Abstract

Scabies is a skin disease caused by the mite Sarcoptei scabei hominis variety. This disease is a public health problem, especially in tropical and subtropical climates. According to the World Health Organization (WHO), globally it is estimated that as many as 300 million people are currently infected with scabies with a prevalence ranging from 0,2% to 71%. Scabies attacks in groups. Because easy transmission occurs through skin contact either directly or indirectly. Appropriate holistic management using a family medicine approach is needed to achieve therapeutic success and prevent transmission to the community. Patient R, a male student aged 10 years, complained of itching between the fingers, especially at night, since 3 weeks ago. Internal risks in the form of lack of personal hygiene, lack of knowledge, curative treatment patterns, and early adolescence. External risks include similar complaints in the surrounding environment, poor family hygiene, humid houses and poor ventilation, low socioeconomic status, lack of family knowledge. A holistic and comprehensive management effort uses a family medicine approach in the form of education about the causes, transmission, treatment and prevention of disease. The results obtained in the form of a better understanding of the disease and changes in behavior that have an impact on the success of therapy. Holistic management with a family medicine approach is needed for patients with scabies to identify problems in various aspects in order to support the success of therapy and prevent transmission.

Abstract

The prevalence of chronic energy deficiency (CED) in Lampung Province in 2013 was 21,3% for pregnant women and 17,5% in non-pregnant women, in Bandar Lampung was 24,5%. The problem of chronic energy deficiency (CED) in women is influenced by various factors, namely food intake, incorrect understanding of nutrition, the entry of undernourished food products from outside and chronic infectious diseases.This study was conducted to determine the relation of food intake with CED in pregnant women in working area of Sukaraja health center in Bandar Lampung.This type of researchwas quantitative using cross sectional approach with consecutive sampling technique. The inclusion criteria were pregnant women in the first and second trimesters, aged 20-35 years, and willing to be respondents. The exclusion criteria were women with chronic infection, anemia, and hyperemesis gravidarum. The research respondents were 71 pregnant women. Food intake data were obtained from the Semi quantitative food frequency questionnaire and CED data by measuring the upper arm circumference (LILA). Data were analyzed by fisher exact test.The results showed that there were 8.5% of respondents suffering from CED. Respondents who have less energy intake (36.6%), less carbohydrate intake (69.0%), less protein intake (11.3%), and less fat intake (39.4%). Based on the results of the analysis, it was found that energy and protein intake was associated with CED (p = 0.022) and (p = 0.017), while carbohydrate and fat intake was not related to CED (p = 0.167) and (p = 0.204). The conclusion of this study is that there is a significant relationship between energy and protein intake with the incidence of CED in pregnant women in the work area of Sukaraja Health Center, Bandar Lampung City.

Abstract

Tinea corporis is an infectious skin infection caused by dermatophytes that colonize and obtain nutrients from keratin, causing infections in the skin in the form of inflammatory reactions. The incidence of tinea corporis is high in tropical and subtropical climates as well as in populations with low socioeconomic status who live in crowded environments and low hygiene. The diagnosis of tinea corporis is based on history taking and physical examination. This case will discuss the identification of risk factors, management and prevention of tinea corporis in cracker makers based on patient-centered and family approach. This study is a case study with data obtained from history taking, physical examination, home visits, and patient medical records. Case, 30-year-old woman works as a cracker maker, lives in a main family, with a moderate daily activities, lack of personal hygiene and environment, and good relations between family members. The patient complained of itching accompanied by a burning sensation and redness around the side folds of the breast, back and right thigh since about 1 week before coming to the health center. After a holistic intervention with the educational method, it can be seen that there are good developments in patients ranging from decreased clinical symptoms to changes in good behavior towards personal and environmental hygiene, conclusions, family medicine services are effective in the management of tinea corporis. Providers resolve clinical problems and also deal with internal, external, psychosocial and environmental risks.

Abstract

Adequacy of nutrition for pregnant women is very important because it can contribute to both the mother and the baby. Pregnant women with poor nutritional status can cause a risk of fetal development, disability in infants, low birth weight, and death before birth. Nutrition in pregnant women is influenced by the mother's nutritional knowledge during pregnancy. This study aims to determine the relationship between nutritional knowledge in pregnant women and weight gain during pregnancy. The design of study was cross sectional analytic using primary data obtained from the questionnaires and secondary data obtained from the KIA / KMS book that include maternal weight. Sampling was examined at Puskesmas Bandar Lampung which is Kedaton, Way Kandis, Satellite, Sukaraja, Sukabumi, and Simpur Puskesmas in 2018. The total sample is 130 pregnant womans with purposive sampling technique. The data analysis using Chi Square with α = 0.05 and CI = 95%. The results of the research showed that there is a relationship between nutritional knowledge in pregnant women and weight gain during pregnancy in Bandar Lampung Primary Health Center with a value of p = 0.003. From the results of this study it can be concluded that nutritional knowledge in pregnant women influences the weight gain during pregnancy.

Abstract

Chronic obstructive pulmonary disease (COPD) is a slow progressive and irreversible or partially irreversible lung disease. Risk factors for COPD include exposure to cigarette smoke, air pollution, exposure to dust and smoke in the work environment. The goal of COPD management is to reduce symptoms, prevent acute exacerbations and decreased lung function. This study is a case report with data obtained through history taking (autoanamnesis and alloanamnesis), physical examination and home visits to complete family data, psychosocial and environmental data. Patient Mr. J 49-year-old diagnosed with COPD. Patients have aspects of internal risk in the form of a history of smoking for 16 years, lack of patient knowledge about the illness, feeling bored and stressed out of not working. The external risk aspects are in the form of lack of family knowledge about the disease and its complications as well as curative treatment patterns, a history of vehicle smoke exposure at work for 27 years. Home visits were done three times to intervene in the form of education and counseling in patients and families about the causes, risk factors, clinical symptoms, prognosis, to prevent complications. In the evaluation, it was found that the patient had shown improvement. The diagnosis and management of these patients has been done holistically, patient centered, family approached, community oriented and based on several theories and current research. In the process of behavior change, Mr. J has reached the trial stage for COPD diet and physical exercise after being given an intervention.
