Abstract

Nausea and vomiting should be further checked and prevented, because these conditions can be a cause or complication during hemodialysis patients. Electrolyte and water imbalance (dehydration) can be considered as the main complication of nausea and vomiting itself. The aim of this study was to determine the factors that influence the severity of nausea and vomiting in patients undergoing hemodialysis. This type of research is quantitative with analytic descriptive method, with an approach using a retrospective cohort research design. The sampling technique in this study was a total sampling technique with a total sample of 57 patients. The research instrument used questionnaires and observations, data analysis using the Chi Square test and simple logistic regression analysis. The location of the study was the hemodialysis therapy site at the Islamic Hospital in Purwokerto in January-February 2020. The results showed that most of the respondents with the severity of nausea and vomiting, the patient's diet was not balanced (61.4%), the excess fluid had edema (63.2%), the gender was male (59.6%), female (40.4%). %), age 18-65 (57.9%), duration of HD <12 months (64.9%) and 58 respondents> 12 months (35.1). The results of bivariate analysis using logistic regression, obtained p value of the patient's diet 0.009, excess fluid, age and sex 0.001 and the duration of HD 0.003. From this analysis, there are 5 factors that can cause the severity of nausea and vomiting, namely the patient's diet, excess fluidity, gender, age, and duration of HD.

Abstract

Therapeutic communication in intensive rooms was a therapeutic communication initiated by nurses to patients with intensive conditions whichrequired critical thinking, fast and precise measures in coping with each patient's needs. To figure out the nurses’ perceptions of the application of therapeutic communication in Intensive room.The study was qualitative phenomenological study, involving eight intensive care nurses participants from IGD, HCU, ICU and NICU. The inclusion criteria were nurses who had worked at least 1 year in the intensive unit, with a minimum education level of bachelor in nursing, through in-depth interviews. The data were analyzed using the Colaizzi method. There were 4 themes identified in the nurses’ perception of the application of therapeutic communication: communication to help the healing process, adaptation and information, stages, technical and benefits of communication. The communication which had not been widely explored wasthe one to help the patients' adaptation process to illness and the communication in emergency situations. The therapeutic communication model is adjusted to the goals and characteristics of patients. Especially for the patients in emergency condition, the nurses are required to have sensitivity and communication skills which have not been sufficiently explored. The communication tends to be directional, not interactive, while the non-verbal communication has not been carried out optimally.

Abstract

Background: Blended learning is a new system that combines face-to-face and online learning. This new system in the Muhammadiyah Purwokerto University (UMP) requires evaluations from the students’ feedback. Students' motivation was the influential factor on both accomplishments and presences in online learning. Students’ motivation, satisfaction, and knowledge of blended learning were the components for the effectiveness of the course.Objective: The study aims to describe the level of motivation, knowledge, and learning satisfaction, and also to observe the correlation of motivation on blended learning with knowledge and learning satisfaction.Methods: This quantitative research used a cross-sectional approach. Data collection techniques used were questionnaires in the google form. The researcher made the questionnaire and has been tested for validity on 30 respondents with r-value of >0.3 (Pearson product-moment). Cronbach alpha for the reliability was ranged from 0.750 to 0.828, so it is valid and reliable to use. The sample was 254 nursing students from the 3rd, 5th, and 7th semesters selected by quota sampling. Data were analyzed by Pearson product-moment correlation.Results: The results showed that students obtained moderate motivation (71.3%), knowledge (81.5%), and satisfaction (70.5%). There was a significant correlation between motivation with knowledge (r=0.650, p<0.01) and learning satisfaction (r=0.636, p<0.01) on blended learning.Conclusion: The blended learning method can be used as a teaching and learning method for future nursing education, followed by evaluation. Furthermore, the lecturer should consider the students’ characteristics and expectations toward blended learning.

