Abstract

Program Keluarga Berencana (KB) merupakan salah satu upaya mengatasi masalah jumlah penduduk. Program ini mendukung kesehatan dan kesejahteraan masyarakat Indonesia dengan cara mengatur usia kehamilan, jumlah anak, dan jarak kelahiran anak. Salah satu problematika saat ini adalah rendahnya pengetahuan dan sikap mengenai program KB di Indonesia. Penelitian ini bertujuan untuk mengetahui pengetahuan dan sikap pria di Surabaya terhadap kontrasepsi. Penelitian ini melibatkan subjek laki-laki yang berusia 17-45 tahun. Variabel dalam penelitian ini berupa pengetahuan dan sikap pria terhadap kontrasepsi dilakukan metode survey secara cross sectional. Pengambilan data dilakukan secara non-probability sampling dengan sampling kuota berdasarkan kategori usia pada 100 responden. Penelitian ini mengidentifikasi 55% responden masuk dalam tingkat pengetahuan kurang, 73% responden tidak mengetahui efek samping dari kontrasepsi, dan 86% responden mengetahui tujuan penggunaan kontrasepsi. Selain itu, 58% responden masih memiliki sikap positif terhadap penggunaan kontrasepsi, 25% responden berpendapat penggunaan kontrasepsi mengurangi kepuasan seksual, dan 84% responden pria berpendapat bahwa suami dan istri bersama-sama bertanggung jawab dalam penggunaan kontrasepsi. Berdasarkan penelitian ini, walaupun mayoritas responden bersikap terhadap penggunaan kontrasepsi, tetapi tingkat pengetahuan mereka masih kurang sehingga pengetahuan pria terhadap penggunaan kontrasepsi perlu ditingkatkan.

Abstract

Menyajikan informasi obat dan konsisten pada etiket obat memiliki potensi untuk meningkatkan kepatuhan dan hasil terapi. Etiket obat harus memuat informasi penting yang dibutuhkan pasien untuk menjamin obat yang aman dan efektif. Diperlukan panduan pelabelan obat untuk mengurangi potensi kesalahan pengobatan. Penelitian ini bertujuan untuk melihat perbedaan sikap, pemahaman, dan praktik penggunaan obat oleh pasien yang mendapat sediaan sirup kering dengan 2 macam desain etiket, yaitu etiket yang umum digunakan di Indonesia dan etiket yang dikembangkan berdasarkan panduan dari beberapa negara, yaitu FIP, USP, dan ACSQHC. Dalam penelitian ini, diperoleh total 80 responden. Hasil yang diperoleh adalah tidak ada perbe- daaan sikap dari kedua kelompok responden mengenai desain etiket kecuali dalam hal kelengkapan isi informasi (P = 0,044). Pada variabel pengetahuan juga tidak terdapat perbedaan bermakna pada kedua kelompok responden, namun pada variabel praktik penggunaan terdapat perbedaan bermakna (P = 0,020) dimana kelompok responden satu mempunyai skor yang lebih tinggi dari kelompok dua yaitu masing-massing 2,68 ± 0,474 dan 2,28 ± 0,784. Kebenaran dalam mengukur dosis dapat dipengaruhi oleh keterampilan motorik seseorang, namun tetap dibutuhkan kelengkapan informasi yang dapat mengurangi potensi medication error, sehingga diperlukan panduan yang mengatur informasi minimal yang harus ada dalam etiket obat pada pelayanan resep di Indonesia. Presenting drug information and consistency on drug labels has the potential to improve compliance and therapeutic outcomes. Drug labels must contain important information needed by patients to ensure safe and effective drugs. Drug labeling guidelines are needed to reduce the potential for medication errors. This study aims to see the differences in attitudes, understanding, and practices of drug use by patients who receive dry syrup preparations with 2 types of label designs, namely labels commonly used in Indonesia and labels developed based on guidelines from several countries, namely FIP, USP, and ACSQHC. In this study, a total of 80 respondents were obtained. The results obtained that there was no difference in attitudes from the two groups of respondents regarding label design except in terms of the completeness of the information content (P = 0.044). In the understanding variable, there was also no significant difference in the two groups of respondents, but in the practice variable there was a significant difference (P = 0.020), where respondent group one had a higher score than group two, namely 2.68 ± 0.474 and 2.28 ± 0.784 respectively. The accuracy in measuring the dose can be influenced by a person's motor skills, but complete information is still needed to reduce the potential for medication errors, so a guideline is needed that regulates the minimum information that must be included in drug labels in prescription services in Indonesia. Abstract

