Abstract

Antiretroviral treatment is one of the biggest parts of the treatment for people infected with HIV. This study aims to determine the effectiveness of drugs and the cost efficiency of ARV combination drug therapy. The study used a non-experimental research design that was observational to determine the Cost Effectiveness Analysis of 2 combinations of ARVs, namely the combination of TLD (Tenofovir + Lamivudin + Dolutegraph), and the combination of TLE (Tenofovir + Lamivudin + Efavirenz). Data collection was carried out retrospectively from the medical record data of patients who started ARV treatment in the period 2021 to 2023. Total of 189 patients were actively treated as outpatients at VCT polyclinics and 64 who met the inclusion criteria were used as study subjects. The effectiveness of treatment uses the parameters of the Viral Load test results. Treatment is said to be effective if the Viral Load test result is <40 copies/mL. Based on the results of the Viral Load test, the TLD combination was more effective (75.9%) compared to the TLE combination (66.7%). Based on Cost Effective Analysis, the combination of TLD (Tenofovir + Lamivudin + Dolutegrafir) is better with an ACER value of IDR 39,871 compared to the combination of TLE (Tenofovir + Lamivudin + Efavirenz) with an ACER value of IDR 68,599.

Abstract

Human Immunodeficiency Virus (HIV) is a disease that is contagious to humans so that it can lower the immune system and be susceptible to diseases. Drugs given to treat HIV infection and improve the quality of life of People With HIV/AIDS (ODHA) are antiretrovirals (ARVs). Adherence to taking antiretrovirals is an important factor in the success of therapy and reducing the amount of HIV virus in the body of ODHA, so that a high level of adherence has a fairly good or undetectable viral load result. The purpose of this study is to find out if there is a relationship between compliance with viral load levels. The research method was conducted with a cross sectional design, starting from May to June 2024. The data used are the results of the MMAS-8 questionnaire that has been validated and filled out directly at the Putri Ayu Health Center in Jambi City and the results of the viral voad level laboratory. Sampling with consecutive sampling. The data obtained in the analysis by bivariate analysis used the Cramer's V with a degree of significance of p-value <0.05. The results in this study were obtained by 11 (52.4%) respondents with a high level of compliance so that the results of viral load were not detected, there were 10 respondents with a moderate and low level of compliance so that 8 (38.1%) respondents had a detected viral load result and 2 respondents were not detected. There was a significant relationship with adherence to taking antiretroviral drugs to viral load levels with a result of 0.000 p-value <0.05 or 5%.

Abstract

Diabetes mellitus (DM) type 2  is a chronic disease that requires continuous treatment, resulting in drug interactions. Drug interactions are one of the drug related problems identified as drug therapy events that can affect the patient's clinical outcome. The aim of the research is to determine the number of potential drug interactions in type 2 DM patients and the relationship between drug interactions and clinical outcomes in the form of achieving blood glucose control targets. This research was conducted using a descriptive analytical method with a cross-sectional design. Data were collected on type 2 DM patients in outpatient care at Hospital Data analysis of the relationship between therapy and clinical outcomes used the chi-square test. The results of the study found that 41 patients (89%) experienced drug interactions, 5 patients (11%) did not experience drug interactions. Based on this data, 138 cases could be identified, consisting of 91 cases (66%) of pharmacodynamic interaction mechanisms, 46 cases (34%) of pharmacokinetics, with 18 cases (13%) of minor severity, 114 cases (83%) of moderate severity. and major in 6 cases (4%). The most frequent interactions were candesartan and lantus with 9 cases (7%) and candesartan and novorapid with 9 cases (7%). Patients achieved clinical outcomes for blood glucose in 22 patients (48%) and 24 patients (52%) achieved uncontrollable blood glucose clinical outcomes. The results of the analysis carried out stated that there was a relationship between drug interactions and patient clinical outcomes in the form of controlled blood glucose control targets, p-value <0.013.

