The issue of patient-centered care remains the focus of various parties. This study aimed to explore the expectations of patients who have experienced a stroke, their family caregivers as recipients of home-based care services and healthcare workers as home-based care providers. This study used a qualitative explorative model. Semi-structured interviews were conducted with 19 participants, in addition to 4 focus group discussions. Data was analyzed using a content analysis approach with open code 13 software and the results were presented in COREQ systematics for qualitative studies. There were four main themes present in the results: physical well-being, self-actualization, psychological condition and balance between family burden and coping. Physical limitations were highlighted as the most burden some for the majority of the participants. At the time of the study, the majority of the patients, their family and health workers were diligently working towards to achieving the best outcome regarding physical well-being. Physical well-being is a fundamental need. Reaching this goal, could lead to better self-actualization, better physiological condition and improving the burden on families. Abstract

This study examines how taxpayer egoism affects the relationship between tax evasion intention and its determinants. Tax evasion intentions are assessed both as a composite variable and a latent variable. This study evaluates the direct relationship between attitude, subjective norm, & perceived behavioral control on tax evasion intentions. Furthermore, this study used taxpayer egoism as a mediating variable. All variables used are in accordance with the perspective of the Theory of Planned Behavior. This research uses Ordinary Least Square (OLS) using SPSS and Partial Least Square using smartPLS to examine the mediation between the independent variable and the dependent variable. The results showed that taxpayer egoism has a positive effect on tax evasion intentions. This result is consistent with OLS Results that show the positive effect of taxpayer egoism on tax evasion intention. The direct effect of all seven determinants of tax evasion intention on tax evasion intention is statistically significant. Moreover, all seven factors that determine the intention to engage in tax evasion have a statistically significant effect on tax evasion intention through the phenomenon of taxpayer egoism.

Abstract

Background: Hemodialysis is a kidney replacement therapy, but it has many complications identified, one of them is pain. Thus, we need non-pharmacological interventions that can be done by nurses, among them is progressive muscle relaxation (PMR), which is economically-efficient and no side effects. Objective: To determine the effect of progressive muscle relaxation on pain in hemodialysis patients. Methods: This is a quasi experimental design of time series design. The number of samples was 100 patients taken by simple random sampling. Progressive muscle relaxation (PMR) was given every day for 4 weeks. Research instrument  applied was Brief Pain Inventory. The data analysis employed independent t-test.Results: There was a decrease in pain level from week to week after the intervention, from the scale of 6 (moderate pain) to scale 3 (mild pain). There was a difference in the level of pain between the intervention group and the control group p <0.001.Conclusion: The therapy of non-pharmacological using a progressive muscle relaxation is proven to reduce pain in hemodialysis patients.

Abstract

Background: Non-compliance with both medical and non-medical therapy regimens, such as dietary management, can lead to elevated blood glucose levels in patients with diabetes mellitus. Additionally, disturbances in circadian rhythms can impact glucose metabolism. The combination of these factors requires further investigation to understand their role in increasing glucose levels. Understanding how dietary non-adherence and circadian rhythm disturbances affect glucose metabolism may help improve glycemic control. Objective: This study aims to assess dietary non-adherence and circadian rhythm disturbances in a patient with type 2 diabetes mellitus and uncontrolled blood sugar. Case: A 65-year-old male patient was admitted to the inpatient unit from the emergency department. He had been diagnosed with type 2 diabetes mellitus since 2002. Blood sugar monitoring was performed every 12 hours using a sliding scale. Despite receiving a hospital-prescribed diet, the patient consumed additional foods such as bread, donuts, biscuits, and milk from outside sources. The patient also exhibited an irregular sleep pattern, sleeping only about 5 hours per night, going to bed after 9 p.m., waking up frequently during the night, and eating snacks (e.g., small bread) before sleep. On the first day, the morning glucose level (GDS) was 313 mg/dL and 140 mg/dL in the evening. On the second day, the morning GDS was 228 mg/dL, and 138 mg/dL in the evening. On the third day, the morning GDS was 145 mg/dL. The patient was discharged in the afternoon with an increased insulin dose from 6 to 8 units. Conclusion: Dietary non-adherence and circadian rhythm disturbances are potential risk factors for elevated blood glucose levels in patients with type 2 diabetes mellitus. Providing health education, continuous monitoring, and support to both the patient and family can encourage adherence to medical regimens and improve health outcomes.
