Abstract

ABSTRACTEarly marriage is a holly/sacred relationship between a man and a woman who had age under 19 years or were currently enrolled in high school and the relationship have been legally recognized in law and religion.The purpose of research was to know the influence of internal and external factors on early marriage among teens at Seumadam Village, Kejuruan Muda Subdistrict, Aceh Tamiang District. This was observational research with a cros-sectional approach. Population were all teens of 15-19 years as many as 556 people. Samples of the research were married teens aged 15-19 years, or who have a boyfriend or were engaged as many as 95 people. Data were obtained through interview using a questionnaire and analyze with the step of univariate, bivariate using chi square test and multivariate using multiple logistic regression test.The result showed that respondents marriaged at age 15-19 year were 26,3%. There was an influence of knowledge, emotional maturity, culture, exposure to mass media and free sex on early marriage among teens. The variable that most influenced on early marriage among teens was culture with the coefficient of B = 3.404.It is suggested to the Seumadam Village to cooperate with the local government such as institution of religion affair office, education and health to reactivate PIK-KRR (information center of counseling adolescence reproductive health) as a forum to get information about complication a pregnancy problem.Keyword : Early Marriage, Teens

Abstract

ABSRACT Teenagers is a potency in the continuing development of a nation. The existence of the various development efforts carried out, caused changes in the life of teenagers. This happens due to an imbalance in the development efforts made against teenagers, giving rise to problems for the construction it self. One of the effects of the imbalance in development is the occurrence of a fundamental change in the attitudes and sexual behaviour among teenagers. Many of the factors that led to a change in attitude and sexual behavior among teenagers, among others, the influence of access porn sites and peers. This research aimed to know the influence of access porn sites and peer group on teenagers sexual behavior in SMA Yayasan Perguruan Kesatria Medan. Type of this research is a descriptive analytic survey with cross sectional approach. The population of this research were all of students at SMA Yayasan Perguruan Kesatria Medan who have been dating, by using totally 107 students were being sample. This study uses multivariate analysis. The results of this research showed that teenagers sexual behavior in the severe category numbered 32 students (29.9%). Variables related to teenagers sexual behavior is an access porn sites (p = 0.042) and adaptation to the peer group (p = 0.034). The results of research using multiple logistic regression test shows that the most influential variables on teenagers sexual behavior is adaptation with peer group, with the regression coefficient β= 1.324. Based on research result suggested to the schools and parents to provide appropriate information and guidance to teenagers about sex education and reproductive health. To teenagers should be more wise in using the internet as a medium to increase knowledge in order to be more useful. Keywords: Free Porn Sites, Peer group, Teenagers Sexual Behavior

Abstract

Cervical cancer is a kind of dangerous tumor that found in cervical area which can cause death to women in Indonesia. In the Medan Hajj General Hospital from October to December of 2013 there were 722 mothers of productive-age couples  who visited to do health check at the obstetrics poly, only 4 (0.5%) mothers performed pap smears check. This research aimed to reduce variables that influenced mothers of productive-age couples who did not do the pap smear check as the cervical cancer early  detection at Medan Hajj General  Hospital period October- December 2013. Data were analyzed by using exploratory factor analysis. Mothers of productive-age couples who did not do pap smear check were the population of this research and were being sample numbered 80 mothers. Data were collected by interviewing using questionnaire. The result of data analysis showed that out of  8 variables (knowledge, attitude, culture, economic, education, husband's support, trust and fear) after being analyzed it formed two (2) factors namely factor 1 (knowledge, education, attitude economic and trust) as internal factors and  factor 2 (culture, husband's support, and fear) as external factors. It is suggested for future studies that will use factor analysis method should increase  the number of variables which will be reduced/summarized. Abstract

ABSTRACT The early complementary feeding is foods or drinks that contain nutrients given to infants under 6 months of age. The early complementary feeding at this time will reduce the coverage of exclusive breastfeeding. The coverage of exclusive breastfeeding in the Puskesmas Teluk Karang was still low about 32,8% (in 2014) while the national target was 80%. It is suspected related to factors of social culture and family support. The purpose of this study was to understand factors influenced the mothers in giving the early complementary feeding in working area of UPTD Puskesmas Teluk Karang, Subdistrict of Bajenis, City of Tebing Tinggi in 2015. Type of this research was analytical observation study with the cross sectional design. The research was done since November 2013 until January 2015. Population in this research were all mothers who had infant aged between 7-12 months in the working area of UPTD Puskesmas Teluk Karang, Subdistrict of Bajenis, City of Tebing Tinggi which was being as samples as 106 mothers. The multiple logistic regression is used to analyze the data. The result of the research showed that the early complementary feeding is high enough i.e 89,6%. From the multivariate analysis, it was proved that the factors influenced the early complementary feeding are the social culture (p=0,008) and the family support (p<0,001). The dominant factor influenced this early complementary feeding was family support (coefficient of B=3,021). It’s suggested that breastfeeding mothers are not easily influenced by the social culture in the communities and families are expected to provide support so that mothers still exclusively breastfed, by fulfilling the nutritional needs during lactation so that it produced the qualified breastfeeding in the sufficient quantities. Abstract

