Abstract

Tuberkulosis (TB) adalah penyakit infeksi yang disebabkan oleh Mycobacterium tuberculosis. Kasus resistensi M. tuberculosis terhadap rifampisin sudah banyak dilaporkan di dunia termasuk Indonesia. Resistensi terhadap rifampisin pada  M. tuberculosis sebagian besar disebabkan mutasi gen rpoB yang menyandi RNA polymerase subunit ß. Mutasi gen rpoB Ser531Leu yang berhubungan dengan resistensi terhadap Rifampisin paling sering terjadi. Adanya mutasi pada rpoB akan  menyebabkan perubahan pada struktur dan aktivitas target obat. Penelitian ini bertujuan untuk mengidentifikasi mutasi gen rpoB Ser531Leu Mycobacterium tuberculosis pada sampel yang diambil dari penderita tuberkulosis paru di RSUP dr. Mohammad Hoesin Palembang. Penelitian ini merupakan penelitian deskriptif terhadap 40 penderita tuberkulosis paru di RSUP dr. Mohammad Hoesin Palembang. Identifikasi mutasi gen rpoB Ser531Leu Mycobacterium tuberculosis dilakukan dengan teknik Multiplex Polymerase Chain Reaction menggunakan primer rpoB531. Dari 40 isolat gen rpoB kodon 531 didapatkan 70% (21 dari 30) terjadi mutasi, wild type sebanyak 9 isolat (30%) dan isolat yang tidak menghasilkan pita sebanyak 10 isolat. Telah ditemukan mutasi gen rpoB Ser531Leu Mycobacterium tuberculosis pada penderita tuberkulosis paru di RSUP dr. Mohammad Hoesin Palembang.

Abstract

Pendahuluan: Hiperurisemia pada pasien kanker meningkatkan angka kematian terutama pada stadium lanjut. Metode: Studi observasional analitik korelatif dengan rancangan cross sectional, untuk mengetahui korelasi kadar asam urat dan derajat keganasan kanker kolorektal. Subjek penelitian 35 orang penderita kanker kolorektal yang dirawat di RSUP Mohammad Hoesin (RSMH), terdiri atas 15 laki-laki dan 20 perempuan. Stadium kanker kolorektal berdasarkan pemeriksaan histopatologi di Laboratorium Patologi Anatomi RSMH/FK Unsri Palembang. Pemeriksaan kadar asam urat serum di Laboratorium Biokimia FK Unsri. Hasil: Stadium klinis penderita terdiri dari stadium II (17,1%), stadium III (48,6%) dan stadium IV (34,3%). Rata-rata kadar asam urat penderita adalah 11,8±3,5 mg/dL, 88,6% mengalami hiperurisemia. Kadar asam urat pada pasien kanker kolorektal stadium awal adalah 10,6 mg/dL, pada pasien kanker kolorektal stadium akhir adalah 12,3 mg/dL. Korelasi antara kadar asam urat dan stadium kanker kolorektal sangat lemah (r=0,072) dan tidak bermakna (p=0,647). Simpulan: Hiperurisemia terjadi pada hampir semua penderita kanker kolorektal. Kadar asam urat tidak berkorelasi dengan stadium kanker kolorektal.Introduction: Hyperuricemia in cancer patients increases mortality, especially in advanced phase. Methods: An observational analytic correlative study with cross sectional design on correlation between uric acid level and cancer stage in colorectal cancer patients. Subjects were 15 male and 20 female colorectal cancer patients treated at RSUP Mohammad Hoesin (RSMH). Stage of colorectal cancer based on histopathology examination in Anatomy Pathology Laboratory of RSMH/Medical Faculty of Sriwijaya University, Palembang. Serum uric acid level was measured in Biochemistry Laboratory of Medical Faculty of Sriwijaya University. Results: The cancer stage of the patients were stage II (17.1%), stage III (48.6%) and stage IV(34.3%). The mean uric acid levels were 11.8±3.5 mg/dL. Approximately 88.6% of colorectal cancer patients have hyperuricemia. The mean level of uric acid in patients with early stage colorectal cancer was 10.6 mg/dL, in patients with end-stage colorectal cancer was 12.3 mg/dL. The correlation between uric acid levels and colorectal cancer stage was very weak (r=0.072) and was not significant (p=0.647). Conclusion: Hyperuricemia occurs in almost all colorectal cancer patients. Uric acid level did not correlate with colorectal cancer stage.

