Abstract

Leptospirosis is primarily a disease of wild and domestic animals, and may be transmitted to humans through direct or indirect contact with infected animal urine. Leptospirosis is endemic in part of Indonesia territory. This work aimed to find the leptospirosis cases in Semarang city, one of the endemic leptospirosis in Indonesia by active and passive case finding. The leptospirosis were screened in the community base active case finding and passive case finding in primary health care center and hospitals, by using modified WHO-SEARO criteria and laboratory confirmation test. There were 191 cases detected with modified WHO-SEARO criteria, and among those cases only 31 cases confirmed by laboratory test, either by using Rapid Detection Test (RDT), Microscopic Agglutinations Test (MAT) or Polymerase chained reaction (PCR). It was found that leptospirosis was not correlated with rainfall season. Fever, headache and myalgia were the most common symptoms experienced by >90% patients. Active case finding add no more than 7% probable leptospirosis and 14% confirmed cases to the traditional passive case finding. It was conclude that leptospirosis case finding is still challenging in the endemic area, such as Semarang city. The application of modified WHO-SEARO criteria as diagnostic tool may be needed to be further evaluated. Keywords: Leptospirosis, Surveillance, Case finding, Endemic, Detection

Abstract

Leptospirosis is still a global health problem because it affects human health in rural and urban areas, both in industrialized and developing countries. The aim of the study was to detect Leptospira spp. bacteria in kidney tissues isolated from rats in the Napu and Bada Highlands of Poso District, Central Sulawesi Province. Kidneys sample from 63 rats were collected from Napu and Bada Highlands of Poso District, Central Sulawesi Province in MayJune 2018. Polymerase Chain Reaction (PCR) was used to detect Leptospira. The molecular characterizations were conducted based on the 16SrRNA and LipL32 genes. Data were analyzed descriptively to describe the presence of pathogenic Leptospira DNA. Analysis phylogenetic was performed using MEGA 6.2 software. A total of 63 rats was successfullycaught during the study consisting of males and female for 36 (57.1%) and 27 (42.9%), respectively. The species of rats were R. exulans, R. tanezumi, R. argentiventer, R. norvegicus, M. Musculus, Paruromys dominator, Maxomys sp., and Rattus sp. The pathogenic of Leptospira DNA was detected in rats with R. argentiventer and Paruromys dominatorspecies using the 16S rRNA and LipL32 gene. Sample sequences using LipL32 target gene is a close similarity with L. interrogans serovar Hardjo, serovar Autumnalis, Lai, Icterohaemorrhagiae, Balico, Grippotyphosa, Mini, Canicola, Hebdomadis; L. noguchii serovar Pomona and L. kirschneri whereas the sample sequence using 16S rRNA targetgene showed similarity with L. interrogans serovar Canicola, Copenhagen, Autumnalis, Pyrogenes, Javanica, Icterohaemorrhagiae, Manilae, Bratislava, Linhae, Hebdomadis, and L. kirschneri serovar Grippotyphosa. The PCR method with the target gene 16SrRNA and LipL32 are able to detect Leptospira spp. in rats R. argentiventer and P. dominator species Keywords: Leptospira, 16S rRNA, LipL32, PCR, Kidney’s Rat Leptospirosis masih merupakan masalah kesehatan global karena mempengaruhikesehatan manusia di daerah pedesaan dan perkotaan, baik di negara industri maupun mnegara berkembang. Tujuan penelitian adalah untuk mendeteksi bakteri Leptospira spp di jaringan ginjal dari tikus di Dataran Tingi Napu dan Bada Kabupaten Poso, Provinsi Sulawesi Tengah. Ginjal tikus sebanyak 63 sampel dikoleksi dari Dataran Tinggi Napu dan Bada Kabupaten Poso, Provinsi Sulawesi Tengah pada bulan Mei – Juni 2018. PCR digunakan untuk mendeteksi Leptospira. Karakterisasi molekuler dilakukan berdasarkan gen 16SrRNA dan LipL32. Data dianalisis secara deskriptif untuk menggambarkan keberadaaN Leptospira yang patogenik. Analisis filogenetik dilakukan dengan menggunakan perangkat lunak Mega 6.2. Sebanyak 63 tikus berhasil ditangkap selama penelitian yang terdiri dari jantan dan betina, masing masing 36 ekor (75,1%) dan 27 ekor (42,9%). Spesies tikus adalah R. exulans, R. tanezumi, R. argentiventer, R. norvegicus, M. Musculus, Paruromys dominator, Maxomys sp, dan Rattus sp. DNA Leptospira patogenik terdeteksi pada tikus dengan spesies R. argentiventer dan Paruromys dominator menggunakan gen 16SrRNA dan LipL32 Sekuen sampel dengan target gen LipL32 menunjukkan kesamaan dengan L. interrogans serovar Hardjo, serovar Autumnalis, Lai, Icterohaemorrhagiae, Balico, Grippotyphosa, Mini, Canicola, Hebdomadis; L. noguchii serovar Pomona dan L. kirschneri. Sedangkan sekuen sampel dengan target gen 16S rRNA menunjukkan kesamaan dengan L. interrogans serovar Canicola,Copenhagen, Autumnalis, Pyrogenes, Javanica, Icterohaemorrhagiae, Manilae, Bratislava, Linhae, Hebdomadis, dan L. kirschneri serovar Grippotyphosa. Metode PCR dengan target gen 16SrRNA dan LipL32 mampu mendeteksi Leptospira spp. pada tikus dengan spesies R. argentiventer dan P. dominator. Abstract

