Abstract

Pain is an unpleasant sensory and emotional experience that can affect the quality of life and leads to decreased productivity in patients. Low back pain (LBP) is one of the significant causes of disability worldwide with lifelong incidence. The purpose of this literature review describes the potential of anthocyanin-based Poly (Methyl Methacrylate) (PMMA)   nanoparticles as the management of inflammatory pain in the Hernia Nucleus Pulposus (HNP ). The method used is a literature study by entering the keyword. Of the 77 journals reviewed, 47 journals were found by the topic and used as a reference for this work. The anthocyanin-based PMMA nanoparticles act as anti-nociceptors by inhibiting microglia that produce inflammatory mediators in HNP. Poly (Methyl Methacrylate) nanoparticles have specific targets in microglia. Anthocyanins have the effect of inhibiting inflammatory pain through many destinations. Anthocyanin inhibits the synthesis of nitric oxide (NO ) and prostaglandin E2 (PGE 2) and inhibits the activation of p38 MAPK and NF-kB pathways that express TNF-α and IL-1β genes as anti-nociceptive. The anthocyanin-based PMMA nanoparticles have potential as a novel therapy for inflammatory pain in HNP. There has been no research between these modalities. Therefore, further research is needed to find out the exact potential of anthocyanin-based PMMA nanoparticles.

Abstract

Background: One of the adverse effects of chemotherapy in the neurological field is chemotherapy-induced peripheral neuropathy (CIPN), it was an adverse effect caused by many chemotherapeutic regimens and a major cause of continuous pain in patients who survived cancer. When the symptoms get worse, survivor’s quality of life is declining and they are often having problems with mental health, insomnia, cognitive functioning, fatigue, physical functioning, and pain. So far the choice of therapy for CIPN is still limited. Now, there was no drug approved to treat pain due to CIPN. Duloxetine are a SNRI (serotonin norepinephrine reuptake inhibitor), which it’s useful in CIPN are highly supported by evidence.Methods: We conducted a systemactic-computer based literature search on January 4, 2019 in PubMed database for article published between 2013 and 2018. We searched for a literature related with used a duloxetine for management of chemotherapy induced peripheral neuropathy.Results: From the 4 studies that included in this review, the total participants are 478 patients with neuropathic pain and already get a chemotherapy regiment. From the meta-analysis showed one study had no effect, two studies had small effect, and one study had medium effect.Conclusion: Duloxetine is a great option for the treatment of CIPN in especially reducing neuropathic pain.

Abstract

Industrial revolution 4.0 has changed the health industry. Medical treatments derived from the latest technologies are expected to improve health and economic status. The rapid development of technology has influenced the learning and clinical practice of medicine. Virtual Reality (VR) is one of the latest technologies that is currently being implemented as a treatment for neurological disorders. This article seeks to give an overview of the development of VR implementation in neurorehabilitation. The data source of this article is research and studies published in NCBI, PubMed, Cochrane, and other relevant online databases. From this article, it is found that VR can be used as a pain reliever and motor function rehabilitation for patients with balance and gait deficits. VR is used to distract patients’ attention to pain for a short time period. VR also improves motor function recovery in stroke patients. Cybersickness is usually reported as a side effect of using VR and it depends on each individual. The implementation of VR for patients with neurological disorders has showed advantages in reducing pain and improving motor function but still need further research about applicability and authorization of virtual reality in the world of medicine.

