Abstract

Biliary Atresia (BA) is extrahepatic cholestasis that results in death within the first two years if the diagnosis and intervention are delayed. The etiology and pathogenesis of BA are still undetermined. Viral infections, including Cytomegalovirus (CMV), are presumed to be one of the causes. Cytomegalovirus infection is more common in intrahepatic than extrahepatic cholestasis such as BA. There are limited data about Cytomegalovirus infection in cholestatic infants with BA. This study compared the incidence of CMV infection in cholestatic infants with biliary atresia and non-biliary atresia. A cross-sectional study was performed in December 2017 - August 2018 in cholestatic infants aged 1-6 months. Liver biopsy, histopathological examination followed by PCR CMV examination were performed on cholestatic infants. The results of the PCR examination were compared between BA and non-BA infants. Statistical analysis of Chi-Square, t-test independent and Mann-Whitney U resulting in p<0.05 were stated as significant. Thirty-seven children were obtained during the study period, consisting of sixteen children with BA and twenty-one children with non-BA. Biliary atresia was predominantly found in female than male children, despite no differences were found between the groups (p=0.163). There were differences in body weight (p=0.002) age (p=0.009), birth weight (p=0.02) and gestational age (p=0.03) between children with BA and non-BA. There was no significant difference in the incidence of CMV infection in cholestatic infants with BA and non-BA (p=0.338). Cytomegalovirus infection in cholestatic infants with BA was less than non-BA cholestatic infants.

Abstract

Highlights:1. In this study, new primers designed using the semi-nested polymerase chain reaction (PCR) method were utilized to identify MAGE-A11 and MAGE-A12 expressions in specimens collected from core biopsy, forcep biopsy, and bronchoalveolar lavage. 2. The histopathological analysis revealed positive expressions of MAGE-A11 and MAGE-A12 in specimens diagnosed with non-small cell lung cancer (NSCLC) as well as in specimens with no malignant cells. 3. This study provides evidence indicating that the detection of messenger ribonucleic acid (mRNA) of MAGE-A11 and MAGE-A12 by nested reverse transcription PCR can improve the accuracy of lung cancer diagnosis.   AbstractThe melanoma antigen gene (MAGE) belongs to the group of cancer-testis antigens that are exclusively expressed in germ cells but may be re-expressed in cancer cells. The highly expressed MAGE-A subfamily in lung cancer may potentially be a diagnostic and prognostic biomarker. This study aimed to identify MAGE-A11 and MAGE-A12 expressions in lung tumors obtained from core biopsy, forceps biopsy, and bronchoalveolar lavage specimens. A cross-sectional observational study was conducted on 90 patients clinically diagnosed with lung tumors. These patients received core biopsy, forceps biopsy, and bronchoalveolar lavage interventions after ethical approval was obtained. The complementary deoxyribonucleic acid (cDNA) quality was assessed by the polymerase chain reaction (PCR) of glyceraldehyde-3-phosphate dehydrogenase (GAPDH). The assessment was performed to ascertain if all specimens exhibited positive PCR amplification of the GAPDH gene. MAGE-A11 and MAGE-A12 were identified through a semi-nested reverse transcription PCR. The positive results were detected by measuring the PCR products, with MAGE-A11 and MAGE-A12 at base pairs (bp) of 858 and 496 in the first and second rounds, respectively. The expressions of MAGE-A11 and MAGE-A12 were observed in 3 (3.33%) and 40 (44.44%) out of 90 specimens, respectively. The prevalence rate of non-small cell lung cancer (NSCLC) was 31.11% (28/90). Among these cases, 3.57% (1/28) showed the expression of MAGE-A11, while 32.14% (9/28) exhibited the expression of MAGE-A12. Sixty-two (68.89%) out of 90 patients were diagnosed with no tumor cell malignancy. Out of 62 cases, 2 (3.23%) exhibited the expression of MAGE-A11, while 31 (50%) demonstrated the expression of MAGE-A12. MAGE-A11 and MAGE-A12 were detected in NSCLC and certain specimens with a pathological diagnosis that indicated the absence of malignant cells. In conclusion, MAGE A11 and MAGE A12 have potential markers to improve the pathological diagnosis of lung cancer. Further investigation is necessary to explore the expression of MAGE-A in correlation with lung cancer progression.

