Abstract

HIV infection is a chronic infection of the immune system with a target of CD4 cells. Total lymphocyte count (TLC) can be done in resource-limited areas and are able to be used as a substitute to CD4 count. An increase in CD8 count can disturb the correlation between CD4 and TLC especially during the late clinical stage.Objective of this research is to find out the correlation of total lymphocyte count with CD4 count and to find out the influence of the clinical staging on the correlation of total lymphocyte count and CD4 count.This study is an observational, analytical and cross sectional study using the medical records of Klinik Teratai RSHS Bandung. The data is sorted according to the WHO clinical staging and are analyzed using Pearson’s correlation and Fisher’s transformation with α=0.05. The results showed that TLC have a correlation with CD4 count in all stadiums (r: 0,501-0,684, p<0,01). There is no significant difference of the correlation coefficients between the clinical stages (p>0.05). There is a correlation between TLC and CD4 count on HIV infected patients and there is no significant decrease of correlation of TLC and CD4 count on HIV infected patients with worsening of the WHO clinical stages. Keywords: CD4 count, total lymphocyte count, HIV/AIDS

Abstract

MicroRNA (MiRNA) adalah RNA pendek (sekitar 22 nukleotida) yang tidak mengkode pembentukan protein tetapi mengatur banyak fungsi penting dalam tubuh manusia. MiRNA pertama ditemukan pada tahun 1993 dan sekarang telah ditemukan mempengaruhi hampir semua penyakit manusia. MiRNA menjalani jalur pemrosesan terpisah dengan beberapa langkah di dalam nukleus dan sitoplasma. Proses ini juga dikontrol dengan ketat di banyak titik, menciptakan jalur regulasi yang sangat kompleks. Gangguan jalur ini dapat menyebabkan fenotipe penyakit seperti yang ditunjukkan oleh banyak pola ekspresi MiRNA yang terkait dengan keadaan penyakit. Namun, perkembangan mikroRNA masih relatif baru, dan ada banyak hal yang masih belum kita ketahui. Oleh karena itu, artikel reviu ini bertujuan untuk memberikan pendahuluan mengenai MiRNA mulai dari sejarahnya hingga perkembangan terbaru bagi para klinisi dan peneliti. Dalam pembuatan reviu ini dilakukan pencarian dengan kata kunci MiRNA melalui sistem pencarian PubMed pada pangkalan data MEDLINE. Hasil pencarian yang relevan akan dipilih oleh peneliti untuk diikutsertakan dalam artikel ini. Berdasarkan pencarian didapatkan bahwa MiRNA memiliki regulasi yang sangat kompleks dengan efek yang luas pada tubuh manusia. Efek dari MiRNA ini dapat ditemukan pada kondisi infark myokard, diabetes, hingga kanker. Suatu hari nanti, terapi berbasis RNA mungkin menjadi hal yang umum sehingga peneliti dan klinisi harus terbiasa dengan subjek yang berkembang pesat ini.

Abstract

Pure red cell aplasia (PRCA) is a rare disorder caused by failure or abnormalities in erythropoiesis only, among all cell lines of the bone marrow. Patients with PRCA usually present with no specific clinical presentation, thus its recognition depends on a clinician’s high index of suspicion, to facilitate early diagnosis and treatment to provide a better prognosis. Tuberculosis (TB) is the infectious disease with the highest casualty rate worldwide and is still a problem in many countries, including Indonesia. This study is a case report of a pulmonary TB patient with PRCA as a complication. PRCA was confirmed by the bone marrow puncture (BMP) examination as the gold standard. In this case, PRCA was inferred to occur due to the use of Isoniazid in the TB treatment regimen. The discontinuation of Isoniazid administration subsequently results in the normalization of Hb levels. In this paper we would highlight PRCA as a differential diagnosis that should be taken into consideration in symptomatic anemia in patients with tuberculosis under standard treatment.

Abstract

Background: The study investigates the combined effects of 7,12-dimethylbenz[a]anthracene (DMBA) and a high-fat diet (HFD) on hepatic gene expression in a breast cancer mouse model. Both DMBA and HFD are associated with oxidative stress, inflammation, and fibrosis, which may contribute to cancer progression.Methods: Female C57BL/6 mice were divided into three groups: standard diet (ND), ND + DMBA, and HFD + DMBA. Mice in the ND + DMBA and HFD + DMBA groups received DMBA injections weekly for six weeks. The HFD + DMBA group was also fed a high-fat diet for four weeks prior to DMBA administration. After a 12-week treatment period, liver tissues were collected and analyzed for NF-κB and CCN1/CYR61 gene expression using semi-quantitative RT-PCR.Results: The study revealed significant upregulation of NF-κB and CCN1/CYR61 expression in the ND + DMBA and HFD + DMBA groups compared to the control group. Notably, the HFD + DMBA group exhibited the highest expression levels of these genes, suggesting that the combination of HFD and DMBA synergistically exacerbates hepatic inflammation and fibrosis.Conclusion: The findings indicate that a high-fat diet in combination with carcinogen exposure can significantly enhance hepatic gene expression related to inflammation and fibrosis, highlighting the importance of dietary management in cancer prevention and treatment.

