Abstract

BACKGROUND: Andrographis paniculata has anti-inflammatory, anti-cancer, and immunomodulatory effects. Intercellular adhesion molecule-1 (ICAM-1) and E-selectin are released in inflammatory conditions due to sepsis. In sepsis-associated fatalities, E-selectin is strongly expressed on pulmonary microvasculature and ICAM-1 is strongly expressed on pulmonary microvasculature endothelial cells, pulmonary macrophages, and lymphocytes. Therefore, this study was conducted to analyze the protective effect of A. paniculata leaf extract against lung injury in lipopolysaccharide (LPS)-induced sepsis mouse model.METHODS: Thirty male Wistar rats were divided into 1 control group and 4 sepsis groups. The control group was fed ad libitum for 14 days, followed by an intraperitoneal injection of 5 mg/kgBW 0.9% NaCl on day-21. While the sepsis groups were fed ad libitum and orally administered 0, 200, 400, and 500 mg/kgBW of A. paniculata leaf extract, respectively, for 14 days. All four sepsis groups were intraperitoneally injected with 5 mg/kgBW LPS on day-21. Mice termination and measurement of ICAM-1 and E-selectin were performed on day-25. The immunohistochemical examination of ICAM-1 expression was performed using ICAM-1/CD54 antibody, meanwhile E-selectin expression was examined using primary antibody E-Selectin (D-7) rabbit IgG antibody. RESULTS: All groups given A. paniculata leaf extract showed lower expression of ICAM-1 (p<0.001) and E-selectin (p<0.001) than the group without extract. A. paniculata leaf extract 500 mg/kgBW gives the best effectiveness compared to other doses.CONCLUSION: A. paniculata leaf extract has a protective effect against lung injury by lowering the expression of ICAM-1 and E-selectin in lung tissue.KEYWORDS: ICAM-1, E-selectin, sepsis, Andrographis paniculata, andrographolide

Abstract

BACKGROUND Niemann-Pick disease (NPD) types A result from the deficient activity of sphingomyelinase. NPD type A is characterized by early-onset, progressive neurodegenerative course; systemic disease manifestations, including massive hepatosplenomegaly, interstitial lung disease, and cherry-red macula; and death in early childhood. The objective: to enhance the recognition of health care providers about the potential undiagnosed NPD because nonspecific clinical manifestation CASE PRESENTATION A 18-months-old boy was admitted to Dr. Kariadi Hospital with enlarged abdomen since seventh month old with failure to thrive. He also showed progressive loss of neurologic function, microcephaly with open major fontanelle, recurrent pulmonary and systemic infection. Physical examination revealed facial dysmorphic, milestone regression, under-nutrition, crackles in both lungs, hepatosplenomegaly with petechial in extremities and floppy infants. Laboratory investigations showed anemia (9.4 g/dL) and thrombocytopenia (73.000/mm3). The lactate dehydrogenase (482 U/L) and alkaline phosphatase (159, 03 IU/L) were higher than normal. Abdominal ultrasound revealed hepatomegaly with normal parenchyma and splenomegaly without nodule. Skeletal survey revealed Erlenmeyer flask deformity. Foam cell are detected in bone marrow puncture. Retcam examination showed cherry red spot at the macula. Bera revealed auditory neuropathy. The enzyme activity showed normal ?–Glucosidase (5.55 uM/hr) and chitotriosidase (105,8 nmol/ml) but low sphingomyelinases activity (0.30 uM/hr) which confirmed the diagnosis of NPD. DISCUSSION Niemann-Pick disease (NPD) types A result from the deficient activity of sphingomyelinase. NPD type A is characterized by early-onset, progressive neurodegenerative course; systemic disease manifestations, including massive hepatosplenomegaly, interstitial lung disease, and cherry-red macula; and death in early childhood. Type A is very rare and a severe infantile form with neurologic degeneration resulting in death usually by 3 years of age. No treatment available for type A and It’s a rare disease in Indonesia. CONCLUSION These investigations were able to diagnose this child as a NPD-Type A. Patient was closely monitored and symptomatic treatment was provided.

