Abstract

Community Acquired Pneumonia (CAP) merupakan infeksi yang terjadi di paru-paru yang didapatkan dari komunitas dan sering meningkatkan angka rawat inap di rumah sakit. Bakteri yang paling umum menjadi penyebab CAP adalah Streptococcus pneumoniae, Mycoplasma pneumoniae, dan Chlamydophila pneumoniae. Berdasarkan pada pedoman American Thoracic Society (ATS) dan Infectious Disease Society of America (IDSA), penggunaan terapi antibiotik pada pasien CAP dinilai berdasarkan stabilitas klinisnya. Ulasan artikel ini bertujuan untuk membandingkan hasil evaluasi penggunaan antibiotic yang dilihat dari stabilitas klinis dan keamanannya. Ulasan artikel bertujuan untuk membandingkan hasil penelitian yang diperoleh dari PubMed sebagai literatur dan diagram PRISMA sebagai guideline pada proses seleksi artikel. Berdasarkan hasil ulasan artikel diperoleh 10 artikel yang sesuai dengan topik dan tujuan. Dapat disimpulkan bahwa penggunaan antibiotik golongan betalaktam atau fluorokuinolon memiliki efikasi dan keamanan yang baik untuk pasien CAP

Abstract

Latar belakang: Ketidakpuasan pasien di Rumah Sakit Kabupaten Sidoarjo khususnya pasien BPJS dan umum masih terdapat beberapa keluhan terkait pelayanan yang diberikan oleh rumah sakit yang disampaikan melalui ulasan website. Tujuan: untuk mengetahui mengetahui perbedaan kepuasan pasien BPJS dan Pasien Umum di bagian pendaftaran rawat jalan. Metode: penelitian ini menggunakan deskriptif kuantitatif dengan pendekatan cross sectional. Populasi pada penelitian ini berjumlah 10.272 responden, besar sampel 100 responden, teknik Pengambilan sampel dengan menggunakan teknik rancangan acak dengan sederhana (Simple Random Sampling), waktu penelitian dilaksanakan pada bulan Januari 2024, teknik analisis yang digunakan uji beda Mann Whitney U-test. Hasil: Responden BPJS yang merasa puas 28 orang (56%), dan yang merasa sangat puas 22 orang (44%). Responden Umum yang merasa puas 6 orang (12%), dan yang  merasa sangat puas 44 orang (88%). Selain itu, hasil penelitian ini juga mennjukkan bahwa ada perbedaan kepuasan Pasien BPJS dan Pasien Umum di bagian pendaftaran rawat jalan di Rumah Sakit Kabupaten Sidoarjo dengan nilai p value 0,000 (p < 0,05). Kesimpulan: ada perbedaan kepuasan Pasien BPJS dan Pasien Umum di bagian pendaftaran rawat jalan di Rumah Sakit Kabupaten Sidoarjo, Rumah Sakit Kabupaten Sidoarjo disarankan untuk meningkatkan kepuasan pasien dengan memberikan pelatihan  kepada petugas pendaftaran tentang pelayanan.

Abstract

Patients with diabetic ulcers are susceptible to infection because the tissue in their extremities is constantly exposed to various pathogenic bacteria, so biofilm formation plays an important role in developing the disease. Based on previous studies, ambroxol can inhibit the formation of biofilm bacteria. The purpose of this study was to identify biofilm-forming bacteria, prove that ambroxol has the potential to inhibit and eradicate biofilms formed by bacteria in diabetic ulcers and analyse the potential of ambroxol in eradicating biofilms formed by bacteria in diabetic ulcer patients based on PEDIS and SEM scores. The method used was a 96-well microtiter plate assay (MtPA) with crystal violet and MTT staining techniques, a test using human samples with a quasi-experimental method with sampling carried out by nonprobability sampling. The samples used were diabetic ulcer patients with culture results forming biofilms. The results obtained identified 3 clinical isolates that formed biofilms with 100% Gram-negative bacteria, including two isolates of Pseudomonas aeruginosa and one isolate of Escherichia coli with the ability to form biofilms are moderate. Ambroxol effectively inhibits and eradicates 50% of biofilms formed by diabetic ulcer bacteria at a minimum concentration of 1 mg/ml. Based on the PEDIS score and SEM images of diabetic ulcer patients, there was a decrease in the PEDIS score in each patient, with an average score of 2. Several studies have demonstrated that combining Ambroxol with antimicrobials can synergistically enhance the antibiotic's efficacy against biofilms. The co-administration of Ambroxol with other antimicrobial agents represents a promising approach to improving antibiotic effectiveness. Nevertheless, careful consideration must be given to the concentration of Ambroxol used.