Abstract

Growing evidence suggests that microRNAs (miRNAs) play a pivotal role in viral infection. The objective of this study was to assess the association between the expression of miR-150, hsa-let-7e, and miR-146a on cytokine expression during dengue infection. Dengue virus (DENV) strain SJN-006, a serotype 2 DENV strain of the Cosmopolitan genotype, isolated in Bali, Indonesia, was used to infect peripheral blood mononuclear cells (PBMCs) isolated from healthy individuals. The relative gene expressions of miR-150, hsa-let-7e, and miR-146a as well as the gene expression of cytokines (IL-6, IL-8, IP-10, and MIP-1β) were determined using quantitative real time - polymerase chain reaction (qRT-PCR) at 6, 12 and 24 hours post infection (hpi). Correlations between the microRNAs and cytokines were analyzed by means of causality tests. Our data suggests that miR-150 and hsa-let-7e were significantly higher in infected-PBMCs after 12 hpi compared to the uninfected-PBMCs (p<0.05). The causality tests demonstrated that miR-150 and hsa-let-7e were negatively correlated with IL-8 expression, meanwhile miR-146a was the contrast. DENV infection was negatively and positively correlated with miR-150 and hsa-let-7e, respectively, after 24 hpi. In conclusion, our data demonstrates the vital role of miR-150, hsa-let-7e, and miR-146a in regulating IL-8 expression with possible different pathways.

Abstract

Inability to understand the pathogenesis of severe dengue, in particular the control mechanism of immune responses, has led to high mortality rate for patients with dengue shock syndrome (DSS). The aim of this study was to determine the control mechanism of cytokine production by mediator suppressor of cytokine signaling (SOCS), toll-like receptor 3 (TLR-3) and nuclear factor kappa B (NFκB) during DENV infection. Peripheral blood mononuclear blood cells (PBMC), isolated from healthy individuals, were infected with dengue virus (DENV)-2 strain SJN-006 Cosmopolitan genotype (isolated from Bali, Indonesia). The relative gene expression of SOCS-3, TLR-3, NFκB, and the cytokine genes (interleukin (IL)-6, IL-8, interferon inducible protein 10 (IP-10), and macrophage inflammatory protein-1 beta (MIP-1β)) were measured using qRT-PCR at 6, 12 and 24 hours post infection (hpi). Student t-test and Mann-Whitney test were used to compare the gene expressions while causal correlations were analyzed using regression test and path analyses. DENV-2 infection increased the gene expression of SOCS-3, TLR-3, and NFκB after 12 and 24 hpi. The expression of IL-6, IL-8, IP-10, and MIP-1β genes was increased and peaked at different times post-infection. NFκB and SOCS-3 genes likely have role in the upregulation of IL-8 and IL-6 gene expression, respectively. MIP-1β gene expression was significantly induced by both NFκB and SOCS-3. In conclusion, our study suggested that SOCS-3, TLR-3, and NFκB are important in regulating the production of IL-6, IL-8, IP-10, MIP-1β during early phase of DENV-2 infection. This enriches our understanding on pathogenesis pathway of DENV-associated cytokine storm.

Abstract

Breast cancer is the most common cancer found in the world and this cancer is the second cause of death due to cancer after lung cancer in Indonesia. This high mortality rate can be prevented by carrying out early detection. However, this is still not done well, especially in communities living in remote areas with low to medium economic and educational levels. The aim is to convey knowledge about breast cancer and training in the self-examination of the breast. It was held on Tuesday, 12 September 2023 at the Ngluyu Community Health Center, Nganjuk, from 07.00 to 14.00 WIB and was attended by 111 people. Data on breast cancer risk factors was collected through questionnaires. There were 96 participants who filled out the questionnaire, aged between 22-63 years. Risk factors data showed that 3.13% did not have children, 15.63% breastfed children less than 6 months, 5.21% were exposed to X-rays more than once a year; 1.04% had breast tumors, and 4.17% had family members suffering from cancer. In addition, 89.58% were over 25 years old and 42.71% had doing self-examination of breast. There was an increase in public understanding at the Ngluyu Community Health Center from 77.70% to 84.12%, namely an increase of 6.42%. This can increase public understanding about breast cancer and procedures to self-examination of breast and contribute in early detection of breast cancer as well as reducing the incidence of breast cancer. The results of this counseling can be used as recommendations by health centers in the Nganjuk district area.