Abstract

Pregabalin ((S)-3-aminomethyl-5-methyl hexanoic acid) merupakan obat baru yang memiliki struktur analog neurotransmiter penghambat, yaitu GABA (g-aminobutyric acid). Pregabalin berikatan dengan α-2-δ subunit saluran kalsium (voltage-gated calcium channels), sehingga dapat mengurangi pelepasan beberapa neurotransmiter eksitatorik dan menghambat terjadinya hiperalgesia dan sensitisasi sentral. Pregabalin memiliki efek antikonvulsan, antihiperalgesik dan ansiolitik yang mirip gabapentin, namun memiliki sifat farmakokinetik lebih baik. Pregabalin secara umum diindikasikan untuk beberapa kondisi nyeri kronis, seperti nyeri pasca-trauma medula spinalis, post-herpetic neuralgia, fibromialgia, neuralgia trigeminal, diabetic peripheral neuropathy (DPN), dan penyakit neuropati lainnya.Pregabalin ((S)-3-aminomethyl-5-methyl hexanoic acid) is a new drug with structure similar to GABA (g-aminobutyric acid), an inhibitory neurotransmitter. Pregabalin binds to α-2-δ subunit of voltage-gated calcium channel to inhibit the release of excitatory neurotransmitter to prevent hyperalgesia and central sensitization. Pregabalin has anticonvulsant, antihyperalgesia and anxiolytic properties similar to gabapentin, with better pharmacokinetic profile. It is indicated for several chronic condition as post-spinal trauma pain, post-herpetic neuralgia, fibromyalgia, trigeminal neuralgia, diabetic peripheral neuropathy (DPN), and other neuropathies

Abstract

Latar Belakang: Pemeriksaan Kecepatan Hantar Saraf (KHS) adalah bagian dari prosedur elektrodiagnostik untuk menegakkan diagnosis penyakit sistem saraf perifer. Pengukuran KHS dapat dipengaruhi oleh berbagai faktor fisiologis dan non-fisiologis, sehingga acuan nilai normal di tiap tempat dapat berbeda; oleh karena itu, sebaiknya setiap tempat pemeriksaan memiliki nilai normalnya sendiri. Saat ini di RSUP Sanglah belum memiliki acuan nilai normal KHS. Tujuan: Mengetahui nilai rata-rata normal KHS di RSUP Sanglah Denpasar. Metode: Penelitian potong lintang consecutive sampling selama bulan Juli 2013. Setiap orang dewasa normal berusia 20-30 tahun yang memenuhi kriteria diperiksa di lengan dan tungkai kanan, untuk menilai latensi, amplitudo, dan KHS. Data diolah menggunakan SPSS 16. Hasil: Dari 30 sampel pemeriksaan Compound Muscle Action Potential (CMAP), rerata hasil secara berurutan latensi, amplitudo, KHS nervus medianus 2,95±0,34 mdet; 4,33±1,54 mV; 67,16±6,73 m/det; nervus ulnaris 2,41±0,29 mdet; 3,46±0,89 mV; 66,65±7,36 m/det; nervus radialis 3,48±0,86 mdet; 1,00±0,33 mV; 59,34±10,01 m/det; nervus tibialis 4,47±1,15 mdet; 6,59±1,85 mV; 53,95±6,65 m/det; nervus peroneus 3,29±1,15 mdet; 2,25±0,84 mV; 54,67±8,25 m/det. Rerata hasil pemeriksaan Sensory Nerve Action Potential (SNAP) secara berurutan latensi, amplitudo, KHS nervus medianus 2,62±0,31 mdet; 12,15±5,24 µV; 65,16±10,29 m/det; nervus ulnaris 2,49±0,3 0 mdet; 13,88±7,21 µV; 63,44±9,79 mdet; nervus radialis 2,38±0,61 mdet; 11,37±5,58 µV; 70,40±10,33 m/det; nervus suralis 4,17±0,67 mdet; 6,73±2,98 µV; 59,06±9,12 m/det. Simpulan: Didapatkan nilai rata-rata normal KHS motorik dan sensorik di RSUP Sanglah Denpasar yang dapat digunakan sebagai nilai acuan.Background: Nerve conduction study is electrodiagnostic procedures important in the diagnosis of peripheral nervous system disease. Nerve conduction velocity (NCV) measurements can be affected by various physiological and non-physiological factors, so the reference normal values can be different in each place; therefore, each center is encouraged to have their own normal reference value. Currently in Sanglah General Hospital not have a reference normal values of NCV. Objective: To obtain normal mean value of nerve conduction velocity (NCV) in Sanglah General Hospital, Denpasar. Method: A cross-sectional study with consecutive sampling technique was done during July 2013. A sample of 30 eligible normal adult, 20-30 year-old, were examined in the right arm and right leg, assessed for latency, amplitude, and NCV. The data is analyzed with SPSS 16.Result: The mean of Compound Muscle Action Potential for latency, amplitude, NCV of median nerve is 2,95±0,34 msec; 4,33±1,54 mV; 67,16±6,73 m/sec; ulnar nerve 2,41±0,29 msec; 3,46±0,89 mV; 66,65±7,36 m/sec; radial nerve 3,48±0,86 msec; 1,00±0,33 mV; 59,34±10,01 m/sec; tibial nerve 4,47±1,15 msec; 6,59±1,85 mV; 53,95±6,65 m/sec; peroneal nerve 3,29±1,15 msec; 2,25±0,84 mV; 54,67±8,25 m/sec. The mean of Sensory Nerve Action Potential for latency, amplitude, NCV of median nerve is 2,62±0,31 msec; 12,15±5,24 µV; 65,16±10,29 m/sec; ulnar nerve 2,49±0,30 msec; 13,88±7,21 µV; 63,44±9,79 m/sec; radial nerve 2,38±0,61 msec; 11,37±5,58 µV; 70,40±10,33 m/sec; sural nerve 4,17±0,67 msec; 6,73±2,98 µV; 59,06±9,12 m/sec. Conclusion: Normal average value of motor and sensory NCV in Sanglah General Hospital Denpasar were obtained, and can be used as a reference.

