Abstract

Background: Alzheimer`s disease (AD) is a kind of dementia that causes impair of memory, intuition, and social life in elderly particularly. This study aims to know the public awareness, attitude, and knowledge of AD in Denpasar, BaliMethods: A cross-sectional study was conducted among 70 participants at Sanglah General Hospital by using self-administered questionnaire during April-May 2018 period. Subjects selection using simple random sampling method. Statistical analysis was carried out by using SPSS ver. 17 to know the percentage or proportion regarding with attitude and knowledge of AD.Results: About 39% of participants have a good knowledge, followed by 20% poor knowledge, and 41% don’t know about AD. The study found that 50% of participants have a good attitude, 25% poor attitude, and don’t have attitude regarding with AD. Data comparison showed that students scored higher for attitude than knowledge about 11%.Conclusion: Most of the participants at Sanglah General Hospital have a poor knowledge and attitude regarding with HD

Abstract

Post-herpetic neuralgia (PHN) is chronic neuropathic pain that is felt for at least 3 months or more at the site of the rash due to herpes zoster infection. Epidemiological studies get a prevalence of 0.49 cases of PHN per 1000 people per year. The clinical manifestation of PHN is chronic pain on the skin lesions due to herpes zoster infection. The therapy that is commonly used as the first line is the provision of pharmacotherapy such as gabapentin or pregabalin. An 83-year-old male patient with a diagnosis of PHN that had been felt for 5 years. He has been taking drugs for 5 years but the pain has not improved. The patient is given high-intensity laser therapy. After 5 treatments with an interval of 1-2 weeks, pain was significantly reduced from an 8/10 scale to 4/10. The frequency of occurrence of severe pain is also reduced from 5 times per day to only 1 time per day. The use of high-intensity laser therapy is not commonly used as a therapy for PHN. Based on its mechanism of action, the use of high-intensity laser therapy is worth considering because of its effectiveness in treating pain in a shorter time than low-intensity laser therapy.

Abstract

Introduction: A subsequent headache within seven days of a head injury (or after regaining consciousness after the head trauma)is referred to as a post-traumatic headache (PTHA); it is referred to as chronic or chronic post-traumatic headache (CPTHA) if it lasts longer than three months after the injury. Case : A 17-year-old male with headache since 3 months ago, 3 days after suffered a blow to his left head from falling from a chair. At that time, the patient fainted for about 15 minutes but had no complaints after regained consciousness. Pain is felt on the left side of the head, throbbing, mild-moderate intensity, and feels heavier with a loud sound or a too-bright light. Discussion: Post-traumatic headache is clinically diagnosed. Laboratory and routine diagnostic imaging studies are unnecessary and have minimal clinical utility. Conclusion: Chronic post-traumatic headaches often occur, especially after minimally traumatic brain injury. The clinical picture is variable and may be similar to tension-type headaches and/or migraines.   Pendahuluan: Nyeri kepala dalam tujuh hari setelah cedera kepala atau setelah sadar kembali dari trauma kepala disebut nyeri kepala pasca-trauma (post-traumatic headache/PTHA); disebut sakit kepala pasca-trauma kronis atau kronis (CPTHA) jika berlangsung lebih dari tiga bulan setelah cedera. Kasus: Seorang laki-laki berusia 17 tahun dengan keluhan nyeri kepala sejak 3 bulan, 3 hari setelah kepala kiri terbentur karena jatuh dari kursi. Saat itu, pasien pingsan sekitar 15 menit, tidak ada keluhan setelah sadar. Nyeri dirasakan di sisi kiri kepala, berdenyut, intensitas ringan-sedang, terasa lebih berat jika ada suara keras atau cahaya terlalu terang. Diskusi: Nyeri kepala pasca-trauma didiagnosis secara klinis. Laboratorium dan studi pencitraan diagnostik rutin tidak diperlukan dan memiliki utilitas klinis minimal. Simpulan: Nyeri kepala pasca-trauma kronis sering terjadi, terutama setelah cedera otak traumatis minimal. Gambaran klinisnya bervariasi dan dapat mirip nyeri kepala tipe tegang dan/atau migrain

Abstract

The headache has been felt since 3 months ago, 3 days after the patient suffered a blow to his left head from falling from a chair. Pain is felt on the left side of the head, throbbing, mild-moderate intensity, and feels heavier when there is a loud sound or seeing a too-bright light. Discussion: Like other primary headaches, post-traumatic headache is diagnosed clinically. Laboratory and routine diagnostic imaging studies are unnecessary and have minimal clinical utility. Brain MRI did not reveal any structural changes in patients with PTHA following injury. Conclusion: Chronic post-traumatic headaches often occur after brain injury, especially after minimally traumatic brain injury. The clinical picture is variable and may be similar to tension-type headaches and/or migraines.

