Abstract

ABSTRACT   A kidney transplant is the ideal option for End Stage Renal Disease (ESRD). A successful kidney transplant will improve the excretory function of the kidney so that it can return the quality of life for patients with Chronic Kidney Disease (CKD) to normal. However, in some cases of transplant recipients, the grafted kidney does not function optimally due to several complications. Post-kidney transplant complications may occur, which are classified into two groups, namely surgical and medical complications. Cases of obstruction characterized by hydronephrosis of the graft kidney, a post-transplant surgical complication. The most common location is where the ureter implants into the bladder, namely the distal 1/3 ureter. The most important medical or non-surgical complications are rejection or rejection reactions ranging from acute, hyperacute and chronic. Chronic rejection is the gradual loss of function of the transplanted organ over several months to years after the organ is functioning normally. Chronic rejection is difficult to treat where the use of immunosuppression is of little use because the damage has already occurred, so the patient again needs another renal replacement therapy, namely hemodialysis. The following is a case report of a patient after a kidney transplant six years ago, experienced decreased kidney function again and had to receive hemodialysis therapy due to a kidney graft/hydronephrosis graft.

Abstract

Background: Pre-eclampsia (PE) is a hypertensive disorder in pregnancy and a significant cause of maternal and perinatal mortality and morbidity. Failure of spiral artery remodeling due to abnormal apoptosis, triggers disturbances in the mother and the baby’s growth. This study aimed to identify the profile of apoptotic marker genes and histopathological features of the placenta in pregnancies with pre-eclampsia.Methods: This study had used case-control method. Samples were taken from normal pregnancies (n=25) and pregnant women with pre-eclampsia (n=25) using a purposive sampling method from several hospitals in Jambi. qRT-PCR was used to examine apoptotic gene expression from placental tissue and hematoxyline eosin staining to view the placenta’s microscopic appearance. The targeted genes were BCL2-associated X (BAX) and B-cell lymphoma 2 (BCL-2). Histopathological changes of the placenta observed were syncytial node, cytotrophoblast, villous edema, hypervascularization, fibrosis stroma, atherosis, infarction, and thrombosis.Results: Relative BAX genes expression were increased once in placenta pre-eclampsia compared to controls, but not statistically significant (p-value>0.05). There was no difference between the decline of BCL-2 gene expression in pre-eclampsia placenta compared to the control (p-value >0.05). Histopathological changes in the placenta were syncytial node and cytotrophoblast (25 of 25), villous edema (19 of 25), hypervascularization (24 of 25), fibrosis stroma (22 of 25), atherosis (12 of 25), infarction (17 of 25), and thrombosis (24 of 25).Conclusion: The expression of BAX genes in pre-eclampsia tends to increase compared to normal pregnancy, and the expression of BCL-2 decreases.  The histopathological features of pre-eclampsia pregnancy placenta are mostly syncytial nodes, cytotrophoblasts, stromal fibrosis, and thrombosis.

Abstract

ABSTRACT Diabetes Mellitus (DM) type 2 is a common metabolic disorder characterized by hyperglycemia. The most frequent microvascular complication in diabetes patients is diabetic nephropathy. Dietary management is one of the approaches in DM treatment. Intermittent fasting as a dietary intervention has been shown to improve overall health, weight management, blood pressure, and glomerular filtration rate (GFR). This study aims to investigate the effects of intermittent fasting on urea and creatinine levels in Wistar rats with a diabetes model. This study is a quantitative experimental analysis conducted in vitro in a laboratory setting, using a pretest and post-test group design. There was no difference in urea and creatinine levels before and after the intermittent fasting diet treatment in Wistar white rats. The one-way ANOVA and Kruskal-Wallis tests indicated no significant differences among the groups, as evidenced by the statistical test results with p-values > 0.05 (not significant). There was no difference in urea and creatinine levels before and after the intermittent fasting diet treatment in Wistar white rats. Abstract

