Abstract

Background: Tuberculosis (TB) is one of most crucial public health issues around the world. TB is an entity of a complex disease with the socio-economic aspect that has very strong correlation in regard to combat this disease. Migration from developing country to developed country inevitably possesses big influence on global epidemiologic of TB. In Australia, TB still becomes the main threat not only in native population but also regarding the migrant movement into Australia. Indonesia is one of among the TB endemic countries with high TB cases, in which not merely due to its high TB prevalence and incidence but also influenced by very high and dense population.Aim: This literature aims to review the clinical, epidemiological, and microbiological aspects of tuberculosis as a comparison between developed country (Australia) and developing country (Indonesia).Conclusion: Tuberculosis cases in Australia remains low compared to Indonesia, however, the close proximity to adjacent developing countries with high endemic of TB contributes significantly to increase number of TB in Australia. Tuberculosis can be cured by following the treatment guidelines with proper monitoring. Moreover, the collaboration between public and private sector along with active collaboration from the family or people surrounding the patients is required to eliminate TB disease.

Abstract

Vitamin B6 or pyridoxine has been shown to modulate hypoxia and inflammation, but it has not yet confirmed their local effects on the peritoneum. Although there was no study regarding this issue, pyridoxine effects on suppressing these two pathways look promising as a preventing agent against peritoneal adhesion. The development of intraperitoneal adhesion post-operation is one of the most common complications after abdominal surgery. Approximately 95% of patients who underwent laparotomy can develop adhesions in the future. Pathogenesis of adhesion consists of three important processes which were induced by trauma: trauma or direct tissue injury-induced suppression of degradation of fibrinolytic matrix and extracellular system, inflammation response with cytokine production, particularly TGF-? and interleukin, also tissue hypoxia as the impact from circulation disorder toward mesothelial cell and sub-mesothelial fibroblasts, which results as increasing hypoxia-inducible factor-alpha (HIF-1?) and vascular endothelial growth factor (VEGF), which are responsible for generating collagen and angiogenesis process. Pyridoxine has biological activity and interesting characteristics in avoiding intra-peritoneal adhesion by dealing with inflammation effects and hypoxia. From the inflammation process, pyridoxine has been shown can hinder macrophage, decrease IL-6, suppress TLR-mediated TAK1 phosphorylation, JNK, IKK-I?B?, which can cause a reduction in NF-?B activation and NLRP3-mediated caspase-1, also suppress iNOS and cyclooxygenase-2 (COX2), which contributed to developing intra-peritoneal adhesion. Vitamin B6 (Vit B6), atau piridoksin telah ditunjukkan dapat memodulasi efek hipoksia dan inflamasi, tetapi belum dikonfirmasi efek lokalnya pada peritoneum. Walaupun belum ada penelitian langsung, efek supresi vitamin B6 terhadap kedua jalur tersebut sangat menjanjikan sebagai agen prevensi adhesi peritoneal. Perkembangan adhesi intraperitoneal pasca operasi adalah salah satu komplikasi paling umum setelah operasi abdominal. Sekitar 95% pasien yang menjalani laparotomi akan mengalami perlengketan di kemudian hari. Patogenesis adhesi melibatkan tiga proses penting yang diinduksi trauma: trauma atau kerusakan jaringan langsung menginduksi penghambatan sistem degradasi matriks fibrinolitik dan ekstraseluler, respons inflamasi dengan produksi sitokin, terutama TGF-? dan interleukin, serta hipoksia jaringan sebagai akibat dari gangguan suplai darah ke sel mesothelial dan fibroblas submesothelial, yang mengarah pada peningkatan hypoxia inducible factor-alpha (HIF-1?) dan faktor pertumbuhan endotel vaskular (VEGF), yang bertanggung jawab untuk pembentukan kolagen dan angiogenesis. Vitamin B6 menyajikan sifat dan aktivitas biologis yang menarik untuk mencegah adhesi intraperitoneal dengan mengatasi efek inflamasi dan hipoksia. Pada jalur inflamasi, vitamin B6  terbukti menghambat makrofag, menurunkan IL-6, menghambat fosforilasi TLR-mediated TAK1, JNK, serta IKK-I?B?, yang menyebabkan penurunan aktivasi NF-?B and NLRP3-mediated caspase-1, serta menghambat iNOS dan cyclooxygenase-2 (COX2) yang berkontribusi terhadap pembentukkan adhesi intraperitoneal.

