Abstract

Urological surgical procedures, such as urolithiasis and benign prostatic hyperplasia (BPH), continue to increase with high recurrence rates. Limited treatment facilities and the risk of antibiotic resistance can affect the cost burden. This study aims to determine the alignment of direct medical costs with INA-CBG’s claim rates and to identify variables influencing total direct medical costs. The study employs an observational approach with a retrospective design. Samples were selected using purposive sampling with inclusion criteria. Data were collected from electronic medical records and financial reports. Direct medical costs were calculated based on all cost components using a hospital perspective approach. Data analysis used univariate, bivariate, and multivariate tests. The average total direct medical costs for urology patients were Rp 9,962,325 ± Rp 1,699,707.79, while the INA-CBG’s claim rates were Rp 9,600,328 ± Rp 1,853,291.26. There were differences in direct medical costs based on age (p-value 0.024), care class (p-value 0.035), and length of stay (LOS) (p-value 0.000). Regression analysis shows that LOS and care class have a statistically significant impact on total direct medical costs (p-value < 0.05). There is a difference between the actual costs borne by the hospital and the INA-CBG’s claim rates.

Abstract

Background: Heart failure (HF) often coexists with multiple comorbidities, which may substantially increase healthcare resource use. However, economic evaluations often overlook the specific cost impact of comorbid conditions in HF patients. Objective: To systematically review and quantify the excess direct medical costs and markup factors associated with comorbidities in patients with heart failure. Methods: A structured literature search was conducted across four databases (PubMed, Embase, ScienceDirect, and Google Scholar) on June 28, 2025. Observational studies reporting direct medical costs of HF patients with and without comorbidities were included. All cost values were standardized to 2024 USD using the Consumer Price Index (CPI) and converted using average exchange rates. The methodological quality of included studies was assessed using criteria adapted from Huber et al. (2015). A total of six studies, all conducted in the United States, met the inclusion criteria. Narrative synthesis was performed due to heterogeneity in cost reporting. Results: Across the six studies, comorbidities significantly increased HF-related costs. Mark-up factors ranged from 0.64 to 2.15, with the highest excess costs observed in patients with hyperkalemia (USD 39,543), drug use disorder (USD 27,783), and cancer (USD 19,379). In contrast, some comorbidities, such as Alzheimer’s disease, ischemic heart disease, and type 2 diabetes mellitus, were associated with lower costs, possibly due to end-of-life care patterns or system-level cost controls. Conclusion: Comorbidities impose a substantial and variable economic burden on HF patients. Findings highlight the need for multi-country studies, standardized cost methodologies, and the inclusion of indirect costs and HF severity stratification in future research. Keywords: Comorbidities; Direct medical costs; Economic burden; Heart failure; Mark-up factor.

Abstract

Chronic diseases require continuous pharmacotherapy and generate sustained demand for essential medicines, particularly within universal health coverage systems. In Indonesia, pharmaceutical utilization under the National Health Insurance program is documented through administrative claims data, which provide an important basis for demand analysis and planning. This study aims to forecast chronic drug demand in Gunungkidul Regency, Special Region of Yogyakarta, using health insurance claims data and a time-series forecasting approach. A retrospective analysis was conducted using weekly aggregated claims data. Drug utilization patterns were examined, and demand forecasting was performed using the Autoregressive Integrated Moving Average model following standard time-series procedures. Forecast accuracy was assessed by comparing predicted values with observed utilization. The results indicate that the model effectively captures weekly demand patterns and short-term fluctuations, producing forecasts that closely align with actual utilization trends. These findings demonstrate that time-series forecasting based on claims data can provide reliable estimates of chronic drug demand. The study highlights the potential value of integrating forecasting models into pharmaceutical inventory planning to support timely drug availability and improve logistics efficiency within regional health insurance implementation.
