Abstract

Kinerja perawat menjadi isu penting dalam organisasi rumah sakit dalam upaya meningkatkan efisiensi dari pelayanan kesehatan. Laporan Kinerja Rumah sakit TK II Putri Hijau Medan berdasarkan Angka Bed Occupancy Ratio (BOR) yaitu mengalami peningkatan setiap tahunnya mulai tahun 2020 yaitu 16, 47%, mengalami peningkatan menjadi 28, 26% tahun 2021, meningkat kembali menjadi 33, 28% tahun 2022 dan meningkat 50, 98% tahun 2023. Namun, angka ini belum sesuai dengan Standar Pelayanan Minimum yaitu 60-80%. Rata-rata lama pasien dirawat. (ALOS) yaitu 2, 47 hari tahun 2020 dan menurun menjadi 5, 71 hari tahun 2021 dan meningkat menjadi 5, 16 hari tahun 2022 dan 5, 81 tahun 2023. Angka ini belum juga sesuai dengan Standar Pelayanan Minimum (SPM) yaitu 6-9 hari. Rata-rata tempat tidur tidak terpakai (TOI) yaitu 14, 58 hari tahun 2020 dan meningkat menjadi 17, 05 hari tahun 2021 dan menurun menjadi 11, 88 hari tahun 2022 serta 6, 91 hari tahun 2023. Angka tersebut belum sesuai dengan Standar Pelayanan Minimum yaitu 1-3 kali. Jenis penelitian adalah survei analitik dengan tipe cross sectional. Jumlah populasi 133 perawat, sampel penelitian seluruh perawat berstatus pengawai ASN, PPPK dan Militer. Penelitian dilakukan bulan Mei sampai dengan Agustus 2024 melalui penyebaran kuesioner. Data dianalisis secara univariat, bivariat dan multivariat menggunakan uji regresi logistik berganda pada taraf kemaknaan 95%. Hasil penelitian menunjukkan bahwa faktor internal yang meliputi: Prestasi, pengakuan, tanggumg jawab dan pengembangan diri berpengaruh signifikan terhadap motivasi kerja perawat pelaksana (p 0, 15 < 0, 05). Hasil penelitian membuktikn bahwa factor eksternal meliputi: Keamanan dan keselamatan, Kondisi kerja dan Supervisi berpengaruh terhadap motivasi kinerja perawat pelaksana (p 0, 021<0, 25). Dapat disimpulkan bahwa factor eksternal. yaitu keamanan dan keselamatan, kondisi kerja dan supervisi paling berpengaruh terhadap motivasi kinerja perawat. Dari hasil penelitian di peroleh Kesimpulan bahwa motivasi kerja perawat sangat di perlukan untuk meningkatkan kualitas asuhan keperawatan di Rumah Sakit TK II putri Hijau Medan.

