Abstract

Employee performance is the level of achievement of a person's work results according to the tasks and standards set, which is seen from how he or she carries out his or her work and meets organizational demands. This study aims to empirically test and analyze the influence of work-life balance, organizational culture and work discipline on organizational commitment and its implications for employee performance. The research method used in this study is a quantitative research method with a descriptive and verification approach. The data sources used are primary data sources and secondary data sources. Data collection techniques in this study are interviews, questionnaires, observations, and library research. The population in this study is all employees totaling 105 people. The sampling technique used in this study is a purposive sampling technique. The analytical tools in this study are SPSS 25 and SmartPLS 4. Testing of measurement models or outer models in this study are convergent validity, discriminant validity, Average Variance Extracted (AVE), composite reliability, Cronbach alpha. Testing of structural models or inner models in this study is the coefficient of determination (R2) and Predictive relevance (Q2). The results of this study indicate that work-life balance, organizational culture, and work discipline have a positive and significant effect on organizational commitment, both partially and simultaneously, with a 92% effect, with the remaining 8% being influenced by other variables not examined in this study. Organizational commitment has a positive and significant effect on employee performance, and organizational commitment can mediate the relationship between work-life balance and employee performance. Meanwhile, organizational commitment cannot mediate the relationship between organizational culture and work discipline on employee performance.

Abstract

ABSTRACT Pressure ulcers are prevalent in older patients who are immobile and can substantially impact their quality of life and add to the burden on the health care system. Factors that can slow wound healing are infections, necrotic tissue, moisture, and blood supply. The TIME model, which stands for tissue management, control infection or inflammation, moisture balance, and epithelial edge advancement, can be used to improve the healing of chronic wounds. This study aims to evaluate the effectiveness of the TIME approach in accelerating the healing of stage III pressure ulcers in elderly patients. This case included an older woman, 70 years of age, with a sacral (Stage 3) pressure ulcer. Treatment consisted of sharp debridement, irrigation (0.9% saline), and dressings of Cutimed Sorbact, cadexomer iodine (0.9%), then polyurethane foam over four treatment sessions. High-protein nutrition and scheduling, turning every three hours, complemented the treatment. Progress was monitored using the Bates-Jensen Wound Assessment Tool (BWAT). After four treatment sessions, necrotic tissue had decreased from 75% to 10% and granulation tissue had increased up to 90%. Clinical signs of infection improved. The initial BWAT score of 49 (indicating degeneration of the wound) was closer to partial regeneration by the final session. in facilitating wound healing in a high-risk patient. More studies are needed with a comparative study design and larger populations to verify this and enable wider clinical adoption. Keywords: Debridement, Geriatric Nursing, Pressure Injury, Wound Healing.  ABSTRAK Ulkus tekan sering terjadi pada pasien lansia yang mengalami kesulitan bergerak, dan hal ini dapat memperburuk kualitas hidup mereka serta menambah beban pada sistem layanan kesehatan. Proses penyembuhan luka dapat terhambat oleh berbagai faktor, termasuk infeksi, jaringan mati, kelembapan yang berlebihan, serta masalah aliran darah. Pendekatan TIME, yang mencakup manajemen jaringan, pengendalian infeksi atau peradangan, keseimbangan kelembapan, serta stimulasi tepi epitel, telah diterapkan untuk mempercepat proses penyembuhan luka kronis.Penelitian ini bertujuan untuk menilai efektivitas metode TIME dalam mempercepat penyembuhan ulkus tekan tahap III pada pasien lansia. Laporan kasus ini membahas seorang pasien wanita berusia 70 tahun yang mengalami ulkus dekubitus di area sakrum. Intervensi dilaksanakan dalam empat sesi, yang mencakup debridemen tajam, irigasi dengan NaCl 0,9%, serta penggunaan balutan Cutimed Sorbact, cadexomer iodine, dan foam poliuretan. Dukungan nutrisi yang kaya protein dan tindakan mobilisasi tersedia setiap tiga jam. Untuk evaluasi, digunakan alat penilaian luka Bates-Jensen (BWAT). Setelah melakukan empat intervensi, area nekrotik mengalami penurunan dari 75% menjadi 10%. Selain itu, jaringan granulasi meningkat mencapai 90%, dan gejala infeksi menunjukkan perbaikan. Skor BWAT awal sebesar 49 menunjukkan adanya kerusakan degeneratif, yang kemudian menunjukkan pergeseran menuju fase regenerasi parsial. Pendekatan TIME yang diterapkan dengan sistematis dan secara individu terbukti dapat mempercepat proses penyembuhan ulkus tekan pada pasien yang memiliki risiko tinggi. Penting untuk melakukan studi tambahan dengan desain komparatif dan melibatkan populasi yang lebih besar guna memperkuat temuan ini. Kata Kunci: Debridemen, Keperawatan Geriatri, Cedera Tekanan, Penyembuhan Luka.

Abstract

Background: Diabetic foot disease remains a major cause of ulceration and amputation, yet neuropathy and peripheral arterial disease are often assessed separately, limiting consistent identification of neuro-ischaemic risk. Purpose: To analyze the concept of the neuro-ischaemic phenotype in diabetic foot ulceration based on the method introduced by Walker and Avant Methods: In a Walker and Avant concept analysis of published literature from PubMed, Embase, Scopus, and Web of Science from November 2020 to 2025, we extracted concept uses, defining attributes, antecedents, consequences, and empirical referents for neuro-ischemic DFU. Boundaries were refined using model, borderline, related, and contrary cases, then synthesised into an operational framework. Results: The analysis defined the phenotype as coexisting loss of protective sensation and objectively confirmed lower-limb ischaemia. Five defining attributes were identified, including silent progression and the need for dual-domain measurement. A tiered minimum dataset was proposed, utilizing the ankle-brachial index, toe-based measures, and Doppler waveform, to reduce misclassification and guide escalation pathways. Conclusion: An integrated neuro-ischaemic phenotype closes single-domain gaps and enables standardised DFU risk stratification. It supports nursing-led triage and referral pathways and requires prospective validation for clinical outcomes. Relevance to clinical practice: Neuro-ischaemic DFU triage: nurses pair Loss of Protective Sensation (LOPS) screening with perfusion tests (ABI plus toe indices or Doppler) to stratify risk, speed early vascular review, tailor offloading/infection surveillance, and standardise communication.