Abstract

Puskesmas sebagai salah satu lini terdepan pelayanan kesehatan bagi masyarakat Indonesia sudah seharusnya menerapkan penggunaan obat yang rasional. Tidak tepatnya penggunaan obat pada tingkat puskesmas mengakibatkan kerugian pada masyarakat. Hal ini dikarenakan masyarakat yang lebih memilih untuk berobat di puskesmas. Ketidakrasionalan penggunaan obat di tingkat puskesmas mengakibatkan meningkatnya proporsi masalah terkait obat di Indonesia yang bersifat sangat merugikan di kalangan masyarakat. Penelitian ini bertujuan untuk mengetahui ketepatan pemakaian obat antibiotik pada pasien infeksi saluran pernafasan di Puskesmas Rawat inap Kampung Laut dengan parameter tepat indikasi obat, tepat pasien, tepat dosis obat yang diberikan, dan interaksi obat. Penelitian ini adalah penelitian observasional bersifat deskriptif dengan pengambilan data secara retrospektif dengan sampel meliputi seluruh pasien penderita infeksi saluran pernafasan di Puskesmas Rawat Inap Kampung Laut tahun 2019 yang termausuk ke dalam kriteria inklusi. Pengambilan sampel pada penelitian dengan teknik purposive sampling. Rasionalitas pada penggunaan antibiotik pada pasien infeksi saluran pernafasan di Puskesmas Rawat inap kampung laut tahun 2019 berdasarkan analisa kualitatif yang telah dilakukan yaitu tepat indikasi obat sebesar 80%, tepat pasien sebesar 100%, tepat dosis obat sebesar 100%, dan interaksi obat antibiotik sebanyak 8 kejadian.

Abstract

Skizofrenia merupakan salah satu penyakit kejiwaan yang dapat menyebabkan gangguan kronis pada fungsi otak dan mempengaruhi populasi manusia sebesar 1%. Terapi pengobatan skizofrenia terdiri dari pemberian antipsikotik generasi pertama dan kedua baik secara tunggal maupun kombinasi. Keberhasilan pengobatan sangat bergantung pada pemakaian antipsikotik yang diberikan secara rasional serta bijak. Tujuan dari penelitian ini adalah untuk mengetahui rasionalitas penggunaan antipsikotik pada pasien skizofrenia di Rumah Sakit Jiwa Daerah Provinsi Jambi berdasarkan indikator ketepatan obat, ketepatan indikasi, ketepatan dosis, ketepatan frekuensi pemberian, cara pemberian dan lama pemberian yang tepat serta interaksi obat yang terjadi. Jenis penelitian ini adalah non eksperimental dengan desain deskriptif, bersifat retrospektif, menggunakan data rekam medis pasien rawat inap periode April-Mei 2022. Hasil penelitian menunjukkan bahwa dari 66 sampel, penderita skizofrenia dengan jenis kelamin pria sebanyak 47 pasien (71%) dan jenis kelamin wanita sebanyak 19 pasien (29%), dengan penggunaan antipsikotik tipikal sebesar 3,03%, antipsikotik atipikal 72,73% dan kombinasi 24,29%. Hasil penelitian menunjukkan ketepatan dari penggunaan antipsikotik dengan indikator tepat obat, tepat indikasi, tepat cara pemberian, dan tepat waktu pemberian adalah 100%, tepat dosis 88%, frekuensi pemberian 70%, dan interaksi obat antibiotik 16 kasus berdasarkan pedoman Drug Information Handbook 22nd Edition dan Pharmacotherapy Handbook 9th Edition. Kesimpulan: Masih ditemukan ketidakrasionalitasan dalam penggunaan antipsikotik terutama pada indikator tepat dosis dan tepat frekuensi.

Abstract

Coronavirus Disease 2019 (COVID-19) is an infectious disease caused by Severe Acute Respiratory Syndrome Coronavirus2 (SARSCoV2). Treatment for COVID-19 is still uncertain, so various treatment efforts have been carried out, including administering antibiotics. This research is a type of non-experimental research and uses a descriptive research design. Data collection was carried out retrospectively. Data collection from medical records of COVID-19 patients in 2022 at Hospital X Muara Bulian. The population of 86 patients, a sample of 46 patients. The research results showed that 69.57% of patients were male, 32.61% of patients were aged 36–45 years, and Covid-19 patients received the most antibiotic therapy, namely azithromycin in the macrolide group at a dose of 500 mg for 5 days with a percentage of 69.57%, The accuracy of the indication, dose, and duration of administration of antibiotics is 100% following the COVID-19 Management Guidelines, Edition 2 of 2020, and no drug interactions have occurred. This research is based on existing literature. It is hoped that future researchers will use a larger sample size than this study so that they can understand demographic characteristics, patterns of antibiotic use, rational use of drugs, and drug interactions that occur.

Abstract

Pharmaceutical Wholesaler (PBF) is a company that distributes medicines to hospital installations, health centers, and pharmacies so that they can be given directly to patients or the public. One of the conditions that has been determined by existing regulations is that each PBF must have a Responsible Pharmacist who is responsible for implementing the provisions for procurement, storage, and distribution of medicines and/or medicinal substances. Good Medicine Distribution Method (CDOB) is the distribution of medicines or medicinal substances that have the aim of ensuring quality when distributing or dispensing medicines and medicinal substances so that they can be properly maintained under the requirements and intended use. Implementation of Good Medicine Distribution Methods at two PBFs, namely PBF X and Y, Jambi City. This research used a non-experimental descriptive design using questionnaires and direct interviews with the pharmacist in charge (APJ) and warehouse head. The results obtained by Good Medicine Distribution Methods from the 56-question PBF X and Y questionnaire fulfill the implementation of Good Medicine Distribution Methods. Abstract

