Abstract

Abstract

Introduction: Tuberculosis (TB) remains a significant public health challenge in Indonesia, ranking among the highest global TB burden countries. The financial burden of TB is substantial, encompassing direct medical costs, indirect income losses, and catastrophic household expenditures. Although systematic reviews on TB-related cost-of-illness (COI) have been conducted in various settings, this study aims to fill critical gaps in Indonesian TB financial data, provide comparative regional insights, and offer actionable policy recommendations. Methods: This study used the Systematic Review (SR) method on articles relevant to: COI, disease cost evaluation, or therapy costs for tuberculosis. Articles are examined in English or Indonesian; obtain full text through several search engines such as: PUBMED, Science Direct, Google Scholar, and SCOPUS; research coverage in Indonesia; and in an unlimited time frame. Results: A total of 11 eligible articles were obtained based on inclusion and exclusion criteria. The direct cost of TB-DS ranges from Rp2.2-5.8 million, while TB-MDR reaches Rp190 million. Indirect costs due to loss of revenue reached Rp481.5 million for MDR-TB. Catastrophic costs occurred in 83% of TB-MDR households and 36% of TB-DS, exceeding 20% of annual income. Most patients were men of productive age (18-45 years), with 82.33% losing their jobs during treatment. These findings highlight the need for strategic interventions to reduce the economic impact of TB. Conclusions: The impact of moderate financial burdens and other medical aspects on institutions and individuals is caused by tuberculosis. Further research should be conducted related to this study to obtain a larger estimate.

Abstract

An elderly person is someone who has reached the age of 60 years and above. From a health perspective, the elderly will experience an aging process characterized by a decrease in physical endurance, making them susceptible to disease. Based on the problem of the elderly in Sidoarjo not understanding healthy lifestyles, how to manage medication has the potential to decrease the quality of life of the elderly. Therefore, it is necessary to empower the elderly through training to improve the quality of life of the elderly. This community service activity targets the category of Sustainable Development Goals (SDGs), namely SDGs 3, which is ensuring a healthy life and improving the welfare of all residents of all ages. The empowerment activity was carried out at the RW.07 Hall, Magersari Village, Sidoarjo District on Saturday, June 28, 2025. Attended by 90 elderly people. The activity began with morning exercise, providing material and demonstrations on making herbal medicine, providing material on infectious diseases (TB) and degenerative diseases (DM and hypertension), providing material on how to properly manage medication and ending with a post-test. The results of the activity showed that the elderly were very enthusiastic about participating in the entire series of activities. The increase in elderly knowledge was seen from the increase in post-test scores. The paired-sample statistical test results yielded a P-value of 0.000. This indicates a significant difference between the pre-test and post-test results. Participants' understanding improved both before and after the educational material was provided.

Abstract

Thyroid hormone disorders, such as hyperthyroidism and hypothyroidism, are common endocrine diseases that impose a considerable economic burden. This study aimed to analyze the components of treatment costs among outpatients with thyroid hormone disorders at RSPAL Dr. Ramelan Surabaya and to examine the relationship between patient characteristics and total treatment costs. A cross-sectional design was employed from both patient and hospital perspectives, involving 86 outpatients during the period of May to July 2025. Data on direct medical costs were obtained from the hospital billing system and out-of-pocket expenses through patient interviews, while direct non-medical and indirect costs were collected through structured interviews. The results showed that most patients were female (69 patients; 80.23%) with a mean age of 42 years, and hyperthyroidism was the most common diagnosis (n=54). The total annual cost reached IDR 1,618,002,478, with an average of IDR 18,813,982 per patient. Direct medical costs represented the largest component, with laboratory examinations accounting for the highest expenditure (an average of IDR 8,245,814 per patient per year). Statistical analysis revealed significant associations between age (p=0.007) and BPJS insurance class (p=0.049) with direct medical costs, employment status (p=0.025) with direct non-medical costs, and both age (p=0.014) and employment status (p=0.000) with indirect costs. In conclusion, the treatment costs of thyroid disorders are predominantly driven by direct medical expenses, with variations influenced by age and insurance class.

Abstract

Background: Diabetic foot ulcers (DFU) are a consequence of diabetes mellitus (DM) that cause a significant financial burden on the healthcare system, society, and individuals. The average prevalence of DFU was 6.3%. The cost of diabetic foot care is 50% to 200% higher than that of primary diabetes care. Objective: This systematic review aimed to provide an overview of the variations in the economic burden of DFU in lower middle-income countries (LMICs). Methods: This systematic review examined studies published in English that investigated the costs associated with evaluating diabetic foot disease in LMICs. COCHRANE, CINAHL, DOAJ, EMBASE, GOOGLE SCHOLAR, Ovid MEDLINE, PUBMED, SCIENCE DIRECT, and SCOPUS were used as literature sources without publication date restrictions. Results: A systematic search identified six eligible studies conducted across multiple countries. The studies were published between 2000 and 2024, with study durations ranging from 6 to 12 months. These studies showed large variations in cost components, estimating both direct and indirect costs of the disease. The total costs incurred for DFU vary among LMICs, ranging from USD 379.39 to USD 60,014.81 per year. Conclusion: DFU impose a significant economic burden in LMICs, with substantial variations in cost estimates owing to differences in methodology and data availability. Future studies on the cost burden of DFU should be conducted on a larger scale to obtain more accurate cost estimates.