Abstract

The objective of this study was to analyze the elevation of osteoblast activity in bone marrow mesenchymal stem cells (BM-MSCs) in osteogenic medium by physiological doses of melatonin administration by measuring alkaline phosphatase (ALP) and osteocalcin level.This studyused BM-MSCs from Rattusnorvegiccus femur bone. Rat BM-MSCs were cultured in a-Mem medium, differentiated in osteogenic medium, and administrated melatonin up to 21 days. This study was divided into 4 groups, K0 (control group), K1 (administrated of 25 nM melatonin), K2 (administrated of 50 nM melatonin), and K3 (administrated of 100 nM melatonin). Rat BM-MSCs were characterized CD 45- and CD 105+ marker using imunocytochemistry analysis and stained with Alizarin red after 15 days treatment. ALP and osteocalcin level were measured using ELISA Kit in days 21st.There weren't differences ofALP level beetwen groups and there are differences ofosteocalcin level between control groups (K0) withK1, K2, dan K3, and beetwen K1 and K2. The conclusion of this study was that there were an elevation of osteoblast activity in rat BM-MSCs in osteogenic medium by physiological doses of melatonin administration characterized by the elevation of osteocalcin level.

Abstract

The objective of this study is to analyze the expression of E6 Humanpapilloma virus (HPV), p53, and p16INK4A in cervical adenocarcinoma grade well differentiated (WD), moderately differentiated (MD), and poorly differentiated (PD). A cross sectional study conducted at Department of Anatomic Pathology, Dr. Soetomo General Academic Hospital Surabaya Indonesia using formalin fix paraffin embedded (FFPE) from cervical normal and cervical adenocarcioma grade WD, MD, and PD. The expression of E6 HPV, p53, and p16INK4A was performed by immunohistochemistry (IHC) staining. Data were analyzed with Kruskal-Wallis and continued with Mann-Withney test. The expression of E6 HPV in the cervical adenocarcinoma showed 35.9% specimens represented negative and 64.1% specimens represented positive. There was no significant difference in the expression of E6 HPV and p53 in cervical adenocarcinoma between grade WD, MD, and PD. The p16INK4A was overexpressed, shown as diffuse appearance in 89.7% of the specimens. There was a significant difference in the expression of p16INK4A between grade WD and MD with PD. In conclusion, some of cervical adecarcinoma were not caused by infection of HPV type 16 or 18 and the expression of p16INK4A might take a role in the developing of malignancy that caused by infection of HPV.

Abstract

Human papillomavirus (HPV) infection is one of the most common sexually transmitted infections. HPV high risk (HPVHR) were HPV16,18 related with invasive penile carcinomas, and HPV low risk (HPVLR) were HPV6,11 related to anogenital warts. Male infection is usually asymptomatic that it would be explain increasing the incidence of HPV associated cancers. Identification HPV genotype is very important for predicting the development of the diseases, to be benign or malignant cancer. The objective of this study was to identify the genotype of HPV that infect men with anogential warts. This research used 12 biopsy specimens from men patient with anogenital warts at Outpatient clinic of Department Dermatology and Venereology, Dr. Soetomo General Hospital period 2016-2017. The specimens were diagnozed by pathologist and HPV gentoyping was done to detect 40 HPV genotype including HPVHR and HPVLR. The result showed that 58% (7/12) were positive for HPVLR and 42% (5/12) were positive for HPV LR/HR. The genotype HPV that infected men patient with anogenital warts is HPVLR (HPV6,11) and HPVHR (HPV18,51,52,82) with single infection of HPVLR or mutiple infection HPVLR/LR or HPVLR/HR. The infection of HPVHR would be develops to be malignant transformation. It suggested that HPV genotype needs to be checked the for the anogenital warts cases for predicting the development of the diseases.

