Abstract

Abstrak Latar Belakang: Leukemia Granulositik Kronik (LGK) merupakan penyakit mieloproliferatif yang ditandai dengan adanya proliferasi seri granulosit tanpa ada gangguan diferensiasi. Direct Coomb’s Test (DCT) digunakan untuk mendekteksi adanya antibodi imun, baik IgG maupun komponen komplemen (umumnya C3d) yang menutupi atau mensensitisasi sel eritrosit. Pemeriksaan coomb’s test pada pasien LGK dilakukan apabila adanya kecurigaan anemia dan terjadinya retikulositosis tanpa ditemukan adanya sumber perdarahan yang jelas. Objektif: Tujuan penelitian ini untuk mengetahui karakteristik pasien LGK berdasarkan hasil direct coomb’s test di RSUP Dr. M.Djamil Padang. Metode: Penelitian deskriptif dilakukan terhadap semua pasien yang didiagnosis LGK yang memiliki hasil direct coomb’s test. Pengambilan sampel dilakukan secara consecutive sampling dengan 30 sampel yang memiliki data berupa usia, jenis kelamin, hasil direct coomb’s test, derajat anemia, hitung retikulosit, dan jenis terapi. Hasil: Penelitian berdasarkan jenis kelamin pada pasien leukemia granulositik kronik didapatkan lebih banyak perempuan yaitu 18 pasien (60%) dan laki-laki 12 pasien (40%) dengan rerata usia 44 tahun. Direct coomb’s test menunjukkan hasil positif pada 21 pasien (70%) dan negatif pada 9 pasien (30%). Anemia berat paling banyak ditemukan pada direct coomb’s test negatif dengan 55,6%. Retikulositosis lebih banyak ditemukan pada hasil direct coomb’s test positif. Kesimpulan: Pasien LGK dengan hasil direct coomb’s test positif lebih banyak ditemukan anemia berat, anemia sedang, dan retikulositosis. Abstract

Background: Venous thromboembolism (VTE) is a significant cause of morbidity and mortality in acutely ill medical patients. Both unfractionated heparin (UFH) and low molecular weight heparin (LMWH) are effective for VTE prophylaxis, but their relative efficacy and safety remain unclear. Methods: We conducted a meta-analysis of randomized controlled trials (RCTs) comparing UFH and LMWH for VTE prophylaxis in acutely ill medical patients. We searched MEDLINE, EMBASE, and Cochrane Central Register of Controlled Trials from 2013 to 2024. The primary outcome was the incidence of VTE. Secondary outcomes included major bleeding and mortality. Results: Seven RCTs with a total of 5,412 patients were included. LMWH was associated with a significantly lower risk of VTE compared to UFH (relative risk [RR] 0.68; 95% confidence interval [CI] 0.52-0.88; p = 0.004). There was no significant difference in major bleeding (RR 0.91; 95% CI 0.65-1.27; p = 0.58) or mortality (RR 0.93; 95% CI 0.78-1.11; p = 0.43) between the two groups. Conclusion: LMWH is more effective than UFH for VTE prophylaxis in acutely ill medical patients without increasing the risk of major bleeding or mortality. LMWH should be considered the preferred agent for VTE prophylaxis in this population.

Abstract

Seorang pasien laki-laki usia 34 tahun yang dirawat dengan dengan diagnosis deep vein thrombosis tungkai kanan dan sindrom nefrotik primer relaps. Trombosis pada sindrom nefrotik dapat timbul dari kebocoran protein dengan berat molekul yang tinggi sehingga menyebabkan hiperkoagulabilitas, peningkatan sintesis faktor yang mendorong trombosis, aktivasi dan agregasi trombosit, aktivasi sistem koagulasi, atau penurunan antikoagulan endogen. Pasien sudah diterapi dengan heparin dan terapi untuk sindrom nefrotik. Pasien dipulangkan dengan perbaikan klinis dan kontrol rutin ke poliklinik.

Abstract

Latar Belakang: leukemia mieloblastik akut merupakan salah satu keganasan darah yang terjadi akibat gangguan proses pertumbuhan sel mieloid, yang menyebabkan terjadinya penumpukan sel blast di sumsum tulang, dan menekan pembentukan sel darah lain yang menyebabkan anemia. Pengobatan yang diberikan pada penderita LMA adalah kemoterapi. Kondisi anemia dapat menyebabkan hipoksia di jaringan, yang dapat mengakibatkan berkurang nya efektivitas kemoterapi. Objektif: Tujuan dari penelitian ini adalah untuk menganalisis hubungan antara derajat anemia dengan tercapainya remisi komplit pada pasien LMA yang di kemoterapi induksi. Metode: Penelitian ini dilakukan dengan menggunakan desain cross sectional dengan pendekatan observasional analitik retrospektif. Sampel pada penelitian ini berjumlah 15 sampel yang diambil dari data rekam medik. Analisis univariate dilakukan untuk mendeskripsi frekuensi variable dan analisis bivariate dengan uji statistic fisher exact test. Hasil: Hasil yang didapat dari penelitian ini adalah pasien LMA banyak yang berusia >40 tahun, berjenis kelamin perempuan, dengan klasifikasi LMA M4, keluhan utama lemah letih, derajat anemia ringan dan berhasil remisi komplit yang lebih banyak. Hasil uji statistik memperlihatkan tidak adanya hubungan derajat anemia dengan tercapainya remisi komplit pada pasien. Hasil analisi LMA dengan nilai p=1,000. Kesimpulan: kesimpulan yang didapatkan adalah tidak adanya hubungan antara derajat anemia dengan tercapainya remisi komplit pada pasien leukemia mieloblastik akut dewasa yang menjalani kemoterapi induksi di RSUP Dr. M Djamil Padang.