Abstract

AbstrakGangguan kognitif sering dijumpai pada penderita gagal jantung sistolik dan tidak terdeteksi sejak awal. Hal ini memperburuk kondisi dan luaran klinis penderita. Penelitian ini bertujuan untuk mengetahui adanya peningkatan risiko terjadinya gangguan kognitif pada penderita gagal jantung sistolik dengan penurunan fraksi ejeksi ventrikel kiri dalam enam bulan. Metode penelitian ini adalah kontrol kasus, di Poli Jantung dan Poli Saraf RSUP Sanglah, Denpasar, periode Oktober 2018 hingga Desember 2018. Analisis data dilakukan dengan SPSS versi 22. Penelitian ini menggunakan 76 subjek, dengan rerata usia 50,3 ± 4,5 tahun. Karakteristik yang lebih dominan dijumpai pada jenis kelamin laki-laki 44 orang (57,9%), pendidikan ≥ 12 tahun 40 orang (52,6%), dan pekerjaan formal 40 orang (52,6%) Dari hasil analisis statistik didapatkan persentase penurunan fungsi kognitif pada kelompok kasus sebesar 89,5% sedangkan pada kelompok kontrol didapatkan 13,2%. Analisis bivariat dengan Chi-square, didapatkan nilai OR=56,1; IK 95% 13,844-227,338; p<0,001. Pada analisis multivariat didapatkan penurunan FEVK dalam enam bulan merupakan faktor risiko yang kuat, memiliki risiko mengalami gangguan kognitif pada penderita gagal jantung sistolik dengan adjusted OR=23,1, setelah mengendalikan jenis kelamin, pendidikan dan pekerjaan, OR=23,1; IK 95% 4,7-114,03; p<0,001. Penelitian ini menyimpulkan bahwa penurunan FEVK dalam enam bulan sebagai faktor risiko gangguan kognitif penderita gagal jantung sistolik. Penulis menyarankan para klinisi perlu melakukan pemeriksaan fungsi kognitif pada penderita gagal jantung sistolik .Kata kunci: gangguan kognitif, penurunan FEVK, gagal jantung sistolik
