Abstract

Introduction: Diabetes mellitus (DM) is one of the major non-communicable diseases with a steadily increasing prevalence in Indonesia, including in island regions with limited access to healthcare services. This community service program was conducted on Lancang Island, Kepulauan Seribu. Objective: The purpose of this service was of improving elderly participants’ knowledge of diabetes management, conducting early detection through health screening, and providing education on healthy lifestyle practices. Method: The methods included interactive health education sessions and physical examinations covering random blood glucose, blood pressure, and cholesterol levels. Result: The results showed that the majority of the 101 elderly respondents exhibited significant chronic health risk factors. The average blood pressure was 152/88 mmHg, indicating a tendency toward hypertension. The mean random blood glucose level of 140 mg/dL suggested a predisposition to prediabetes and diabetes mellitus, while the mean cholesterol level reached 253 mg/dL, categorized as high. Conclusion These findings indicate that most elderly participants have not yet implemented optimal dietary management and physical activity, further exacerbated by limited health facilities and access in island areas. The educational intervention effectively increased participants awareness of disease risks and complications, as well as the importance of early detection. Overall, the promotive and preventive activities had a positive impact on improving knowledge, awareness, and motivation among elderly participants to adopt healthy lifestyles. This program is expected to serve as a foundation for sustainable community health development efforts in island regions.

Abstract

Background: Anxiety in patients with chronic kidney disease (CKD) is not only a psychological problem but also a serious clinical threat because it triggers hemodynamic instability. When patients experience intense anxiety before an invasive procedure such as double-lumen catheter (DLU) placement, the sympathetic nervous system is activated, increasing blood catecholamine levels, leading to spikes in blood pressure and tachycardia. This condition is particularly risky for CKD patients who already have cardiovascular complications. Several factors can trigger anxiety and depression in hemodialysis patients, including frequent comorbidities, frequent hospitalizations, new experiences with invasive procedures, chronic pain, sleep disturbances, chronic inflammation, increased fatigue, decreased sexual function, uremia, and lack of family support in daily life. Purpose: To determine the effectiveness of music therapy in reducing anxiety in patients with chronic kidney disease undergoing venous access placement. Method: This study used an evidence-based nursing practice (EBNP) approach with a quasi-experimental pre-post-test design, conducted on 14 participants selected through purposive sampling based on inclusion criteria. Music therapy was administered during the venous access/CDL surgical procedure for 30 minutes. Anxiety was measured using the HARS and VAS questionnaires. Data were analyzed using univariate and bivariate tests, including paired t-tests and Wilcoxon tests. Results: The prevalence of anxiety decreased from 60% to 40%, with HARS and VAS measurements decreasing from 48% to 43% after the intervention. The paired t-test and Wilcoxon test showed p-values ​​of 0.01 and 0.00, respectively. Conclusion: Music therapy can reduce anxiety in hemodialysis participants. Applied systematically and supported by scientific evidence, music therapy has a positive effect on anxiety in patients with chronic kidney disease (CKD) undergoing venous access. Suggestion: Further exploration of the physiological and psychological mechanisms of music therapy is needed, as well as consideration of other factors, such as pain, sleep quality, depression levels, and medication adherence, which may influence anxiety levels in hemodialysis patients undergoing first-time venous access.   Keywords: Anxiety; Chronic Kidney Disease Patients; Music Therapy; Venous Access Insertion.   Pendahuluan: Kecemasan pada pasien Penyakit Ginjal Kronis (PGK) bukan sekadar masalah psikologis, melainkan ancaman klinis yang serius karena memicu instabilitas hemodinamik. Saat pasien mengalami kecemasan hebat menjelang tindakan invasif seperti pemasangan catheter dual lumen (CDL), sistem saraf simpatis akan teraktivasi, meningkatkan kadar katekolamin dalam darah yang berujung pada lonjakan tekanan darah dan takikardia. Kondisi ini sangat berisiko bagi pasien PGK yang sudah memiliki komplikasi kardiovaskular. Beberapa faktor yang bisa memicu kecemasan dan depresi pada pasien hemodialisis, antara lain penyakit penyerta yang sering timbul, seringnya dirawat di rumah sakit, pengalaman baru terhadap tindakan invasif, nyeri kronik, gangguan tidur, peradangan kronik, peningkatan kelelahan, penurunan fungsi seksual, uremia, dan kegagalan dukungan keluarga dalam kehidupan sehari-hari. Tujuan: Untuk menerapkan terapi musik efektif menurunkan kecemasan pada pasien penyakit ginjal kronis yang menjalani pemasangan akses vena. Metode: Penelitian ini menggunakan pendekatan evidence-based nursing practice (EBNP) dengan desain quasi-experiment pre-posttest, dilakukan pada 14 partisipan terpilih berdasarkan purposive sampling sesuai kriteria inklusi. Terapi musik diberikan selama proses operasi pemasangan akses vena/CDL selama 30 menit, kecemasan diukur menggunakan kuesioner HARS dan VAS. Data dianalisis menggunakan uji univariat dan bivariat dengan uji paired t-test dan uji Wilcoxon. Hasil: Prevelansi tingkat kecemasan 60% menurun menjadi 40% dengan pengukuran HARS dan VAS 48% menurun menjadi 43% setelah intervensi. Uji paired t-test dan Uji wilcoxon menunjukkan nilai p=0.01 dan p=0.00. Simpulan: Terapi musik dapat menurunkan kecemasan pada partisapan hemodialisis, diaplikasikan secara sistematis dan didukung oleh bukti ilmiah untuk menurunkan kecemasan terapi musik memiliki efek positif terhadap kecemasan pasien penyakit ginjal kronis yang menjalani pemasangan akses vena. Saran: Perlu mengeksplorasi lebih jauh mekanisme fisiologis maupun psikologis dari penerapan terapi musik, serta mempertimbangkan faktor-faktor lain, seperti nyeri, kualitas tidur, tingkat depresi, dan kepatuhan terhadap pengobatan yang mungkin berpengaruh terhadap tingkat kecemasan pada pasien hemodialis yang akan menjalani pemasangan akses vena pertama kali.   Kata Kunci: Kecemasan; Pasien Penyakit Ginjal Kronis; Pemasangan Akses Vena; Terapi Musik.

