Abstract

Latar Belakang: Hiperparatiroid primer merupakan suatu kondisi klinis yang disebabkan peningkatan sintesis hormon paratiroid (HPT) akibat tumor paratiroid. Hiperparatiroid primer dapat mengganggu metabolisme kalsium. Dengan demikian, diagnosis hiperparatiroid primer secara umum dapat ditentukan melalui pemeriksaan kadar kalsium serum dan HPT serta ditemukannya tumor paratiroid melalui pemeriksaan pencitraan. Secara garis besar tatalaksana hiperparatiroid primer terdiri dari tatalaksana medikamentosa dan intervensi pembedahan. Tatalaksana medikamentosa meliputi pemberian suplementasi vitamin D, kalsimimetik dan bifosfonat. Intervensi pembedahan untuk mengangkat tumor paratiroid merupakan terapi definitif hiperparatiroid primer. Klinisi perlu meningkatkan pengetahuan dan keterampilan terhadap hiperparatiroid primer agar pelayanan pasien lebih komprehensif.

Abstract

ABSTRACT Background: Type 2 diabetes mellitus (T2DM) is caused by reduced insulin sensitivity, which can lead to insulin shortage. Several recent investigations have found that vitamin D is connected with insulin secretion and sensitivity. Vitamin D insufficiency is linked to poor glucose regulation. Aim: To determine the correlation between serum 25-hydroxyvitamin D levels and glycemic control in Type 2 DM patients. Method: T2DM patients' serum levels of 25-Hydroxyvitamin D, Fasting Blood Glucose (FBG), 2-hours postprandial blood glucose (2-h PBG), and HbA1c were examined in this cross-sectional analytical observational study. Statistical analysis was also performed. Result: From 49 samples included, the number of male subjects was almost the same as female, mean age was 58 years. There was a decrease in serum 25-Hydroxyvitamin D levels with a median of 27.87 ng/ml (12,626 -94,367). Glycemic control was found to be poor, the median of FBG levels was 134 mg/dl (61-339), 2-h PBG levels was 208 mg/dl (93-488) and HbA1c levels was 7.5% (5.6-12.8). Analysis using Spearman correlation between serum 25-Hydroxyvitamin D levels with levels of FBG, 2-h PBG and HbA1c showed values of r = -0.538 and p = 0.000, r = -0.354 and p 0.013, r = -0.501 and p 0.000. Conclusion: There was a statistically significant negative correlation between serum 25-Hydroxyvitamin D levels and levels of FBP, 2h-PBG and HbA1c in T2DM patients. Abstract

Anemia is a serious public health problem and has an impact on the life cycle of adolescents, especially adolescent girls. Adolescent girls who suffer from anemia have limitations in the growth process, learning capacity, ability to focus on daily activities, are susceptible to infection, have a tendency to drop out of school, physical fitness levels, and work productivity. The risk of anemia in adolescent girls tends to increase with age and coincides with the teenage years when the growth process occurs most rapidly. The ages between 12 and 15 years, when nutritional needs are very high, are known to have a very high risk of suffering from anemia. One of the educational media that can be easily understood by adolescents is animated videos. The purpose of this community service is to increase knowledge about preventing anemia in adolescents. The activities carried out are in the form of education, using educational videos to prevent anemia in adolescents. The method used is socialization and health education to adolescent girls through videos and explanations of the consumption of iron tablets distributed to all adolescent girls. The location of the activity was carried out at SDN 001 Nongsa. The stages of the activity include the stages of preparation, implementation and reporting. Data from the results of community service activities showed an increase in the capacity of adolescent girls, showing a change of 22 points in the pre-post test education, HB level examination from 30 people. The examined adolescent girls who experienced anemia were 16 people (53%) and 14 people did not experience anemia. All adolescents showed high enthusiasm for this activity and hoped that anemia and nutritional status education activities could be carried out sustainably to improve the fitness and health of adolescent girls.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a growing global health threat, where chronic hyperglycemia and insulin resistance can affect coagulation profiles.Objective: This study aims to examine coagulation factor profiles and platelet counts in T2DM patients. Methods: A descriptive cross-sectional observational study using data from prothrombin time (PT), activated partial thromboplastin time (APTT), international normalized ratio (INR), and platelet counts of T2DM patients admitted to the Internal Medicine Ward of Dr. M. Djamil General Hospital, Padang, in October 2023. Data analysis was performed descriptively using SPSS v26 software. Results: Of the 39 patients analyzed, the distribution of characteristics showed a male predominance of 64.1%, with the largest age group being 45–59 years (48.7%). The most common complication associated with T2DM was chronic kidney disease, identified in 82.1% of the study population. Evaluation of hemostasis parameters showed that APTT was prolonged in 56.4% of patients. Conversely, PT (87.2%), INR (87.2%), and platelet count (66.7%) were within reference ranges. Conclusion: This study found most coagulation parameters normal, but a relatively high proportion had prolonged APTT, underscoring the need for individualized coagulation monitoring in T2DM, especially with renal complications

Abstract

Marine-Lenhart syndrome (MLS) is an uncommon disorder characterised by the concurrent presence of Graves’ disease and hyperfunctioning thyroid nodules. Its global prevalence ranges between 2.7- 4.1% of Graves’ disease cases, presenting significant challenges in diagnosis and management. MLS is often difficult to detect due to limitations in autoimmune serology tests, thyroid ultrasonography, and access to nuclear medicine facilities. Diagnosis relies on a combination of clinical hyperthyroidism symptoms, the presence of specific thyroid antibodies, and scintigraphy findings that reveal "hot" or "cold" nodules. The primary therapeutic options for MLS include antithyroid drugs, radioactive iodine (RAI) therapy, and surgery. RAI is commonly employed as definitive treatment, often requiring higher doses to address the resistance of hyperfunctioning nodules. Surgery is indicated in cases involving large nodules, compressive symptoms, or suspected malignancies. Treatment strategies should be tailored to the individual clinical characteristics of each patient to minimize complications and ensure optimal outcomes. Advancing the diagnosis and management of MLS requires enhanced access to advanced diagnostic technologies and improved healthcare provider proficiency in identifying and treating the syndrome. A multidisciplinary and integrated approach is essential for achieving favorable clinical outcomes for MLS patients.