Abstract

Background Problems: Currently, there is still limited identification of nutritional intake, hormonal, and physical performance in adolescent athletes in Indonesia. Research Objectives: This study aims to identify anthropometric, nutritional intake, IGF-1 hormone level, and adolescent athletes’s performance. Methods: The research method used a cross-sectional study. A convenient sample of 58 athletes under Balai Pemusatan Pendidikan dan Latihan Pelajar. The research instrument used Bioelectrical Impedance Analysis (BIA), leg dynamometer, hand grip dynamometer, tape measure, height stature, SQ-FFQ Form, and body fat caliper. Blood samples were collected and tested by a professional laboratory. Data analysis techniques used Pearson’s test for normally distributed data and Spearman’s rank for non-normally distributed data. Findings and Results: The result of this study found a significant (p < 0.05) correlation between energy intake, anthropometric measurement of z score (BMI/Age), and endomorph score with an athlete’s muscle strength. We also found significant differences (p < 0.05) in macronutrient and micronutrient consumption in male and female athletes, and we also found significant differences in somatotype score and muscle strength in male and female athletes. Conclusion: We conclude that many athletes are not consuming the recommended levels of carbohydrates, fat, protein, and some micronutrients. We found a correlation between energy intake and fat intake with leg muscle strength. However, there was no correlation between IGF-1 hormone levels and athlete strength. Intake is thought to indirectly affect grip strength and leg strength.

Abstract

Inflammation is the body's response to tissue damage and infection. Prolonged inflammation can increase the risk of non-communicable diseases, including cardiovascular disease. Some factors that can trigger inflammation are dietary patterns, high physical activity, and body composition. This study examined the correlation between dietary inflammatory index (DII), physical activity, body composition, and high sensitivity C-reactive protein (hs-CRP) levels. It examined differences in characteristics based on gender. This study was a cross-sectional design of 55 adolescent athletes at the Student Sports Education and Training Center in Central Java. The exclusion criteria were an athlete who had a physical injury (did not participate in training) and a smoker. The DII score was calculated based on the dietary pattern using SQFFQ, anthropometric data (weight, height, and nutritional status based on BMI/age), body composition (body fat percentage, fat mass, and fat-free mass) using the BIA tool, physical activity data for the last seven days using IPAQ and hs-CRP levels using the FIA method. Our result showed that the DII score was significantly correlated with hs-CRP (p=0.013). There was no correlation between physical activity, body composition, and anthropometry with hs-CRP levels (p>0.05). There were differences in body height, body weight, body fat percentage, and fat-free mass between males and females (p<0.001). The diet of athletes had anti-inflammatory potential (83.6%), as evidenced by the intake of vitamins and minerals, and athletes had low hs-CRP levels <1 mg/L (85.5%), indicating that no inflammation occurred. Future longitudinal research is needed with a large population of adolescent athletes to explain the role of anti-inflammatory and pro-inflammatory diets on the incidence of inflammation and measure physical exercise in athletes to reduce inflammatory bias.

Abstract

Background Osteoporosis is a complication that often occurs in thalassemia major patients. This complication is still not widely known. This community service program aims to increase the knowledge of families of major thalassemia patients in Rembang Regency and Grobogan Regency regarding the complications of osteoporosis in thalassemia patients. Methods Education about osteoporosis, risk of osteoporosis in thalassemia patients, and prevention of osteoporosis. Extension method with presentation and discussion. The activity was attended by 20 thalassemia families in the Rembang Regency and 21 families of thalassemia patients in the Grobogan Regency. Before and after counseling, pretest and posttest questionnaires were completed to measure the knowledge of families of thalassemia major patients regarding bone complications in thalassemia patients. Results and Discussion Nearly half (46.34%) of respondents did not know the risk of osteoporosis in thalassemia. After counseling, 82.93% of respondents knew the risk of osteoporosis in thalassemia, but there were still 17.07% of respondents who still did not know this risk. Conclusion There is a significant increase in family knowledge of thalassemia major patients about the risk of osteoporosis in thalassemia. Further efforts to increase knowledge are needed with written information media (leaflets) given to families of thalassemia major patients