Abstract

Background: The results of a comprehensive diagnosis, combined with the individual's general state, establish the appropriate therapeutic care for breast cancer patients. These management disparities have an impact on clinical outcomes, which may influence patient survival. The evaluation process of breast cancer management is essential as a strategy to boost the therapy's effectiveness in the future year. Objectives: Assessing survival rate, one-year overall survival, among stage III breast cancer patients.Methods: An observational study was conducted comprising a retrospective cross-sectional study. Data collected retrospectively from the medical record followed a total sampling technique. This study included patients with breast cancer as a main diagnosis with or without comorbidities and aged ≥ 18 years when undergoing their first diagnosis. Moreover, the exclusion criteria were patients who had incomplete medical record data, undergoing breast cancer treatment at other healthcare facilities, unspecified clinical stage, and patients who had been diagnosed with non-stage III breast cancer. The survival time of the participants is defined as the number of months from initial diagnosis to their last status (alive, died, or loss of follow-up). One-year overall survival was projected using the Kaplan-Meier curve and presented as a percentage with a 95% confidence interval. Results: Of the 23 patients with stage III breast cancer and the mean age was 57.39 ± 10.417 years. The one-year overall survival for stage III breast cancer was 78.3%. In addition, metastases conditions were associated with worse one-year overall survival compared to patients without metastases (40.0% vs 88.9%, p=0.028).Conclusion: The survival rate of breast cancer patients remains lower than in another recent study from Indonesia. This indicates the requirement of treatment strategies evaluation.

Abstract

Sleep is a basic need for everyone. Sleep quality is said to be good if it does not show signs of sleep deficiency, such as swelling in the eyelids, and dark areas around the eyes. Many sleep conditions are found among teenagers who can later cause many effects, such as reduced learning concentration, memory, thinking and health disorders. The purpose of this research is to know the relationship between sleep quality with adolescent learning achievements. The research uses the collation analytic design with the cross-sectional retrospective approach. Samples used as much as 60 respondents. The sampling technique used is purposive sampling. Data collection using questionnaires and report value. The results of the study gained good sleep quality with good learning achievement as many as 11 respondents (18.3%), while poor sleep quality with good learning achievement as many as 45 respondents (75.0%). Based on the statistical test there is a value of rs = 0.000 with P = 1,000 where p > 0.05 so that HI rejected means there is no relationship the quality of sleep and learning performance of young women at Manba'ul Hikam Boarding School Putat Tanggulangin Sidoarjo. Counseling guidance is required and motivates students to be more active in learning.

Abstract

Ulcerative Colitis (UC) is a chronic inflammatory disorder affecting the colon, characterized by recurrent episodes and involving multifaceted etiological factors in its pathogenesis. This study aims to explore the pharmacological effect of Morinda citrifolia Linn. fruit ethanolic-extract (MFE) in UC. The UC mice model was induced using 2% acetic acid administered per-rectal. Subsequently, mice were categorized into six groups, comprising a normal control, negative control, positive control, and three MFE treatment groups (100mg/kgBW; 200mg/kgBW; 400mg/kgBW). Colitis severity was assessed by scoring system to obtain the Disease Activity Index (DAI) and Hematoxylin-Eosin (HE) staining. The 2,2-diphenyl-1-picrylhydrazyl (DPPH) assay was employed to determine the free radical scavenging activity. MFE exhibited notable antioxidant activity, with an IC50 value of 0,553 mg/ml. Notably, MFE administration led to a discernible reduction in DAI scores, with MFE treatment demonstrating improved inflammation condition compared to the negative control from day 6 through day 11. Furthermore, immune cell infiltration within colonic tissue was markedly attenuated in the MFE-treated group compared to the negative control group. Evaluation of colon tissue damage revealed scores of 5.7, 4.2, and 3 for MFE doses of 100 mg/kg, 200 mg/kg, and 400 mg/kg, respectively, whereas the positive control group exhibited a score of 3, indicating superior tissue preservation. Notably, these scores contrasted starkly with the negative control group, which registered a damage score of 11.8. These findings exhibit the MFE potential as a therapeutic intervention for UC owing to its pharmacological properties, underscoring its potential as a natural remedy in UC management.

