Abstract

Remote digital health services, including Video Directly Observed Therapy (VDOT) and Wirelessly Observed Therapy (WOT), offer a promising alternative to traditional Directly Observed Therapy (DOT) for tuberculosis (TB) treatment adherence. This systematic review, following PRISMA guidelines, analyzed 10 randomized controlled trials (RCTs) involving 2,226 participants across seven countries. Results showed remote interventions improved medication adherence by 93% and increased sputum conversion rates to 73%. Most participants (84–100%) preferred digital services due to convenience, reduced travel burden, and better communication with healthcare providers. Remote digital health enables flexible, patient-centered care, particularly beneficial in areas with limited healthcare access, while addressing barriers of conventional DOT. However, successful implementation depends on robust technological infrastructure, continuous training for healthcare workers, and patient engagement strategies. Further multicenter studies are needed to validate these findings and guide policy adoption. This review highlights the efficacy and acceptability of remote digital health in TB care, advocating for its integration into treatment programs to enhance compliance and outcomes. Keywords: Remote digital health, pulmonary tuberculosis, medication adherence, health risks, health service

Abstract

Nursing assessment in critical patients in the ICU is a complex process that is essential for supporting standard interventions to improve the safety and quality of nursing care. This study aims to identify and analyze various literature related to nursing assessments and nursing care based on ICU service standards. A total of 293 articles were initially identified from various databases: 52 from PubMed, 145 from ScienceDirect, 83 from Google Scholar, 10 from Scopus, and 3 from ProQuest. After removing 85 duplicate articles, 208 articles remained. The screening process based on title identification, further reduced 55 articles. Subsequent abstract screening yielded 31 articles. Finally, a full text review of these articles resulted in 10 articles selected for inclusion. The literature review found that pre-arrival assessments help prepare for patient care needs before ICU arrival. Quick checklist based assessments can quickly and systematically identify early critical conditions in patients. Comprehensive assessments, using an integrated and ongoing patient assessment format, combined with progress notes and sophisticated monitoring tools, can enhance the efficiency of patient care. The conclusion is that implementing Nursing Assessment through a systematic approach to ICU services significantly contributes to optimizing the quality and safety of critical patient care.

Abstract

Introduction: Tuberculosis (TB) is a chronic infectious disease that can affect various organs and often leads to psychological stress. Spiritual Emotional Transcendence Care (SET Care), which integrates the SEFT approach with self-transcendence theory, is a relaxation technique aimed at reducing stress in TB patients.  Methods: This quasi-experimental study used a non-randomized pretest-posttest control group design. A total of 20 pulmonary TB patients were selected through purposive sampling and divided into treatment (n=10) and control (n=10) groups. Stress levels were measured using a modified DASS-42 stress subscale. Data were analyzed using the Wilcoxon signed-rank test and the Mann-Whitney U test with a significance level of p ≤ 0.05. Results: Demographic data showed equal gender distribution (50% male, 50% female), with most participants aged 44–55 years and educated up to senior high school. A significant reduction in stress levels was found in the treatment group after the intervention (p = 0.004), while the control group showed no significant change (p = 0.655). The Mann-Whitney U test confirmed a significant difference in post-test scores between groups (p = 0.000). Conclusions: SET Care effectively reduces stress in pulmonary TB patients and can be considered a complementary nursing intervention. Its integration into nursing practice supports holistic care by addressing emotional and spiritual well-being, potentially enhancing treatment adherence and patient outcomes.

Abstract

Introduction: Excessive and uncontrolled use of smartphones in adolescents can cause addiction to users. One negative influence on uncontrolled smartphone use is the occurrence of social anxiety. The purpose of this study was to analyze the relationship between smartphone addiction and social anxiety in adolescents.Method: This research method uses descriptive analysis design with cross sectional approach. The population in this study were adolescents of smartphone users in SMA Antartika Sidoarjo. The number of samples of this study were 289 respondents using probability sampling techniques with a type of simple random sampling. The independent variable in this study is smartphone addiction, while the dependent variable is social anxiety. Data obtained by questionnaire instruments using Smartphone Addiction Scale (SAS) and Social Anxiety Scale for Adolescent (SAS-A), and analyzed using spearmans rho with significance <0.05. Result: The results of the statistical test show the value of p = 0,000 and the value of r = 0,255. The conclusion of this study, there was a relationship between smartphone addiction and social anxiety in adolescents of SMA Antartika Sidoarjo. Conclusion: Adolescents who experience smartphone addiction always fail to control the use of smartphone and assume that smartphone is important, while smartphone addiction can cause social anxiety in the form of fear of negative evaluation by others. Nurses need to do health education for adolescents related to smartphone addiction and the negative effects it causes.

Abstract

During the COVID-19 pandemic which is a non-natural disaster, the ARDS cases (Acute Respiratory Disease Syndrome) rocketed. Despite the advantages of prone positioning for ARDS care, ICU staff seldom use it due to the unavailability of positioning tools, making it a challenging task for nurses. This study aims to investigate nurses' experiences with manual prone positioning and propose ICU-friendly prone position tools. This research method used a qualitative phenomenology theory study to 15 ICU nurses who were experienced in prone positioning at least 10 times to intubated patients and had complaints about the intervention. The experience was gathered using structured questions and recorded. The data was analyzed following Colaizzi’s thematic method, and all the emerged themes were collected and reported. The research results show that fifteen interviewed nurses noted that prone positioning is physically demanding and requires a coordinated team with a minimum of three staffs, 2 handles patient positioning, 1 leader securing the intubation tube. Before starting, the team assesses the difficulty level. Recommended tools include a slide sheet with handles, a modified Vollman prone positioner, and a crane-like prone harness. The conclusion is nurses find tools highly beneficial for prone positioning but will improvise with available resources like underpads as a ring-shaped pillow if tools are unavailable.
