Abstract

Aims: Critically ill patients' severe symptoms and higher mortality risk can impact sleep quality. Non-pharmacological therapy, including lavender aromatherapy and earplugs, can improve sleep quality in these patients. To determine the effect of the combination of lavender aromatherapy and the use of earplugs on sleep quality in critically ill patients. Methods: This study uses a Quasy Experiment design with a one-group pretest-posttest design, involving 33 participants aged 18-65, with The Richards-Campbell Sleep Questionnaire, and uses Paired Sample t-Test for data processing. Results: In this study, an overview of the sleep quality of critical patients showed an improvement. An analysis of the sleep quality of critically ill individuals revealed a notable enhancement. The study found a significant improvement in sleep quality after the lavender aromatherapy intervention and the use of earplugs in the intervention group, as indicated by a p-value of 0.001 (p<0.05). Conclusion: The findings of this study indicate that the use of lavender aromatherapy intervention with earplugs can positively impact the sleep quality of critically ill patients. Furthermore, nurses can independently use lavender aromatherapy and earplug use as crucial nursing treatments.

Abstract

This study aims to analyze cases of myasthenia gravis (MG) in pregnancy using the Roy Adaptation model approach. The research method used a case study approach with a case of a patient experiencing MG-weighted flaccid tetraparesis during pregnancy. The research results showed that the nursing care plan used the Roy adaptation model. The main needs found are nutrition (swallowing disorders), mobility (impaired physical mobility), sensation (acute pain) and protection (risk of infection). Interventions provided include self-care assistance: eating, exercise promotion: muscle strength, pain management and infection control. In conclusion, Roy's adaptation model approach is useful in this case because it focuses on the patient's ability to adapt to his illness. The patient had a disease that had been going on for years and had worsened due to her pregnancy. Providing appropriate nursing care can speed up patient recovery so that the length of stay is shorter. Keywords: Roy Adaptation, Nursing Care, Myasthenia Gravis

Abstract

The aim of this research is to determine the effectiveness of breathing exercises, which is one of the independent nursing interventions to reduce anxiety in patients with lung cancer. The research method used was a systematic review carried out on journals originating from three databases, namely, ScienceDirect, Scopus and PubMed which were published between 2020-2023. The research results showed that there were seven articles that showed the effectiveness of breathing exercise programs on anxiety in patients with lung cancer. The conclusion obtained is that breathing exercises using the three-ball apparatus exercise technique, ACBT (Active Cycle Of Breathing Technique), ADOPT-Breathing Training and Qigong Breathing Exercise are considered effective in reducing anxiety significantly in patients with lung cancer. Abstract