Abstract

Penelitian ini bertujuan untuk meningkatkan pemahaman masyarakat tentang deteksi dini kanker serviks dan evaluasi Pap smear, serta meningkatkan keterampilan pengambilan sampel Pap smear pada penyedia layanan kesehatan di Kecamatan Mawaddah, Kabupaten Mojokerto. Studi deskriptif observasional dengan desain cross sectional dilakukan pada 100 wanita berusia 20-70 tahun dari Rumah Sakit Mawaddah. Penilaian dilakukan sebelum dan sesudah pemaparan materi. 100 sampel Pap smear yang dikumpulkan oleh bidan setempat dievaluasi untuk menilai kecukupan spesimen menggunakan sistem Papanicolaou dan Bethesda. Kegiatan ini secara signifikan meningkatkan pemahaman tentang kanker serviks (8,05% menjadi 82,26%), sementara kegiatan pelatihan tenaga medis meningkatkan pengetahuan mengenai pengambilan sampel Pap smear (51,62% menjadi 100%). 99% dari sampel Pap smear yang dikumpulkan oleh tenaga medis di Kecamatan Mawaddah dinilai memadai dan memuaskan. Program pengabdian masyarakat yang berfokus pada deteksi dini kanker serviks melalui Pap smear, secara efektif meningkatkan pemahaman masyarakat, meningkatkan pengetahuan dan keterampilan pengambilan sampel oleh tenaga medis, yang dapat meningkatkan pula efektifitas layanan Pap smear di Rumah Sakit Mawaddah pada masyarakat.

Abstract

Kegiatan pengabdia masyarakat ini bertujuan untuk meningkatkan pemahaman tenaga kesehatan dan masyarakat mengenai deteksi dini serta pencegahan kanker serviks. Pelatihan tenaga kesehatan dilakukan agar mereka dapat melakukan prosedur Pap smear dengan benar dan sesuai standar, sementara penyuluhan masyarakat bertujuan untuk meningkatkan kesadaran akan pentingnya deteksi dini dan pencegahan kanker serviks. Studi ini menggunakan metode deskriptif observasional dengan pendekatan cross-sectional, mencakup dua kegiatan utama: pelatihan tenaga medis secara daring dan penyuluhan masyarakat secara langsung. Pelatihan Pap smear diikuti oleh 51 peserta yang terdiri dari perawat, bidan, dan dokter. Evaluasi sebelum dan sesudah pelatihan menunjukkan peningkatan pemahaman yang signifikan, di mana peserta dengan skor 80-100 meningkat dari 58,82% menjadi 82,35%. Penyuluhan kanker serviks diikuti oleh 74 peserta. Sebelum penyuluhan, hanya 12,16% peserta yang memperoleh nilai 80-100, namun setelahnya meningkat menjadi 100%. Sebanyak 62,2% peserta telah menikah, 8,1% memiliki riwayat hubungan seksual sebelum usia 17 tahun, dan 9,5% memiliki lebih dari satu pasangan. Selain itu, 98,6% belum pernah menerima vaksin HPV, dan 78,4% jarang melakukan Pap smear, menunjukkan rendahnya kesadaran pencegahan kanker serviks. Setelah penyuluhan, 60 peserta menjalani skrining kanker serviks. Mayoritas (41,7%) berusia 31-40 tahun. Hasil skrining dengan sistem Bethesda menunjukkan 100% NILM. Berdasarkan klasifikasi Papanicolaou, 88,1% termasuk kelas II (96,2% inflamasi, 1,9% bacterial vaginosis, 1,9% atrofi), dan 11,1% kelas I. Program ini terbukti efektif dalam meningkatkan pemahaman tenaga kesehatan dan masyarakat, serta keterampilan tenaga kesehatan dalam prosedur Pap smear, yang berkontribusi pada peningkatan kualitas layanan deteksi dini kanker serviks di RSIA Hikmah Sawi Bangkalan.