Abstract

Sanitasi merupakan salah satu hal yang mendasar dalam upaya pemenuhan kehidupan manusia demi tercapainya tingkat kesehatan yang optimum. Data Dinas Provinsi Sumatara Utara bahwa jumlah kasus diare tahun 2020 mencapai 70.243 penderita dengan Incidence Rate (IR) 31,35%=. Tujuan dalam penelitian ini adalah untuk mengetahui dan Faktor-Faktor Yang Memengaruhi Stop Buang Air Besar Sembarangan Di Wilayah Kerja Puskesmas Pangkatan Kecamatan Pangkatan Kabupaten Labuhanbatu Sumatera Utara Tahun 2024. Metode Penelitian ini adalah penelitian modifikasi (mix methode). Penelitian dengan menggunakan kombinasi antara penelitian kuantitatif dan kualitatif. Penelitian ini dilakukan di wilayah kerja Puskesmas Pangkatan Kecamatan Pangkatan Kabupaten Labuhanbatu Sumatera Utara pada bulan Januari - Maret tahun 2024.Sampel dalam penelitian ini berjumlah sebanyak 94 kepala keluarga. Hasil Penelitian Dari hasil uji statistik, Chi – Square di peroleh ada pengaruh tingkat pendidikan dengan perilaku stop buang air besar sembarangan dengan p= 0,000 (<0,05), ada pengaruh tingkat pendapatan dengan perilaku stop buang air besar sembarangan dengan p=0,000 (<0,05), da pengaruh tingkat pengetahuan dengan perilaku stop buang air besar sembarangan dengan p=0,000 (<0,05), ada pengaruh sikap dengan perilaku stop buang air besar sembarangan. dengan p=0,000 (<0,05), ada pengaruh kepemilikan jamban sehat dengan perilaku stop buang air besar sembarangan dengan p=0,000 (<0,05), ada pengaruh peran kepala keluarga dengan perilaku stop buang air besar sembarangan dengan p=0,000 (<0,05), ada pengaruh peran tenaga kesehatan dengan perilaku stop buang air besar sembarangan dengan p=0,000 (<0,05), ada pengaruh peran kader dengan perilaku stop buang air besar sembarangan dengan p=0,000 (<0,05) dan ada pengaruh peran tokoh/pemuka masyarakat dengan perilaku stop buang air besar dengan p=0,000 (<0,05). Kesimpulan dari penelitian ini adalah ada pengaruh pendidikan,pendapatan, perilaku , sikap, kepemilikan jamban, peran kepala keluarga, kader dan tokoh masyarakat dengan perilaku stop buang air besar sembarangan. Saran dalam penelitian ini Diharapkan petugas kesehatan Puskesmas pangkalan dapat rutin atau memiliki jadwal yang jelas dalam memberikan penyuluhan kepada masyarakat yang di bantu oleh kader dan tokoh masyarakat memberikan.

Abstract

The performance of inpatient nurses at AmaliaMedika Riau Hospital has not been implemented optimally, this is due to ineffective two-way communication both to superiors and fellow nurses. Teamwork that is not well established, so that there is work that is not carried out optimally. The purpose of this study is to determine the influence of communication and teamwork on the performance of nurses at Amalia Medika Riau Hospital. The research design used in this study is quantitative research. The population in this study is all nurses in the inpatient room at Amalia Medika Riau Hospital as many as 45 people. The sampling technique uses the saturated sample technique so that the sample totals 45 people. The data collection technique uses a questionnaire. Data analysis was carried out by univariate and bivariate analysis with chi square and multivariate tests. The results of the study showed that there was an influence of communication on the performance of nurses at Amalia Medika Riau Hospital and there was an influence of teamwork on the performance of nurses at Amalia Medika Riau Hospital. A multivariate analysis was obtained that the communication variable had the most influence on the performance of nurses at Amalia Medika Riau Hospital. The conclusion in this study is that there is an influence of communication and teamwork on the performance of nurses at Amalia Medika Riau Hospital and the variable that has a higher chance is communication. The implication of this study is that the performance of inpatients at Amalia Media Riau Hospital can be improved by improving two-way communication by creating openness, empathy and mutual support between fellow nurses and superiors. Improving the teamwork of inpatient nurses by contributing both energy and mind to improve performance.

Abstract

Background: Exclusive breastfeeding is an important factor in supporting the growth and development of babies. However, exclusive breastfeeding coverage is still low in various regions, including at the Ujung Batu Health Center, North Padang Lawas Regency. Research Objectives: To analyze the factors that affect the success of exclusive breastfeeding. Research Method: Using the Analytical Survey method with a Cross Sectional approach. Research Population: this is all mothers who have babies 6-12 months in the Ujung Batu Health Center as many as 115 people and a sample of 89 people with random sampling techniques. Research Instrument: this is a questionnaire about the success of Exclusive Breastfeeding and several factors related to the success of Exclusive Breastfeeding using univariate, bivariate analysis with chi-square test and multivariate with logistic regression. Data were collected through questionnaires and analyzed using logistic regression tests. The results of the study: showed that the factors that had a significant influence on the success of exclusive breastfeeding were the level of maternal knowledge (p=0.002), family support (p=0.005), the role of health workers (p=0.001), and maternal employment factors (p=0.010). Mothers with a good level of knowledge and strong family support are more likely to breastfeed exclusively. In addition, health workers who actively provide education increase the likelihood of success in exclusive breastfeeding. In contrast, working mothers have a lower likelihood of exclusive breastfeeding. The conclusion of this study is that increased education about exclusive breastfeeding as well as the support of families and health workers is very important to increase the coverage of exclusive breastfeeding in this region Suggestion: Respondents who have babies aged 6-12 months are expected to be able to give exclusive breastfeeding and can increase awareness of the importance of exclusive breastfeeding, and have high patience when breastfeeding exclusively.

