Abstract

Purpose: The purpose of this study is to predict the in silico molecular interactions of bitter melon quercetin with GLP-1 receptors using the comparable compounds of myricetin and allosteric modulators. Methods: The in silico test was used to predict the quercetin molecular interactions by docking using the Molegro Virtual Docker computer program. The receptor used was human GLP-1, code PDB: 5VEX with allosteric modulators ligand (97V_1201 [A]). As part from predicting molecular interactions, this study also carried out the prediction of its pharmacokinetic properties (ADME) and the toxicity of quercetin and myricetin using the pkCSM online tool program. The data analysis was performed by comparing the docking bond energies between quercetin, the allosteric modulator ligands and the myricetin comparators at the target receptor. The lower the bond energy of the ligands to the target receptor, the more stable the bonds are. This can be used to predict the biological activity of the compound. Results: The in silico test results showed that the bond energy of quercetin = -70.2678 kcal/mol, myricetin = -105,298 kcal/mol and allosteric modulators = -126,992 kcal/mol. Conclusions: The above test results indicate that quercetin has antidiabetic potential by activating the GLP-1 receptor although it is lower than the myricetin and allosteric modulators. The results of the in silico test using the pkCSM online tool program showed that the quercetin compound had good pharmacokinetic properties and a low toxicity level.

Abstract

Pengetahuan sangat penting dalam mengembangkan diabetes self-management untuk mencegah resiko komplikasi jangka panjang dengan diet yang ketat. Tujuan penelitian ini untuk mengetahui hubungan antara pengetahuan dan diabetes self-management dengan tingkat stres pasien diabetes melitus yang menjalani diet. Penelitian ini menggunakan desain cross-sectional. Jumlah total sampel didapatkan sebesar 106 responden dengan multistage sampling, Kriteria inklusi meliputi pasien DM <5 tahun dengan rentang usia 35-55 tahun, tidak menggunakan insulin injeksi, Pasien DM masih berkeluarga dan tinggal dengan keluarganya. Data didapatkan dengan kuesioner Knowledge of Diabetic Diet Questionnaire, Self-Management Dietary Behaviors Questionnaire, dan tingkat stres menjalani diet. Kemudian dianalisis dengan uji Spearman test. Hasil penelitian menunjukkan bahwa tingkat pengetahuan (p=0,049; r=-0,192) dan diabetes self-management (p= 0,000; r= -0,341) memiliki hubungan terhadap tingkat stres saat menjalani diet. Diabetes self-management memiliki hubungan yang sangat kuat dari pada tingkat pengetahuan terhadap tingkat stres pasien diabetes yang menjalani diet. Penelitian selanjutnya diharapkan bisa menggunakan responden dengan wilayah yang lebih luas dan serta mengembangkan intervensi yang lebih baik untuk meningkatkan pengetahuan tentang diet pada pasien diabetes melitus. Abstract The correlation between knowledge level and diabetes self-management towards stress level of patients DM with diet. Knowledge is very important in developing diabetes self-management to prevent the risk of long-term complications with a strict diet. The purpose of this study was to determine the relationship between knowledge and diabetes self-management with stress levels of people with diabetes mellitus who went on a diet. This study uses a cross-sectional design. The total samples obtained were 106 respondents with multistage sampling, inclusion criteria included patients with DM <5 years with a range of ages 35-55 years, did not use insulin injection, DM patients were still married and lived with their families. Data are obtained with questionnaire of Knowledge of Diabetic Diet Questionnaire (KDDQ), a Self-Management Dietary Behaviors Questionnaire (SMDBQ), and a stress level on a diet. Data analyzed by the Spearman Rho. The results showed that the level of knowledge (p=0.049; r=-0.192) and diabetes self-management (p= 0.000; r=-0.341) had a relationship to stress levels while on a diet. Diabetes self-management has a very strong relationship to the level of knowledge of the stress level of diabetics with diet. Future studies are expected to be able to use respondents with a wider area and develop better interventions to increase knowledge about diets in people with diabetes mellitus. Abstract