ABSTRACT Severe malnutrition at this time is a matter of special concern in the city of Lhokseumawe, because it may cause a lost generation. Data of malnutrition cases can be presented spatially and can be drawn in a map by using Quantum GIS Software.This research aimed to make a map of the distribution of under five malnutrition cases in the city of Lhokseumawe in 2012. This is a descriptive research. The population were all of under five malnutrition cases and registered at Lhokseumawe Health Office as many as 43 cases in 2012, and all of them were selected as samples. The result showed that out of 43 under five malnutrition cases in Lhokseumawe city, the 15 cases (34.88%) were found in Banda Sakti subdistrict and they were near the beach, the cases with clinical symptom were 15 cases, the cases without clinical symptom were 2 cases (4.65%) and they were spread out the hill and on the side of the road, 8 cases (18.60%) were in radius < 1 km from their houses to health center, 12 cases (27.91%) were in radius < 1 km to the hospital, and 1 case (2.33%) died in radius < 1 km to the hospital. By using Quantum GIS software through Geographic Information System (GIS), it would be easy for Health Office of Lhokseumawe City and health centers in making intervention to malnutrition countermeasure.Keywords: Malnutrition, Under Five, Mapping, Quantum GIS, Geographic Information System

Abstract

Introduction: Fibrinogen is a prognostic biomarker in septic children, lower fibrinogen levels are closely related to higher risk of death. Organ dysfunction was diagnosed based on PELOD-2 score. The aim of this study was to determine the corelation between fibrinogen levels and PELOD-2 score as a predictor of mortality in pediatric patient with sepsis treated in the PICU and HCU at H. Adam Malik General Hospital. Method: A cross-sectional study of pediatric patients with sepsis treated in the PICU and HCU H. Adam Malik General Hospital, Medan. Correlation of fibrinogen levels with PELOD-2 scores was seen on day I and day III of treatment in patient aged 1 month to 18 years with a hospital stay of at least 72 hours and were analyzed using the Spearman test and the relationship between fibrinogen and mortality was analyzed using the Mann-Whitney test. Results: 50 subjects were recruited in this study, fibrinogen levels had no correlation with PELOD-2 score day I (p = 0,074, r = -0,225) and with PELOD-2 score days III (p = 0,110, r = -0,229) and with mortality (p = 0,160). Conclusion: Fibrinogen levels were not correlate with PELOD-2 score as a predictive factor for mortality in septic children.

Abstract

Carbon monoxide (CO), resulting from incomplete combustion such as tobacco smoking, serves as an indicator of nicotine addiction. The aim of this study was to compare the levels of exhaled CO levels between electronic cigarette (e-cigarette) and tobacco smokers and to determine the association between nicotine dependence and exhaled CO levels in e-cigarette and tobacco smokers. A cross-sectional study was conducted using purposive sampling on active smokers, with 70 smokers in each group. The nicotine dependence was measured using the Penn State Nicotine Dependence Index (PSNDI) questionnaire for the e-cigarette group and the Fagerstrom questionnaire for the tobacco smoking group. The CO level was measured using the smokerlyzer piCO. To compare the mean exhaled CO levels between e-cigarettes and tobacco smoker groups, the Mann-Whitney test was used. The Mann-Whitney test and Kruskal-Wallis test were used to assess the associations between nicotine dependence and mean exhaled CO levels in the e-cigarette smoker group and in the tobacco smoker group, respectively. The majority of e-cigarette smokers were 17 years old (65.7%) and male (75.7%). Among tobacco smokers, the majority were also 17 years old (44.3%), and mostly male (91.4%). The mean exhaled CO levels in the tobacco smoker group were significantly higher than the e-cigarette smoker group, 6.86 ppm vs 1.61 ppm with p<0.001. There was a significant association between nicotine dependence and exhaled CO levels among tobacco smokers (p<0.001). Nicotine dependence was not associated with exhaled CO levels among e-cigarette smokers. This study highlights that the smokerlyzer piCO devise could be used to screen nicotine addiction in tobacco smokers.

