Abstract

Stem Cell Transplantation for Hematology Malignancy Abstract The incidence of hematological malignancies continues to increase and has a high rate of morbidity and mortality. Hematological malignancies are conditions of hematological cells such as erythrocytes, leukocytes, and platelets, growing uncontrollably and not dying so that they dominate the population and cannot work normally. Stem cell therapy in hematological malignancies, also known as bone marrow transplantation therapy, is the main therapeutic modality for hematological disorders and malignancies that supports the restoration of cellular components of blood, including monocytes, lymphocytes, basophils, eosinophils, neutrophils, erythrocytes, and platelets. Stem cells that can be used in hematological malignancies are a combination of hematopoietic stem cells (HSC) and mesenchymal stem cells (MSC) for bone marrow transplantation and immune restoration for hematological disorders. The use of HSCs with MSC co-therapy can help revive grafts from HSC transplants. Key words: malignancy, hematology, stem cell.

Abstract

Leprosy is a chronic granulomatous disease. Leprosy is caused by M. leprae. However, not all exposure to M. leprae causes the disease. The condition of the host immune system determines the pathogenesis of leprosy. Interleukin-10 work as a pro- and anti-inflammatory cytokine. Polymorphism in the IL-10 gene promoter affects the amount of IL-10 secretion. The amount of IL-10 secretion determines the body’s response to M. leprae. The purpose of this study was to determine the distribution of -2849 IL-10 gene promoter polymorphism in leprosy patient at RSUP dr. Mohammad Hoesin Palembang. This research is an observational descriptive study with cross sectional design. Polymorphism identification was performed using PCR-RFLP. A total of 50 samples were identified. Most of the leprosy patients at RSUP dr. Mohammad Hoesin for the period January – February were < 50 years old (73.47%), male (66%), MB leprosy (92%), and came from Malay-South Sumatra ethnicity (53.06%). The genotype frequency distribution was GG 93.88%, AG 6.12%, and AA 0%. The frequency of allele G was 96.94% and allele A 3.06%. The majority of leprosy patients at RSUP dr. Mohammad Hoesin had wild-type genotypes.

Abstract

Osteogenesis imperfecta is a disorder of the formation of collagen tissue that functions as connective tissue and is caused by a gene mutation that causes disturbances in the formation of type 1 collagen. This study aimed to describe the diagnosis of osteogenesis imperfecta. A 23-year-old man came to the rheumatology polyclinic of Dr. Mohammad Hoesin General Hospital Palembang with a complaint of recurrent fractures since ± 17 years ago. The patient also complained that the hearing in the left ear was slowly decreasing. The patient's right thigh was still pinned 2 years ago, but after being controlled by orthopedics, it was said that the bones were still not fused. Then the patient was referred to the rheumatology polyclinic for further examination and management. On examination, the patient was 145 cm tall and weighed 40 kg. He had blue-gray sclera, triangular facial appearance, right leg length 86 cm, left leg length 78 cm, blue sclera, and scoliosis, while secondary sex growth was within normal limits. From the examinations of bone survey and bone age, it was found osteoporotic bone structure, scoliosis, plate-screw in the middle 1/3 of the right femur, fracture union of the femur, bowing of the left femur, BMD Z-Score -3.0 means very low compared to the same age and gender. Management is more focused on supportive therapy with the aim of minimizing the occurrence of fractures, minimizing disability, and helping people with osteogenesis imperfecta to be independent and maintain overall health. The goal of orthopedic management is to treat osteogenesis imperfecta with fractures and prevent or correct bone deformity. In conclusion, osteogenesis imperfecta is a complex hereditary disease characterized by striking clinical variability necessitating a logical classification system. Disease management requires multidisciplinary experts and further research on therapeutic approaches such as bisphosphonates.

Abstract

Pulmonary tuberculosis is a global health problem that requires early and precise diagnosis for effective control. In this literature review, we compare several molecular diagnostic techniques used in the detection of pulmonary tuberculosis, such as Polymerase Chain Reaction (PCR), Whole Genome Sequencing (WGS), and Next Generation Sequencing (NGS), as well as Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-based Diagnostic Techniques. Polymerase Chain Reaction stands out for its high sensitivity, although time-consuming and high operational costs. Meanwhile, whole genome sequencing and next generation sequencing have detailed tuberculosis (TB) strain identification capabilities but have high costs and limited availability. On the other hand, CRISPR-based diagnostic techniques offer speed and low cost but are still in the advanced stages of development. Challenges in implementing new techniques include technical barriers, logistics, and improving sensitivity and specificity. Suggestions for future research include the development of more effective, faster, and affordable techniques, especially in developing portable diagnostic tests for accessibility of pulmonary tuberculosis diagnosis in various regions.

Abstract

For several years, COVID-19 has been a major problem in human health and also caused a global socio-economic crisis. However, the impact of viral load on patient mortality has not been thoroughly investigated. This study analyzes the association between CT value and the mortality of COVID-19 patients in inpatients at Mohammad Hoesin Hospital. This study used observational analysis with a longitudinal retrospective design. This study had 280 subjects. Fifty-four of the 280 patients died (19.3%) 33% had a CT <22.93; 16.7% had a CT 22.93-29.93; 31.5% had a CT 29.94-34.9, 18.5% had a CT >35 with p-value >0.05 for patient mortality. Analysis of age, comorbidities, and clinical severity of patient mortality showed a p-value <0.05. Meanwhile, gender analysis of patient mortality showed a p-value >0.05. These results indicate that the CT value cannot predict patient mortality. Patients with older age, comorbidities, and admission with severe clinical severity have a higher risk of death.