Abstract

Prebiotik adalah komponen makanan yang tidak dapat dicerna di dalam tubuh dan mendorong pertumbuhan atau aktivitas mikroba usus yang bermanfaat seperti bakteri asam laktat (BAL). Namun, kemampuan Lactobacillus yang diisolasi untuk memanfaatkan prebiotik masih sedikit diketahui. Penelitian ini bertujuan untuk mengetahui pengaruh berbagai jenis prebiotik sebagai sumber karbon terhadap metabolisme isolat Lactobacillus indigenous dari teh kombucha, dadih (susu fermentasi tradisional) dan bambu yang digunakan untuk wadah dadih. Kemampuan isolat Lactobacillus dalam memetabolisme berbagai prebiotik sebagai sumber karbon dilakukan dengan menumbuhkan BAL dalam media MRSB dimana kandungan glukosa diganti dengan prebiotik yaitu fruktooligosakarida (FOS), galaktooligosakarida (GOS) dan inulin, kemudian penurunan kadar glukosa diukur setelah inkubasi 24 jam. Pengukuran total glukosa dilakukan dengan metode fenol sulfat. Hasil penelitian menunjukkan keseluruhan isolat Lactobacillus mengalami peningkatan jumlah sel selama inkubasi 24 jam yaitu dari 0,52 menjadi 1,26 log. Penurunan kadar glukosa berkisar antara 3,80 ± 0,44% sampai 91,33 ± 4,83%. Penurunan kadar glukosa tertinggi terjadi pada isolat L. plantarum1 RB210 yaitu 91,33 ± 4,83% dan tidak berbeda nyata (P>0,05) dengan L. paracasei RK41 dan L. paracasei RL2 yang ditumbuhkan pada media inulin. L. plantarum1 MK2 menunjukkan penurunan pH tertinggi yaitu 1,91 ± 0,08 dan tidak berbeda nyata (P>0,05) dengan L. rhamnosus MY2 pada media yang mengandung GOS. Hasil penelitian ini menunjukkan bahwa isolat Lactobacillus Indigenus secara definitif dapat menggunakan prebiotik sebagai sumber karbon untuk metabolismenya. Hasil penelitian ini memberikan dasar pengaruh penggunaan probiotik dan prebiotik untuk aplikasi sinbiotik sehingga dapat memodulasi mikrobiota usus.

Abstract

The increase in mask waste and a lack of understanding about the management of single-use mask waste raises the potential for environmental pollution and COVID-19 transmission. Aim: This study aimed to assess the level of understanding of medical mask waste management and strengthening health protocols in controlling COVID-19 infection in Banjar Purwa Santhi, Benoa, Badung Regency. Methodology and Results: A cross-sectional method and descriptive study design were used with a sample of 100 respondents selected via stratified sampling. The research and service activities at Banjar Purwa Santhi Benoa include public campaign and assessment of the level understanding of medical mask waste management. Based on univariate analysis, 91% of respondents know that mask waste contains hazardous and toxic substances. If mask waste is disposed carelessly, 98% of respondents stated that it can pollute the environment, and 98% of respondents also stated that it has the potential to become a means of transmitting COVID-19. However, 85% of respondents disposed of the masks in the household trash. Conclusion, significance and impact study: Respondents had a good level of knowledge about health protocols and mask waste management but have not separated masks from household waste. By holding the community service activity, it is hoped that changes in community behavior will occur.

