Abstract

Human resources are a key factor in the success of healthcare organizations, particularly hospitals, which are required to provide quality services on an ongoing basis. This study aims to analyze the influence of work environment, salary, and job promotion on employee performance through job satisfaction as an intervening variable at RSU X. This study uses a quantitative approach with a survey method. Data were collected through questionnaires distributed to RSU X employees and analyzed using Structural Equation Modeling with a Partial Least Squares approach (SEM-PLS) through SmartPLS 4.0 software. Model evaluation included testing the validity and reliability of constructs, testing the structural model, and testing direct and indirect hypotheses. The results showed that the work environment, salary, and job promotion had a positive and significant effect on employee job satisfaction. Job satisfaction was also proven to have a positive and significant effect on employee performance. In addition, job satisfaction was able to significantly mediate the effect of the work environment, salary, and job promotion on employee performance. The coefficient of determination (R²) value shows that the research model has strong explanatory power, and the predictive relevance (Q²) value indicates that the model has excellent predictive capability. These findings confirm that improving employee performance in hospitals can be achieved more effectively through efforts to improve the work environment, fair salary management, and a transparent promotion system that takes employee job satisfaction into account.

Abstract

Midwives are legally required to support postpartum women in exercising their right to breastfeed. In practice, many midwives provide formula milk, ignoring the mother's right to breastfeed and potentially harming both mother and baby. The purpose of this paper is to investigate the legal and ethical considerations surrounding midwives and mothers in relation to breastfeeding rights, emphasizing the need for transparency between midwives and mothers. This research uses empirical methods and a qualitative approach. The findings in this paper indicate that mothers are willing to provide informed consent, including information about any conditions that prevent breastfeeding immediately after delivery. Health Law Number 17 of 2023 mandates exclusive breastfeeding and imposes strict sanctions for those who prohibit breastfeeding without medical reasons. Despite this regulation, some midwives still provide formula milk with the verbal consent of postpartum mothers for medical reasons without adequate medical examination.

Abstract

Purpose: This study aims to examine the adoption, hospital integration, and revenue implications of telemedicine, with specific emphasis on Indonesia where hospital-based telemedicine services remain less developed than commercial platforms. The review focuses on identifying key drivers of user acceptance, organizational readiness, and financial outcomes to support strategic telemedicine development in hospitals.Methodology: A systematic review was conducted using a comprehensive search of open-access empirical studies published from 2020 to 2025. Twenty-six eligible articles were identified and analyzed. Data were synthesized thematically across three domains: factors influencing user adoption, hospital-level implementation processes, and the economic impact of telemedicine services. The review integrates both global and Indonesian evidence to provide a comparative perspective. Results: Findings show that perceived usefulness, trust, and digital readiness consistently shape patient and provider adoption of telemedicine. Successful hospital integration is supported by adequate infrastructure, clear governance, and alignment with clinical workflows. Several studies report cost-effectiveness and increased service utilization, indicating potential for enhanced hospital revenue, although economic evidence from Indonesian hospitals is still emerging.Applications/Originality/Value: This review offers practical insights for policymakers and hospital administrators planning long-term institutionalization of telemedicine. It is original in combining global and Indonesian contexts while emphasizing the financial sustainability of hospital-based telemedicine services.

Abstract

The National Health Insurance (JKN) program is threatened with a deficit and potential default in 2026 due to inefficiency and the risk of fraud in healthcare facilities. In the context of JKN, which uses a package system (INA-CBG), Clinical Pathway (CP) is a strategic instrument within the JKN system that has dual benefits for both Payers (BPJS Kesehatan) and Providers (Hospitals). For BPJS Kesehatan, CP primarily functions to ensure service quality and prevent undertreatment fraud, the practice of reducing services to benefit hospitals. This prevention ultimately achieves cost efficiency in the JKN program by avoiding payment of complication claims and readmissions arising from substandard services. On the hospital side, CP helps control real cost effectiveness (cost) to maintain efficiency. High CP compliance can reduce inefficiencies (e.g., resource overutilization), thereby reducing the gap in hospital losses under the INA-CBG package system. This indirectly reduces the incentive for hospitals to commit fraud. This study aims to synthesize comprehensive evidence regarding the impact of CP compliance on cost efficiency (Hospital Cost) from a provider perspective and its impact on the sustainability of JKN. The method used was a Systematic Review based on the PRISMA framework, analyzing 28 articles from hospitals in Indonesia (2016-2025) that discussed the impact of CP on costs and/or Length of Stay (LOS) as well as the quality of clinical services. During the snowballing and abstract screening stages, we also found that compliance was an important variable measured by the studies we reviewed. The results show that the level of CP compliance in Indonesia remains low; 80% of hospitals have compliance below 50%, which triggers service variations and the risk of fraud. CP compliance is consistently positively correlated with reduced Hospital Costs (thus increasing internal hospital efficiency) and accelerated LOS. In addition to cost efficiency, CP has also been shown to improve the quality of clinical services by reducing complications (SSI: Surgical Site Infection) and reducing the risk of readmissions. In conclusion, CP is an essential dual solution; improving CP compliance is a fundamental key to balancing hospital cost efficiency without sacrificing quality, while simultaneously safeguarding the financial sustainability of the JKN Program from the burden of unnecessary complication claims.

Abstract

Abstract Schizophrenia is a chronic mental disorder with a high tendency for relapse, which worsens the patient's prognosis and increases the burden of care. Clinical factors such as insight, as well as demographic factors including age and education level, are thought to play a crucial role in influencing treatment adherence and relapse risk. This study aims to analyze the association between insight, age, and education level with relapse rates in schizophrenia patients at RSJD dr. Arif Zainudin Surakarta. This was an analytic observational study with a cross-sectional design. Subjects were selected using purposive sampling, totaling 96 patients. Data were analyzed using Chi-Square test, Fisher’s Exact test, and Logistic Regression. The results showed a significant association between insight (p=0.045) and education level (p=0.046) with relapse rates. Meanwhile, age (p=0.588) was found to have no significant association with relapse incidence. It is concluded that insight and education level are associated with relapse rates, although no single factor emerged as a dominant independent predictor in the multivariate model. Improving patient insight through psychoeducation is recommended to prevent relapse.

Abstract

The partner for community service activities was the youth posyandu in Tambakboyo Hamlet, Mliwis. Adolescent boys at Mitra Posyandu have a smoking habit. There were 25% of teenage boys who have tried to stop smoking but failed. The problems faced by posyandu partners are the difficulty of generating teenagers' intention to stop smoking, teenagers also don't know how to stop smoking, and there are still many teenagers who relapse or smoke again even though they are already at the action stage. The aim of this program was to provide smoking cessation education and health messages with the stages of change in smoking behavior in each adolescent using a transtheoretical model approach and monitoring and strengthening smoking cessation by adolescent cadres. The methods used were socializing activities to youth posyandu, providing media on how to stop smoking, providing education regarding efforts to stop smoking, training youth cadres, designing applications for monitoring efforts to stop smoking, and assisting teenagers to stop smoking. The results of this service were that there was an increase in adolescent knowledge by 0.17% after being given smoking cessation education, the formation of peer counselors consisting of adolescent cadres, the existence of health media in the form of pocket books and booklets, and the creation of the Smoke Stop Step (3S) application to monitor efforts of quit smoking. After using the application and providing health messages according to the smoking cessation stages, there were 3 teenagers who started to intend to stop smoking and 1 teenager entered the preparatory stage (preparing to stop smoking).