Demam dengue atau demam berdarah dengue merupakan salah satu masalahkesehatan di daerah tropis dan subtropis. Penyakit ini disebabkan oleh virus dengue(genus flavivirus, famili flaviviridae). Vektor pembawa virus dengue adalah nyamukAedes aegypti dan Aedes albopictus. Genom virus dengue tersusun atas tiga proteinstruktural (protein nukleokapsid, protein envelope, dan protein pre-membran) dan tujuhprotein nonstruktural (NS1, NS2a, NS2b, NS3, NS4a, NS4b, dan NS5). Usaha untukmengontrol penyakit ini tergantung pada pemahaman patogenesis virus dengue. Tetapipengetahuan mengenai patogenesis virus dengue masih belum banyak diketahui karenabelum adanya model yang cocok baik in vitro maupun in vivo. Sehingga dilakukananalisis sekuen nukleotida E/NS1 gene junction virus dengue tipe 2 asal DKI Jakarta,Indonesia untuk mengetahui hubungan filogeni virus dengue yang beredar saat ini.Sekuen nukleotida (240 bp) dari E/NS1 gene junction merupakan segmen yang biasadigunakan untuk analisis perbandingan sekuen. Hasil analisis menunjukkan strain virusyang diteliti berkerabat dengan virus DEN-2 asal Asia yaitu Myanmar, Pakistan, SriLanka, Brunei, dan Malaysia. Tetapi kelima virus ini berada dalam kelompok yangberbeda dengan virus asal Amerika latin (Brazil). Virus Dengue dari Asia diduga lebihvirulen dibandingkan virus dengue dari daerah lain. Abstract

Aedes aegypti is a primer vector of dengue virus (DENV) and chikungunya virus (CHIKV). The susceptibility of mosquitoes to DENV and CHIKV depends on their recognition receptor of pathogens. C-type lectins (CTLs) are an important mediator of virus infection in A. aegypti. This study aims to identify potential receptors and determine the binding affinity between ligand–receptor interaction, CTLs and virus envelopes (DENV-1, 2, 3, and 4 and CHIKV) interaction based on in silico analysis. Sample sequences were obtained from GenBank (NCBI), and 10 CTLs were acquired from VectorBase. Homology modeling based on a minimum standard of 20% was processed using the SWISS-MODEL server. Molecule interaction was visualized as tertiary protein structure in PyMol V1.7.4. Homology modeling revealed all 3D protein model scores to be at 20%. The highest homology modeling score of 98.72% is obtained for CHIKV envelope protein (KF925315.1). The CTL GenBank ID: AAEL014382-PA has the lowest score at −1407.20 kcal/mol. The predicted mechanism of DENV-1, 2, 3, and 4 co-infections with CHIKV-1 is through the binding of the viral envelope to the CTL. Interaction analysis showed that the viruses enter through an endocytic mechanism. The results of homology modeling and lowest interaction energy could serve as the basis for in vivo studies on virus co-infection in A. aegypti. This information illustrates that CTL receptors facilitate DENV and CHIKV co-infection in A. aegypti.