ABSTRACT Background Obesity is a global epidemic problem that can increase the risk of metabolic syndrome and insulin resistance. Modification of diet and moderate exercise can control metabolism. Alternate Day Fasting is one of the intermittent fasting diet modifications where the day alternates between normal and fasting diets. When fasting can consume food substances 20-25% of total energy. Aim this research objective was to determine the effect of the Alternate Day Fasting diet on body weight, and liver function. Method this research is an experimental study on male Sprague Dawley rats aged 2-3 months, weighing 150-250 grams, and no anatomical abnormalities, which are divided into 2 groups, namely control (n = 10) and Alternate Day Fasting (n = 10). Results the Alternate Day Fasting can reduce body weight (mean = 197.5) compared to controls (mean = 253.6). There was no difference in blood glucose (Mann Whitney; p> 0.05), Serum Glutamic Oxaloacetate Transaminase (Mann Whitney; p> 0.05), Serum Glutamic Pyruvic Transaminase (Mann Whitney; p> 0.05) between the control group and the Alternate Day Fasting diet. Conclusion Alternate Day Fasting Diet can lose weight, and does not result in an increase in Serum Glutamic Oxaloacetic Transaminase and Serum Glutamic Pyruvic Transaminase. Abstract

ABSTRACT Background Peak flow respiratory rate is one of the tests used to assess lung function, monitor for diseases caused by pulmonary obstruction, and respond to pulmonary treatment. Many factors affect the peak expiratory flow which will also affect lung function mainly due to anthropometric differences. Aim The research objective was to determine the effect of body mass index on peak expiratory flow rate. Method forty-three women, inclusion criteria were in good health, did, n't have obstructive respiratory disorders, and did not smoke. The students measured height, weight, waist circumference, and peak flow respiratory rate using the Mini Wright Flowmeter. Result in the peak expiratory flow rate of female students (mean=228.37). There was no significant correlation between female students' peak expiratory flow and body mass index (p = 0.28, r = -0.19). Conclusions The effect of peak expiratory flow on body mass index is not statistically significant for students of the Faculty of Medicine, Jambi University,

Abstract

Latar Belakang. Preeklampsia (PE) merupakan penyebab mortalitas dan mobirditas utama pada maternal dan perinatal. Kegagalan remodelling arteri spiralis pada sel trofoblas akan menyebabkan keadaan hipoksia dan memicu terjadinya gangguan pada pertumbuhan bayi selama di dalam rahim yang outputnya dapat berupa berat badan lahir rendah (BBLR) ataupun persalinan prematur. Tujuan Untuk mengidentifikasi profil usia ibu dan berat badan lahir bayi pada kehamilan dengan preeklampsia dan kehamilan normal di Kota Jambi. Metode Penelitian ini adalah penelitian deskriptif analitik pendekatan Case- control. Sampel data berat badan lahir bayi diambil dari 50 ibu melahirkan (25 ibu PE dan 25 ibu normal) dengan metode purposive sampling di RSUD Raden Mattaher Jambi. Identifikasi profil usia ibu dan berat badan lahir bayi dilakukan analisis data menggunakan pearson chi square. Hasil Pada penelitian ini didapatkan, berdasarkan usia ibu, kasus terbanyak pada kehamilan PE adalah usia <20 dan >35 tahun yang tergolong usia berisiko tinggi yaitu sebanyak 55,55 %. Berdasarkan berat badan lahir bayi, kasus BBLR didapatkan terbanyak pada kelompok PE yaitu sebanyak 92,30% dibandingkan dengan kelompok kehamilan normal dan bermakna secara uji statistik. Kesimpulan: Usia ibu berisiko tinggi lebih banyak pada kelompok kehamilan PE dibandingkan kelompok kehamilan normal, serta berat badan bayi lahir rendah lebih tinggi pada kelompok kehamilan PE dibandingkan kelompok kehamilan normal, dan bermakna secara statistik (chi-square<0,05).