Background: A brain tumor is an unnatural or abnormal mass growth of brain tissue that develops uncontrollably. Petroclival meningioma represents a serious challenge in neurosurgical cases. Surgical procedures or tumor removal via craniotomy have a significant risk of the patient's post-operative condition which will affect the patient's quality of life in the future, so holistic and comprehensive nursing care is needed. Purpose: To analyze cases of brain tumors in patients with petroclival meningioma after digital subtraction angiography (DSA) was performed in preparation for tumor removal via craniotomy. Method: Case study involving respondents through a nursing care analysis approach using the Roy adaptation model. The patient was a 51-year-old woman who underwent a digital subtraction angiography (DSA) procedure in preparation for surgical craniotomy to remove the tumor. Results: The main needs found from the nursing assessment process were sensation (chronic pain), neurological (risk of ineffective perfusion of brain tissue), activity or mobility (barriers to physical mobility), protection (risk of injury or falls), and personal. protection. concept (anxiety). Providing appropriate nursing care can help patients prepare for surgery, speed up the post-operative recovery process, and help patients adapt to post-operative conditions with the aim of shortening the treatment period and improving the quality of life of post-operative patients. Conclusion: Nursing care using the Roy adaptation model approach is quite useful in handling petroclival meningioma cases because it focuses on the patient's ability to adapt to the condition of the disease. Suggestion: Future researchers need to apply nursing care using the Roy adaptation model approach which is supported by the development of nursing interventions for petroclival meningioma patients through the application of evidence based nursing (EBN), so that it will further improve the quality of nursing services.   Keywords: Roy Adaptation; Nursing care; Petroclival Meningioma; Craniotomy.   Pendahuluan: Tumor otak merupakan pertumbuhan massa jaringan otak yang tidak wajar atau abnormal dan berkembang secara tidak terkontrol. Meningioma petroclival menjadi salah satu tantangan berat pada kasus bedah saraf. Tindakan pembedahan atau craniotomy removal tumor memiliki risiko yang cukup besar terhadap kondisi pasien pasca tindakan pembedahan yang akan memengaruhi kualitas hidup pasien ke depannya, sehingga diperlukan asuhan keperawatan yang holistik dan komprehensif.   Tujuan: Untuk menganalisis kasus tumor otak pada pasien meningioma petroclival pasca tindakan digital subtraction angiography (DSA) dalam persiapan craniotomy removal tumor. Metode: Studi kasus yang melibatkan seorang responden melalui pendekatan analisis asuhan keperawatan menggunakan model adaptasi Roy. Pasien berjenis kelamin perempuan berusia 51 tahun pasca tindakan digital subtraction angiography (DSA) untuk persiapan operasi craniotomy removal tumor. Hasil: Kebutuhan utama yang ditemukan dari proses pengkajian keperawatan adalah sensasi (nyeri kronik), neurologis (risiko ketidakefektifan perfusi jaringan serebral), aktivitas atau mobilitas (hambatan mobilitas fisik), perlindungan (risiko cedera atau jatuh), dan konsep diri (ansietas). Pemberian asuhan keperawatan yang tepat dapat membantu pasien mempersiapkan dalam menghadapi tindakan operasi atau pembedahan, mempercepat proses pemulihan pasca pembedahan, dan membantu proses adaptasi pasien terhadap kondisi pasca pembedahan dengan tujuan dapat mempersingkat masa perawatan dan meningkatkan kualitas hidup pasien pasca tindakan pembedahan. Simpulan: Asuhan keperawatan dengan pendekatan model adaptasi Roy cukup bermanfaat dalam penanganan kasus meningioma petroclival karena berfokus pada kemampuan pasien untuk beradaptasi terhadap kondisi penyakitnya. Saran: Bagi peneliti selanjutnya, perlu adanya penerapan asuhan keperawatan dengan pendekatan model adaptasi Roy yang didukung dengan pengembangan intervensi keperawatan pada pasien meningioma petroclival melalui penerapan evidence based nursing (EBN), sehingga akan semakin meningkatkan kualitas asuhan keperawatan.   Kata Kunci: Adaptasi Roy; Asuhan Keperawatan; Meningioma Petroclival; Kraniotomi.

Abstract

This study aims to identify the effect of exercise and physical activity on post-stroke recovery. The method in this study is a literature review by collecting international databases, namely PubMed, Proquest, Science Direct, NCBI, Sage Journal, and Taylor & Francis. The results showed that the total number of studies reviewed was 12 studies, with 701 patients. The types of stroke included in this study were; recurrent stroke, ischemic, infarction, intracerebral hemorrhage, and transient ischemic attack. Physical activity interventions in this literature study are aerobic exercise, walking exercise, isokinetic training, balance exercise, and aquatic-based exercise. In conclusion, regular physical activity and exercise affect chronic stroke recovery, significantly improving motor function, emotional, cardiorespiratory health, and quality of life. Abstract

This study aims to explore the effectiveness of modifying drug-taking control using digital communication media on Tuberculosis survivors. The research method used is a literature review by searching five databases. Namely, Google Scholar, Clinical Key, EbscoHost, and Pubmed, using the keywords "Tuberculosis," "adherence," "internet", "Smartphone," and "Directly Observed." The results of the study found that the monitoring method for taking medication involved digital information technology, namely 1) a smartphone that can play videos and the "Video Observed Treatment" application can be installed, 2) "E monitor via a smartphone that is used as a reminder to take medication, 3) a device that functions as a monitoring box to remind people to take medication 4) Reminders that are carried out using telephone devices that are intervened directly from the center. In conclusion, patient adherence to treatment in the control group resulted in a lower estimate of the effectiveness of the intervention with the test case group using digital communication media. Abstract

This study aims to describe the implementation of nursing care in post-endoscopic transsphenoidal surgery (ETSS) patients with indications of pituitary adenoma. This research method is a case study in post-ETSS patients with Roy's Adaptation theory approach. This study showed that the patient was free of postoperative complications. The patient could mobilize in stages and went home in a condition that he could walk even with assistance. During treatment, the patient's blood sugar levels were controlled, and signs of dehydration were not found. In conclusion, strict control and good postoperative care can prevent the risk of postoperative complications in patients. Keywords: Roy Adaptation, Pituitary Adenoma, ETSS

