Abstract

Ventilator-associated pneumonia (VAP) occurs in patients who have been on mechanical ventilation with an endotracheal tube (ETT) for at least 48 hours. This type of pneumonia typically arises as a result of nosocomial infections or Healthcare-Associated Infections (HAIs) and is commonly linked to the use of ventilators in hospital settings. This study was to identify the factors associated with the incidence of VAP among patients admitted to the Intensive Care Unit of Abdoel Wahab Sjahranie Hospital, Samarinda. Method: A cross-sectional study was conducted. The study population consisted of all patients who had undergone mechanical ventilation in the ICU without a prior history of pneumonia, totaling 118 respondents, using the HAIs bundle observation tool and direct observation. Chi-square and multiple logistic regression were used for statistical analysis, along with a predictive model. The results showed that the prevalence of VAP among respondents in the ICU was 5.9%. Factors significantly associated with the incidence of VAP included duration of ventilator use (p = 0.000), oral hygiene procedures (p = 0.000), head-of-bed elevation to 30–45° (p = 0.007), hand hygiene compliance (p = 0.015), and aseptic suctioning procedures (p = 0.043). The multivariate analysis identified duration of ventilator use ≥96 hours as the most dominant factor, with the highest Odds Ratio (OR = 13.975; 95% CI: 0.753–227.435). Duration of ventilator use was the most significant factor associated with an increased risk of VAP, with patients ventilated for ≥96 hours being 13 times more likely to develop VAP compared to those ventilated for <96 hours. Proper oral hygiene, appropriate head-of-bed elevation (30–45°), compliance with hand hygiene, and aseptic suctioning procedures were also found to be significantly associated with VAP incidence among ICU patients at Abdoel Wahab Sjahranie Hospital

Abstract

Human Immunodeficiency Virus (HIV) remains a major global public health issue, with antiretroviral (ARV) therapy playing a critical role in its management. Adherence to ARV therapy is essential to suppress viral load, prevent drug resistance, and improve the quality of life of people living with HIV/AIDS (PLWHA). However, cases of non-adherence persist. At RSUD A. Wahab Sjahranie, a referral hospital in East Kalimantan, 16% of patients have been found to be non-adherent to their ARV medication. Ensuring adherence is vital to avoid complications and prevent HIV transmission. This study aims to identify and analyze the factors influencing ARV adherence among HIV/AIDS patients at the Anyelir Outpatient Clinic, RSUD A.W. Sjahranie, Samarinda. This study employed a quantitative analytical approach with a cross-sectional design. It involved 106 HIV/AIDS patients who were actively receiving antiretroviral therapy (ART). The independent variables included knowledge, self-efficacy, duration of therapy, side effects, family support, social support, and healthcare staff attitude. Data were collected using a structured questionnaire administered directly to respondents and supported by secondary data from medical records. A total sampling technique was applied, and data were analyzed using chi-square tests and logistic regression at a 95% significance level. A total of 85.8% of respondents were classified as adherent to ARV treatment. Bivariate analysis showed that knowledge (p = 0.019), family support (p = 0.018), social support (p = 0.011), healthcare staff attitude (p = 0.006), and self-efficacy (p = 0.002) were significantly associated with ARV adherence. Multivariate analysis identified family support (OR = 4.275), healthcare staff attitude (OR = 6.834), and self-efficacy (OR = 20.241) as the most influential factors. Knowledge, side effects, family support, social support, healthcare staff attitude, and self-efficacy are significant determinants of ARV adherence among PLWHA. Among these, self-efficacy emerged as the most dominant factor.

Abstract

The purpose of this study is to implement automatic waterfill innovation for fuel efficiency, time effectiveness, and minimizing the potential for hazardous and toxic waste contamination in coal mining. This research method is quantitative with a research and development approach. This research was conducted at PT. Antareja Mahada Makmur site Multi Harapan Utama. The research was conducted in June 2024. This research began with an analysis of the current conditions regarding the water truck process in the waterfill; setting targets with Specific, Measurable, Achievable, Relevant, and Time-Bound indicators. After the implementation of automatic waterfill, the operational time required to turn the waterfill on and off was drastically reduced because automation eliminates manual steps. It also significantly reduced the total waterfill running time per shift from 480 minutes to just 144 minutes, which means significant energy savings, increased time efficiency, and reduced fuel waste and potential damage due to excessive operations. The application of automatic waterfill in coal mines brings many benefits, including improved operational quality, cost savings, increased safety, and reduced environmental impact. In addition, this system also has a positive impact on operator motivation and job satisfaction.

Abstract

The aviation industry operates in a high-risk environment where safety is paramount. Despite the implementation of Safety Management Systems (SMS), aviation incidents often highlight gaps not in compliance, but in safety culture—how individuals and organizations perceive and prioritize safety. A mature and robust safety culture can significantly reduce accidents and improve operational reliability. This study aims to explore and describe the safety culture profile in the aviation sector, particularly focusing on the maturity of safety practices at the individual, group, and organizational levels. A cross-sectional study was conducted using the Nordic Occupational Safety Climate Questionnaire (NOSACQ-50), a validated instrument to assess safety culture. A total of 183 aviation workers from various operational units participated in the survey. In addition to quantitative analysis, focus group discussions (FGDs) were held with selected management and operational staff to provide contextual insights into survey findings. The results showed that the overall safety culture maturity level among respondents was categorized as proactive, with the highest scores in management commitment (85.0) and safety procedures (85.0). However, relatively lower scores were observed in risk perception (70.5) and work environment (70.0), indicating areas for targeted improvement. These findings suggest that while aviation organizations have made considerable progress in building a strong safety culture, further efforts are needed to enhance risk awareness at the individual level and improve environmental factors influencing safety behaviour. Interventions should emphasize participatory safety practices, continuous communication, and reinforcement of risk-based thinking

Abstract

Pulmonary function impairment remains a critical occupational health issue among coal industry workers due to prolonged exposure to airborne pollutants. Despite the implementation of occupational health regulations, respiratory disorders continue to be prevalent, leading to decreased work capacity and increased health risks. Identifying key risk factors is essential to developing targeted preventive measures. Objective: This study aims to analyze the relationship between occupational risk factors and pulmonary function impairment among workers at PT X Coal Division, East Kalimantan. A cross-sectional study was conducted on 183 coal division workers selected through cluster random sampling. Data were obtained through structured questionnaires, spirometry tests, and workplace dust level measurements. Multivariate logistic regression analysis was used to determine the most influential factors associated with pulmonary function impairment. The analysis identified significant associations between pulmonary function impairment and a history of other diseases (p = 0.001; OR = 11.709; 95% CI: 2.901–47.257), work tenure of ≥3 years (p = 0.032; OR = 3.319; 95% CI: 1.109–9.934), heavy physical workload (p = 0.036; OR = 2.807; 95% CI: 1.071–7.361), and inadequate personal protective equipment (PPE) usage (p = 0.015; OR = 2.566; 95% CI: 1.205–5.463). Other factors such as age, body mass index (BMI), work location, dust exposure, exercise habits, and smoking were not significantly associated with pulmonary function impairment. Work-related exposures and individual health conditions play a crucial role in pulmonary function impairment among coal workers. Strengthening workplace safety measures, enforcing PPE compliance, and implementing regular health screenings are necessary to mitigate respiratory risks. Ensuring a safer work environment through targeted preventive strategies is essential for protecting workers from long-term pulmonary complications while enhancing occupational health policies in the coal industry.
