Abstract

Prestasi belajar adalah suatu penguasaan atau keterampilan dan dikembangkan oleh mata pelajaran, biasanya dilihat dalam hasil tes atau nilai. Selain itu, terdapat faktor internal dan eksternal yang berpengaruh terhadap prestasi belajar seorang mahasiswa. Seorang mahasiswa tentunya tidak terlepas dari permasalahan yang terjadi dalam kehidupan. Permasalahan tersebut juga memiliki dampak pada kualitas hidup. Seseorang yang memiliki pandangan psikologis, kesejahteraan emosional, kesehatan fisik, dan kesehatan mental yang baik, dikatakan memiliki kualitas hidup yang positif. Terjadinya penurunan prestasi belajar dan gangguan kualitas hidup mahasiswa disebabkan oleh berbagai faktor, salah satunya gangguan kesehatan fisik seperti nyeri kepala. Nyeri kepala merupakan keluhan yang umum terjadi pada hampir semua orang. Ada tiga kategori utama nyeri kepala, salah satunya nyeri kepala primer, dengan salah satu klasifikasinya yaitu nyeri kepala tipe tegang yang memiliki prevalensi tertinggi. Penelitian ini bertujuan mengetahui perbedaan prestasi belajar dan kualitas hidup dengan kejadian nyeri kepala tipe tegang episodik infrekuen di kalangan mahasiswa dan mahasiswi Fakultas Kedokteran Universitas Udayana. Penelitian dilakukan dengan metode analitik menggunakan studi potong lintang. Data diperoleh menggunakan kuesioner nyeri kepala, prestasi belajar, dan SF-36, dibagikan secara daring dengan 100 responden. Data dianalisis menggunakan IBM SPSS Statistics 25. Berdasarkan analisis, didapatkan hasil p>0,05, yang tidak signifikan pada prestasi belajar dan p<0,05, yang signifikan pada kualitas hidup dengan kejadian nyeri kepala. Disimpulkan bahwa tidak ada perbedaan rerata yang bermakna pada prestasi belajar namun signifikan pada kualitas hidup terhadap kejadian ada atau tidaknya nyeri kepala tipe tegang episodik infrekuen pada mahasiswa.

Abstract

Myogenic low back pain is one type of low back pain that is common but often undiagnosed. Medical students are packed with lecture and study activities so they are more at risk of experiencing low back pain. This study is to determine the proportion, characteristics of sufferers and characteristics of myogenic low back pain in medical students at Udayana University class of 2020. This study is a descriptive cross-sectional study with a sample size of 90 people who meet the inclusion criteria. Sampling using consecutive sampling method. Data collection by filling out an online questionnaire via Google Form and checking the presence of MPS through palpation by a neurology resident. Data analysis was performed using SPSS version 25. The proportion of myogenic low back pain sufferers in medical student class of 2020, Udayana University was 26.7%. The characteristics of the majority of sufferers were 20 years old (41.7%), female (70.8%), normal BMI (91.7%), occasional exercise (62.5%), sitting duration ?4 hours (95.8%), and hunched sitting position (75%). The characteristics of myogenic low back pain were mostly acute (62.5%), pain was felt gradually (62.5%), and the pain scale was mild (54.2%). Respondents mostly chose non-pharmacological therapy by lying down or stretching (95.8%). Out of the 90 respondents who were included in the inclusion criteria in this study, there were 24 (26.7%) respondents who experienced myogenic low back pain with different characteristics from previous studies. Further research with a larger population and causal relationship studies needs to be done to find out the exact cause of myogenic low back pain and how to avoid it.