Abstract

Permasalahan yang dihadapi oleh kelompok tani di desa Sembungan, Kejajar, Wonosobo adalah banyaknya buah carica yang tidak layak diolah menjadi sirup carica. Buah carica yang rusak tidak dapat diolah sehinggah menjadi sampah yang mengganggu lingkungan pariwisata setempat. Permasalahan kedua adalah mahalnya harga pupuk dan obat tanaman yang digunakan petani untuk bercocok tanam sayuran. Tujuan pengabdian ini adalah membantu kelompok tani dalam mengolah limbah buah carica menjadi pupuk organic cair dan obat hama tanaman. Teknologi yang digunakan adalah proses limbah carica menjadi eco-enzyme. Metode pengabdian yang digunakan adalah sosialisasi tentang pemanfaatan limbah organik, pelatihan produksi pupuk cair, fasilitasi teknologi, pelatihan pemasaran pupuk organic, dan pendampingan. Pengabdian ini menghasilkan luaran berupa pupuk organic cair yang siap diterapkan untuk penyubur tanaman sayuran dan obat hama penyakit tanaman.

Abstract

Human papillomavirus (HPV) L1 major capsid protein generates a highly immunogenic virus like particles (VLPs), which have been used as the main component of its prophylactic vaccine. However, the neutralizing antibodies against L1 VLPs are mostly type specific and may not be effective to prevent infection from different strains of HPV. On the other hand, HPV L2 minor capsid protein has low antigenic variation, thus can induce cross-neutralization. This study aims to obtain HPV 52 L1/L2 chimeric protein, which is designed based on HPV type 52 as one of the most circulated high-risk types in Indonesia, to develop a broad-spectrum HPV vaccine. Substitution of HPV 52 H4 helix L1 region with an HPV 52 L2 epitope was carried out using overlap extension PCR. HPV 52 L1/L2 chimeric gene was constructed into pET-SUMO expression vector and expressed in Escherichia coli BL21 (DE3). Bioinformatics modeling suggested that L2 epitope was located inside of the loop region in monomer form, and on the contrary, it was located outside of the pentamer surface. Furthermore, B cell and T cell epitopes predictions were conducted using Immune Epitope Database (IEDB) analysis. The B cell epitopes prediction revealed eleven potential epitopes, whereas the T cell epitopes prediction showed seven potential epitopes for each MHC class I and MHC class II. This study showed that HPV 52 L1/L2 chimeric protein has the potential to induce cross-neutralizing antibodies and can be developed as a promising candidate for a new HPV vaccine.

Abstract

UMKM Happy Feet di Yogyakarta, adalah produsen sandal kulit dan vinyl, menjadi tumpuan ekonomi bagi masyarakat sekitar. Namun, masalah produktivitas dan kesehatan pekerja muncul akibat stasiun kerja pembuatan pola yang tidak ergonomis dan pencahayaan yang kurang memadai. Kondisi ini dapat menyebabkan ketidaknyamanan, kelelahan, bahkan gangguan kesehatan pada pekerja, serta menghambat produktivitas secara keseluruhan. Berdasarkan observasi dan diskusi dengan pemilik, permasalahan yang dihadapi meliputi: (1) postur kerja yang tidak ergonomis akibat desain stasiun kerja yang kurang tepat, (2) pencahayaan yang tidak memenuhi standar, (3) kurangnya pemahaman pekerja tentang ergonomi. Solusi yang ditawarkan dalam pengabdian ini meliputi: (1) perancangan ulang stasiun kerja sesuai prinsip ergonomi, (2) perbaikan sistem pencahayaan, (3) pelatihan dan sosialisasi pencahayaan yang baik. Implementasi solusi ini terbukti meningkatkan produktivitas dan kesejahteraan pekerja di UMKM Happy Feet melalui perbaikan postur kerja dan lingkungan kerja yang lebih nyaman.