Abstract

Background: Segmental zoster paresis (SZP) is a very uncommon complication characterized by localized weakening of the upper or lower limbs. Case report: A 69-year-old man came with complaints of weakness in the right leg two months ago. He had weakness in his right leg after two days of having herpes on his right leg. There was paresthesia and hypesthesia on neurological examination according to the L1-L3 dermatome. Neurophysiology examination revealed a neuropathy in the right femoral nerve. Discussion: Varicella-zoster virus, being a neurotrophic virus, primarily invades and incubates in the dorsal root or cerebral ganglion nerve cells. Etiological investigations in SZP patients have revealed that the anterior root is the most prevalent location of inflammation and degeneration. Conclusion: SZP is a rare complication following VZV infection that can lead to misdiagnosis and overtreatment in clinical practice. The neurophysiological examination is very important in diagnosing segmental zoster paresis.

Abstract

    DIFFERENCE IN OUTCOME OF ACUTE ISCHEMIC STROKE PATIENT WITH NORMAL LEUKOCYTE AND LEUKOCYTOSISABSTRACTIntroduction: Inflammation is an independent risk factor for ischemic stroke, which needs to be inhibited to reduce worsening occlusion of arteries due to atherosclerotic plaques. Increased leukocyte count is considered an acute damage marker of brain tissue that experiences ischemia and is used to determine the severity and prognosis of acute ischemic stroke.Aims: To analyse association between the number of leukocytes in acute onset ischemic stroke patients during admission and discharge.Methods: A prospective analytical observational study with a cohort design on acute ischemic stroke patients in Wangaya Regional Hospital, Denpasar-Bali between January and September 2018. Clinical manifestations were assessed using National Institutes of Health Stroke Scale (NIHSS). Data was collected through medical records and analysed using Chi-square test and Mann-Whitney.Results: Seventy-six subjects devided into 2 groups;  normal  leucocytes and  leucocytosis group,  38 patients respectively. The normal leucocytes group was dominated with male subject (78.9%) and mean leucocytes count was 7,612/mm3. While leucocytosis group was dominated with female subjects (63.2%) and mean leucocytes count was 12,294/ mm3. Mean NIHSS at admission in normal leucocytes group and leucocytosis group was 5 (moderate) and 8 (moderate) respectively. Mean NIHSS at discharge in normal leucocytes group and leucocytosis group was 3 (low) and 11 (moderate) respectively.Discussion: Patients with acute onset ischemic stroke with normal leukocyte count during admission, tend to have better clinical outcome two times fold compared to the patients with leucocytosis.Keywords: Leukocytes, leucocytosis, acute ischemic stroke, NIHSSABSTRAKPendahuluan: Inflamasi merupakan faktor risiko independen terjadinya stroke iskemik, yang perlu dihambat untuk mengurangi perburukan oklusi pada pembuluh darah akibat plak aterosklerotik. Peningkatan kadar leukosit dianggap menunjukkan kerusakan akut jaringan otak yang mengalami iskemia dan digunakan untuk mengetahui tingkat keparahan dan prognosis stroke iskemik akut.Tujuan Mengetahui hubungan antara kadar leukosit terhadap manifestasi klinis pasien stroke iskemik onset akut pada saat masuk dan keluar perawatan.Metode: Penelitian observasional analitik prospektif dengan rancangan kohort terhadap pasien stroke iskemik onset akut yang dirawat di RSUD Wangaya, Denpasar, pada bulan Januari hingga September 2018. Manifestasi klinis subjek dinilai berdasarkan derajat beratnya stroke menggunakan National Institutes of Health Stroke Scale (NIHSS). Data dikumpulkan melalui rekam medis dan di analisis dengan uji Chi-square dan Mann-Whitney.Hasil: Didapatkan 76 subjek yang dibagi menjadi 2 kelompok, yaitu masing-masing 38 subjek pada kelompok dengan leukosit normal dan kelompok dengan leukositosis. Kelompok subjek leukosit normal, didominasi oleh subjek laki- laki (78,9%) dan rerata kadar leukosit 7.612/mm3, sedangkan pada kelompok leukositosis mayoritas perempuan (63,2%) dan rerata kadar leukosit 12.294/mm3. Rerata skor NIHSS saat masuk pada kelompok leukosit normal adalah 5 (moderat) dan  pada kelompok dengan leukositosis adalah 8 (moderat). Adapun rerata skor NIHSS pada kelompok leukosit normal saat pasien keluar adalah 3 (ringan) dan 11 (moderat) pada kelompok leukositosis.Diskusi: Subjek stroke iskemik akut dengan kadar leukosit awal yang normal cenderung memiliki perbaikan manifestasi klinis pada saat keluar RS dua kali lebih baik daripada subjek dengan leukositosis.Kata kunci: Leukosit, leukositosis, stroke iskemik akut, skor NIHSS