Abstract

Background: Coronavirus Disease 2019 (COVID-19) is a respiratory system disease transmitted to humans and has infected humans in almost every country. Plasma cytokine levels, such as C-reactive protein (CRP), are elevated inflammatory markers in most COVID-19 patients and are routinely examined, especially in the intensive care Unit (ICU). Elevated CRP levels are associated with higher rates of severity and death. Objective: This study aimed to determine the correlation between CRP levels and outcomes in COVID-19 patients at COVID ICU RSUP. Dr. M. Djamil Padang Period July – December 2021. Method: This research was conducted using by analytic observational method with a retrospective cohort design. Secondary data was collected from the medical records of COVID-19 patients treated in the ICU COVID RSUP. Dr. M. Djamil Padang from July – December 2021, using total sampling as a technique, 107 samples met the inclusion criteria. Data analysis was carried out using univariate and bivariate methods. Result: The result shows more male patients (51.4%) than women (48.6%). From the age range, most age was found in the range > 59 years (54.2%). The most common type of comorbid disease was diabetes (32.7%). The highest CRP level at the initial ICU admission was >160 mg/L (70.1%), and the highest ferritin level was >434 ng/mL (86.0%). The most common patient outcome was death (65.4%). There is a relationship between CRP levels (p = 0.029) and the outcome of COVID-19 patients in the COVID ICU. Conclusion: This study concludes that there is a correlation between C-Reactive Protein (CRP) levels and Outcomes in COVID-19 patients at COVID ICU RSUP. Dr. M. Abstract

Background: Warm autoimmune hemolytic anemia (wAIHA) is caused by erithrocytes destruction by IgG or IgG plus C3d-mediated autoantibody. Objective: To present a case of warm autoimmune hemolytic anemia (wAIHA) in a patient with chronic hepatitis B infection. Methods: This is a case report that showed a special case. Results: Man, 64 y.o, hospitalized with diagnosis wAIHA and chronic hepatitis B. Diagnosis of wAIHA is built by: severe normocytic normochromic anemia, reticulocytosis 42.29%, increasing lactate dehydrogenase 397 U/L, direct Coombs test (+4), antibody screening IgG (+4). Anothers laboratorium findings: HbsAg (+), albumin 3.5 g/dL, ALT U/L, rapid HIV (-), anti HCV (-). There is no blast found in peripheral blood and bone marrow smear. Patient also diagnosed chronic hepatitis B inactive phase with negative HbeAg, HBV DNA 547 IU/L, normal liver finding in abdominal USG and moderate fibrosis from fibroscan. Washed red cell transfussion was administered, 2 x 125 mg methilprednisolone iv (3 days) tapering off until to 40 mg/day doses. He did not get antiviral prophylaxis and be planned to reevaluate in next 6 months. Conclusion: Steroid is first line therapy in wAIHA. When wAIHA is diagnosed, determination of chronic hepatitis B status should confirmed.  There is a risk of reactivation of chronic hepatitis B infection so monitoring is needed.

Abstract

Background: Diabetic ketoacidosis (DKA) is a state of metabolic decompensation/disorder characterized by the triad of hyperglycemia, acidosis and ketosis, caused by absolute or relative insulin deficiency and increased counter-regulatory hormones. Objective: To report a case of diabetic ketoacidosis (DKA) in a young adult patient with type 1 diabetes mellitus and scalp abscess, emphasizing the importance of early diagnosis and prompt management to achieve clinical improvement. Methods: This case report describes the clinical presentation, diagnostic approach, and therapeutic management of a 19-year-old male with DKA. Results: A 19-year-old male treated in the internal medicine ward of Dr. M djamil Padang with diabetic ketoacidosis, type 1 DM, and Abscess capitis. The patient was treated with rehydration and intravenous insulin infusion can show clinical improvement. Conclusion: The diagnosis in patients is based on anamnesis, physical examination and supporting examinations. Immediate therapy is needed in the management of DKA in patients. After DKA is resolved, look for the causes of the risk of DKA. On the third day of treatment the patient's condition improved clinically and the laboratory.