Abstract

Introduction: In the field of medical education, the evaluation process of communication and clinical skills training is known as Objective Structured Clinical Examination (OSCE). OSCE can be assessed using the global rating method, which is a method of subjective evaluation by examiners and standardized patients of student performance. Objective: The study aims to examine the relationship between the subjectivity of examiners and standardized patients in OSCE scores using the global rating method. Methods: The study involved 985 students from the Faculty of Medicine at Maranatha Christian University who participated in the Local OSCE from December 2021 until January 2022. Secondary data were analyzed using logistic regression in SPSS software and presented in tables and multivariate analysis. Results: The global rating assessment by examiners showed a significant relationship with OSCE scores (p<0.001), while the global rating assessment by standardized patients did not show a significant relationship with OSCE scores (p>0.05). This indicates that the evaluation by examiners has a greater influence on OSCE results compared to the assessment by standardized patients. Recommendations: More comprehensive training is needed for standardized patients in conducting assessments using the global rating method to minimize bias. Furthermore, future research is recommended to explore other factors that may influence OSCE assessments to improve the validity and reliability of the evaluation.

Abstract

Minat terhadap tindakan kosmetik telah mengalami peningkatan dalam beberapa tahun terakhir. Namun tindakan kosmetik dapat menimbulkan rasa nyeri yang berlebih. Telah tersedia berbagai metode untuk mengurangi rasa nyeri, salah satunya adalah pemberian stimulus vibrasi. Tujuan dari literature review ini adalah untuk mengetahui efek dan signifikansi anestesi vibratorik dalam mengurangi rasa nyeri pada tindakan kosmetik. Metode yang digunakan adalah review terhadap tujuh artikel dari jurnal internasional yang diterbitkan pada tahun 2014-2024. Artikel didapatkan dari mesin pencarian PubMed dengan kata kunci ‘vibratory anesthesia’, ‘cosmetic procedure’, dan ‘vibration anesthesia device’. Dari studi pustaka didapatkan hasil bahwa anestesi vibratorik aman dan efektif dalam mengurangi nyeri pada berbagai tindakan kosmetik seperti dermal filler di wajah, injeksi steroid interlesional pada keloid, staple removal, dan suture removal. Stimulasi vibrasi dapat diberikan secara tunggal maupun bersamaan dengan metode analgesik lain, baik dalam waktu bersamaan maupun beberapa saat sebelum dilakukan tindakan. Stimulai vibrasi diberikan dengan alat Vibration Anesthesia Device (VAD).

Abstract

The global prevalence of postpartum depression is increasing annually. This condition has been associated with polymorphisms in the catechol-O-methyltransferase (COMT) enzyme, which plays a critical role in the degradation of catecholamines. Therefore, a literature review was conducted to examine the influence of COMT enzyme variants on postpartum depression, with a focus on specific time periods and genetic variants implicated in the disorder. The review methodology involved a systematic search of articles on the PubMed database using the keywords "catechol-O-methyltransferase," "postpartum depression," and "role of COMT in postpartum depression." Inclusion criteria required full-text availability in either Indonesian or English. Four relevant studies were identified that investigated the relationship between COMT enzyme variants and postpartum depression. Evidence suggests that the COMT Val158Met polymorphism may affect the risk of postpartum depression in the context of elevated estrogen levels, as high estrogen concentrations are known to downregulate COMT expression. Additionally, certain single nucleotide polymorphisms (SNPs), including rs2020917 (TT genotype), rs737865 (GG genotype), and the COMT rs4680 Met variant allele, have been shown to modulate the influence of COMT on postpartum depression independently of estrogen levels, possibly through interactions with other enzymatic pathways.

Abstract

Preeklamsia merupakan kondisi terjadinya disfungsi plasenta karena adanya inflamasi yang bersifat sistemik, yang dapat ditandai secara umum dengan adanya hipertensi dan gangguan organ. Homosistein merupakan salah satu faktor yang berperan dalam terjadinya disfungsi endotel. Pada beberapa penelitian yang ada, dipercaya asam folat dapat menurunkan kejadian preeklamsia pada ibu hamil. Studi pustaka ini dibuat untuk membahas efek asam folat terhadap pencegahan terhadap kejadian preeklamsia. Penelitian ini dijalankan dengan metode literature review, artikel didapatkan melalui PubMed dengan kata kunci preeklamsia, dan asam folat. Ditemukan terdapat 6 penelitian yang membahas mengenai suplementasi asam folat terhadap pencegahan terhadap preeklamsia. Studi pustaka ini menyimpulkan bahwa pemberian asam folat dosis tinggi 4 hingga 5 mg memilki efek terhadap penurunan insidensi preeklamsia, tetapi juga terdapat penelitian yang menyatakan tidak terdapat pengaruh pemberian asam folat terhadap pencegahan preeklamsia. Waktu pemberian asam folat yang paling efektif pada usaha pencegahan preeklamsia masih perlu diteliti lebih lanjut.

Abstract

Background: The government implements a high-quality assurance system in the health sector to strengthen the national education system. One of the steps is the Indonesian Medical Doctor National Competency Examination (known as UKMPPD in Indonesia), which follows regulations on higher education and health workers. The entrance exam for medical students at the Faculty of Medicine, Maranatha Christian University, consists of a biology test and an academic potential test.Aims: The purpose of this study was to determine the relationship between the biology entrance exam, academic potential test, and grade point average with the final score of UKMPPD CBT at the Faculty of Medicine, Maranatha Christian University. Methods: The samples collected consisted of students from the Faculty of Medicine at Maranatha Christian University, from the 2010 to 2016 classes, who took the UKMPPD from 2016 to 2022. Data were analyzed using the Spearman test and examined for both bivariate and multivariate relationships.Results: This study showed that the biology entrance examination, medical undergraduate GPA, and medical professional GPA influenced the UKMPPD CBT final score of Maranatha Christian University Faculty of Medicine students. Nevertheless, there is no relationship between students’ TPA score and UKMPPD CBT.Conclusion: Medical undergraduate GPA had the strongest relationship with CBT scores, compared to medical professional GPA. This study also highlighted the importance of non-cognitive aspects of input selection tests besides the improvement of the quality of the clinical education evaluation process.