Abstract

BackgroundGranulomatous mastitis (GM) is often found in reproductive-age women. These patients are treated by the administration ofantituberculosis drugs. It is difficult to determine the exact cause of GM cytologically and histopathologically. Various etiologies i.e.,tuberculosis, sarcoidosis, foreign bodies and immunological diseases may provide granulomatous inflammation features in thetissue. High-sensitivity tests such as PCR can help to detect the presence of Mycobacterium tuberculosis (MT) as one of possibleetiologic agent. Accurate diagnosis will improve the treatment quality thus a better quality of life. The aim of this study was todetermine whether the granulomatous mastitis patients who referred to the Department of Anatomic Pathology Faculty of MedicineUniversitas Sriwijaya/Dr. Moh. Hoesin Palembang Hospital were caused by MT.MethodsA descriptive observational study was performed. Thirty paraffin blocks of GM patients during the period of 1st October 2018 to 29thSeptember 2020 were collected. DNAs were extracted from tissue scrapings of the slides. Then, PCR analysis was conducted usinga specific primer IS6110 with around 200 bp PCR product.ResultsThe mean age of the patients was 40.8 years (range: 23-67 years). There were six samples (20%) of GM produced the expectedamplicon. However, only three of six samples morphologically consistent with tuberculosis.ConclusionGranulomatous inflammatory reactions in the breast are not only caused by tuberculosis. Further study is needed to determine theexact cause of this lesion. Despite high costs, PCR examination may become an alternative approach in determining the etiology ofGM.

Abstract

Efektivitas Jeruk Nipis (Citrus aurantifolia Swingle) sebagai Zat Antiseptik pada Cuci Tangan. Jeruk nipis (Citrus aurantifolia Swingle) merupakan tanaman yang sangat berpotensi untuk dikembangkan sebagai tanaman berkhasiat. Hal ini disebabkan karena komponen kimia yang terkandung dalam jeruk nipis (Citrus aurantifolia Swingle) antara lain flavonoid, alkaloid, tanin, minyak atsiri, dan saponin yang mempunyai aktivitas antimikroba. Penggunaan air perasan jeruk nipis (Citrus aurantifolia Swingle) sebagai alternatif untuk mencuci tangan serta penelitian mengenai antibakteri di dalamnya, menimbulkan pertanyaan mengenai efektivitas jeruk nipis (Citrus aurantifolia Swingle) sebagai zat antiseptik pada cuci tangan. Tujuan penelitian ini adalah untuk mengetahui perbandingan efektivitas mencuci tangan menggunakan air biasa, air perasan jeruk nipis (Citrus aurantifolia Swingle), dan alkohol 70%. Penelitian ini adalah penelitian eksperimental laboratoris menggunakan rancangan eksperimental sederhana (Pretest-Posttest Control Group Design). Sebanyak 5 mahasiswa Fakultas Kedoketan Universitas Sriwijaya dibagi menjadi 5 kelompok. Tiga kelompok diberikan air perasan jeruk nipis dengan variasi konsentrasi 50%, 75%, dan 100%. Kelompok lainnya adalah kelompok kontrol positif dengan pemberian alkohol 70% dan kontrol negatif diberikan aquadest. Analisis terhadap jumlah koloni atau angka kuman dilakukan melalui pengambilan sampel di permukaan telapak tangan subjek dengan cara di swab menggunakan kapas swab steril. Secara umum, terjadi penurunan jumlah koloni atau angka kuman pada cuci tangan menggunakan air perasan jeruk nipis. Namun, secara statistik tidak ada perbedaan yang bermakna kecuali pada kelompok jeruk nipis konsentrasi 75% postest menit pertama. Jeruk nipis konsentrasi 75% efektif sebagai antiseptik yang mana terdapat perbedaan jumlah koloni atau angka kuman pretest dengan postest menit pertama.