Abstract

Drug-drug interactions among children are a getting along concern in health care settings, specifically intensive care units, as sources of adverse drug events that may affect patient condition. Children admitted to pediatric intensive care unit are more prone to drug-drug interactions owing to the diseases and medications complexity. This condition could put the patient at high risk of harm, particularly with his critical condition, so need intense considerations from clinical practitioners to prevent adverse drug events caused by potential drug-drug interactions. This article’s review attempts to explore the important drug-drug interactions among children, including explaining the drug combination, mechanism, and related adverse drug events to help health practitioners recognize it earlier before prescribing the medication. This article’s review explored previous research results from PubMed and Google Scholar as literature resources and PRISMA flow chart as protocol for article selection process. A total of 9 articles discussed comprehensively about the type of drug combinations, mechanism of drug-drug interactions, and associated adverse drug events with significant drug-drug interactions that commonly occurred in children’s patient during the treatment. The drug-drug interaction including midazolam-phenobarbital, cannabidiol-clobazam, Paxlovid-tacrolimus, inhaled fluticasone propionate-lopinavir/ritonavir, rifampicin-warfarin, clofazimine-moxifloxacin, benzatropine-haloperidol, and enalapril-spironolactone. In conclusion, gaining a better understanding of drug-drug interactions among children will empower healthcare professionals to develop useful strategies to recognize, manage, and prevent various types of pharmacokinetic and pharmacodynamic interactions. Especially at different stages in terms of age, physiology, and complexity of the disease in children.

Abstract

Infeksi parenkim pada paru merupakan salah satu penyebab terjadinya penyakit pneumonia, tepatnya dikantung kecil yang disebut alveoli. Untuk mengurangi hal itu terjadi diperlukan adanya intervensi perawatan yaitu penggunaan antibiotik sebagai salah satu terapi utama untuk penyakit yang disebabkan oleh bakteri Streptococcus pneumoniae. Pada penelitian ini bertujuan untuk mengetahui jumlah penggunaan antibiotik empiris dan/atau definitif pada pasien anak dengan pneumonia di Rumah Sakit Akademik Universitas Gadjah Mada. Penelitian ini merupakan penelitian observasional deskriptif dengan desain penelitian cross-sectional pada pasien rawat inap yang berusia 0 hingga £18 tahun. Hasil penelitian ini diperoleh penggunaan antibiotik pasien anak dengan pneumonia periode 1 Januari hingga 31 Desember 2022 didapatkan nilai total DDD sebesar 1248,35 DDD/100 hari rawat dengan nilai DDD antibiotik tertinggi adalah gentamisin sejumlah 538,16 DDD/100 hari rawat dan nilai DDD antibiotik terendah adalah seftazidim 2,39 DDD/100 hari rawat inap. Segmen yang termasuk dalam DU 90% diantaranya gentamicin, sefiksim dan azitromisin, sefotaksim serta segmen yang temasuk dalam DU 10% diantaranya seftriakson, ampisillin, siprofloksasin, amoksisillin klavulanat, amoksisillin, amikasin, kotrimoksazol, dan seftazidim.

Abstract

Background: Augmented renal clearance (ARC) is a common condition in critically ill patients, particularly in intensive care units, characterized by enhanced renal elimination. The accelerated elimination of drugs can significantly impact therapeutic efficacy, potentially resulting in suboptimal treatment outcomes. This review aims to provide a comprehensive overview of drugs impacted by ARC, offering a better understanding of the impact of ARC on drug therapy.Method: A narrative review was conducted to explore previous studies from Scopus, PubMed, and ScienceDirect. PRISMA flow chart was used to guide the article selection process.Result:  A total of 14 articles were comprehensively reviewed and discussed regarding drugs affected by ARC, the impact of ARC on the pharmacokinetic/pharmacodynamic properties of drugs, and their clinical outcomes. Classes of drugs affected by ARC include beta-lactam antibiotics, glycopeptide antibiotics, anticoagulants, and anticonvulsants.Conclusion: Antibiotics are the most frequently reported drugs to be impacted by ARC, followed by anticoagulants and anticonvulsants. The impact of ARC on anticoagulants is inconsistent. ARC reduces free drug concentration, requires a higher dose to achieve the therapeutic target, and is associated with a higher risk of treatment failure. However, no significant differences were found in clinical response and mortality compared to non-ARC.

Abstract

The main objective of this study is to explore the role of university students in empowering the community of Lau Simomo Village through educational, religious, and social activities carried out during the Community Service Program (KKN). This research employed a descriptive qualitative approach with a field study method to provide a direct understanding of the conditions in the community. The data were obtained from both primary and secondary sources. Primary data were collected from the residents of Lau Simomo Village, Karo Regency, while secondary data included documentation, personal notes, and related references. Data collection techniques consisted of interviews, observations, and documentation. The findings reveal that the KKN program brought positive outcomes for both the villagers and the students. In the educational field, tutoring in the village library encouraged children’s motivation to learn, enriched teaching variations at schools, and introduced entrepreneurial insights through the socialization of banana chips production. Anti-bullying counseling helped foster respect among children. In terms of religion, activities such as wirid, Friday sermons, and Qur’an recitation lessons strengthened the community’s spiritual values. Socially, collective work, mango tree planting, and visiting non-Muslim residents during bereavement enhanced solidarity and tolerance. Overall, the KKN program provided tangible benefits for society while offering valuable experiences for students in community engagement.