Salah satu penyebab terjadinya peningkatan infeksi cacing adalah rendahnya pengetahuan dan kesadaran masyarakat tentang personal hygiene. Tujuan dari penelitian ini adalah mengetahui tingkat pengetahuan ibu terhadap cacingan, kepatuhan anak minum obat cacing di Kota Surabaya, dan mengetahui hubungan kedua variabel tersebut. Responden penelitian adalah ibu yang mempunyai anak usia prasekolah (5-6 tahun). Metode penelitian yang digunakan adalah survei cross sectional menggunakan kuesioner yang telah dilakukan uji validitas dan uji reliabilitas. Untuk menilai pengetahuan jawaban benar=1 dan salah=0, kepatuhan menggunakan skala likert 1-3 yaitu sering, kadang-kadang, tidak pernah. Digunakan metode sampling dengan purposive sampling pada 130 responden. Uji statistik menggunakan uji korelasi Spearman’s rho. Hasil penelitian menunjukkan pengetahuan responden 90,7% (n=118) baik, 8,5% (n=11) cukup, 0,8% (n=1) kurang. Selain itu, 80,8% (n=105) patuh, 19,2% (n=25) tidak patuh terhadap pemberian obat cacing pada anak. Terdapat hubungan yang bermakna (p=0,001) antara tingkat pengetahuan ibu dengan kepaatuhan memberikan obat cacing pada anak dengan hasil korelasi positif sebesar 0,300, hubungan antara kedua variabel tersebut tergolong lemah atau rendah. Berdasarkan hasil penelitian, peningkatan pengetahuan diharapkan dapat menjadi upaya untuk menurunkan angka cacingan.

Abstract

Imunisasi Dasar Lengkap merupakan pemberian vaksin kepada bayi berumur 0-11 bulan agar terhindar dari penyakit menular. Saat ini, target capaian 100% imunisasi dasar pemerintah belum terpenuhi. Terdapat beberapa faktor yang mempengaruhi kelengkapan imunisasi dasar. Penelitian ini bertujuan untuk mengetahui hubungan pengetahuan ibu tentang imunisasi dasar terhadap status lengkapnya imunisasi dasar anak di Kecamatan Kenjeran Surabaya. Penelitian ini menggunakan desain cross-sectional dan teknik pengambilan sample yaitu purposive sampling. Pengetahuan imunisasi dasar diukur menggunakan kuesioner yang telah dilakukan uji validitas dan reliabilitas. Kriteria inklusi responden antara lain seorang ibu yang memiliki anak berusia ≤ 6 tahun. Pada penelitian ini, didapatkan 145 responden yang memenuhi kriteria inklusi. Responden dikategorikan berpengetahuan tinggi jika skor pengetahuannya ≥ 8 dan rendah jika skornya < 8. Tingkat pengetahuan ibu terhadap imunisasi dasar yang tinggi sebanyak 75 responden (51,7%). Status lengkapnya imunisasi dasar diperoleh dari Buku Kesehatan Ibu dan Anak (KIA). Dari seluruh responden, terdapat 78 responden (53,8%) yang memiliki Buku KIA. Dari jumlah tersebut, terdapat sebanyak 71 orang responden (91%) yang anaknya telah mendapatkan imunisasi dasar secara lengkap. Dari hasil analisis tidak ditemukan korelasi signifikan antara pengetahuan ibu dengan kelengkapan imunisasi dasar (p=0,127). Faktor selain pengetahuan sebaiknya juga perlu diteliti sehingga perilaku orang tua dalam melaksanakan imunisasi anak dapat dijelaskan dengan lebih baik. Edukasi masyarakat mengenai pentingnya imunisasi dasar juga harus dilanjutkan untuk meningkatkan pengetahuan masyarakat khususnya ibu dalam hal imunisasi dasar anak.