Abstract

Background: Over the past decade, numbers of Carbapenemase Producing-Carbapenem Resistant Enterobacteriaceae (CP-CRE) has been increasing worldwide and it has been becoming a threat because of its resistance against carbapenem which is considered as the “last resort” antibiotic. Therapy options for its infection are still limited. Aminoglycoside serves as one of the most commonly used antibiotics, but the resistance against it has already been presented for a long time. Aminoglycoside Modifying Enzyme (AME) is the most important resistance mechanism against aminoglycoside. AAC(6’)-Ib enzyme is one of the most common AME produced by the gram-negative bacteria. Objectives: This study wished to identify the gene of this enzyme among CRE isolated from infected Indonesian patients in Dr. Mohammad Hoesin Hospital Palembang. Methods: Twenty-eight isolates collected from CRE-infected patients identified by Vitek 2 Compact (bioMerieux, USA) in dr. Mohammad Hoesin Hospital Palembang during September—November 2017. AAC(6’)-Ib gene was identified using PCR method, then visualize by electrophoresis. The result is then analyzed by comparing it with a susceptibility test. Results: Out of 28 samples, AAC(6’)-Ib is identified in 22 (78.57%) samples. Samples with AAC(6’)-Ib showed to be less resistant to various antibiotics, significantly to amikacin (p=0.023). Conclusion: AAC(6’)-Ib gene is found in most of samples implying its frequent occurrence in Indonesian patients.

Abstract

Abstract Infectious disease is one of the most common diseases in the world. Staphylococcus aureus and Escherichia coli are two common causes of infection and are resistant to many antibiotics, so the new agents are needed to overcome antibiotic resistance. Cinnamon is often used as a preservative because it has antibacterial activity. Cinnamomum burmannii is kind of native cinnamon from Indonesia. The antimicrobial active compounds cinnamaldehyde and eugenol are the main reasons for its antibacterial activity. This study observed the efficacy of the cinnamon extract (Cinnamomum burmannii) as antibacterial against Staphylococcus aureus and Escherichia coli. An experimental study, in vitro using Post-test Only Control Group Designed, has been done in Microbiology and Biotechnology Laboratory of Medical Faculty of Sriwijaya University. Cinnamon was extracted, then tested for its antibacterial activity using well diffusion and serial dilution to determine diameter of inhibition zone and minimum bactericidal concentration. Phytochemical tests were also conducted to determine the antibacterial compounds of cinnamon extract. Ethanol extract of cinnamon was able to inhibit the growth of Staphylococcus aureus with MBC 5% and inihibitory zone 6,84±0,68 mm and Escherichia coli with MBC 10% and inhibitory zone 5,69±0,69 mm. Cinnamon extract which has the greatest effectiveness is concentration of 40% with inhibition zone 15,69±0,80 mm (Staphylococcus aureus) and 9,63±0,59 mm (Escherichia coli). This ability is due to the antibacterial compounds as evidenced by positive results in various phytochemical tests. Cinnamon extract is effective as antibacterial against Staphylococcus aureus and Escherichia coli in vitro. Abstract

Abstract Background Aloe vera is a plant that has been used as an alternative drug. This plant contains various compounds, like anthraquinone, saponin, flavonoid, alkaloid, and tannin that has an antibacterial effect against Staphylococcus aureus and Escherichia coli. Both of it were responsible for the infection incident. This study aims to determine the efficacy of Aloe vera sap as an antibacterial against Staphylococcus aureus and Escherichia coli. Methods An experimental study, in vitro using post-test only control group design, has been done at laboratory of Medical Faculty of Sriwijaya University, by examining the antibacterial activity of Aloe vera sap in five different concentration (5%, 10%, 20%, 40%, dan 80%) using well diffusion and solid dillusion method to determine the minimum bactericidal concentration (MBC). And then continued with the phytocemical screening to determine the compound inside the Aloe vera sap. Results Aloe vera sap were able to kill Staphylococcus aureus at 5% and Eshcerichia coli at 80%. Compatibility test showed that Aloe vera sap with concentration of 10%, 20%, 40%, and 80% are compatible with amoxicillin, therefore 80% is compatible with cefotaxime. This ability due to the compound that it contains, which is alkaloid, flavonoid, tannin, quinone, and saponin. Conclusion Aloe vera sap is effective as an antibacterial against Staphylococcus aureus and Escherichia coli. Keyword: Aloe vera sap, antibacterial, efficacy, Staphylococcus aureus, Escherichia coli.

