Abstract

The Maternal Mortality Rate (MMR) is still high globally and nationally, with bleeding and infection due to perineal rupture as the main causes. Primigravida are at greater risk of experiencing perineal rupture. Perineal massage has been proven to be effective in increasing tissue elasticity, thereby reducing the incidence of perineal rupture and labor complications. This study aims to determine the effect of perineal massage on primigravida on the incidence of perineal rupture during labor. This study is a quantitative study with a quasi-experimental method with a non-equivalent control group design. The population is all primigravida pregnant women with a gestational age of 34-36 weeks who visit the UPTD Baloi Permai Health Center. The sampling technique used purposive sampling and obtained a sample of 18 respondents. The data obtained were analyzed using SPSS. The results showed that most respondents in the treatment group did not experience perineal rupture (77.8%), while in the control group most experienced second-degree perineal rupture (66.7%). The statistical test obtained a p-value of 0.002 < α 0.05, indicating that perineal massage significantly reduces the incidence of perineal rupture. Healthy pregnant women of reproductive age (20–35 years) are recommended to perform regular perineal massage starting at 34 weeks of gestation using proper technique. This practice increases muscle elasticity, improves blood flow, and prepares the tissues for childbirth, thereby reducing the risk of perineal rupture. Abstract

The post partum period is the period after the birth of the placenta and ends when the uterus returns to its pre-pregnancy state. This period lasts 6 weeks or 42 days. In the post partum period, it is not uncommon for mothers to experience various unpleasant conditions. Some of the problems frequently encountered include post-partum bleeding, vaginal discharge, infections, urinary incontinence, swollen breasts, Baby Blues Syndrome and Post-Partum Depression. If this condition is not treated properly, it can become a risk factor for complications that can endanger the mother and baby. The writing method was carried out using a journal review approach from Google Scholar with the keywords post partum, risk factors and complementary in each journal. The search yielded 10 studies reviewed. Writing this journal review is to find out the application of complementary therapies that can reduce risk factors in the post partum period through searching journal reviews. The studies reviewed show that providing complementary therapy can reduce various complaints and risk factors in post partum mothers. It is hoped that midwives can provide midwifery care with a complementary therapy approach to postpartum mothers.

Abstract

High-risk pregnancy represents a condition that increases the likelihood of complications for both the mother and the fetus and therefore requires appropriate and comprehensive management. Midwives, as frontline healthcare providers, perform strategic roles in early detection, continuous monitoring, health education, and referral coordination in the management of high-risk pregnancies. This study analyzes the role of midwives in high-risk pregnancy care management using a Systematic Literature Review (SLR) approach. The review examined scientific articles retrieved from national and international databases, including Google Scholar, PubMed, ScienceDirect, SINTA, and Garuda. The search process identified 275 articles published between 2019 and 2025. Application of predefined inclusion and exclusion criteria resulted in the selection of 10 relevant articles for detailed analysis. The findings indicate that midwives contribute to the reduction of pregnancy complications and the improvement of maternal and neonatal safety through early detection of risk factors, routine monitoring of maternal and fetal conditions, provision of health education, and coordination of referral services. Continuity of midwifery care improves adherence to antenatal care visits and supports earlier management of pregnancy complications. Educational interventions delivered by midwives increase pregnant women’s knowledge, awareness, and healthy behaviors during pregnancy. Improved maternal understanding of pregnancy health contributes to prevention of delays in complication management and supports better pregnancy outcomes for both mothers and infants