Abstract

Lack of physical exercise and inadequate nutritional intake in chronic obstructive pulmonary disease (COPD) patients could lead to low muscle mass and function, which causes reduced respiratory function, increased incidence of exacerbations, exercise intolerance, increased visits to health facilities, and low health status. Therefore, physical exercise and good nutritional intake are the main foundations of a comprehensive pulmonary rehabilitation program for COPD patients. The aim of the study was to determine the effect of chicken egg white supplementation on dyspnea degree and the incidence of exacerbations in stable COPD patients who perform physical exercise independently at home. A quasi-experimental study was conducted in 38 stable COPD patients based on the Global Initiative for Chronic Lung Disease (GOLD) 2021 classification criteria. The patients were divided into two groups: the control group who did smartphone application-assisted physical exercises and the intervention group who did the same physical exercises plus chicken egg white supplementation for 12 weeks. Performed physical exercises included both strength and endurance training. The degree of dyspnea was assessed with modified Medical Research Council (mMRC) dyspnea scale and the incidence of exacerbations was assessed by direct interviews at the beginning and at the end of the intervention. A Fisher’s exact test was used to assess the association between the intervention with dyspnea degree and exacerbation incidence. In the intervention group, chicken egg white supplementation for 12 weeks lowered the dyspnea degree by 3.16 times than the control (relative risk 3.16 with p=0.001). Furthermore, the incidence of exacerbations was also reduced by 1.8 times compared to the control group (relative risk 1.8, p=0.001). This study highlights a significant improvement in dyspnea degree and exacerbation incidence in COPD patients with the combination of protein supplementation and physical exercise. Therefore, adding chicken egg white intake could be beneficial for stable COPD patients practicing physical exercises.

Abstract

Asthma is a chronic inflammatory airway disorder and is among the top ten causes of morbidity and mortality in Indonesia, highlighting the need for effective management strategies. This study aimed to evaluate changes in asthma knowledge levels before and after using the "Paru Sehat" smartphone application for digital education. A quasi-experimental study using a pre- and post-test design was conducted over three months at the outpatient clinic of Universitas Sumatera Utara Hospital, Medan, Indonesia, among asthma patients aged over 18 years. The patients completed the Patient Asthma Knowledge Questionnaire (PAKQ) before installing the "Paru Sehat" application to establish baseline knowledge. Once a week, patients received reminders to access materials covering an asthma overview, peak flow meter use, medication differences, and management techniques. After four weeks of intervention, the participants' knowledge was reassessed using the PAKQ. Data were analyzed using either paired Student's t-test or Wilcoxon test to assess the improvement in knowledge. The results showed increases in knowledge about asthma in general, asthma triggers, and asthma diagnosis domains, with the median score in each domain rising by one point (p=0.003, p=0.001, and p=0.002, respectively). The asthma management domain exhibited the largest change, with a median increase of two points (p<0.001). The total scores increased by an average of five points (p<0.001), indicating an overall improvement in patient knowledge about asthma. The "Paru Sehat" application demonstrated significant potential in enhancing asthma knowledge across multiple domains. Further research is warranted to explore the long-term effectiveness of the application in diverse populations and settings, ultimately contributing to improved asthma management and patient outcomes.

Abstract

The severity of coronavirus disease 2019 (COVID-19) may be measured by interleukin-6 (IL-6) and chest X-rays. Brixia score of the chest radiographs is usually used to monitor COVID-19 patients’ lung problems. The aim of this study was to demonstrate the relationship between IL-6 levels and chest radiographs (Brixia score) that represent COVID-19 severity. A retrospective cohort study was conducted among COVID-19 patients who had a chest X-ray and examination of IL-6 levels at H. Adam Malik General Hospital, Medan, Indonesia. A multinomial logistic regression analysis was conducted to evaluate the association between IL-6 levels and the severity of the chest radiograph. A total of 76 COVID-19 patients were included in the study and 39.5% of them were 60–69 years old, with more than half were female (52.6%). A total of 17.1%, 48.7%, and 34.2% had IL-6 level of <7 pg/mL, 7–50 pg/mL and >50 pg/mL, respectively. There were 39.5%, 36.8% and 23.7% of the patients had mild, moderate and severe chest X-rays based on Brixia score, respectively. Statistics analysis revealed that moderate (OR: 1.77; 95% CI: 1.05–3.32) and severe (OR: 1.33; 95% CI: 1.03–3.35) lung conditions in the chest X-rays were significantly associated with IL-6 levels of 7–50 pg/mL. IL-6 more than 50 pg/mL was associated with severe chest X-ray condition (OR: 1.97; 95% CI: 1.15–3.34). In conclusion, high IL-6 levels significantly reflected COVID-19 severity through chest X-rays in COVID-19 patients.