Abstract

Background: Toddlers who experience wasting will experience delayed growth and development if it occurs in the long term, a decrease in the immune system, are susceptible to infectious diseases, and can cause death if they experience severe wasting for a long time. This study aims to determine the family determinants of wasting in toddlers in Jambi City. Method: This study is a descriptive-analytical study with a Cross-Sectional research design. The study was conducted on 35 toddlers experiencing wasting who were treated at the Community Health Center in Jambi City and did not experience congenital abnormalities, were not undergoing steroid treatment, or did not experience other chronic diseases. The mothers of children suffering from wasting were given questionnaires, and interviews were conducted. Result: The results of the study showed that 45.7% were aged 31 to 40 years, 48.6% of mothers' education graduated from high school, 57.1% family income was less than 2 million, 82.9% of mothers did not work, 45.7% of drinking water sources came from PDAM water, the number of children in the family ≤ 2 children was 62.9%, and there was a relationship between maternal age, maternal education, family income, employment history, drinking water sources and number of children with wasting events (p-value <0.05). Conclusion: This study concludes that family determinants influence the incidence of wasting in toddlers in Jambi City. Keywords: Determinant; Family ; Todldlers; Wasting;

Abstract

ABSTRACT Background: The government's stunting prevention measures begin with improving nutrition, namely during adolescence, in the case of prospective brides, pregnant women, postpartum mothers, and toddlers. Empowerment for women in the community or at home is expected to reduce stunting problems and various child health problems. This study aims to screen for risk factors and prevent stunting in women of childbearing age. Methods: Thirty-six women of childbearing age attended this research activity in the Olak Kemang Health Center working area. The activities included pre-marital nutrition education for women of childbearing age, examination of weight, height, arm circumference, and measurement of blood haemoglobin. Results: The body mass index of Women of Childbearing Age was the most in the thin category, namely 39% (BMI <18.5), haemoglobin levels >12 as much as 58,33%, upper arm circumference above 22 as much as 78% and there was an increase in knowledge assessed from the results of the pretest and posttest, it is known that the average pretest value is 60, and the posttest value is 80. Conclusion: Management of iron tablet administration is needed for women of childbearing age who experience anaemia and increased nutritional intake in women of childbearing age. Abstract

Background: Hypertension is a silent killer that causes 10.44 million people to die every year. The prevalence of hypertension based on the 2018 Riskesdas results has also increased, as well as in Jambi Province. In pre-clinical and clinical studies, intermittent fasting and physical activity are non-pharmacological therapies for hypertension and reducing risk factors for cardiovascular disease. This study aims to determine the effect of intermittent fasting and moderate physical activity on systolic blood pressure and creatinine in hypertension sufferers in Jambi City. Methods: This research is a quasi-experimental research with a cross-sectional research design. The research was conducted on 45 patients suffering from hypertension who received treatment at the Community Health Center in Jambi City, and did not suffer from heart disease, kidney failure, history of stroke or diabetes mellitus. The research sample had urea and creatinine checked before and after the study, carried out intermittent fasting twice a week for 1 month, and had their blood pressure measured once every three days. Respondents received guidance regarding moderate physical activity by providing exercise guidance and sports videos of moderate physical activity. Result: The results of the study showed that intermittent fasting and moderate physical activity in hypertensive patients can reduce systolic pressure, systolic blood pressure after intermittent fasting (mean=135.2+19, 9) compared to before (mean=144.1+21.1) (t-test; p=0.001) as well as reducing urea levels, after the intermittent fasting diet, the average creatinine level was 23.6 (11.9 - 65.7) compared to previously 20.4 ( 11 - 74) (t-test; p=0.001). Conclusion: The conclusion of this study is that intermittent fasting and moderate physical activity can reduce systolic blood pressure and urea in hypertensive sufferers.. .

Abstract

Overweight and obesity is a condition in which there is an imbalance between the entry and exit of energy for a long time. Intermittent Fasting is one of the dietary interventions that can affect body composition such as body fat percentage and thick folds of fat under the skin. The purpose of this study was to determine the effect of Intermittent Fasting type 5:2 on body fat percentage and thick folds of fat under the skin in individuals with overweight and obesity. This study is an experimental study using pretest and posttest group design. The number of samples was 50 people with a consecutive sampling method. The statistical test used was the paired t-test. The results showed that intermittent fasting had a significant effect on body fat percentage (p<0.001), thick folds of fat under the skin of the biceps (p=0.009), and triceps (p<0.001), while the supra iliac and subscapular parts decreased but statistically insignificant (p=0.093) and (p=0.217).