Abstract

Introduction: Diffuse large B cell lymphoma (DLBCL) is the most common type of non-Hodgkin lymphoma among B cell lymphomas. The interaction of tumor cells with their microenvironment (tumor microenvironment, TME) leads to progressivity of malignancy. CD163 + macrophages known as components of TME. Nuclear factor of activated T cell (NFATc1) and MYC are important transcription factors in malignant transformation and progression. Therapeutic strategies were fast developed, nevertheless, efforts to decrease DLBCL morbidity and mortality are unsatisfied, therefore,new markers for prognosis and or therapeutic options of the patients are necessary. This study was aimed to investigate NFATc1 expression in DLBCL and its TME. Methods: Thirty-two paraffin blocks were selected then immunostained for expression of NFATc1, MYC, and CD163. Clinopathologic data i.e. ages, gender, and proliferation index Ki-67 were obtained. Data was analyzed by statistics Result: Positive expression of CD163 and NFATc1 was among 55% and 45% of cases respectively. All DLBCL cases in this study were non-GCB subtype and more patients were under 60 years (66%). Positive expression of CD163 was higher in males (69%) and in patients under 60 years (63%). Tissues positive for both NFATc1 and CD163 was observed higher among males and patients under 60 years. Conclusion: NFATc1 may affect development and or progression of certain subsets of DLBCL non-GCB subtype.

Abstract

Tujuan: Penelitian ini bertujuan untuk mengetahui korelasi  antara indeks massa tubuh  dan profil lipid pada remaja obesitas di Kota Palembang. Metode: Penelitian ini adalah penelitian analitik observasional dengan desain potong lintang terhadap 143 remaja di Kota Palembang. Indeks massa tubuh dihitung berdasarkan berat badan (kg) dan tinggi badan (m). Kadar profil lipid berupa kolesterol total, trigliserida, LDL-kolesterol dan HDL-kolesterol darah diperiksa menggunakan kit dari human® di Laboratorium Kimia Dasar Fakultas Kedokteran Universitas Sriwijaya. Analisis data menggunakan uji korelasi Pearson. Hasil: Dari penelitian ini didapatkan sekitar 29,4% remaja di Kota Palembang mengalami obesitas dan sekitar 17,5% remaja mengalami dislipidemia. Uji korelasi Pearson antara indeks massa tubuh dan profil lipid menunjukkan nilai p=0,093 r=0,141, p=0,002 r=0,260, p=0,983 r=0,002, dan p=0,256 r=0,096 masing-masing untuk kolesterol total, trigliserida, LDL-kolesterol dan HDL-kolesterol. Korelasi indeks massa tubuh dengan kadar kolesterol total, LDL-kolseterol dan HDL-kolesterol tidak bermakna (p>0,05). Namun, korelasi indeks massa tubuh dengan kadar trigliserida bermakna (p<0,05) dengan arah korelasi postif dan kekuatan lemah (r=0,2-0,4). Simpulan: Pada remaja, tidak ada korelasi indeks massa tubuh dengan kadar kolesterol total, LDL-kolseterol dan HDL-kolesterol. Terdapat korelasi positif antara indeks massa tubuh dan kadar trigliserida pada remaja.

Abstract

Tujuan: Untuk mengetahui peran asam folat dalam metabolisme dan metode pemeriksaan asam folat serum. Metode: Artikel ini dibuat dengan mengumpulkan sumber rujukan dari Sinta, Google Scholar, Pubmed, dan ScienceDirect.  Artikel-artikel yang berisi mengenai asam folat (folic acid) atau folat (folate) dan metode pemeriksaan  asam folat (folate detection method, folate analysis method) tahun 2005-2020 dikumpulkan kemudian dibahas dan ditarik kesimpulan secara umum. Hasil: Bentuk aktif asam folat, tetrahidrofolat, berperan dalam metabolisme purin, pirimidin (timin), dan metionin dengan cara mendonorkan unit satu karbon (metil, metilen, metenil, formimino atau formil). Kadar asam folat serum normal adalah 6-20 ng/ml. Beberapa metode digunakan untuk memeriksa kadar asam folat serum yakni mikrobiologi, HPLC dan protein binding assay. Metode mikrobiologi merupakan baku emas, tetapi membutuhkan waktu yang lama, mudah terkontaminasi dan perlu tenaga mikrobiologi khusus.  Metode HPLC memberikan hasil yang lebih cepat, tetapi mahal dan kurang sensitif. Metode protein binding assay merupakan metode baru yang menggunakan protein ikat folat. Metode ini memiliki sensitivitas dan spesifisitas yang baik. Kekurangan pada metode ini adalah penyimpanan reagen dan kalibrasi. Kesimpulan: Asam folat dibutuhkan dalam metabolisme nukleotida dan asam amino. Metode pemeriksaan asam folat diharapkan dapat dikembangkan untuk pemeriksaan asam folat yang lebih sederhana, murah dan cepat.