Abstract

Preeklamsi disebabkan hipoperfusi plasenta, pelepasan sitokin dan disfungsi endotel sebagai respons inflamasi. Neutrofil/Limfosit Ratio (NLR) dan Platelet/ Limfosit Ratio (PLR) merupakan petanda inflamasi sistemik pada berbagai penyakit inflamasi. Membandingkan NLR dan PLR sebagai petanda inflamasi pada kehamilan normal, Preeklamsia Ringan (PER) dan Preeklamsia Berat (PEB). Penelitian observasional analitik dengan pendekatan belah lintang terhadap 102 pasien kehamilan di RSUP dr. Kariadi yang terbagi menjadi 3 kelompok (normal, PER, PEB). Neutrofil dan limfosit dihitung dengan hitung jenis nilai absolut trombosit berdasarkan hematology analizer. Analisis data menggunakan uji Kruskal Wallis dilanjutkan uji post hoc Mann-Whitney. Median NLR pada kehamilan normal ialah 4,15(1,34;5,86) pada PER 5,56 (1,78;8,5) dan PEB 10,98 (5,57;18,8) terdapat perbedaan NLR kehamilan normal dengan PEB dan PER dengan PEB. Median PLR pada kehamilan normal 151,44 (92,34;381,54) PER 134,72 (30,84;491,38) dan PEB 153,35 (26,34;441,14) tidak terdapat perbedaan PLR pada kehamilan normal dan PER (p=0,359) kehamilan normal dan PEB (p=0,965) PER dan PEB(p=0,535). Terdapat perbedaan NLR antara kehamilan normal dengan PEB dan PER dengan PEB, tidak terdapat perbedaan PLR antara ketiganya

Abstract

BACKGROUND Dyslipidemia is characterized by an increase in low-density lipoprotein (LDL) and triglyceride (TG) levels and a decrease in high-density lipoprotein (HDL). Cholestyramine as an antidyslipidemia has several side effects, so an alternative is needed. Pectin is a natural substance with a mechanism of action similar to that of cholestyramine. Mango peel is one of the sources of pectin, containing 10–15% of this substance. This study aimed to prove the effect of arum manis mango (Mangifera indica L.) peel extract on LDL, HDL, and TG levels in dyslipidemic Sprague-Dawley rats. METHODS 25 Sprague-Dawley rats were divided into 5 groups. All groups were given high-fat diet for the first 18 days, followed by standard feed (negative control group), cholestyramine (Sequest®) 80 mg/200 g body weight (standard treatment group), and mango peel extract (M-90 [90 mg/day], M-180 [180 mg/day], and M-360 [360 mg/day] groups) for the next 15 days. LDL and HDL levels were analyzed using the cholesterol oxidase-phenyl aminopyrazolone method and TG level using the glycerol-3-phosphate-oxidase-phenol-aminophenazone method. RESULTS The M-360 group reduced the LDL level (p = 0.015), while the standard treatment group increased the HDL level (p = 0.042). Although significant TG level changes were found in the negative control, standard treatment, and M-360 groups (p = 0.042), the mean differences of LDL, HDL, and TG levels between groups were not significantly different (p = 0.245, 0.328, and 0.454, respectively). CONCLUSIONS M. indica peel extract reduced LDL and TG levels at 360 mg/day.