Abstract

Latar Belakang: Metode kontrasepsi jangka panjang (MKJP) merupakan metode untuk mencegah kehamilan lebih dari tiga tahun. Prevalensi MKJP nasional tahun 2022 sebesar 22,41 % dari target RPJMN 28,39% pada tahun 2024. Penggunaan kontrasepsi masih didominasi oleh non-MKJP.  Penggunaan MKJP di Kecamatan Lubuk Kilangan tahun 2022 sebesar 15,3%. Objektif: Penelitian ini bertujuan untuk mengetahui hubungan karakteristik ibu dan dukungan suami dengan penggunaan MKJP pada akseptor KB di Puskesmas Lubuk Kilangan. Metode: Penelitian ini merupakan penelitian analitik dengan desain cross sectional. Penelitian ini dilakukan di Puskesmas Lubuk Kilangan dengan jumlah sampel 200 akseptor KB. Pengambilan sampel dengan consecutive sampling menggunakan kuesioner. Analisis data dilakukan menggunakan uji chi square dan multivariat regresi logistik. Hasil: Hasil penelitian menunjukkan lebih dari separuh ibu dengan usia tidak berisiko (51,0%) dan paritas tinggi (55,5%); sebagian besar ibu memiliki tingkat pendidikan tinggi (71,0%), pengetahuan baik (80,0%), dan mendapatkan dukungan suami (73,0%). Kesimpulan: Terdapat hubungan antara karakteristik ibu (usia, paritas, dan pengetahuan) dan dukungan suami dengan penggunaan MKJP di Puskesmas Lubuk Kilangan. Faktor paling dominan mempengaruhi penggunaan MKJP adalah dukungan suami.

Abstract

Hypothyroidism is a clinical condition characterized by reduced production of thyroid hormones and is among the most common endocrine disorders, affecting about 3% of the general population, with a higher prevalence in women (5.1%) than in men (0.9%). In Indonesia, the prevalence of hypothyroidism is estimated at 5-10%. Despite achieving normal thyroid stimulating hormone (TSH) levels with levothyroxine (LT4) therapy, many patients continue to experience symptoms like fatigue, depression, and cognitive impairment, which impact their quality of life. A study of 969 hypothyroid patients found that 77.6% were dissatisfied with their treatment and reported low quality of life scores. Although LT4 has been the standard therapy for nearly 50 years due to its stability and the peripheral conversion of thyroxine (T4) to triiodothyronine (T3), recent recommendations from experts in developed countries suggest using liothyronine (LT3) monotherapy or a combination of LT3 and LT4 in selected cases.

Abstract

Diabetes Mellitus is a chronic metabolic disorder caused by impaired insulin secretion and/or insulin resistance. Genetic testing through genomic sequencing is one of the modalities available for diabetes mellitus. Genomic sequencing is the process of analysing DNA obtained from a blood sample (or alternatively from tissue samples). Broadly, genomic sequencing plays two major roles in diabetes mellitus: supporting diagnosis and guiding therapeutic approaches. In its development, genomic sequencing has proven valuable for diagnostic investigation in type 1 diabetes and maturity-onset diabetes of the young (MODY), as well as for assessing the polygenic risk score (PRS) in type 2 diabetes. Establishing a definitive diagnosis allows for the selection of individualised therapy. Several randomised controlled trials have demonstrated that the risk of developing diabetes can be reduced by up to half if detected at an earlier stage. Moreover, genomic sequencing can identify genetic variants that influence responsiveness to antidiabetic treatments. At present, several potential antidiabetic agents targeting novel pathways are under development and in various stages of clinical trials. The application of genomic sequencing thus facilitates the implementation of individualised therapy, ultimately contributing to the realisation of precision medicine.

Abstract

Seorang perempuan usia 34 tahun datang dengan keluhan lemah pada kedua tangan dan kaki, benjolan di leher, riwayat berdebar-debar, penurunan berat badan dan siklus menstruasi yang tidak teratur. Pasien didiagnosis dengan tiroiditis Hashimoto dan sindrom Gitelman. Pasien diterapi dengan levotiroksin dan koreksi elektrolit. Pasien kemudian pulang dalam keadaan perbaikan klinis.

Abstract

This study aims to determine the impact of monetary policy on the operational and financial stability of Bank Sumut Syariah KCP Stabat, as well as the strategies employed in response to these changes. The method used is qualitative descriptive with data collection techniques in the form of interviews, observations, and documentation. The results of the study indicate that monetary policies such as changes in interest rates, inflation, and the rupiah exchange rate have a significant impact on financing activities and the mobilisation of public funds. In financing activities, changes in margins and economic conditions cause customers to delay applications or experience payment difficulties, which the bank addresses through strict selection, restructuring, and product innovation.