Abstract

Diabetes melitus (DM) merupakan salah satu penyakit metabolik menahun yang paling sering di dunia, dengan tren yang semakin meningkat dan juga berdampak pada masyarakat di Indonesia. Komplikasi dari penyakit DM dapat berdampak pada angka morbiditas dan mortalitas yang tinggi. Penyakit DM memiliki komplikasi yang berdampak ke sistem saraf seperti stroke, gangguan fungsi kognitif, gangguan tidur, serta nyeri neuropati diabetik (NND). Komplikasi diabetes sering tidak diketahui oleh masyarakat. Kurangnya pemahaman masyarakat indonesia terhadap DM, komplikasi dan upaya pencegahan menjadi salah satu kendala yang masih terjadi. Komplikasi ini semakin meningkat risikonya pada kelompok usia lanjut (lansia) dan menganggu kualitas hidup hingga segi ekonomi di masyarakat. Peran fase kesehatan pertama yaitu Puskesmas sangat berperan penting dalam upaya pencegahan penyakit diabetes, seperti deteksi dini sehingga dapat pencegahan dan penanganan kepada pasien diabetes. Tujuan dari kegiatan ini yaitu untuk meningkatkan pengetahuan masyarakat tentang komplikasi DM pada sistem saraf di area kerja Puskesmas I Denpasar Selatan. Metode kegiatan yang akan dilakukan yaitu ceramah, diskusi, hand-on dalam penegakan diagnosis dan kuisioner pre dan post-test terselenggara pada tanggal 30 Oktober 2022 di Banjar Pegok, Denpasar. Hasil kuisioner pre dan post-test didapatkan adanya peningkatan nilai pengetahuan peserta setelah dilakukan pelatihan. Simpulan dan implikasi yang didapatkan yaitu diharapkan setelah adanya pelatihan ini, para masyarakat di Puskesmas I Denpasar Selatan dapat lebih memahami tentang penyakit DM dan komplikasinya terhadap sistem saraf.

Abstract

Introduction: Vascular parkinsonism has a prevalence 4.4% to 12% of parkinsonism. Until now there are no specific diagnostic criteria to establish diagnosis of vascular parkinsonism, so it is often misdiagnosed with idiopathic parkinsonism. Case Report: Male, 82 years old, with gait disorder 3 years ago, appeared stiff with small steps. The patient appears to be slower, drooling frequently, and has difficulty swallowing food. The patient was diagnosed with Parkinson's disease and given levodopa/benserazide. However, the progress of the disease was getting worse, so the dose was increased. Since 2 years, patient became forgetful and more irritable. History of ischemic stroke 3 years ago with no sequelae. History of hypertension and gout arthritis (GA). Patient was compos mentis without signs of focal deficit or lateralization, however, there was a cogwheel-type rigidity in all extremities without any tremor. MoCA-Ina (Montreal Cognitive Assesment Indonesian Version) test was 21. The head CT (Computer Tomography) scan revealed chronic lacunar infarct with brain atrophy and small vessel ischemic changes. Then patient was diagnosed with vascular parkinsonism and treated with levodopa, acetosal, candesartan and consulted to medical rehabilitation department. Discussion: Based on criteria of Zijlmans et al., and Winikates and Jankovic, patient is included in vascular parkinsonism, namely the presence of bradykinesia, rigidity and postural instability which indicates a parkinsonism accompanied by 3 vascular scores. Conclusion: Diagnosis of vascular parkinsonism is very complex and challenging. Multidisciplinary management from neurologists and medical rehabilitation specialists must be applied to improve the quality of life, reduce morbidity and mortality.

Abstract

Background: Uremic syndrome in chronic kidney disease (CKD), particularly in hemodialysis (HD) patients, is a growing global health problem. The majority of patients with this condition experience one or more neurological disorders, such as neuropathic pain. One of the adjuvant therapies in the treatment of neuropathic pain is vitamin B12, which is important for the regeneration and myelination of peripheral nerves and also plays an important role in the restoration of functional nerves. Objective: The aim of this research is to evaluate the role of vitamin B12 in the relief of pain symptoms Method: This research was conducted at General Hospital in Bali use a case-control design and consecutive sampling. This was carried use 21 patients with CKD on regular hemodialysis with neuropathic pain, while 22 patients without neuropathic pain were used as the control group. All patients were tested for serum vitamin B12 levels. Neuropathic pain was assessed using the DN4. Results: The data values of total vitamin B12 levels in blood serum were obtained within a range of 81.72 pmol/L to 2337.5 pmol/L, and a median value of 1306.25 pmol/L. A cut-off value of 1509.375 pmol/L was obtained with 76.2% sensitivity and 59.1% specificity. Furthermore, an odd ratio between low serum vitamin B12 and the incidence of neuropathic pain was also obtained, namely OR= 3.611 (95% CI= 1.012-12.9; p=0.044). Conclusion: It was found that low serum vitamin B12 in CKD patients on hemodialysis increased the risk of experiencing neuropathic pain by 3.611 times compared to those with normal serum vitamin B12.