Background: The rising prevalence of coronary heart disease (CHD) has highlighted the need for supportive-educative efforts, particularly in nursing. These initiatives are crucial for improving self-care management in CHD patients, enhancing their quality of life. Clinical nurses are key in this role, leveraging their direct patient interaction and expertise to provide tailored education and support. Purpose: This study aimed to investigate the supportive-educative needs of CHD patients from the perspective of clinical nurses. Methods: A descriptive qualitative study was conducted for this research. Participants included 30 clinical nurses responsible for caring for CHD patients in both inpatient wards and outpatient polyclinics. Interview guidelines were utilized to collect data through in-depth interviews, triangulation, and the obtained interview data were analyzed using the thematic analysis approach.  Results: We identified four themes related to the supportive-educative needs of CHD patients: 1) engaging and informative media; 2) culturally sensitive approaches; 3) nurses’ attitudes; and 4) family empowerment. Conclusion: Supportive education, tailored to the needs of CHD patients, is vital for effective disease management. This study's findings highlight the importance of incorporating four key elements into supportive education: engaging and informative media to enhance learning, culturally sensitive approaches to address diverse patient needs, the positive impact of nurses’ empathetic attitudes, and the empowerment of families in the care process. Integrating these aspects can significantly aid CHD patients in developing self-care skills that positively influence their quality of life.

Abstract

Hypertension is a severe medical condition and is the leading cause of premature death worldwide, significantly increasing the risk of heart, brain, kidney, and other diseases. However, not all people with hypertension are aware of their disease, and it is only discovered after complications occur. Therefore, hypertension must be prevented and controlled. Prevention and control of hypertension aims to prevent and reduce the probability of morbidity, complications, and death. Hypertension can be prevented by controlling risky behaviors. This study aims to analyze the risk factors for hypertension in coastal communities in Tarakan City. This research is quantitative research with a cross-sectional study design. The study sampled people in the working area of the Amal Beach Health Centre, Tarakan City, and met the sample criteria obtained using a purposive sampling technique. The data obtained will be processed univariate and bivariate. The results showed that there was a significant relationship between obesity (p-value 0.001), family history (p-value 0.013), and sports activity (p-value 0.009) with the incidence of hypertension. Smoking habits did not show a significant relationship (p-value 0.230) with the incidence of hypertension. This study concludes that obesity, family history, and exercise activity are risk factors for the incidence of hypertension in coastal areas of Tarakan City. Therefore, it is hoped that health promotion efforts in the form of scheduled counseling on risk factors for hypertension and preventive efforts in the form of early detection so that people can prevent or control hypertension to avoid complications.

Abstract

Introduction: Families play a critical role in the care and support of individuals with schizophrenia. However, this responsibility often leaves caregivers with significant physical and psychological burdens. Objective: The study aimed to evaluate the family caregiver, patient, environmental factors, and family function as predictors of family caregiver burden in schizophrenia patients. Methods: This study used a cross-sectional design. The population consisted of family caregivers of schizophrenia patients who had experienced at least one episode in the past year, lived in the same house, and had been caregivers for at least one year. We recruited 220 family caregivers of schizophrenia patients. The variables in this study were family caregiver, patient, environmental factors, family function, and caregiver burden. The data was collected using a self-report questionnaire and analyzed using partial least squares. Results: Most respondents were predominantly between the ages of 46 and 65. The model showed that caregiver burden was influenced by the patient factor (t= 4.259, path coefficient: 0.088), environment factor (t= 6.540, path coefficient: 0.288), and family function (t= 10.977, path coefficient: 0.497). These findings showed that family function was the dominant factor in caregiver burden.   Conclusion: Patient factors, environmental factors, and family function significantly affected the family caregiver burden, except for the family caregiver factor. This model can help family caregivers decrease their burden by managing family functioning.

Abstract

Introduction: The coaching clinic is one of the strategies designed to increase nurses' knowledge in conducting genetic counseling for patients and families of thalassemia patients. Nurses will not only be given information, but they will also be taught how to communicate and how to use genetic counseling to make sure the coaching clinic's goals are met. Objective: This study aims to describes the effect of a simple coaching clinic on nurses' knowledge of genetic counseling implementation in thalassemia care. Methods: This research used a quantitative approach and was conducted on August 20th–21st , 2022. The design of this study was quasi-experimental without a control group. This study obtained a total sample of 29 nurses using a proportional sampling technique and inclusion criteria. The inclusion criteria in this study were nurses with a diploma degree, an undergraduate education background, and registered nurses. The data collection instrument was a 30-questions genetic counseling questionnaire. IBM SPSS version 25.0 was used to analyze the data using the Wilcoxon test. Results: Simple coaching clinic proved to have a significant effect in improving the knowledge level of nurses in general with p-value = 0.004. Knowledge level improvement can be seen from Mean (+SD) 53.67 (+13.64) to 69.67 (+8.26). Likewise with nurses' knowledge about thalassemia, genetic counseling and bio-informatics, the majority of respondents experienced positive changes. In contrast to the genetic topic, the majority of respondents did not experience a change in their level of knowledge so that a p-value >0.05 was obtained. Conclusions: Changes in level of knowledge indicated that the coaching clinic's effects on level of knowledge are significant and have an impact on health services across a variety of health institutions, including clinics and hospitals. As a result, it is critical to have a health strategy that promotes the accessibility of genetic counseling services for patients with thalassemia and other genetic illnesses.

