Abstract

Transfusion-related acute lung injury (TRALI) is defined as the onset of respiratory distress in a patient after receiving a blood component transfusion. So far, TRALI is considered a rare complication in the field of blood transfusion. However, in the last decade, there has been a shift in perspective. Currently, the US Food and Drug Administration recognizes the syndrome as the leading cause of transfusion-related death. This literature review aims to describe TRALI and its prevention strategies. Understanding the pathogenesis of TRALI drives prevention strategies from a blood bank perspective. A major breakthrough in efforts to reduce the incidence of TRALI was excluding female donors from the high plasma volume product, which resulted in an approximately two-thirds reduction in incidence. However, this strategy has not completely eliminated complications of transfusion. In recent years, research has identified patient-associated risk factors for the development of TRALI and empowered clinicians to take an individualized approach to patients requiring transfusion.

Abstract

Background: Malignant pleural effusion (MPE) has a variable survival rate and prognosis. The LENT score is one method for assessing survival rates in patients with MPE. This study aimed to investigate the LENT score as a prognostic factor for overall survival (OS) and progression-free survival (PFS) of patients with MPE at a tertiary hospital in West Sumatera. Methods: This study was an observational analytic study involving several tertiary hospitals in West Sumatera with a minimum observation period of 2 years. Data were collected from medical records. We used Kaplan Meier analysis to assess OS and PFS. Results: A total of 198 subjects met the inclusion criteria. Most MPE patients in this study were aged ≥60 years, male, smokers, pleural fluid lactate dehydrogenase value <1500, ECOG 1, serum NLR value <9, and high-risk cancer, namely lung cancer. The distribution of LENT scores for MPE patients was evenly distributed among the low, medium, and high-risk groups. Kaplan Meier analysis showed that the median OS based on LENT scores were 804 days, 275 days, and 161 days, respectively (log-rank test p = 0.000). The median PFS based on LENT scores were 715 days, 202 days, and 106 days, respectively (log-rank test p=0.000). The OS and PFS findings are longer than previous studies. Conclusion: Based on LENT scores, overall survival and progression-free survival MPE patients at tertiary hospitals in West Sumatera have a better prognosis compared to previous studies.

Abstract

Latar Belakang: COVID-19 merupakan penyakit saluran pernafasan oleh SARS-CoV-2. Perubahan terapi COVID-19 bersifat dinamis. Solidarity trial oleh WHO yang melibatkan RSUP M. Djamil Padang dilakukan menggunakan 4 alternatif terapi yaitu remdesivir, gabungan lopinavir/ritonavir, lopinavir/ritonavir ditambah interferon (ß1b), dan klorokuin. Objektif: Untuk mengetahui gambaran terapi antivirus pada pasien COVID-19 di RSUP Dr. M. Djamil Padang periode Maret sampai Agustus 2020. Metode: Penelitian ini menggunakan data dari rekam medik dengan metode total sampling. Variabel penelitian adalah umur, jenis kelamin, derajat penyakit, komorbid, obat antivirus, durasi pemberian, cara pemberian obat dan luaran perawatan. Data dianalisis secara univariat. Hasil: Total rekam medis pasien COVID-19 adalah 105. Hasil penelitian menunjukkan rerata umur pasien yaitu 44,9 ± 18,8 tahun. Kelompok terbanyak 45-64 tahun (41,9%), jenis kelamin perempuan (54,3%), derajat sedang (37,1%), dan tanpa komorbid (58,7%). Antivirus yang diberikan adalah oseltamivir (26,4%), remdesivir (0,9%), hidroksiklorokuin/ klorokuin (2,8%), oseltamivir diikuti hidroksiklorokuin/ klorokuin (17,9%), kombinasi lopinavir dan ritonavir (7,6%) serta tanpa antivirus (44,3%). Rute pemberian terbanyak adalah oral; durasi terapi oseltamivir 5-10 hari, hidroksiklorokuin/klorokuin 5-13 hari, serta kombinasi lopinavir dan ritonavir 12-15 hari. Pasien yang mendapatkan oseltamivir sebagian besar sembuh (96,4%). Kesimpulan: Terdapat perubahan terapi antivirus yang dinamis sejak awal pandemi hingga Agustus 2020 di RSUP M.Djamil Padang dengan oseltamivir sebagai antivirus yang paling banyak digunakan.