Bronkopneumonia merupakan jenis infeksi yang menyerang saluran udara menuju paru-paru, yang dikenal sebagai bronkus. Penyakit ini umumnya disebabkan oleh infeksi bakteri, tetapi juga dapat disebabkan oleh infeksi virus dan jamur. Penelitian ini bertujuan untuk mengetahui rasionalitas penggunaan antibiotik pada pasien bronkopneumonia pediatri di Instalasi Rawat Inap Rumah Sakit X Kabupaten Bungo tahun 2024 dengan metode Gyssens. Penelitian ini didasarkan pada tingginya tingkat peresepan antibiotik dalam terapi penyakit bronkopneumonia sehingga harus digunakan secara rasional agar tidak menimbulkan terjadinya resistensi antibiotic Penelitian ini menggunakan metode deskriptif analitik dengan pendekatan retrospektif. Data dikumpulkan melalui unit rekam medis di Rumah Sakit X Kabupaten Bungo dari bulan Januari – Desember 2024 yang berjumlah sebanyak 42 pasien dan kemudian dianalisis dengan metode Gyssens. Hasil analisis penggunaan antibiotik berdasarkan metode Gyssens diperoleh penggunaan antibiotik tepat dan rasional (kategori 0) sebesar 40,5%, penggunaan antibiotik tidak tepat dosis (kategori II A) sebesar 47,6%, dan penggunaan antibiotik tidak tepat karena ada antibiotik lain yang lebih efektif (kategori IV A) sebesar 11,9%. Kesimpulan dari penelitian ini adalah penggunaan antibiotik di instalasi rawat inap Rumah Sakit X Kabupaten Bungo tahun 2024 beberapa diantaranya masih belum rasional.

Abstract

Good Manufacturing Practice (GMP) merupakan pedoman dalam industri pangan untuk menjamin mutu dan keamanan produk. Penelitian ini bertujuan mengevaluasi penerapan GMP pada industri rumah tangga roti di Kota Sarolangun (roti R dan S) dan Kota Jambi (roti A dan B). Metode yang digunakan adalah kuantitatif non-eksperimen dengan desain observasional. Data diperoleh melalui observasi dan wawancara langsung dengan karyawan selama April–Mei 2025. Hasil menunjukkan bahwa penerapan GMP pada roti R dan S di Kota Sarolangun telah memenuhi sebagian besar prinsip GMP, namun masih ditemukan kekurangan pada aspek fasilitas fisik seperti pintu, jendela, dan ventilasi. Sementara itu, roti A dan B di Kota Jambi telah menerapkan GMP dengan baik, namun terdapat beberapa ketidaksesuaian seperti toilet yang kurang terawat, alat pencuci tangan tidak lengkap, dan tempat sampah yang belum memadai. Temuan ini dianalisis untuk mengidentifikasi kekurangan dan merumuskan rekomendasi. Disarankan adanya peningkatan pelatihan karyawan, perbaikan fasilitas sanitasi, serta penyusunan standar operasional prosedur yang lebih ketat. Implementasi perbaikan ini diharapkan dapat meningkatkan kualitas dan keamanan produk roti, sehingga mampu memenuhi standar GMP dan bersaing di pasar.

Abstract

Hypertension is also called the silent killer characterized by continuously high blood pressure in the ratio of systolic blood pressure (TDS) ≥ 140 mmHg and diastolic blood pressure (TDD) ≥ 90 mmHg. This hypertension requires long-term treatment, therefore patient non-compliance in therapy can cause failure of hypertension treatment. To determine the level of patient compliance in hypertension treatment, it can be seen from the compliance of drug retrieval by analyzing the relationship to gender, age, and comorbidities. This research method is carried out non-experimentally and uses analytical descriptive methods. Data collection from the poly registration book for patients with the July 2023 reverse referral program which will be measured 6 times from July - December 2023 obtaining patients with a population of 411 patients and a sample of 218 patients. The results showed that women suffered more from hypertension (62.8%), the most age of hypertension ≥60 years (78.4%), and patients suffered from hypertension without comorbidities (60.6%). The most commonly used hypertension treatment was monotherapy (56.9%). The level of compliance of hypertensive patients in the referral program was high in the category of re-taking antihypertensive drugs (67.4%). The results of the analysis stated that there was a relationship between gender and age on compliance with taking antihypertensive medication. The conclusion of this study is that the level of compliance is included in the high category, and there is a relationship between gender and age on compliance with re-taking antihypertensive drugs.