Abstract

HIGH SUBSTANCE P SERUM LEVELS AS RISK FACTOR FOR CHRONIC TENSION TYPE HEADACHE AMONG MEDICAL STUDENT FACULTY OF MEDICINE UDAYANA UNIVERSITYABSTRACTIntroduction: Several studies revealed that serum substance P (SP) levels increase in people with headache, particularly chronic tension-type headache (CTTH). One of the examples of population at risk of CTTH is medical students because of their stressful academic load. Aim: To determine whether high serumSP levels are the risk factors of CTTH among medical students.Methods: A case-control study was conducted to medical students, aged 19 to 21, of the Faculty of Medicine, Udayana University from December 2018 to January 2019. Subjects were classified into case group (with CTTH) and control group (without CTTH). SP serum levels were measured on both groups. Analyses included univariate, bivariate with Fisher's test to calculate relative risk (RR), and multivariate logistic regression analysis.Results: Sixty-two subjects, predominantly female (51.6%), were involved in this study. In the case group, most of the subjects had mild depression (71%), mild stress (93.5%), pericranial tenderness (61,3%), and elevated SP serum levels (83.9%). From the multivariate analysis, a high level of SP serum (≥ 349,06ng/L) was the risk factor of CTTH (adjusted Odds ratio 9.9; 95% CI 1.06-93.16). Besides that, other factors, for example, mild stress, mild anxiety, mild depression, pericranial tenderness, and sleep disturbances, were related to the prevalence of CTTH(p=0.001).Discussion: This study concluded that high serum SP levels significantly increased the risk of CTTH occurrence among medical students.Keywords: Chronic tension-type headache (CTTH), medical student, SP serum levelsABSTRAKPendahuluan: Beberapa penelitian menunjukkan kadar substasi P (SP) serum meningkat pada penderita chronic tension type headache (CTTH). Salah satu populasi yang rentan mengalami CTTH adalah mahasiswa kedokteran karena beban akademik relatif tinggi yang diduga berkaitan dengan stres psikologis.Tujuan: Mengetahui apakah kadar SP serum merupakan salah satu faktor risiko CTTH pada mahasiwa kedokteran.Metode: Studi kasus kontrol dilakukan terhadap mahasiswa Fakultas Kedokteran Universitas Udayana berusia 19-21 tahun pada bulan Desember 2018 hingga Januari 2019. Subjek dikelompokkan ke dalam kelompok kasus (dengan CTTH) dan kontrol (tanpa CTTH), kemudian seluruhnya diperiksakan kadar SP serum. Analisis data dilakukan secara univariat, bivariat dengan uji Fisher untuk menghitung risiko relatif (RR), dan analisis regresi logistik multivariat.Hasil: Sebanyak 62 subjek yang mayoritas perempuan (51,6%) ikut serta dalam studi ini. Pada Pada kelompok kasus, sebagian besar mengalami depresi ringan (71%), stres ringan (93,5%), nyeri tekan perikranial (61,3%), dan memiliki kadar SP serum yang tinggi (83,9%). Berdasarkan analisis multivariat, kadar SP serum tinggi (≥ 349,06ng/L) merupakan faktor risiko CTTH (adjusted rasio Odds 9,9; IK 95% 1,06-93,16). Abstract