Abstract

Fatigue and inadequacy dialysis are common problem in hemodialysis patients. The dialysis inadequacy can cause an increased progression of impaired renal function, as well as the increased morbidity and mortality, and declining productivity of hemodialysis patients. Fatigue prevalence ranged from 44,7–97% from mild to severe. Fatigue is a common complaint of hemodialysis patients that can lower physical function and life quality. To determine the correlation between adequacy and the fatigue level of the patients with End Stage Renal Disease (ESRD) undergoing hemodialysis. This study used a descriptive analytic and cross sectional approach involving 75 respondents and the FACIT-G Questionnaire was used to collect the data. The inclusion criteria are male and female patients aged 18–70, undergoing hemodialysis for more than 3 months with a frequency of 2 times at least 4 hours, composmentis patients. The adequacy hemodialysis was assessed using the Kt/V formula. All data were collected during the session of hemodialysis. Pearson Product moment test wes used to analyze the data. The mean dialysis adequacy was 1.43±0.380, 57(76%) only 13 (17.3%) patients had adequate dialysis (minimum laboratory standard Kt / v = 1.8) and inadequate were 62 (82.7%) patients. The mean of fatigue was 20.07 and 62 (82.7%) respondents experienced severe fatigue. There was no significant correlation between adequacy and the fatigue level of the patients with ESRD undergoing hemodialysis with p value 0.504 (α> 0.05). Mostly patients had inadequate dialysis, both adequate and inadequate dialysis patients had experience fatigue from mild to severe. Multiple individuale and personnel factors affect dialysis adequacy directly or conversely.

Abstract

The purpose of this study to determine the relationship of fatigue with depression. This  descriptive correlative analytic research with 78 samples. The questionnaire used was Functional Assessment of Chronic Illness Therapy-G and Zung Self-rating Depression Scale. The mean fatigue score was 24.21 and 44.50 for depression. Pearson correlation result showed r=0.646 (p0.05). Logistic regression analysis showed R=4.18%, which means depression predicted about 40% by fatigue, and 60% influenced by other variables. There is a strong relationship between fatigue and depression. Management of depression and fatigue should be standard in patient care.

Abstract

Desa Melung terletak di lereng kaki Gunung Slamet, dan merupakan desa berisiko mengalami bencana, baik tanah longsor, gempa, maupun terkena letusan gunung Api Slamet. Dalam pengembanganya desa Kedungbanteng adalah bagian desa binaan Fakultas ilmu Kesehatan Universitas Muhammadiyah Purwokerto dengan keunggulan Keperawatan Kegawatdaruratan. Sangat penting keberlanjutan pembinaan desa Melung sebagai desa Mitra, sinergisitas kerjasama dalam pembinaan desa mitra ini akan menjadi tolak ukur keberhasilan dan perwujudan desa Melung sebagai perwujudan desa siaga bencana. Metode pelaksanaan kegiatan dilakukan dengan mempersiapkan sumber daya yang kompeten dalam mengelola manajemen kesiapsiagaan dan mitigasi menghadapi bencana. Pemetaan kebencanaan secara komprehensif baik dari risiko dan dampak bencana (Analisis kerentanan Bencana). Penyediakan sarana dan prasarana yang mendukung peningkatan simulasi kesiapsiagaan menghadapi bencana dan menyusun RENSTRA sebagai Desa Siaga Bencana. Kegiatan dilaksanakan selam dua kali yaitu pada tanggal 8 Maret 2020 dan 29 Juli 2020. Setelah dilaksanakan kegiatan PPDM, terjasi peningkatan pengetahuan warga desa Melung, Pengaktifan kembali Forum Pengurangan Risiko Bencana (FPRB) desa Melung dalam aktifitas desa siaga bencana dan pemetaan bencana juga pembuatan RENSTRA desa siaga bencana.

Abstract

Lack of sleep in patients in the Intensive Care Unit (ICU) is associated with dysfunction of the immune, cardiovascular, metabolic, memory, and cognition systems, as well as increased mortality. Various factors that are thought to be related to patient sleep quality come from patient and environmental factors. The purpose of this study was to determine the factors that affect the sleep quality of ICU patients. This research is a quantitative study, using a cross-sectional approach, with as many as 44 patients involved in this study. The research instrument used the observation sheet and The Richards-Campbell Sleep Questionnaire (RCSQ) is also equipped with a thermohygrometer to measure temperature and humidity and a sound level meter to measure noise. Data analysis used the Spearman Rank test, and research ethics was obtained from the Ethics Commission of RSMS Margono. The results showed that most of them were women (75%), aged ≥40 years (68.2%) with the most days of stay less than 4 (93.2%), postoperative (95.5), and with poor sleep quality (72.7%). Factors that affected the sleep quality of the patient are heart rate (0.030), MAP (0.042), patient pain score (0.031), and sleep position (0.002). There was no affecting the SpO2 factor, temperature humidity, noise, patient perceptions of noise, light, and action with p value> 0.05. In conclusion, most of the respondents have poor sleep quality. There is an effect of the heart rate, sleep position, and pain score on the patient's sleep quality. It is necessary to apply a combination of pharmacological management and nursing measures to improve sleep quality.