Abstract

Background: The purpose of this study was to determine whether the cARV therapy correlates with the serum level of IL-6, ferritin, and hepcidin in HIV infected patients with ACD.Methods: A cross-sectional study conducted in HIV patients with ACD, aged 15-65 years with exclusion criteria such as chronic renal disease, chronic liver disease, treated for anemia in the last 3 months, taking iron supplements in the last 3 months, history of blood transfusion in the last 1 year, suffering from acute infection, tuberculosis infection, malignancy, hepatitis C virus infection, acute hypersensitivity reaction and pregnancy. Blood specimen examined by high sensitivity human IL-6 In vitro ELISA, DRG Hepcidin- 25 ELISA, serum ferritin by agglutination method and CD4 cell count by flow cytometry. Results: A total of 86 HIV patients with ACD consist of 42 subjects with experienced cARV and 44 subjects with naive cARV. There are several differences between cARV naïve and experienced cARV including: Hb (g/dl) [ 10.8 ( 9.88 – 11.75) Vs. 11.60 (11.00 – 12.34) p: 0.058 ], serum iron (mg/dl) [ 50.00 (31 – 66.8) Vs. 71.40 (56.00 – 108.00) p: 0.121 ), serum ferritin (ug/dl) [ 630 ( 194 – 1101) Vs. 195.40 (128.25 – 589.47) p: 0.008 ], IL-6 (pg/mL) [7.20 (3.88 – 10.76) Vs. 4.03 (1.69 – 8.49) p: 0.009), serum hepcidin (ng/mL) [45.54 (16.74 – 95.31) Vs. 23.35 (6.58 – 49.99) p: 0.007] , CD4 cell count (cell/uL) [31.5 (14.25 – 90.75) Vs. 307 (168.5 – 458.75) p : < 0.001)] There is a significant inverse correlation between the cARV therapy with level of serum IL-6 ( r: - 0. 285, p: 0.008), ferritin (r: - 0.321, p: 0.007) and hepcidin (r: - 0.293, p: 0.006). While there was a significant positive correlation between the cARV therapy with CD4 cell count (r: 0.676, p: 0.001). There was no correlation between the cARV therapy with serum iron levels ( r: 0.176, p: 0.121).Conclusion: There is an inverse correlation between the cARV therapy with serum level of IL-6, ferritin, and hepcidin in HIV patients with ACD.  Keywords: cARV, ACD, IL-6, Ferritin, Hepcidin.

Abstract

Bali cattle is known as pioneer’ cattle resulted by their high ability to adapt in poor quality of feed so that was assumed for finding the specific species of lactic acid bacteria.  The study aimed to investigate the potency of lactic acid bacteria isolated from the gastrointestinal tract of Bali’ cattle as a candidate for new probiotic. Fifteen isolates of lactic acid bacteria isolated from 15 intestinal tracts of Bali cattle were grown on the specific medium deMan Rogosa Sharpe (MRS) broth followed by Gram staining, catalase test, the formation of CO2 gas, and antimicrobial activities against Bacillus cereus. The result of this study confirmed 15 isolates were Gram-positive bacteria, and 14 out of 15 isolates were categorized as homofermentative. Probiotic potency test showed 11 out of 15 isolates were tolerant against lower pH (pH 2, 4 and 6), and also resistant to bile acid with NaDC concentration 0.2, 0.4, and 0.6 mM.  Identification of isolate using API 50CH test kit found 4 out of 5 isolates as Lactobacillus sp. and the remainder as Lactococcus sp. The study concluded isolates of lactic acid bacteria isolated from the gastrointestinal tract of Bali cattle showed potential as a probiotic candidate.

Abstract

To date, pre-eclampsia (PE) still a problem of Maternal Fetal Medicine related to high incidence, maternal and neonatal morbidity and mortality. Pre-eclampsia is caused by pregnancy; however, the mechanism has not been established so it is still a disease of theories. This relates to differences in treatment, resulting in different ways of prevention and output of PE itself. Recently, the role of F2-IsoP ,SREBP-2 and 2-ME was suspected to be very important in the mechanism of the PE. Meanwhile, the placenta acts as a source regulatory protein production, so that the material of this study was taken from placental tissue. The study objective was to prove the high F2I-soP, high SREBP-2 and low 2-ME expression in the placenta as risk factors for PE Case control study has been conducted in the department of obstetrics and gynecology Sanglah Hospital with 62 samples in 2015. The case group consisted of 31 mothers with PE and control groups consisted of 31 non-PE mother. The study material is a placental tissue. F2-IsoP and SREBP-2 expression study was performed using immunohistochemistry and 2-ME with ELISA techniques in Pathobiology Laboratory Faculty of Veterinary Medicine Udayana University. Data were analyzed with chi square test and discriminant using SPSS. The statistical test results are presented in tabular form and narrative. In this study, it was found that high expression of F2-IsoP increased the risk of PE  4 times higher (OR = 4.44; 95% CI = 1.53 to 12.94; p = 0.005) ; high expression of SREBP-2 increased the risk of PE 8 times higher  (OR = 8.19, CI95% = 2,311 to 29.073; p = 0.001) and low expression of 2-ME increased the risk of PE 5 times higher (OR = 5.23; CI95% = 1.75 to 15.55; p = 0.002). On the discriminant test, we obtained contributing risk factor for the occurrence of PE were SREBP-2, F2Isop and 2-ME (p = 0.002) respectively. Conclusion, high F2-IsoP expression, high SREBP-2 expression and low 2-ME expression in placenta were risk factors for PE. Abstract