Abstract

Background: Thalassemia major is associated with ferritin levels and SII values with impaired liver function. Furthermore, the measurement of impaired liver function is carried out using Serum Glutamate Oxaloacetate Transaminase (SGOT) and Serum Glutamate Pyruvate Transaminase (SGPT) levels. Increased SGPT and SGOT levels are indicators of liver necrosis. Objective: This study aimed to prove the association between SII and ferritin with SGPT and SGOT levels in thalassemia major patients. Methods: An observational analysis with a cross-sectional approach was adopted to analyze the medical records of pediatric patients at the Kasuari Clinic of Dr. Kariadi Hospital Semarang from September to October 2023. SII value and ferritin level were the independent variables, while SGOT and SGPT were dependent. Data were collected and analyzed univariately for characteristic description and bivariately using the Spearman test. Results: The median SII value, ferritin level, SGPT level, and SGOT level in thalassemia major patients were 451.73/L, 2097.30 ng/mL, 20.00 U/L, and 40.00 U/L. The results showed there was a moderately significant association between SII values and SGOT levels (p=0.010, r=-0.457) as well as ferritin and SGPT (p=007, r=0.475). There was no significant association between SII values and SGPT levels, as shown by a p-value of 0.122. Similarly, there was no association between ferritin and SGOT levels (p=0.088). Conclusion: A moderately significant association was found between SII values and SGOT levels, as well as ferritin levels and SGPT levels in thalassemia major patients.

Abstract

BACKGROUND: Thalassemia patients with repeated transfusions (multi-transfusion) are often at risk of experiencing early osteoporosis. Several studies have demonstrated that osteoporosis in these individuals was associated with altered bone remodeling, characterized by decreased insulin-like growth factor-1 (IGF-1) and serum calcium ions, as well as increased serum β-Cross Laps (β-CTx) levels. Despite the prevalence of this condition, there is limited literature on the relationship between IGF-1 levels and calcium ions with β-CTx. Therefore, this study was conducted to examine the relationship between IGF-1 and calcium ions levels with serum β-CTx in multi-transfused β-thalassemia major patients.METHODS: A cross sectional study involved 29 thalassemia patients with multiple transfusions, aged 2-18 years, that were selected from the electronic medical records. Calcium ion levels were examined using ion selective electrode method. Subsequently, IGF-1 and serum β-CTx levels were examined by enzyme-linked immunosorbent assay (ELISA), and the data were analyzed using the Pearson correlation test.RESULTS: The results showed that the mean serum IGF-1 levels, calcium ion, and β-CTx were 20.11±20.763 ng/mL, 1.26±0.07 mmol/L, and 9330.40±1696.76 ng/mL, respectively. Statistical analysis showed a significant relationship between the levels of IGF-1 (r=-0.573; p=0.001) and calcium ion (r=-0.373; p=0.046) with serum β-CTx. A moderate negative relationship was found between IGF-1 levels and β-CTx, while calcium ion levels and β-CTx showed a weak negative relationship.CONCLUSION: A moderate negative correlation between IGF-1 and serum β-CTx, and a weak negative correlation between calcium ion and serum β-CTx suggest that IGF-1 and calcium ions may serve as potential indicators of bone turnover and osteoporosis risk in multi-transfused β-thalassemia major patients, underscoring their potential role in routine clinical evaluations.KEYWORDS: miR-200a expression, NO level, early-onset preeclampsia, late-onset preeclampsia

Abstract

Background: Blood glucose levels over normal limits are a hallmark of diabetes mellitus (DM), a chronic metabolic disease with several causes. Disorders in lipid metabolism can result from type 2 diabetes mellitus. This study investigated the correlation between fasting blood glucose (FBG) levels and serum triglyceride and HDL cholesterol in elderly people with type 2 diabetes. Objectives: To prove that in elderly people with type 2 diabetes mellitus, FBG levels are correlated with serum triglyceride and HDL cholesterol levels. Methods: A cross-sectional study of 46 secondary data on elderly patients with diabetes mellitus at Diponegoro National Hospital in Semarang. data analysis using Pearson's correlation test (p<0.05). Results: The Pearson’s correlation test between fasting blood glucose levels and triglyceride levels was p = 0.076, and serum HDL cholesterol was p = 0.270 in elderly patients with type 2 diabetes mellitus. Conclusion: There is no correlation between FBG levels and serum triglyceride and HDL cholesterol levels in elderly people with type 2 diabetes mellitus.