Introduction : Leprosy is one of the most common causes of non-traumatic peripheral neuropathy in the world. There are many factors causing neuropathic pain in multibacillary (MB) type leprosy, one of which is an increase in serum levels of glial fibrillary acidic protein (GFAP). Aim: To prove high serum GFAP levels is a risk factor for neuropathic pain in MB type leprosy in Sanglah General Hospital Denpasar. Methods: This study was an observational analytic study with a case control design conducted at the Dermato-Venerology Department outpatient and in Pathological Clinic Laboratory in Sanglah General Hospital from November 2019 to January 2020. The sample of this study is all multibacillary leprosy patients with neuropathic pain who met inclusion criteria. A total of 60 patient were willing to participate in this study, the pain scales were measured using DN4 questionnaire and GFAP serum levels were examined. Chi-square test was performed to prove high serum GFAP levels is a risk factor of neuropathic pain in multibacillary leprosy. Results: High serum GFAP levels increase the incidence of neuropathic pain of MB type leprosy 5 times higher than controls (OR 5.72; p = 0.003), anxiety (OR 2.1; p = 0.24), and leprosy reaction (OR 2.2; p = 0.052). Discussion: High serum levels of GFAP is a risk factor of neuropathic pain in MB type leprosy in Sanglah General Hospital Denpasar. Keywords: GFAP, MB type leprosy, neuropathic pain

Abstract

Tidur dianggap sebagai kebutuhan manusia yang berupa istirahat secara alami yang untuk kesehatan tubuh. Kualitas tidur yang buruk memengaruhi fungsi kognitif, kesehatan fisik, penurunan konsentrasi, serta berbagai masalah lainnya. Kualitas tidur buruk banyak ditemukan pada mahasiswa, disebabkan oleh berbagai hal yang mampu mengganggu kualitas tidur. Penelitian ini tergolong sebagai penelitian deskriptif analitik yang bertujuan agar mengetahui proporsi dan perbedaan karakteristik kualitas tidur mahasiswa Program Studi Sarjana Kedokteran berdasarkan pendekatan cross sectional. Sampel melibatkan mahasiswa Program Studi Sarjana Kedokteran. Kualitas tidur dinilai dengan kuesioner Pittsburgh Sleep Quality Index (PSQI), serta kuesioner berbagai faktor yang memengaruhi kualitas tidur. Data diolah serta dianalisis secara univariat supaya mengetahui distribusi frekuensi, serta analisis chi square untuk melihat perbedaan secara statistik. Total skor PSQI sebanyak 97 sampel dalam rentang 2-16 dengan rata-rata 9,35 yang menunjukkan kualitas tidur buruk. Uji statistik didapatkan bahwa perbedaan kualitas tidur yang signifikan terjadi pada pernyataan olahraga secara teratur (p=0,018), dapat tidur nyenyak walaupun sedang menghadapi masalah (p = 0,008), mengonsumsi kopi sebelum tidur (p=0,015), merokok sebelum tidur (p = 0,001), minum banyak air sebelum tidur (p = 0,030), dan mengonsumsi susu sebelum tidur (p = 0,029). Mahasiswa Program Studi Sarjana Kedokteran memiliki kualitas tidur yang buruk, sehingga ditemukan perbedaan kualitas tidur yang signifikan secara statistik di pernyataan-pernyataan tersebut.

Abstract

The use of social media does have many advantages. But there are some bad effects that accompany it, one of which is sleep quality disturbances. A study shows that use of social media at night affects the quality of sleep worse, causing anxiety disorders, depression and self-esteem. Diagnosis of sleep disorders in adolescents is difficult because it is not often complained about by adolescents. In addition, when adolescents sleep disorders are no longer a concern for parents. In a study conducted by Haryono on junior high school students with 140 respondents, it was found that 63.6% experienced sleep disorders. This study aims to prove that there is a relationship between social media use and adolescent sleep quality disorders at SMP Negeri 1 Mengwi. This study used a cross sectional approach in which the independent and dependent variables were collected at one time and at the same time. The sample in this study amounted to 79 respondents who met the inclusion and exclusion criteria. From the results of the Correlation analysis test, it got a p value <0.05 with a correlation coefficient of 0.633 which indicates a strong positive relationship between the duration of social media use and the quality of sleep of the respondents. Based on the results of the research and discussion carried out, it can be concluded that there is a significant relationship between the duration of social media use and sleep quality in adolescents at SMP Negeri 1 Mengwi.