Bali cattle with its distinctively genetic characteristics, namely simple life or easy to adapt on unfavorable environmental condition was known as cattle pioneer. These characteristics were due to specific type of lactic acid bacteria (LAB). These facts indicate that the LAB identification with specific advantages was absolutely necessary for further research in order to prevent incorrect interpretation. Generally, molecular approach is the accurate method of identifying microorganism, and in this case, we used the analysis of the 16S rRNA gene. Research activities began with cultivation of SR2 isolates on specific media deMann Rogosa Sharpe (MRS) followed by Gram stain, catalase test, and formation of CO2 gas. Molecular analysis was performed by PCR of the 16S rRNA gene using primers 27F and 1492R. The PCR products were sequenced and nucleotides data were analyzed by Mega 5.2 Software tool. The results showed that phenotypical isolates of SR2 showed characteristics as LAB isolates, and the results of the 16S rRNA gene analysis showed that isolates SR2 included into Lactococcus lactis strain with a bootstrap value of 100.Keywords - Bali cattle, lactic acid bacteria, molecular analysis, rumen, SR2 isolate

Abstract

Kopi luwak merupakan kopi yang difermentasi pada saluran pencernaan luwak yang dapat memberikan cita rasa yang khas pada kopi tersebut. Pada proses saluran pencernaan hewan luwak terdapat bakteri yang memegang pernan penting dalam proses fermentasi biji kopi, yaitu bakteri asam laktat. Penelitian ini bertujuan untuk mengetahui karakteristik bakteri asam laktat pada feses luwak yang memegang peranan penting dalam saluran pencernaan hewan luwak. Penelitian ini adalah penelitian deskriptif dengan rancangan penelitian observasional. Penelitian ini adalah penelitian deskriptif dengan rancangan penelitian observasional. Pada penelitian ini digunakan sebanyak 18 sampel yang diambil dari agrowisata di setiap kabupaten di Pulau Bali. Seluruh sampel diperiksa di Laboratorium Mikrobiologi Fakultas Kedokteran Universitas Udayana dan Laboratorium Kesehatan Provisinsi Bali. Sampel dikultur pada media MRS-CaCo3 dan hasil positif bakteri asam laktat akan dilakukan uji gas CO2, uji pH, uji katalase, uji pewarnaan gram, serta diidentfikasi menggunakan metode semi automatic BD BBL Crsytal. Dari 18 sampel feses luwak yang dikultur ditemukan 16 diantaranya merupakan bakteri gram positif dan berkatalase negatif yang merupakan ciri ciri bakteri asam laktat. Pada uji gas CO2 ditemukan 9 koloni bakteri yang bersifat homofermentatif, dan 7 bersifat heterofermentatif. Pada uji pH keseluruhan koloni bakteri berada pasa suasana asam dengan rentang pH 4-5. Pada pengamatan di bawah mikroskop ditemukan bentuk bakteri bulat dan batang. Kemudian terdapat beberapa spesies yang ditemukan, yaitu: Corynebacterium Genitalium, Kytococcus sedentarius, Streptococcus sanguinis, dan Streptococcus faecium. Pada penelitian ini ditemukan beberapa karakteristik bakteri asam laktat yaitu bakteri gram positif dengan katalase negatif, memiliki suasana pH yang asam, memiliki bentuk bervariasi dari batang hingga bulat, serta dapat bersifat heterofermentatif atau homofermentatif.