Abstract

Data from the World Health Organization (WHO) in 2020 states that developing economies have 40% of people with hypertension while developed countries only 35%, the African region holds the highest position of people with hypertension, which is 40% and Southeast Asia 36%. The purpose of the study was to analyze factors that influence adherence to taking medication in hypertensive patients at the Paringgonan Health Center, Ulu Barumun District, Padang Lawas Regency. The research design used Analytical Survey method with Cross Sectional approach. The population in this study were all 95 hypertensive patients, all of whom were sampled using the total population technique. Data analysis used univariate analysis, bivariate with chi-square test and multivariate with logistic regression.The results of logistic regression research show that there is an effect of education p = 0.000, knowledge p = 0.000, distance of residence p = 0.000, emotional support p = 0.006 and duration of taking medication p = 0.000. While the dominant factor influencing adherence to taking medication in hypertensive patients is knowledge p = 0.000 with Exp(B) = 68.614.The conclusion of this study is that there is an influence of education, knowledge, distance of residence, emotional support and duration of taking medication. The dominant factor, namely knowledge, influences compliance with taking medication in hypertensive patients at the Paringgonan Health Center, Ulu Barumun District, Padang Lawas Regency. It is recommended that the Puskesmas be taken into consideration and input in improving the implementation of counseling and improving the performance of health workers in carrying out policies and intervening in adherence to taking hypertension medication in collaboration with health workers from Puskesmas Paringgonan and PMO so that the objectives of the program are implemented and are expected to reduce the number of cases of hypertension.

Abstract

Dental and oral health is an important aspect of children's growth and development because it plays a role in supporting nutritional fulfillment. Basic Health Research data shows that dental problems in Central Aceh are still high, including in the elementary school age group. This research was conducted at SD Negeri 2 Atu Lintang which does not yet have a School Dental Health Unit (UKGS), involving all 24 3rd grade students. The results showed a significant increase in toothbrushing behavior after being given counseling using the demonstration method. Before counseling, most students were in the medium category (29.1%), while after counseling it increased to good (87.5%). The statistical test showed a value of p = 0.000 (p < 0.05), so it can be concluded that the demonstration method is effective in improving toothbrushing behavior in students. Thus, dental and oral health counseling based on the demonstration method has been proven to improve the knowledge and skills of correct brushing practices in school children. It is hoped that this activity will be carried out continuously by strengthening micro-teaching and the use of counseling media so that the effectiveness of the program is more optimal.

Abstract

Nurse performance is a series of activities of nurses in providing an action or nursing care to patients. Good performance is a reflection of the quality of nursing services provided by nurses. Data on the quality of nursing at RSI Malahayati Medan in 2024 is 75.8% of the performance of executive nurses in the good category, 15.9% of the performance of executive nurses in the sufficient category and 8.3% of the performance of executive nurses in the less assessment category including documentation of nursing care. The purpose of the study was to analyze the effect of the leadership style of the head of the room on the performance of the executive nurse in the implementation of nursing care in the inpatient room of RSI Malahayati Medan. The research design used Analytical Survey method with Cross Sectional approach. The population in this study were all executive nurses who served in the inpatient room as many as 70 people, all of whom were used as research samples with total population techniques. Data analysis used univariate analysis, bivariate with chi-square test and multivariate with logistic regression. The results of logistic regression research indicate that there is a democratic influence p = 0.003, laisses faire (free to act) p = 0.003, participatory p = 0.005 and transpormative p = 0.013. While the dominant factor influencing the performance of executive nurses is democratic p = 0.001 with Exp (B) 85.754, meaning that democracy has 85 times the risk compared to other variables. The conclusion of this study is that there is a democratic, free-action, participatory and transformative influence on the performance of implementing nurses in the implementation of nursing care in the inpatient room of RSI Malahayati Medan and variables that have no effect are authoritarian. It is recommended that the head of the room in carrying out the leadership function should be democratic, namely making decisions based on deliberations with other teams that are adjusted to the situation so that it will produce effective leadership so as to make subordinate performance better and of higher quality.

