Abstract

Universal Health Coverage (UHC) is a global commitment to ensure that all people have access to quality health services without facing financial hardship. In Indonesia, the National Health Insurance (JKN) program managed by BPJS Kesehatan aims to provide protection and benefits of health services, including promotive, preventive, curative, and rehabilitative services. However, challenges related to public understanding of this program are still significant. Based on observations in Perbarakan Village, Deli Serdang, the majority of people have minimal knowledge about JKN. Therefore, community service activities in the form of counseling were designed to improve their understanding of JKN. Socialization activities were carried out in Perbarakan Village, Deli Serdang Regency, involving 50 participants in the format of lectures, discussions, and questionnaire distribution. The pretest results showed low public knowledge about the JKN program, with only 35% of participants knowing basic information. After being given education, there was a significant increase in the post-test results, with 100% of respondents answering correctly, recording a 65% increase in knowledge. In addition, understanding of the procedure for moving primary health facilities increased by 48%.These findings suggest that clear and structured education can increase public awareness and understanding, which in turn supports the achievement of UHC goals. Therefore, appropriate and sustainable socialization is essential to ensure active community participation in the JKN program, so that it can expand access and utilization of quality health services. This counseling is expected to encourage public awareness to register as JKN participants, strengthen health protection, and support the achievement of UHC in Indonesia.

Abstract

Non-communicable diseases have become the main cause of death globally and nationally, due to the high number of NCDs which continue to increase every year, so to control PTM risk factors in Indonesia, the government established the Posbindu PTM program. However, the utilization of Posbindu PTM in the working area of the Brand Health Center has not reached the target of 100%. This study aims to determine the factors associated with the utilization of Posbindu PTM. This type of research is quantitative with cross-sectional design and the analysis used is univariate and bivariate. The population in this study were people suffering from non-communicable diseases in the working area of the Brand Health Center. The sample in this study amounted to 100 people. The results showed that out of 100 respondents, 60.0% used Posbindu PTM. Variables related to the utilization of Posbindu PTM were knowledge (p=0.001), mileage (p=0.011), family support (p=0.004), attitude (p=0.001), and health status (p=0.002). The variable that is not related to the utilization of Posbindu PTM is the variable support from health workers (p=0.423). It is recommended for puskesmas to further improve the quality of life of the community through collaboration with health workers such as cadres..

Abstract

Surveillance of Health Care Associated Infections (HAIs) is a dynamic and continuous process of collecting, identifying, analyzing, and interpreting health data. This study aims to analyze the HAIs surveillance system based on input, process, and output components using a mixed-method approach. Phase I (quantitative) assessed input (human resources and infrastructure) and output (epidemiological data and HAIs incidence rates) using secondary data analyzed univariately. Phase II (qualitative) explored input and process components through interviews and FGDs with the IPC team (IPCN and IPCLN), analyzed using thematic analysis. Results from Phase I showed the highest HAIs cases in patients over 41 years, with VAP incidence in February (32.5 ‰) exceeding the standard. Other peaks included non-SC IDO in February (1.58 ‰), SC IDO in April (5 ‰), and phlebitis in March (1.11 ‰). Phase II revealed issues in HR quality, incomplete SOPs, limited infrastructure, and lack of indicators to assess data completeness and accuracy

Abstract

Transgender women are one of the key populations highly vulnerable to HIV/AIDS transmission due to risky sexual behaviors, limited access to friendly health services, and high levels of social stigma. To address this issue, a community service initiative was carried out at the Lubuk Pakam Public Health Center (Puskesmas), Deli Serdang Regency, aimed at increasing knowledge and preventive efforts related to HIV/AIDS among transgender women through educational approaches and integrated services. The methods included needs assessment through observation and interviews, development of locally relevant educational materials, interactive counseling sessions, voluntary HIV testing (VCT), and training for healthcare providers. A total of 25 transgender participants joined the education session, and 15 underwent VCT accompanied by pre- and post-test counseling. The results showed an average increase in knowledge of 35.4% based on pre-test and post-test scores. Additionally, an inclusive HIV service corner was established, and anti-stigma communication training was conducted for healthcare workers. This approach proved effective in enhancing community engagement and improving awareness of HIV prevention among transgender individuals. Success factors included the use of visual media, community-based education, local language delivery, and strong institutional support from the health center. These findings highlight the importance of replicating similar programs in other regions, involving community peer educators as key partners. Sustained, locally tailored education is essential for effective HIV/AIDS control among marginalized populations such as transgender women.

Abstract

Background: Non-Communicable Diseases (NCDs) are the leading cause of death globally, including in Indonesia, which is experiencing an increasing prevalence of hypertension and diabetes. To address this challenge, the Indonesian government developed the Posbindu PTM program as an early detection effort for NCD risk factors at the community level. However, the effectiveness of the program implementation still faces various challenges in the field. Objective: This study aims to analyze the impact of the Posbindu PTM program implementation on the coverage of early detection of Non-Communicable Diseases in the working area of Merek Health Center, Karo District. Methods: This research employed a quantitative approach with a cross-sectional analytic design. Data were collected from 422 respondents who were active participants of Posbindu PTM using structured questionnaires, observations, and document reviews. Data were analyzed through univariate, bivariate (using Chi-Square tests), and multivariate analysis (using logistic regression). Results: The majority of respondents were female (60.2%) and aged 45–59 years (35.1%). The Posbindu activities were conducted ?3 times per year (68.2%), facilities and infrastructure were adequate (71.3%), cadres' competence was good (64.9%), and health workers’ involvement was high (59.5%). Coverage of blood pressure checks reached 74.4%, Body Mass Index (BMI) measurement 68.0%, random blood sugar checks 62.8%, and education on NCD risk factors 71.1%. A significant relationship was found between the program implementation and early detection coverage. Conclusion: The implementation of the Posbindu PTM program at Merek Health Center was fairly optimal and positively influenced the improvement of early NCD detection coverage. This program plays an important role in empowering communities to prevent and control NCDs through regular screening and health education.

Abstract

Background: Type 2 Diabetes Mellitus (type 2 DM) is a chronic disease in adults due to the body's resistance to insulin. Purpose: This study aims to analyze the determinants of the incidence of type 2 DM. Methods: The study design was a case-control study. The case population in this study was patients with type 2 DM who had been diagnosed by doctors based on data available in the medical records of Bintang Bayu Health Center. In contrast, the control population was patients who did not suffer from type 2 DM. The study's sample size was 138 people, with a ratio of cases to controls of 1:2. The case group consisted of 46 people, and the control group comprised 92 people. The sampling technique used was nonprobability sampling, namely, consecutive sampling. Questionnaire distribution was the primary method of data collection. The frequency distribution of each variable was calculated before analyzing the data using Chi-square and logistic regression. Results: Diet (p=0.001; OR=4.7; 2.231–10.154), knowledge (p=0.033; OR=2.3; 1.129–4.785), physical activity p = 0.010; OR = 2.8; 1.342 – 6.032), hypertension (p=<0.001; OR=4.9; 2.089–11.784) were significant to the incidence of type 2 DM. The most dominant variable was hypertension, p=<0.001; OR=7.3 (2.581-20.680). Conclusions: Poor diet, poor knowledge, light physical activity, and hypertension were significant factors in the incidence of type 2 DM.