Abstract

Background: One of the nonpharmacological management that can currently be used to treat Myofascial Pain Syndrome  (MPS) is dry needling (DN). Dry needling is a pain intervention technique by inserting a solid needle into the skin to stimulate myofascial trigger points (TrPs). In a previous study conducted at Central Hospital in Bali, DN was shown to reduce pain and oral analgetic usage in patients with upper trapezius muscle MTrPs, but the effect of DN on overall life quality (such as sleep quality and depression) is still lacking. Objective: This research was conducted to find out whether there was an improvement in pain scale, depression and quality of sleep of health employees in Sanglah Hospital who experienced myofascial pain syndrome after dry needling therapy. Methods: an observational experimental design, which compares the difference of the Numerical Pain rating score, Pittsburg Sleep Quality Index, and Hamilton Depression Scale before and after DN. The study used a consecutive sampling method to reach the designated sample size. Results: A total of 53 samples followed the entire series of studies. The mean difference between NPRS before DN and after DN showed a significant result of p= 0.00 (<0.05), but in the aspect of PSQI and HDS no statistical difference. Conclusion: There is a trend of improvement in the quality of life after DN from pain scale, mood condition and sleep quality. However, this research still cannot show a link between DN therapy with depressive disorders and sleep quality.

Abstract

Background: Study on the correlation of sleep quality with short-term memory function in medical students has a high urgency considering that intelligence and good memory function are essential for their performance and learning ability in the field. There are only a few related studies on medical students in the Faculty of Medicine and Health Sciences of Universitas Udayana. Objective: This study aims to determine the correlation between sleep quality and short-term memory function of medical students in the Faculty of Medicine and Health Sciences of Universitas Udayana. Methods: The sample was determined by the non-probability consecutive sampling method. The Indonesian version of the Pittsburgh Sleep Quality Index (PSQI) questionnaire and the Digit Span Backwards Task assessed sleep quality and short-term memory function respectively. The data was analyzed by Pearson correlative test and logistic regression multivariate analysis. Results: There were 75 subjects in this study. The PSQI average score of the respondents was 9.35 ± 4.45, and the Digit Span Backwards Task average was 3.75 ± 1.77. The Pearson correlative test showed a moderate correlation between sleep quality and short-term memory function (r = -0.44.; p < 0,05). The logistic regression multivariate analysis found that sleep disturbances affect short-term memory function more than other sleep quality components, with a probability value of 98%. Conclusion: There is a correlation between sleep quality and short-term memory function of medical students in the Faculty of Medicine and Health Sciences of Universitas Udayana.

Abstract

Introduction: A subsequent headache within seven days of a head injury (or after regaining consciousness after the head trauma)is referred to as a post-traumatic headache (PTHA); it is referred to as chronic or chronic post-traumatic headache (CPTHA) if it lasts longer than three months after the injury. Case : A 17-year-old male with headache since 3 months ago, 3 days after suffered a blow to his left head from falling from a chair. At that time, the patient fainted for about 15 minutes but had no complaints after regained consciousness. Pain is felt on the left side of the head, throbbing, mild-moderate intensity, and feels heavier with a loud sound or a too-bright light. Discussion: Post-traumatic headache is clinically diagnosed. Laboratory and routine diagnostic imaging studies are unnecessary and have minimal clinical utility. Conclusion: Chronic post-traumatic headaches often occur, especially after minimally traumatic brain injury. The clinical picture is variable and may be similar to tension-type headaches and/or migraines.   Pendahuluan: Nyeri kepala dalam tujuh hari setelah cedera kepala atau setelah sadar kembali dari trauma kepala disebut nyeri kepala pasca-trauma (post-traumatic headache/PTHA); disebut sakit kepala pasca-trauma kronis atau kronis (CPTHA) jika berlangsung lebih dari tiga bulan setelah cedera. Kasus: Seorang laki-laki berusia 17 tahun dengan keluhan nyeri kepala sejak 3 bulan, 3 hari setelah kepala kiri terbentur karena jatuh dari kursi. Saat itu, pasien pingsan sekitar 15 menit, tidak ada keluhan setelah sadar. Nyeri dirasakan di sisi kiri kepala, berdenyut, intensitas ringan-sedang, terasa lebih berat jika ada suara keras atau cahaya terlalu terang. Diskusi: Nyeri kepala pasca-trauma didiagnosis secara klinis. Laboratorium dan studi pencitraan diagnostik rutin tidak diperlukan dan memiliki utilitas klinis minimal. Simpulan: Nyeri kepala pasca-trauma kronis sering terjadi, terutama setelah cedera otak traumatis minimal. Gambaran klinisnya bervariasi dan dapat mirip nyeri kepala tipe tegang dan/atau migrain