Abstract

Pelayanan kesehatan merupakan suatu produk, berupa jasa kepada pasien, aktifitas ini tidak bisa dimiliki namun bisa dirasakan oleh pasien. Indikator yang ingin dicapai pada produk jasa ini adalah kepuasan pasien terhadap pelayanan kesehatan. Untuk mencapai produk jasa yang sesuai dengan standar maka diperlukan kinerja maksimal dari pelaku jasa yaitu perawat. Kinerja perawat merupakan ujung tombak dari keberhasilan jasa pelayanan kesehatan yang jika perawat mengurangi perannya maka kepuasan pasien tidak akan tercapai. Tujuan penelitian untuk menganalisis faktor yang mempengaruhi kinerja perawat di UPTD RSUD Datu Beru Takengon Aceh Tengah. Metode penelitian yang digunakan adalah survey analitik dengan rancangan cross sectional. Uji statistik yang digunakan adalah uji chi square dan regresi logistik. Populasi adalah seluruh perawat sebanyak 236 orang. Tehnik pengambilan sampel menggunakan accidental sampling yang diperkuat dengan rumus slovin didapatkan sampel 148 orang. Hasil penelitian menunjukan bahwa variabel kepemimpinan (P-value 0,001), motivasi (P-value 0,002), Kedisiplinan (P-value 0,001) dan lingkungan kerja (P-value 0,001). Hasil uji regresi logistik berganda menunjukkan bahwa variabel kepemimpinan (OR=1.718), kedisiplinan (OR=3.684) dan lingkungan kerja (OR=1.104) artinya bahwa variabel kedisiplinan mempunyai pengaruh 3 kali lipat mempengaruhi kinerja perawat. Kesimpulan didapatkan bahwa kepemimpinan, motivasi, kedisiplinan dan lingkungan kerja mempengaruhi kinerja perawat di UPTD RSUD Datu Beru, Takengon, Aceh Tengah.

Abstract

Pelayanan kesehatan administrasi di rumah sakit sangat erat kaitannya dengan penggunaan Sistem Informasi Manajemen Rumah Sakit (SIMRS). SIMRS merupakan suatu platform teknologi informasi yang dirancang khusus untuk mengelola dan mengintegrasikan berbagai aspek administrasi dan operasional di rumah sakit. Penelitian ini bertujuan untuk menganalisis dampak implementasi SIMRS terhadap pelayanan administrasi menggunakan metode PIECES. Metode penelitian yang digunakan adalah metode kualitatif dengan melibatkan 11 informan. Pengumpulan data dilakukan melalui wawancara mendalam untuk memperoleh informasi yang komprehensif tentang pengalaman dan pandangan para informan terhadap implementasi SIMRS. Analisis data dilakukan dengan pendekatan deskriptif kualitatif untuk memahami secara mendalam fenomena yang terjadi. Hasil penelitian menunjukkan bahwa implementasi SIMRS di Rumah Sakit Umum Haji Syaiful Anwar telah membawa peningkatan signifikan dalam efisiensi waktu dan kemudahan pengisian data pasien, serta mengurangi penggunaan kertas. Namun, terdapat beberapa kendala yang masih perlu diatasi, seperti kurangnya pemahaman dan keterampilan SDM, keterbatasan komputer, dan modul yang belum terintegrasi. Kesimpulan dalam penelitian ini adalah implementasi SIMRS di RSU Haji Syaiful Anwar yang ditinjau dari Performance (Kinerja), Information/Data (Informasi/Data), Economic (Ekonomi), Control/Security (Kontrol/Keamanan), Efficiency (Efisiensi), Service (Pelayanan) sudah cukup baik namun masih perlu dilakukan perbaikan agar SIMRS dapat berfungsi secara optimal dan memberikan manfaat maksimal bagi rumah sakit. Disarankan evaluasi berkala, pelatihan tambahan, Strategi untuk mengatasi turnover pegawai yaitu perekrutan yang baik, pengembangan karier dan kenaikan gaji, serta peningkatan infrastruktur teknologi untuk mengoptimalkan integrasi modul dan efisiensi pelayanan.
