Abstract

Stunting remains a critical public health issue in Indonesia, with a national prevalence of 19.8% in 2024 and 24.7% in urban areas, such as Palu City. Maternal behaviors play a pivotal role in prevention, yet no validated instrument has measured stunting prevention behaviors based on behavioral theory. To develop and validate a Theory of Planned Behavior (TPB) based instrument to assess stunting prevention behaviors among mothers of children under five in Indonesia. An instrument development and validation study was conducted in Palu City, Sulawesi Tengah, from May to December 2025. The study followed five phases: (1) literature review and blueprint construction (50 items), (2) content validation by five experts using Aiken’s V, (3) pilot testing with 30 mothers, (4) large-scale data collection from 300 purposively selected mothers, and (5) psychometric validation using Exploratory Factor Analysis (EFA) and Confirmatory Factor Analysis (CFA). Data analysis was performed with R software. Expert review confirmed 45 items (90%) valid (Aiken’s V ≥ 0.80). Pilot testing showed high reliability (Cronbach’s α = 0.922). EFA and CFA supported a five-factor structure consistent with TPB, with good model fit (CFI = 0.974, TLI = 0.973, RMSEA = 0.019, SRMR = 0.075). Internal consistency was strong (Cronbach’s α = 0.884). The TPB-Stunting Instrument is a reliable, valid, and culturally relevant tool for measuring maternal behaviors in stunting prevention programs.

Abstract

Background: Human Immunodeficiency Virus (HIV) and Acquired Immune Deficiency Syndrome (AIDS) remain global health challenges, particularly among men who have sex with men (MSM), who face biological and social vulnerabilities. In Indonesia, MSM represent a key population with high HIV prevalence, and Palu City reports the highest number of HIV cases in Central Sulawesi, underscoring the need for targeted prevention. Objectives: This study examined the relationship between knowledge, attitudes, infrastructure availability, healthcare provider support, peer influence, stigma/discrimination, and family support with HIV/AIDS preventive behavior among MSM at Talise Health Center, Palu. Methods: An explanatory mixed-methods design was used. The quantitative phase applied a cross-sectional approach with total sampling of 30 MSM, analyzed using univariate and bivariate (Chi-Square) tests. The qualitative phase involved case studies with in-depth interviews of 10 informants. Results: Overall, 43.3% of MSM showed good preventive behavior. Significant associations were found between infrastructure availability, peer influence, family support, and stigma/discrimination with HIV/AIDS preventive behavior (p<0.05). No significant relationship was observed for knowledge or healthcare provider support (p>0.05). Qualitative findings revealed that although MSM possessed adequate knowledge, this did not consistently lead to preventive actions. Conclusions: HIV/AIDS preventive behavior among MSM in Talise remains suboptimal. Key influencing factors include attitudes, family support, infrastructure, peer influence, and stigma/discrimination. Strengthening empowerment programs to reduce stigma and enhance community support is essential to improve prevention efforts.

Abstract

Background: Stunting is a global malnutrition issue affecting children's development worldwide. Machine learning (ML) has been applied to predict stunting; however, studies on international collaboration and ML application across various geographical contexts remain limited. Objective: This study aims to analyze publication trends and collaboration patterns in stunting prediction research using machine learning and identify the most relevant ML methods. Methods: A bibliometric analysis was conducted using the Scopus database, focusing on publications from 2019 to 2024. The data were analyzed through descriptive statistics and science mapping, including trend topics, co-occurrence networks, and international collaboration analysis. Results: The study found that regression-based algorithms and deep learning are the most widely used machine learning methods for stunting prediction. International collaborations between countries with high stunting prevalence, such as Indonesia, Bangladesh, and Ethiopia, were also identified. Conclusion: This study highlights the importance of developing locally tailored predictive models and strengthening international collaboration to improve the effectiveness of stunting prediction models. Cross-sector and interdisciplinary collaborations are also essential for more holistic solutions.

Abstract

Public hospitals are under sustained pressure to reconcile fiscal sustainability, service quality, and equitable access. New Public Management (NPM) reforms influence tariff setting by expanding managerial autonomy, output-based financing, performance measurement, and market-like incentives. This review aims to synthesize evidence on how NPM-oriented hospital tariff and provider-payment policies influence patient experience and patient satisfaction in public hospitals. Methods: A systematic review was conducted according to PRISMA 2020. Scopus, Web of Science, PubMed/MEDLINE, ScienceDirect, and SAGE Journals were searched for English-language literature published from January 2019 to June 2026, while seminal older works were retained for theoretical framing. After duplicate removal and screening, 45 sources were included in thematic synthesis, comprising 31 primary empirical studies, 8 review articles, and 6 conceptual or policy analyses. Tariff and payment reforms may improve cost control, technical efficiency, and managerial responsiveness, but the included literature also links poorly calibrated tariffs with cost shifting, service selection, shortened length of stay, coding pressure, and distrust when payment rules are opaque or misaligned with actual service costs. Patient experience described what patients actually encountered during care, including access, waiting, communication, billing clarity, and affordability, whereas patient satisfaction reflected evaluative judgment against expectations and perceived value. Hospital tariff reform is a governance intervention rather than a purely technical pricing exercise. Sustainable reform requires periodic cost review, quality-sensitive payment design, transparent patient-facing communication, and safeguards for equity.