Abstract

DM with ulcer can result in a decrease in the quality of life of patients and need to reduce the intensity of pain and odor on the ulcer, preventing amputation and Hyperbaric Oxygen as an adjuvant therapy in accelerating wound healing. This study aims to explain the effect of HBO on the quality of life of patients with diabetic foot ulcers at RSAL Dr. Ramelan Surabaya. Methods: The research used quasi-experimental design with a non-equivalent control group design approach. The samples were 70 patients from population of all diabetes patients who suffered from diabetic foot ulcers at RSAL Dr. Ramelan. Data were collected by WHOQOL-BREF questionnaire. Analyzed byWilcoxon signed rank test and Mann Whitney U test. Result: Mann Whitney in the post test, the physical p-value domain (0.000), psychological domain (0.012), social domain (0.003), and environmental domain (0,001), where the intervention group has a better value than the control group. Wilcoxon signed rank test results on the physical domain (0,000), psychological (0,012), social (0,003), and environmental (0,001), meaning there were significant differences between pre post test in intervention group. Discussion: Diabetes mellitus patients with foot ulcers given hyperbaric oxygen showed improvement on wound condition, thus reducing pain and medical needs. This makes patients motivated, reduce anxiety, and improve interpersonal support and confidence. Therefore, it is necessary to socialize the effect of HBO on DM patients with foot ulcer, as a treatment option in improving the quality of life of DM patients with foot ulcers.

Abstract

Introduction: Pregnant women are a population vulnerable to health problems. The morbidity and mortality of pregnant women with COVID-19 are higher compared with the rest of the population. This study aims to determine the survival rate of pregnant versus non-pregnant women with COVID-19. Methods: The study used a retrospective observational cohort design. Data were collected from Dr Soeradji Tirtonegoro and Penembahan Senopati hospitals on patients treated between March 2020 and June 2022. A total of 923 women diagnosed with COVID-19 were included in the study. Sociodemographic and clinical data on patient characteristics were extracted from medical records. Data analysis was conducted using Kaplan-Meier and Cox regression analyses to estimate survival probability and investigate predictors of death, with a 5% significance level. Results: Among 932 women with COVID-19, 231 deaths occurred. The median survival time for all participants was 30.56 days, while for pregnant women, it was 11.21 days, and for pregnant women with comorbidities, it was 9.68 days. Years of treatment, age, comorbidities, number of comorbidities and pregnancy were associated with survival rate (P-value < 0.001). Consistent results were also obtained for pregnant women’s age and number of comorbidities (P-value < 0.05). Significant differences between pregnant and non-pregnant patients remained after adjustment for year, sex, age and hospital type (P-value < 0.001). Conclusion: Pregnant women with COVID-19 have a higher risk of death compared with non-pregnant women. Keywords: comorbidity; COVID-19, female; pregnant women; survival analysis

Abstract

Background: A fracture is a condition in which the continuity of bone tissue is broken, causing stress and pain during the healing process. Therefore, the management of pain in postoperative bone fracture patients should be addressed.. Objective: The aim is to identify interventions for managing pain in patients with postoperative bone fractures. Design: A systematic scoping review. Data Sources: The search process was conducted from March 8 to March 15, 2022, using the following databases: PubMed, CINAHL, and Science Direct. Review Methods: PRISMA flowcharts were used for the systematic review. Articles published in the last five years (2018-2022) were included, focusing on full-text articles, randomized controlled trials (RCTs), and cohort studies that discussed the topic of postoperative pain management in fracture patients. The search yielded a total of 4,324 articles from the four databases. After screening by year, 819 articles were obtained. Based on the full text and abstracts of 432 articles, and applying exclusion and inclusion criteria, five articles were selected.  Results: The five articles demonstrated that pain management in postoperative bone fracture patients can be achieved through various pharmacological approaches, including peripheral nerve block anesthesia, antioxidants (vitamin C), intravenous tranexamic acid, intravenous dexamethasone, and the use of calculators and comprehensive pain plans as management tools. Conclusions: This study recommends that healthcare professionals apply and further develop the findings as a pain management strategy for postoperative bone fracture patients.
