-type genotype for A2706G, precluding any association analysis for these variants. In conclusion, diabetes mellitus and individual treatment regimens were associated with linezolid-induced TON in this study. In contrast, the investigated mtDNA polymorphisms (G3010A and A2706G) showed no variability; thus, no relationship could be evaluated. Larger prospective studies with expanded genetic markers and multivariate adjustment are warranted to characterize susceptibility factors for linezolid-associated TON better.

Abstract

Background: Fat is a major macronutrient in breast milk, and its concentration varies according to multiple factors, including stage of lactation, time of collection, milk fraction, feeding duration, maternal diet, and maternal body mass index (BMI). Alterations in breast milk fat composition may influence infant growth patterns and long-term metabolic health. This study aimed to examine the association between maternal BMI and the fat content of expressed breast milk. Methods: An analytical observational study with a cross-sectional design was conducted at the Medical Basic Chemistry Laboratory, Faculty of Medicine, Universitas Sriwijaya. Breast milk samples were obtained from 63 breastfeeding mothers in Palembang City, comprising 30 mothers with normal BMI and 33 mothers with obese BMI. Breast milk fat concentration was measured using the glycerol-3-phosphate oxidase–phenol aminophenazone (GPO-PAP) method. Data were analyzed using an independent t-test. Results: The mean fat concentration of breast milk from mothers with normal BMI was 3786.7±743.2 mg/dL, whereas that from mothers with obese BMI was 4339.2±958.0 mg/dL. A statistically significant difference in fat concentration was observed between the two groups (p=0.014). Conclusion: Maternal BMI is significantly associated with the fat content of expressed breast milk, with higher fat concentrations observed in mothers with obese BMI. These findings imply that maternal nutritional status should be considered in lactation counseling and infant feeding practices, particularly to optimize infant growth and metabolic outcomes.

Abstract

Introduction: Oral cancer is the 11th most common cancer worldwide. Previous studies have reported that saliva contains biomarkers for oral cancer. MMP-9, found in saliva, is a marker that indicates and facilitates tumor development. However, comprehensive biomolecular and bioinformatics characterization of salivary MMP-9 remains limited. This study aimed to analyze the biomolecular and bioinformatics characteristics of salivary MMP-9 as an oral cancer biomarker and a potential target for therapy because it plays an important role in tumor development and management. Methods: The features of MMP-9 were obtained from the NCBI database (www.ncbi.nlm.nih.gov). Protein analyses were performed using Protparam site, PROTSCALE, TMM application, PEPTIDE CUTTER, NETNGLYC and TARGETTP. The protein model and structure of MMP-9 were evaluated using SWISSMODEL. Results: Based on the study, it showed that MMP-9 is located on 20q13.12. It is involved in extracellular matrix breakdown, and this enzyme degrades type IV and V collagens. Based on the instability index (II), the protein was found to be unstable. The hydrophobicity range of the MMP-9 protein was 0–7654. Among the 20 amino acids analyzed, the hydrophobicity of arginine was -4.500, while isoleucine was 4.500. Transmembrane helix analysis indicated the MMP-9 protein isoform is predominantly located outside of the cell. The predicted N-glycosylation site was at the 28th amino acid. The structural model analysis indicated that the protein model was of good quality. Conclusion: Biomolecular and bioinformatics characterization of salivary MMP-9 demonstrates strong potential as an oral cancer biomarker. The biomolecular and bioinformatics characteristics identified in this study support the potential utility of MMP-9 as an oral cancer biomarker and provide a foundation for future studies investigating its therapeutic relevance.