Abstract

Hemodialysis is the most successful and most frequent therapy used as form therapy replacement kidney therapy hemodialysis own One of the impacts felt by patients is that is cramps muscle cramps is one of the symptom the most common physical problems experienced patient dialysis. This is contraction muscle sudden that is not deliberate and marked with painful great thing happened repeatedly in patients hemodialysis. The purpose of study This analyze influence implementation implementation stretching exercise intradialytic in repair symptom cramps muscles in patients undergoing hemodialysis at Al Ihsan Regional Hospital Bandung. Data collection uses questionnaire, observation and interviews. Research sample as many as 15 patients shared into 2 groups. Research conducted at Al Ihsan Regional Hospital Bandung, room intermittent hemodialysis May -August 2025. results testing use one-way ANOVA test sig. value < 0.05 so that can concluded that there is influence significant before and after given stretching exercise intervention on cramps. Multivariate test on week first and second p value 1,000 or > 0.05 then No There is influence, on week to three 0.106>0.05 no There is influence and on the week to four p values 0.015<0.05 exist influence. Implementation Intradialytic exercise range of motion (ROM) intervention according to SOP shows success in a way practical All participants who received intervention report feeling more comfortable, calm, relaxed, and not too focus to action will therapy hemodialysis.

Abstract

Background: Diabetes mellitus (DM) is a chronic metabolic syndrome characterized by hyperglycemia resulting from impaired insulin secretion or function. Long-term uncontrolled blood glucose levels carry a risk of triggering microvascular complications such as diabetic peripheral neuropathy (DPN), which is characterized by nerve damage and reduced sensitivity. Objective: This study aims to analyze the relationship between hyperglycemia status and the prevalence of peripheral neuropathy in Type 2 DM patients at the outpatient unit of RSIJ Cempaka Putih. Methods: A descriptive correlational study design with a cross-sectional approach was used. The study population consisted of all Type 2 DM patients at the RSIJ Cempaka Putih Outpatient Unit, with a sample size of 94 respondents selected via purposive sampling. The research instruments included medical record review for blood glucose levels and the 10-g monofilament test for neuropathy detection. Results: The majority of respondents were male (51.1%), with an average age of 58.73 years, and had had DM for >5 years (56.4%). A total of 53.2% of respondents were in a state of hyperglycemia ( mg/dL) and 70.2% of respondents had peripheral neuropathy. Chi-square test results showed a statistically significant association between hyperglycemia status and the occurrence of peripheral neuropathy, with a p-value of 0.001 (p < 0.05). Conclusion: There is a significant association between hyperglycemia and the development of peripheral neuropathy. These findings suggest that unstable blood glucose levels are a major determinant of nerve damage in patients with diabetes, underscoring the need for improved glycemic management and foot care education to prevent further complications.