Abstract

Clinical and laboratory learning is imperative in achieving the competency target of nursing students. Due to the COVID-19 pandemic, learning must be done online; therefore, the practice of laboratory and clinical learning could only be conducted somewhat. The development of virtual learning media that minimizes face-to-face and represents clinical and laboratory situations is urgently needed nowadays. This research aims to develop a VRS storyboard as the media for learning cases of the cardiovascular system. This research followed Research and Development (R&D), which were shortened into 5 steps (literature review, concept creation, concept model development, model testing and evaluation, and content model evaluation). The storyboard and case framework was developed from the results of FGDs and workshops and based on the NLN/ Jeffries Simulation model. The study's findings indicate the availability of nursing care storyboards about cases of Cardiovascular Disorders for student simulations based on best nursing practices consisting of learning objectives, scenes, actions, challenges, and elements of transfer, as well as opportunities to ask questions. The storyboard format allows interactive learning in terms of material, practice in nursing care skills, and nursing actions in a safe and immersive environment during the COVID-19 pandemic.

Abstract

Tersedak merupakan kejadian yang dapat dicegah dan penyebab morbiditas dan mortalitas pada kelompok anak. Tersedak adalah kondisi kegawatdaruratan pada anak yang harus ditangani dengan cepat karena dapat menyebabkan kekurangan oksigen (hipoksia). Keterlambatan dalam penanganan tersedak pada anak menyebabkan kematian. Kegiatan ini merupakan upaya upaya promotif dan preventif dalam menurunkan angka kejadian meninggal dan kasus kegawatdaruratan tersedak pada anak. Tujuan kegiatan adalah untukmeningkatkan kemampuan pemahaman ibu-ibu kader aisyiyah terakit pengetahuan dan penanganan kegawatdaruratan tersedak pada anak. Program Ipteks bagi masyarakat ini dilakukan dengan metode ceramah, diskusi dan simulasi/demonstrasi terkait kegawatdauratan tersedak dan penangannanya. Alat ukur atau evaluasi keberhasilan program dilihat dari peningkatan pengetahuan pre dan post test edukasi dan pelatiahan. Sekaligus peserta dapat mempraktikan tindakan pencegahan dan penanganan kegawatdaruratan tersedak pada anak dengan metode back blow dan heimlick manuver. Hasil kegiatan menunjukkan keberhasilan pelaksanaan kegiatan dilihat dari hasil kuisioner yang disebarkan kepada ibu-ibu kader Aisyiyah yang diberikan sebelum dan setelahdilakukan kegiatan. Berdasarkan hasil didapatkan data, terjadi peningkatan pengetahuan masyarakat dari nilain16 menjadi 19 (gambar 1) dengan target skor maksimal adalah 21. Parameter evaluasi lain menunjukan ibu–ibu kader Aisyiyah dapat memamhami dna mempraktikan tindakan pencegahan dna penanganan kegawatdaruratan tersedak pada anak dengan metode back blow dan heimlick manuver.Â

Abstract

 Discharge planning ini menjadi hal yang wajib dalam akreditasi Rumah Sakit (RS), dan pelayanan kesehatan, setiap asien yang masuk kerumah sakit akan mendapatkan layanan Kesehatan termasuk didalamnya adalah discharge planning yang menjamin keberlangsungan layanan rumah sakit. Adapun permasalahan yang dihadapi oleh mitra adalah Perawat RS sudah melakukan discharge planning namun masih belum sesuai dengan standar kompetensi sehingga kurang komprehensif dan berdampak pada kualitas. Solusi yang ditawarkan diantaranya dengan mengadakan pelatihan discharge planing sesuai dengan standar dan kompetensi terkini dengan berbasis evidence base nursing secara daring dna luring sehingga menjadi solusi mitra dan masukan pada kurikulum keperawatan. Melakukan pelatihan tentang metode discharge planning dan evaluasinya sesuai dengan standar dan kompetensi terkini dengan berbasis evidence base nursing. Evaluasi program pelatihan melalui kuesioner dan demonstrasi. Berdasarkan hasil penerapan IBM didapatkan data, adanya peningkatan pengetahuan kompetensi rata rata discharge planning dari nilai 44,59 menjadi 69,54 dengan target skor maksimal adalah 83. Padas indikator evaluasi lain menunjukan perawat dapat memamahami dan mempraktikan teknis pelaksanan discharge planning pada pasien.
