Abstract

Abstract

Background: Painful diabetic neuropathy (PDN) is a severe complication of type 2 diabetes mellitus (T2DM). Nerve Growth Factor (NGF) is vital for neuronal health, but its status as a systemic biomarker in PDN is contested due to conflicting reports on serum levels. This study aimed to quantitatively synthesize the literature on the association between serum NGF concentrations and the presence of PDN in patients with T2DM. Methods: PubMed, Scopus, Web of Science, and Embase were searched for observational studies from January 2015 to August 2025 that measured serum NGF in T2DM patients with PDN versus control groups (T2DM without PDN or healthy individuals). Data were independently extracted by two reviewers. A random-effects model was used to calculate the pooled Standardized Mean Difference (SMD) and 95% confidence intervals (CIs) to account for assay variability. Heterogeneity was explored using the I² statistic and meta-regression. Results: From 1,482 records, seven cross-sectional studies (485 PDN patients, 511 controls) were included. The meta-analysis revealed that patients with PDN had significantly lower serum NGF concentrations compared to controls, with a pooled SMD of -1.28 (95% CI: -1.79 to -0.77, p < 0.00001). Substantial heterogeneity was present (I² = 84%). Meta-regression showed that a longer duration of diabetes was significantly associated with a greater reduction in NGF levels (p = 0.02). No significant publication bias was detected. Conclusion: This meta-analysis provides consolidated evidence that lower systemic NGF levels are a strong feature of established PDN. The findings support the neurotrophic deficit hypothesis in PDN pathophysiology and identify NGF as a candidate biomarker requiring rigorous validation in longitudinal studies that carefully differentiate painful from painless neuropathy phenotypes.

Abstract

The aging process in the elderly affects not only physical capabilities but also cognitive function. Cognitive impairment in older adults is thought to be associated with physical activity and neurotrophic factors, particularly the serum level of Brain-Derived Neurotrophic Factor (BDNF). Majejaitan—a traditional form of activity involving fine motor coordination and repetitive hand movements—is hypothesized to exert a protective effect against cognitive impairment by increasing serum BDNF levels. This study aimed to examine the role of Majejaitan activity and elevated serum BDNF levels as potential protective factors against cognitive impairment in elderly individuals. An observational case-control study was conducted from November 2024 to January 2025 at the East Denpasar 1 Primary Health Care Center. The participants were women aged 50–60 years, divided equally into two groups: a case group (with cognitive impairment) and a control group (without cognitive impairment), each consisting of 19 participants. Cognitive impairment was assessed using the Indonesian version of the Montreal Cognitive Assessment (MoCA-Ina), with scores below 26 indicating impairment. Majejaitan activity was defined as performing the activity at least three times per week for 30–60 minutes, and participants were classified as either active or inactive. Serum BDNF levels were measured using a sandwich-type ELISA with the Human BDNF ELISA Kit (BT LAB) and were categorized as high or not high based on receiver operating characteristic (ROC) curve analysis.

Abstract

Background: Neuropathic pain (NP) is a severe, chronic complication of Hansen's disease (HD), persisting after antimicrobial therapy and profoundly diminishing quality of life. Its pathophysiology is driven by persistent, complex neuroinflammatory processes within the peripheral nervous system. Circulating biomarkers, especially the glial-derived protein S100B, offer a potential objective window into this underlying pathology. This study aimed to meta-analyze the association between circulating S100B, TNF-α, and IL-6 and the presence of NP in patients with HD. Methods: A systematic search of PubMed, Scopus, and Web of Science databases was conducted for observational studies published between January 2015 and December 2025 that compared biomarker levels in HD patients with and without NP, diagnosed using validated screening instruments. Data from eligible studies were extracted independently, and methodological quality was assessed using the Newcastle-Ottawa Scale. A random-effects meta-analysis was performed to compute the pooled standardized mean difference (SMD) with 95% confidence intervals (CIs) for each biomarker. Results: Seven studies, comprising 812 patients (405 with NP, 407 without NP), met the inclusion criteria. The meta-analysis revealed that serum S100B levels were significantly elevated in HD patients with NP compared to those without (SMD = 1.28, 95% CI [0.95, 1.61], p < 0.001). This finding was accompanied by very high statistical heterogeneity (I² = 78%). Concurrently, the analysis demonstrated significantly higher circulating levels of TNF-α (SMD = 0.89, 95% CI [0.62, 1.16]) and IL-6 (SMD = 0.75, 95% CI [0.48, 1.02]) in the NP group. Conclusion: This meta-analysis establishes a strong statistical association between a distinct neuroinflammatory biomarker profile—characterized by elevated circulating S100B, TNF-α, and IL-6—and the presence of neuropathic pain in Hansen's disease. S100B, as a marker of Schwann cell distress, is a particularly relevant component of this profile. These findings underscore the pivotal role of neuroinflammation in HD-related NP, although the high heterogeneity and non-specific nature of these systemic markers necessitate a cautious interpretation regarding their immediate clinical applicability.

Abstract

Cognitive dysfunction is a common problem associated with ageing and affects quality of life. As a repetitive activity, weaving and brain-derived neurotrophic factor (BDNF) levels are believed to be essential in maintaining cognitive function. This study aims to prove the relationship between weaving activity, BDNF levels, and cognitive dysfunction. This study is an observational study with a case-control design. The study subjects were middle-aged women (45–59 years) divided into two groups: CD (cases) and non-CD (controls). CD status was determined using the Mini-Mental State Examination (MMSE) score. Weaving activity was assessed through a questionnaire, with the criterion for active weaving being at least 3–5 times per week (minimum 60 minutes per session), categorised as weaving (+) and non-weaving (–). Plasma BDNF levels were measured using an ELISA kit and categorised as high BDNF (+) and low BDNF (–) based on the mean value. The researchers performed statistical analysis using the Chi-Square test and logistic regression. The Chi-Square test showed a significant association between weaving activity as a protective factor against CD occurrence (p < 0.001; OR: 0.038) and high BDNF levels as a protective factor against CD occurrence (p < 0.001; OR: 0.013). Multivariate analysis using logistic regression indicated that weaving activity (p = 0.001; AOR: 0.028; CI: 0.003–0.247) and high BDNF levels (p < 0.001; AOR: 0.011; CI: 0.001–0.100) were independent protective factors against CD occurrence. This study indicates that weaving activity and high plasma BDNF levels are independent protective factors against CD.

Abstract

Background: Breast cancer remains one of the most prevalent and lethal malignancies among women globally, which leads to the critical importance of early detection and a more comprehensive approach to breast cancer management. Purpose: To investigate HER2 positivity as a predictor of brain metastases in breast cancer. Methods: A total of 23 peer-reviewed studies published between 2015 and 2025 were analyzed, including randomized trials, retrospective cohorts, prediction model studies, and registry-based evaluations. Eligibility was guided by the PICOTS framework, and databases searched included PubMed, Scopus, MDPI, Elsevier, SagePub, and BMC Cancer. Results: The review confirms that HER2 overexpression significantly correlates with earlier brain metastasis onset, increased incidence, and distinctive treatment response profiles. HER2 status also influences survival outcomes and benefits from targeted agents such as trastuzumab, tucatinib, and pyrotinib. HER2-low expression additionally showed intermediate risk profiles, while imaging and circulating tumor cell data added prognostic value. Conclusion: HER2 positivity is a robust, independent predictor of brain metastasis in breast cancer patients. Routine integration of HER2 profiling in CNS risk assessment models may improve early detection and therapeutic precision, ultimately enhancing clinical outcomes for high-risk populations.