Abstract

Introduction: The Original Intracerebral Hemorrhage (oICH) score has been widely used as a clinical assessment scale that is consistent and reliable for predicting patient mortality. However, the ICH score is sometimes complicated, does not contain an acute complication component of stroke, and is not developed to predict the mortality of patients undergoing surgery. We intended to create a new ICH (nICH) and ICH surgical evacuation (ICH-SE) score to predict 30-day mortality in spontaneous ICH patients. Methods: We conducted a retrospective cohort study involving all spontaneous ICH patients who were hospitalized at Prof. Dr. I.G.N.G. Ngoerah Denpasar between July 2020 and July 2022. To evaluate the relationship between the independent variables and the dependent variable, bivariate analysis will be followed by multivariate logistic regression analysis and the Hosmer and Lemeshow tests. The final model was converted into a scoring system and assessing the final area under the curve (AUC) model to determine predictive mortality value, optimal cut-off point, sensitivity, and specificity value. Results: 265 patients met the requirements, and 83 underwent evacuation surgery. Independent factors associated with ICH patient mortality were high NIHSS on admission, the volume of bleeding (P = 0.016 each), and age >65 years (P = 0.005). Independent factors for good outcomes were pneumonia, sepsis, respiratory failure (p=0.027, p=0.022, p=0.004, respectively), and the absence of mass effect (P<0.001). The nICH score was overall better than the oICH score (AUC 0.864 vs. 0.760). Taking a cut-off value of 2, the nICH score has a sensitivity of 75% and a specificity of 81.4%. Independent factors associated with the mortality of spontaneuous ICH patients who underwent surgery were pneumonia, midline shift >8mm, and intraventricular hemorrhage. By taking a cut-off value of 1, the ICH-SE score has a sensitivity of 71.7% and a specificity of 78.4% with a Youden index (YI) of 0.501. Conclusion: The nICH score proved more effective and reliable than the oICH score in predicting mortality within 30 days of spontaneous ICH patients. This study has also developed an ICH-SE score that can be used to predict 30-day mortality in spontaneous intracerebral hemorrhage patients who are about to undergo an evacuation surgery.

Abstract

Background: Painful diabetic neuropathy (PDN) is a severe complication of type 2 diabetes mellitus (T2DM). Nerve Growth Factor (NGF) is vital for neuronal health, but its status as a systemic biomarker in PDN is contested due to conflicting reports on serum levels. This study aimed to quantitatively synthesize the literature on the association between serum NGF concentrations and the presence of PDN in patients with T2DM. Methods: PubMed, Scopus, Web of Science, and Embase were searched for observational studies from January 2015 to August 2025 that measured serum NGF in T2DM patients with PDN versus control groups (T2DM without PDN or healthy individuals). Data were independently extracted by two reviewers. A random-effects model was used to calculate the pooled Standardized Mean Difference (SMD) and 95% confidence intervals (CIs) to account for assay variability. Heterogeneity was explored using the I² statistic and meta-regression. Results: From 1,482 records, seven cross-sectional studies (485 PDN patients, 511 controls) were included. The meta-analysis revealed that patients with PDN had significantly lower serum NGF concentrations compared to controls, with a pooled SMD of -1.28 (95% CI: -1.79 to -0.77, p < 0.00001). Substantial heterogeneity was present (I² = 84%). Meta-regression showed that a longer duration of diabetes was significantly associated with a greater reduction in NGF levels (p = 0.02). No significant publication bias was detected. Conclusion: This meta-analysis provides consolidated evidence that lower systemic NGF levels are a strong feature of established PDN. The findings support the neurotrophic deficit hypothesis in PDN pathophysiology and identify NGF as a candidate biomarker requiring rigorous validation in longitudinal studies that carefully differentiate painful from painless neuropathy phenotypes.