Abstract

This study aims to evaluate the results of published studies related to nonpharmacological management to reduce complaints of constipation in orthopedic patients. The research method used was Systematic Review which was carried out through a search strategy in 9 electronic databases with the keywords (orthopedic patient OR orthopedic patient) AND (nonpharmacological management OR nonpharmacological therapy) AND (constipation OR obstipation). The results showed that there was various nonpharmacological management used to treat and prevent the severity of constipation in orthopedic patients, including interventions such as chewing gum, abdominal massage, acupressure, hydrotherapy, biofeedback therapy, and nursing interventions based on the patient's active involvement in care. Various nonpharmacological strategies are used to treat constipation in orthopedic patients, and the best method to treat constipation in orthopedic patients cannot be ascertained due to the variety of instruments used and differences in sample characteristics in each study. Selected articles were assessed for quality using the Critical Appraisal Skill Program (CASP) guidelines. The six reviewed articles obtained that the range of patients who were given nonpharmacological interventions was ≥ 18 years. Instruments for measuring the incidence and severity of constipation are the Constipation Assessment Score, ROME III, Bristol Scale, Rasmusen measuring scale, Constipation Score Method, and Constipation Risk Assessment Scale. In conclusion, several nonpharmacological therapies can be applied to orthopedic patients to reduce their constipation complaints, thereby reducing the impact of hospitalization on patients and improving their quality of life. Abstract

The treatment for COVID-19 patients with hypoxia is typically limited to invasive and non-invasive oxygenation therapy without any complementary therapy to accelerate improvement in the patient’s health status. The physiological disorders faced by COVID-19 patients also affect their psychology. Anxiety disorders can interfere with physiological processes, such as respiratory processes, aggravating the patient’s oxygenation problems and leading to worsened conditions and death. Therefore, oxygen therapy and pharmacotherapy must be supported by complementary therapies, such as deep breathing. This research aims to investigate the effects of deep breathing relaxation techniques and nature-based sound therapy on the oxygen saturation and anxiety of COVID-19 patients. This study used a quasi-experimental approach with a pretest-posttest control group design approach with 66 samples selected by non-probability sampling. The sample comprised 33 patients in the control group and 33 in the intervention group. The statistical analysis used the Wilcoxon test. The results showed that deep breathing relaxation techniques and nature-based sound therapy increase the patient’s oxygen saturation values and decrease the anxiety score of COVID-19 patients with a p- value of < 0.001. The control group that received the deep breathing relaxation intervention obtained a p-value of <0.001 for increasing oxygen saturation and reducing anxiety levels. Therefore, deep breathing relaxation interventions and nature sound music therapy positively affect COVID-19 patients’ oxygen saturation values and anxiety scores. Abstract

Postoperative nausea and vomiting (PONV) stands as one of the most prevalent complications within the initial 24 hours following surgery, potentially extending the Length of Stay (LOS) and incurring elevated financial burdens for patients in hospital settings. Aromatherapy emerges as a non-pharmacological approach for alleviating PONV symptoms. This case study aims to evaluate PONV severity and manage symptoms in neurosurgical patients utilizing a nursing process approach. Employing a case study design integrated with a nursing process approach, this study was conducted. The inclusion criteria were respondents aged 18 or older, American Society of Anesthesiologist (ASA) I or II classification, undergoing neurosurgery under general anaesthesia, willing to participate, conscious (GCS: 14-15), with a healthy sense of smell, and PONV scores of 1 or 2. Exclusion criteria comprised discharged or deceased patients, absence of PONV, respiratory issues like asthma/COPD, odor allergies, and severe post-surgical complications. Dropout criteria included sudden emergencies or alternative interventions during PONV assessments. Results showed 4 patients with PONV 1 scores and 6 with PONV 2 scores before aromatherapy. After aromatherapy, 6 patients had PONV 0 scores, 2 had PONV 1 scores, and 2 had PONV 2 scores. Findings underscore alterations in PONV scores pre- and post-aromatherapy administration, highlighting the potential of this evidence-based nursing intervention to enhance patient comfort and mitigate postoperative complications in neurosurgical settings.