Abstract

Introduction: Diabetes mellitus is a global epidemic that continues to rise, including in Indonesia, and it has significant impacts on public health. The role of pharmacists in providing pharmaceutical care for diabetic patients in community health centers is crucial, yet challenges in its implementation persist. However, in-depth studies on pharmacists' practice experiences in this context are still limited in Indonesia. This study aimed to explore pharmacists' experiences in implementing pharmaceutical care for diabetic patients at community health centers in Indonesia. Methods: The cross-sectional study was conducted in community health centers in South Kalimantan Province, Indonesia. Pharmacists practicing at community health centers were recruited between October and December 2023. The survey was distributed in both paper and online (Google Form) formats. The questionnaire used in the survey focused on the provision of assessment, care plan, and follow-up parameters regarding the medication of diabetic patients. Results: A response rate of 60,3% (n=143) was obtained from pharmacists involved in this survey. Regarding the assessment parameter, the indicator of assessment patient trust was the most dominant implemented by pharmacists (93,7%) compared to other indicators. In terms of the care plan parameter, providing information on medication indications was the most dominant indicator implemented (82,9%). Meanwhile, for the follow-up parameter, the indicator of medication adherence follow-up was the most dominantly implemented by pharmacists (79,1%). Factor analysis revealed that assessment, care plan, and follow-up evaluation each constituted one composite. Conclusion: The pharmaceutical care provided by pharmacists at community health centers for diabetic patients is still suboptimal. The factors that caused the suboptimal implementation of pharmaceutical care were resource limitations, inadequate specialized training for pharmacists, and insufficient support from the healthcare system.

Abstract

The Transition of Primary Healthcare Centers to Regional Public Service Agency (BLUD) status allows them to independently manage their finances, including medicine procurement. This change is expected to impact the effectiveness of planning and procurement. This study evaluated the effectiveness of medicine planning and procurement in Primary Healthcare Centers that had transitioned to BLUD status. The study used retrospective observations based on data from 2022 to June 2024. Samples were selected using purposive sampling based on inclusion and exclusion criteria. The independent variable in this study was the implementation of BLUD financial management, while the dependent variables included compliance with FORNAS, medicine stockouts, and excess medicines. The study involved 30 Primary Healthcare Centers and the KruskalWallis test was used for statistical analysis. The results showed that the average scores for all indicators did not meet the expected standards. After BLUD implementation, there was a decrease in medicine compliance with FORNAS, although it remained above the minimum performance standard of 80%, and a significant reduction in stockouts (p < 0.05). The percentage of excess medicines increased, but not significantly (p > 0.05). In conclusion, medicine planning and procurement were ineffective. The BLUD financial management model impacted medicine planning and procurement by reducing compliance with FORNAS, and decreasing stockouts.

Abstract

Introduction: The government targets a significant reduction in antibiotic resistance by 2030 through wise antibiotic management, including implementing the Clinical Decision Support System (CDSS) for Health Providers in Non-Pneumonia acute respiratory infections (ISPA) and Non-specific Diarrhea in Primary Healthcare Centre (Puskesmas). However, the readiness and acceptance of Health Providers to CDSS need to be evaluated. This study evaluates the readiness and acceptance of doctors, nurses, midwives, pharmacists, and pharmacists' assistants in Puskesmas using CDSS. Methods: The method used was a cross-sectional quantitative survey with snowball sampling of 185 respondents at the Probolinggo Puskesmas in July-August 2024. The questionnaire was developed based on a combination of the Technology Readiness Index and the Technology Acceptance Model (TRAM), and the data was analyzed using SEM-PLS (Structural Equation Modeling-Partial Least Squares). Result: This study revealed that optimism contributed positively to the perception of ease of use (PEOU) and perception of benefits (PU) of CDSS. Innovation also contributed positively to PEOU, but not significantly to PU. Optimism increases PEOU and PU, while innovation only increases PEOU. Although innovation is insignificant to PU, it has a more significant impact on PEOU than optimism. Conclusion: These findings also show that PU affects the attitude of Health Providers to use CDSS (? = 0.286, p < 0.001) but does not directly affect behavioral intentions (? = 0.081, p = 0.250). PEOU significantly affected PU (? = 0.617, p < 0.001) and attitude (? = 0.661, p < 0.001). Attitudes towards CDSS greatly influenced the behavioral intentions of healthcare providers to use it (? = 0.851, p < 0.001), making it a strong predictor of CDSS adoption. The integration of TRI and TAM in predicting the readiness of Health Providers has proven helpful in understanding the factors of CDSS adoption in Puskesmas. These findings highlight the importance of training for health providers to improve CDSS readiness and acceptance. In addition, the results of this study can be the basis for policy development in implementing CDSS Primary Healthcare Centers to support a more rational use of antibiotics.