Abstract

Background: Subcutaneous emphysema often occurs in cases of implanted pneumothorax chest tubes and must always be evaluated. Subcutaneous emphysema is a condition where air or gas is found in the tissue under the skin. Case presentation: A 49-year-old man was treated for sudden shortness of breath that occurred after a violent cough accompanied by pain and heaviness in the chest area 1 day before admission to the hospital. The patient had previously received anti-tuberculosis drug treatment for 6 months based on chest X-ray results in 2022, and the patient had undergone a rapid molecular test (TCM) examination, mycobacterium tuberculosis (Mtb), and obtained Mtb results not detected. Lung auscultation obtains sound intensity breath weakness until it disappears in both lung fields. Palpation of the skin revealed widespread crepitus on the face, neck, upper extremities, back, chest, and abdomen. The range of motion areas of the neck, shoulders, and hands are limited due to pain with movement. Evaluation of the chest tube obtained: the chest tube was installed in the anterior axillary line on the right at the level of the 5th intercostal space with number 10 attached to the chest wall and the chest tube well fixed to the chest wall. The end chest tube has been connected to the WSD bottle, and evaluation of the WSD shows that there are undulations and bubbles. Conclusion: The patient was admitted with spontaneous pneumothorax secondary to tuberculosis and was implanted with a chest tube.

Abstract

Liver injury can be caused by coronavirus disease 2019 (COVID-19), characterized by increased levels of transaminase enzymes (aspartate aminotransferase/AST and alanine aminotransferase/ALT). Objective: To determined the correlation between transaminase enzyme levels and the severity of COVID-19 in Dr. M. Djamil Central Public Hospital, Padang. Methods: This was an observational analytic study with a cross-sectional design. Data were obtained randomly from medical records of confirmed COVID-19 patients hospitalized in Dr. M. Djamil Hospital from June to August 2021, with moderate, severe, and critical severity based on Indonesian Ministry of Health criteria. Data were analyzed using the Chi-square test to examine the distribution and correlation between variables and a simple logistic regression test to assess the level of risk between variables. Results: 57,80% had abnormal AST, and 44,95% had abnormal ALT. The median AST was highest in the critical group, while the median ALT was highest in the severe group. The distribution of sex and age group were significantly different based on AST and ALT levels, and the distribution of patients with coronary heart disease was significantly different based on ALT levels. Transaminase enzyme levels had a significant correlation with the severity of COVID-19 (p-value = 0,00). Patients with critical degrees had the highest risk for abnormal AST and severe degrees had the highest risk for abnormal ALT. Conclusion: Transaminase enzymes have a significant correlation with the severity of COVID-19.Keywords:  COVID-19, liver injury, transaminases, disease severity

Abstract

Background: The SARS-CoV-2 virus easily spreads globally in a short period. Pregnant women are one of the vulnerable groups who can be infected with COVID-19, they have a risk of complications during pregnancy Objective: This research aims to identify the traits of expectant mothers with COVID-19 infection at Dr. M. Djamil Padang. Methods: This form of research uses retrospective descriptive approach, utilizing a comprehensive sampling and relying on secondary data sources. The research was conducted at Dr. M. Djamil Padang Hospital by taking data on pregnant women with confirmed COVID-19 infection in 2020-2021. Samples that met the inclusion and exclusion criteria were 302 cases Result: The results of this study showed the most gestational age was 3rd trimester (96.0), no clinical symptoms (58.6%), with the highest pregnancy outcomes being mature (72.5%), the highest birth weight babies being 2500-4000 grams (81,1%) and the highest maternal and infant outcomes being live mothers and babies ( 87.7%). Conclusion: This study concludes is that most pregnant women with COVID-19 infection at Dr. M. Djamil Padang in 2020-2021 with characteristics of third trimester gestational age, no clinical symptoms, mature pregnancy outcome, weight normal birth and high maternal and infant survival rates