Abstract

ABSTRAK Penderita diabetes melitus memiliki risiko tinggi mengalami komplikasi neuropati kaki yang dapat menyebabkan ulkus hingga amputasi jika tidak dideteksi secara dini. Kader kesehatan memiliki peran strategis sebagai garda terdepan dalam pemantauan kesehatan masyarakat, namun seringkali masih menghadapi keterbatasan dalam pengetahuan teknis dan keterampilan deteksi dini. Kegiatan pengabdian masyarakat ini bertujuan untuk mengoptimalkan peran kader kesehatan dalam deteksi dini neuropati kaki diabetes melalui edukasi dan keterampilan pemeriksaan sederhana di RT 06 RW 06 Kelurahan Cempaka Putih Barat, Jakarta Pusat. Kegiatan dilakukan melalui beberapa tahapan yang dimulai dengan penyuluhan kesehatan dan simulasi pemeriksaan fisik kaki. Pasca kegiatan ini, diketahui telah terjadi peningkatan pengetahuan dan keterampilan kader yang signifikan terkait deteksi dini dan manajemen penyakit. Kegiatan pengabdian ini penting dijalankan untuk meningkatkan kemampuan kader dalam mencegah komplikasi diabetes sehingga dapat meningkatkan kualitas hidup masyarakat. Kesimpulannya, pemberian edukasi teknis dan pelatihan keterampilan efektif mengoptimalkan peran kader kesehatan dalam melakukan manajemen dan deteksi dini neuropati kaki penderita diabetes. Kata Kunci: Kader Kesehatan, Diabetes Melitus, Neuropati Kaki, Deteksi Dini, Edukasi.  ABSTRACT Diabetes mellitus patients are at high risk of developing diabetic foot neuropathy complications, which can lead to ulcers and even amputation if not detected early. Health volunteers (kader kesehatan) have a strategic role as the front line in community health monitoring, yet they often face limitations in technical knowledge and skills for early detection. This community service activity aims to optimize the role of health volunteers in the early detection of diabetic foot neuropathy through education and simple examination skills in RT 06 RW 06, Cempaka Putih Barat, Central Jakarta. The activity was conducted through several stages, including health education and simulation of physical foot examination. Following the activity, there was a significant increase in the knowledge and skills of the volunteers regarding early detection and management. This community service is essential to enhance the volunteers' ability to prevent diabetic complications and improve the community's quality of life. In conclusion, providing technical education and skills training effectively optimizes the role of health volunteers in managing and detecting diabetic foot neuropathy. Keywords: Health Volunteers, Diabetes Mellitus, Foot Neuropathy, Early Detection, Education.

Abstract

A Sequential Explanatory Mixed Methods Evaluation of a Clinical Nursing Practice Guideline for Intracranial Pressure Management  Background: Increased intracranial pressure (IICP) is a life-threatening complication of traumatic brain injury (TBI). Previous studies in Thailand have not evaluated the effectiveness of a structured clinical nursing practice guideline (CNPG) in improving nurses’ knowledge, skills, and confidence, highlighting the need for a rigorously developed and evaluated CNPG.Purpose: This study aimed to evaluate the effectiveness of a CNPG in improving nurses’ knowledge, skills, and confidence in managing IICP among patients with TBI and to explore nurses’ perceptions of its feasibility, optimality, and benefits.Methods: A sequential explanatory mixed methods design was employed. The quantitative phase used a pre–post design and was conducted from February to September 2024. The intervention involved implementing an IICP CNPG among 45 registered nurses in two neurological wards at a tertiary hospital in eastern Thailand. Data were collected using four validated instruments: the Nurses’ Knowledge on IICP Questionnaire, Clinical Skills Observation Checklist, Self-Confidence Scale for IICP Care, and Nurses’ Perception of CNPG Questionnaire. In the qualitative phase, 20 nurses participated in two focus group discussions, which were analyzed using conventional content analysis.Results: Nurses showed statistically significant improvements in knowledge, clinical skills, and self-confidence following CNPG implementation (all p < .001). Perceptions of the CNPG in terms of feasibility, optimality, and benefits significantly exceeded the standard benchmark (all p < .001). Eight qualitative themes were identified, highlighting the roles of evidence-based knowledge, repeated training, structured protocols, interprofessional collaboration, and refresher training in enhancing the consistency, confidence, and quality of IICP care.Conclusion: The CNPG was feasible, effective, and well accepted among nurses, leading to significant improvements in knowledge, clinical skills, and self-confidence in IICP management. Future research should examine its long-term impact and effectiveness across diverse clinical settings.     Background: Diabetic peripheral neuropathy (DPN) is associated with older age and an abnormal ankle-brachial index (ABI). However, no previous study has investigated the synergistic interaction between older age and an abnormal ABI in relation to DPN.Purpose: This study aimed to investigate the independent associations and synergistic interaction of older age and an abnormal ABI in relation to DPN.Methods: This analytical cross-sectional study included 416 individuals with type 2 diabetes mellitus in Jakarta, Indonesia. Older age was defined as age >64 years. DPN was assessed using a combination of sensory testing with a 10-g Semmes-Weinstein monofilament and vibration testing with a 128-Hz tuning fork, with the diagnosis validated by a physician. ABI was assessed using continuous-wave Doppler ultrasound. Chi-square tests, independent t-tests, and multivariable logistic regression were performed with a significance level of p < .05.Results: DPN was identified in 34.9% (n = 145) of participants. Older age (adjusted odds ratio [AOR] = 1.74) and an abnormal ABI (AOR = 2.04) were independently associated with DPN. Older participants with an abnormal ABI were 4.03 times more likely to have DPN than adult participants with a normal ABI (p < .001). A positive synergistic interaction between older age and an abnormal ABI in relation to DPN was identified (AOR = 4.03; relative excess risk due to interaction [RERI] = 2.20; attributable proportion [AP] = 0.55; synergy index [SI] = 3.65).Conclusion: Older age and an abnormal ABI were independently associated with DPN, with their co-occurrence showing a positive synergistic interaction in relation to DPN. Further longitudinal studies with larger samples and relevant clinical factors are needed to confirm these findings.