Abstract

Background: The prevalence of type 2 diabetes mellitus (T2DM) is increasing worldwide. Sodium-glucose cotransporter-2 inhibitors (SGLT2i) and dipeptidyl peptidase-4 inhibitors (DPP-4i) are two second-line therapy alternatives for T2DM patients inadequately controlled with metformin. Objective: This study aimed to systematically review the cost-effectiveness of combining metformin+SGLT2i vs metformin+DPP-4i for T2DM treatment. Methods: A systematic search was conducted in PubMed, Scopus, and ScienceDirect for articles published between 2015-2025, using predefined keywords and following the PRISMA and PICOS frameworks (P: T2DM patients uncontrolled on metformin monotherapy; I: Metformin+SGLT2i therapy; C: Metformin+DPP-4i therapy; O: Cost, clinical outcomes (HbA1c% reduction), Incremental Cost-Effectiveness Ratio (ICER) values, Quality Adjusted Life Years (QALY); S: Study with cost-effectiveness analysis design). Additional studies were identified through reference screening. Eligible articles were independently reviewed and assessed for reporting quality using the CHEERS-2022 standards. Results: Five studies met the inclusion criteria. Considerable heterogeneity was observed with mean patient ages ranging from 55-61 years old and baseline HbA1c levels from 7.9%-9.4%. The studies were conducted in the US, UK, Mexico, and Greece, all funded by the pharmaceutical industry, and used economic models. Despite these differences, all studies consistently demonstrated that combining metformin+SGLT2i was more cost-effective than metformin+DPP-4i. SGLT2i improved the quality of life by 0.032–0.04 QALYs, reduced hypoglycemia, and provided additional benefits for patients with cardiovascular risk, although it was associated with higher initial costs. Conclusion: This review showed that the combination of metformin+SGLT2i was more cost-efficient and effective in managing T2DM than the combination of metformin+DPP-4i.

Abstract

Drug safety is a significant concern in many countries, as side effects (AE) and adverse drug reactions (ADR) have caused many deaths worldwide. One of the reasons is the low contribution of pharmacists in spontaneously reporting AE/ADR. This study aims to develop a questionnaire to assess factors that correlate with spontaneous reporting by pharmacists. A questionnaire pilot was tested on 30 pharmacist respondents who worked in type C hospitals in Surabaya and Sidoarjo, Indonesia. Respondents' responses were then evaluated for face validity, construct validity, and reliability. The results showed that the face validity of the questionnaire was ideal. Then, the results of the construct validity of the knowledge section using point biserial correlation showed that two items were invalid because the r-value was smaller than the r-table (r = 0.361). Then, construct validity uses the factor analysis method for psychological, environmental, and practical variables by paying attention to the Kaiser-Mayer-Olkin Measure (KMO) value, which must be greater than 0.5, the significance of the Bartlett test, which must be less than 0.05 and the factor loading value which conditions must be greater than 0.5. As a result, most of the psychological, environmental, and practical variables show valid and reliable results. However, further consideration should be given to eliminating some items that do not meet the requirements. In conclusion, this validated questionnaire can be used to obtain additional information regarding factors influencing spontaneous reporting by pharmacists.

Abstract

The Tuberculosis Information System (SITB) is a strategic platform for optimizing TB logistics management in primary health facilities, but its use is still not optimal. This study aims to analyze the effectiveness of SITB usage and its impact on the availability of TB logistics in community health centers in Jember Regency. This study used a cross-sectional design involving 30 community health centers from July to December 2024. Variables examined included technology, management, and organizational components affecting SITB system impact. The effectiveness of SITB was evaluated using the HOT (Human-Organization-Technology) - Fit model, the relationship between components is analyzed using the Spearman rank test. The results show, despite SITB implementation since 2020, logistics availability remained inequitable is still empthy beyond the standard in primary healthcare (> 4%), but Non- antituberculosis logistic have overstocked with an average availability of more than 5 months in quarterly periode (> 150%). Correlation analysis revealed that information quality and user satisfaction significantly influenced system impact (p < 0,05). In conclusion, SITB implementation has not ensured equitable TB logistics availability across community health centers in Jember Regency, where in the used of SITB, information quality and user satisfaction are key factors influencing system impact.