Abstract

Penyakit infeksi merupakan penyakit dengan insidensi tinggi di Indonesia sebagai negara tropis. Dewasa ini, berbagai penelitian tidak hanya seputar agen penyebab (bakteri, virus, parasit) ataupun faktor lingkungan saja, namun juga banyak melibatkan inang, termasuk genetika manusia. Pengabdian masyarakat ini bertujuan mengedukasi sasaran terbatas yaitu akademisi, mahasiswa, peneliti, dan klinisi di bidang kedokteran dan kesehatan di Indonesia tentang peran genetika manusia dalam penyakit infeksi. Pengabdian masyarakat dilakukan dengan membuat media daring audiovisual berupa tanya jawab bersama pakar dengan evaluasi setelah satu bulan dari pengunggahan video. Hasil pengabdian masyarakat menunjukkan bahwa penggunaan media daring audiovisual efektif dalam meningkatkan pemahaman akademisi, mahasiswa, peneliti, dan klinisi mengenai peran genetika manusia dalam penyakit infeksi. Diskusi dengan responden menghasilkan daftar pertanyaan yang relevan, yang kemudian dijawab oleh pakar melalui video edukatif. Video ini diunggah dan dibagikan kepada sasaran terbatas, memperoleh 48 penayangan serta respon positif dalam satu bulan. Evaluasi menunjukkan perlunya distribusi lebih luas untuk meningkatkan dampak edukasi. Model serupa dapat dikembangkan untuk memperluas jangkauan edukasi kedokteran berbasis daring. Pemanfaatan media daring audiovisual dalam pengabdian masyarakat bagi akademisi, mahasiswa, peneliti, dan klinisi di bidang kedokteran dan kesehatan meningkatkan pemahaman tentang peran genetika manusia pada penyakit infeksi. Untuk memperluas dampaknya, distribusi konten perlu diperluas. Model edukasi serupa dapat dikembangkan lebih lanjut untuk menjangkau masyarakat luas, seiring dengan kemajuan teknologi dan kebutuhan informasi kesehatan yang terus berkembang.

Abstract

AbstractBackground: Non-neoformans cryptococci were previously considered to be saprophytes and nonpathogenic to humans. Cryptococcus laurentii is commonly found in the environment and pigeon feces and has been reported to be an emerging human fungal pathogen that causes infection in the immunocompromised host. Here, we report a unique case of pulmonary cryptococcosis coinfection with Miliary Tuberculosis.Case: A 14-years old adolescent underweight boy was admitted to the Emergency Unit with the chief complaint of fever, cough, chest pain, breathlessness, night sweats, malaise, and weight loss. He was referred from Public Health Center in Sleman Yogyakarta with Miliary Tuberculosis and consumed six weeks of fixed-dose combinations of the first-line anti-tuberculosis drug. Microbiology culture of good quality patient sputum revealed Cryptococcus laurentii along with Klebsiella oxytoca. The patient was diagnosed with Pulmonary Cryptococcosis. The patient's symptoms, especially fever and chest pain, were not improved by oral Fluconazole 200 mg treatment. However, he had an excellent response to Levofloxacin 500 mg during hospitalization and oral Fluconazole therapy at 400 mg/day for eight weeks. After eight weeks of Fluconazole treatment, the patient got well, and then Fluconazole was stopped. Anti-tuberculosis was treated for six months, and no recurrence was found.Conclusion: This article reports the rare pulmonary involvement of Cryptococcus laurentii in adolescent patients with Miliary Tuberculosis patient in Indonesia. A high degree of pulmonary mycosis suspicion and techniques improvement for respiratory specimen collection, culture, and identification contribute to early diagnosis and fungal infection treatment. In addition, interprofessional discussion can improve patient treatment and outcomes.

Abstract

Indonesia is a member of Southeast Asia Regional Office (SEARO) ranked the first in dengue hemorrhagic fever (DHF) problem based on incidence rate (IR) and case fatality rate (CFR). Several provinces in Indonesia experience an outbreak, one of which is the Mataram City in West Nusa Tenggara Province. Mataram City is an endemic area of DHF because the DHF cases are always found in three consecutive years with the number of cases that fluctuate and tend to increase. This study aimed to obtain factors that could be used to improve early warning systems in controlling DHF. This study used a case control design with a ratio of 1:1 to 180 house holds. The results showed that home environmental factors, such as no ceiling, indoor and outdoor temperature that had the potential for breeding places for mosquitoes, no wire net in ventilation, low lighting and high humidity, related to DHF transmission. Vector distibution with entomology index showed that the existence of larvae, eggs and mosquitoes played a role in dengue transmission. The dominant factors affecting the transmission of dengue in Mataram City are the condition of the ceiling and the existence of mosquito eggs in the house.
